Obstructive sleep apnea（OSA） is a serious type of sleep disorder that can lead to cardiometabolic and neurocognitive diseases. We utilized smart device-based photoplethysmography technology to collect sleep data from the Chinese population from 2019 to 2022. Distributed lag nonlinear models combined with a generalized nonlinear model or a linear mixed effects model were used to investigate the shortterm associations between daily temperature and indicators of OSA severity. We included a total of6,232,056 d of sleep monitoring data from 51,842 participants with moderate to severe risk of OSA from313 Chinese cities. The relationships between ambient temperature and OSA exacerbation, apneahypopnea index（AHI）, and minimum oxygen saturation（MinSpO2） were almost linear and present only on the same day. Higher temperatures were associated with a greater risk of OSA exacerbation, with an8.4%(95% confidence interval（CI）: 7.6%–9.3%) increase per 10 °C increase in temperature. A 10 °C increase in daily temperature corresponded to an AHI increase of 0.70 events/h（95% CI: 0.65–0.76）and a MinSpO2decrease of 0.18%（95% CI: 0.16%–0.19%）. Exposure to elevated temperatures during the night can also lead to adverse effects. The effects of higher temperatures on OSA severity were stronger among men, participants with a body mass index ≥ 24 kg/m2, those aged 45 years and older, individuals with a history of hypertension and diabetes, and during the cold season. This large-scale,nationwide, longitudinal study provides robust evidence suggesting that higher ambient temperatures may immediately worsen OSA.

Objective:Observational studies indicate that insomnia may increase the risk of developing and/or dying from cardiovascular diseases,especially coronary artery disease(CAD).Our purpose is to explore the underlying causal relationship between genetic variants susceptible to insomnia and the risk of CAD by Mendelian randomization ***:The study was conducted using publicly available statistical data on genetic variants identified from a genome-wide association meta-analysis of insomnia(n=113,006 individuals)and a genome-wide association meta-analysis of CAD(n=184,305 individuals),which consisted of both cases and *** genetic association between variants and CAD was assessed by the variants’association with insomnia,and estimations were integrated by an inverse-variance weighted ***:Among the Mendelian randomized analytical sample,8 variants were associated with insomnia complaints and *** there was no pleiotropic association with the latent *** addition,in the inverse-variance weighted meta-analysis(the estimations combined from the 8 variants),the odds ratio was 1.15(95%CI:1.05–1.25;P=0.002)for CAD,and in the weighted method analysis,the odds ratio was 1.14(95%CI:1.03–1.27;P=0.015)for ***:All of the data indicated that some valuable variants might involve in the development of CAD by leading the ***,insomnia might be a causal factor for CAD,and improving the quality of sleep might be a new way for populations with insomnia to prevent CAD.

Epidemiological studies showed that night workers are at higher risk of developing chronic metabolic ***,no study has investigated the changes in circadian rhythms caused by a combined effect of sleep and diet in a real-life setting on cardiometabolic health,gut microbiota,and psychological status in healthy people.A 4-week step-wise misaligned-realigned controlled-feeding trial with a 2×2 factorial design(sleep and diet)was conducted on healthy young *** first,subjects experienced a one-week circadian rhythm misalignment with a high-fat fast-food diet,extended eating window,and delayed sleep schedules,then gradually transited to a complete circadian rhythm realignment with a high-fiber balanced diet,8-h timerestricted eating,and normal sleep *** rhythm misalignment led to significantly higher levels of fasting glucose and homeostatic model assessment for insulin resistance(HOMA-IR)of subjects compared to baseline and failed to recover to the baseline level in circadian rhythm ***,the incremental area under the curve(iAUC)of postprandial glucose decreased with circadian rhythm adjustments as compared to that in circadian rhythm misalignment,suggesting circadian rhythm realignment by sleep or/and diet could partly restore glucose metabolism impaired by a short-term circadian rhythm ***,circadian rhythm changes did not result in overall perturbations of gut microbiota diversities.

Background: Segawa syndrome due to GTP cyclohydrolase deficiency is an autosomal dominant disorder with variable expression, that is clinically characterised by L-dopa responsive, diurnally fluctuating dystonia and parkinsonian symptoms. Objective: To delineate the neurological and psychiatric phenotype in all affected individuals of three extended families. Methods: GTP cyclohydrolase deficiency was documented by biochemical analyses, enzymatic measurements in fibroblasts, and molecular investigations. All affected individuals were examined neurologically, and psychiatric data were systematically reviewed. Results: Eighteen affected patients from three families with proven GTP cyclohydrolase deficiency were identified. Eight patients presenting at less than 20 years of age had typical motor symptoms of dystonia with diurnal variation. Five family members had late-presenting mild dopa-responsive symptoms of rigidity, frequent falls, and tendonitis. Among mutation carriers older than 20 years of age, major depressive disorder, often recurrent, and obsessive-compulsive disorder were strikingly more frequent than observed in the general population. Patients responded well to medication increasing serotonergic neurotransmission and to L-dopa substitution. sleep disorders including difficulty in sleep onset and maintenance, excessive sleepiness, and frequent disturbing nightmares were present in 55% of patients. Conclusion: Physicians should be aware of this expanded phenotype in affected members of families with GTP cyclohydrolase deficiency.

Background： Depression is often comorbid with chronic somatic diseases. Few previous studies have investigated the prevalence of somatic diseases in depression or the prescription pattern of antidepressants in comorbidly depressed patients in Asia. This study aimed to investigate the prevalence of somatic comorbidity （SC） in depression and compared the prescriptions of antidepressants in depressed patients with and without SC. Methods： A total of 2320 patients treated with antidepressants in 8 Asian countries were examined, and a diagnosis was based on the International Classification of Disease, 10^th revision. We listed 17 common chronic somatic diseases. Patients＇ socio-demographic and clinical characteristics and psychotropic drug prescriptions were recorded using a standardized protocol and data collection procedure. Results： Of the patients examined, 1240 were diagnosed with depression and 30% of them （n = 375） had SC. The most common comorbid condition was diabetes （23.7%）. The patients with SC were more likely to seek help at a general hospital （74.7% vs. 47.2%）, and had a higher incidence of symptoms involving sadness, disturbed sleep, and poor appetite. Noradrenergic and specific serotonergic antidepressant was prescribed more for patients with SC than for those without SC （30.4% vs, 22.9%）. Conclusions： SC is common in depressed Asian patients. It is important to strengthen the recognition of depression, especially in general hospitals and when patients report some somatic discomfort. It is also a matter of urgency to establish evidence-based guidelines for the use of new antidepressants in depressed patients with SC.

When an animal faces a threatening situation while asleep, rapid arousal is the essential prerequisite for an adequate response. Here, we find that predator stimuli induce immediate arousal from REM sleep compared with NREM sleep. Using in vivo neural activity recording and cell-type-specific manipulations, we identify neurons in the medial subthalamic nucleus(mSTN) expressing corticotropin-releasing hormone(CRH) that mediate arousal and defensive responses to acute predator threats received through multiple sensory modalities across REMsleep and wakefulness. Weobserve involvement of the sameneurons in the normal regulation of REMsleep and the adaptive increase in REMsleep induced by sustained predator stress. Projections to the lateral globus pallidus(LGP) are the effector pathway for the threat-coping responses and REM-sleep expression. Together, our findings suggest adaptive REM-sleep responses could be protective against threats anduncover a critical component of the neural circuitry at their basis.
