Inhibin B:are modified ranges needed for orchiectomised testicular cancer patients?

Inhibin B is a gon adal horm one that down regulates the pituitary production of follicle-stimulati ng horm one (FSH). In recent years, inhibin B has proved to be an excellent marker of spermatogenesis and even a predictive factor for the recovery of fertility in patients undergoing orchiectomy and antineoplastic treatments. We propose to study inhibin B levels in orchiectomised testicular cancer patients, in order to identify a minimum value representative of normal semen quality. This retrospective study evaluates horm onal and semen parameters of 290 no rmozoospermic patie nts atte nding the laboratory of Semi no logy - Sperm Bank "Loredana Gandini"(Rome, Italy) for cryopreservation of seminal fluid following a diagnosis of testicular cancer (TC group) and 117 healthy, normozoospermic men as a control group (CTR group). The percentile distribution of gonadotropin and inhibin B values in the TC and CTR groups was analyzed. There was a statistically significant differenee between the two groups in the levels of all hormones (P ≤ 0.001) and in all semen parameters (P 95^th percentile of CTR group. Orchiectomised patients for testicular cancer presented inhibin B levels lower than healthy patients, despite normozoospermia. Our study revealed the poor sensitivity of the current inhibin B reference range when applied to monorchidic patients, suggesting the need to establish more representative ranges to enable more appropriate counseling in relation to the patienfs new endocrine condition.

Various conditions affecting nerve cells and the nervous system due to the loss of neurons and their connecting networks are described under the superordinate phrase“Neurodegenerative diseases”.Such diseases lead to disability due to gradual neuronal death in both the central nervous system and the peripheral nervous system.While many of these diseases have unknown causes,sometimes these are due to medical conditions such as alcoholism,a tumor,or a stroke,or other causes which may include genetic mutations,toxins,chemicals,and viruses.

Effects of the Huangkui capsule(黄葵胶囊)on chronic kidney disease:a systematic review and Meta-analysis

OBJECTIVE:To collect data and investigate the effects of Huangkui capsule(黄葵胶囊,HKC)on chronic kidney disease(CKD).METHODS:PubM ed,Embase,Cochrane Library,and three Chinese databases(China National Knowledge Infrastructure,Wanfang,and China Science and Technology Journal Database)were searched for articles published until October 2020.Randomized controlled trials(RCTs)of HKCs used for treating CKD were reviewed.Data were organized and analyzed using RevMan 5.3 software.RESULTS:HKC significantly lowered the blood urea nitrogen(BUN)level[mean difference(MD)=-2.26;95%confidence interval(CI),-2.91 to-1.61],serum creatinine level(MD=-17.45;95%CI,-22.29 to-12.60),and 24-h urine protein content(MD=-0.73;95%CI,-1.00 to-0.46)and improved the glomerular filtration rate(GFR)(MD=10.69;95%CI,5.57 to 15.81)and serum albumin level(MD=2.20;95%CI,0.49 to 3.90).CONCLUSIONS:Our findings show that HKC could lower the BUN level,serum creatinine level,and 24-h urine protein content.HKC also improved GFR and serum albumin levels.However,high-quality RCTs in other countries with larger sample sizes are warranted to confirm these findings.

In recent work,we have shown that cell senescence of mouse fibroblasts in vitro associates with a build-up of cryptic exons in selected mRNAs,whose level is usually controlled by the activity of TAR DNA binding protein of 43 kDa(Tdp-43)(Torres et al.,2022).In vivo,we also found traits of cell senescence in the motor neuron disease model achieved by overexpressing SOD-G93A,the SOD1 gene(harboring a single amino acid substitution of glycine to alanine at codon 93).

Why microglia kill neurons after neural disorders? The friendly fire hypothesis

Neuroinflammation plays a fundamental role on the pathophysiology of acute and chronic neural disorders.Microglia activation is a major event following central nervous system inflammation displaying different phenotypes with beneficial and detrimental actions(a Janus face).The reason for this apparent duality is unknown.We have previously shown that following experimental middle cerebral artery occlusion in the rat brain,microglia seem to support and impair adult neurogenesis in the same ischemic striatum.Based on these results,we raised the hypothesis that in the same pathologic environment,gradients of different ligands distributed over different anatomical niches might contribute to both detrimental and beneficial microglial phenotypes.These ligands(“danger signals”)are released by dying cells and bind to microglial receptors in their membranes.Activation of different microglial receptors induces downstream biochemical pathways culminating in a spectrum of microglial phenotypes like M1 and M2 and others.In this paper,we first review the immune functions of microglia and the role of toll-like receptors on the fight against infections.We then briefly revise the dual role of microglia after neural disorders.We then propose a novel hypothesis to explain the Janus face of microglia during the pathophysiology of central nervous system diseases:the“friendly fire hypothesis”.According to this idea“danger signals”or danger associated molecular patterns released by stressed,damaged and/or dying cells during stroke,trauma and other diseases might activate microglial pattern-recognition receptors(i.e.,toll like receptors)or other unidentified receptors normally activated by pathogens.This could activate the same genetic and biochemical machinery used by microglia to fight against pathogens even in the absence of infection.According to this notion,microglia may cause bystander neuronal damage with a kind of blind“friendly fire”,fighting against a non-existing infection during non-infectious disorders,like stroke and trauma

Neuroprotective effects of erythropoietin on neurodegenerative and ischemic brain diseases: the role of erythropoietin receptor

Erythropoietin （Epo） is a fundamental hormone in the regulation of hematopoiesis, and other secondary roles mediated by the binding of the hormone to its specific receptor （EpoR）, which leads to an activation of key signaling pathways that induce an increase in cell differentiation, apoptosis control and neuroprotection. It has been suggested that their ftmction depends on final conformation of glycosylations, related with affinity to the receptor and its half-life. The presence of EpoR has been reported in different tissues including central nervous system, where it has been demonstrated to exert a neuroprotective function against oxidative stress conditions, such as ischemic injury and neurodegenerative diseases. There is also evidence of an increase in EpoR expression in brain cell lysates of Alzheimer＇s patients with respect to healthy patients. These results are related with extensive in vitro experimental data of neuroprotection obtained from cell lines, primary cell cultures and hippocampal slices. Additionally, this data is correlated with in vivo experiments （water maze test） in mouse models of Alzheimer＇s disease where Epo treatment improved cognitive function. These stud- ies support the idea that receptor activation induces a neuroprotective effect in neurodegenerative disorders including dementias, and especially Alzheimer＇s disease. Taken together, available evidence suggests that Epo appears to be a central element for EpoR activation and neuroprotective properties in the central nervous system. In this review, we will describe the mechanisms associated with neuroprotection and its relation with the activation of EpoR in order with identify new targets to develop pharmacological strategies.

Structural basis of signaling of cannabinoids receptors:paving a way for rational drug design in controling mutiple neurological and immune diseases

Cannabinoids(CBs),analgesic drugs used for thousands of years,were first found in Cannabis sativa,and the multiple CBs used medicinally,such as tetrahydrocannabinol(THC),cannabidiol(CBD)and dozens more,have complex structures.In addition to their production by plants,CBs are naturally present in the nerves and immune systems of humans and animals.Both exogenous and endogenous CBs carry out a variety of physiological functions by engaging with two CB receptors,the CB1 and CB2 receptors,in the human endocannabinoid system(ECS).Both CB1 and CB2 are G protein-coupled receptors that share a 7-transmembrane(7TM)topology.CB1,known as the central CB receptor,is mainly distributed in the brain,spinal cord,and peripheral nervous system.CB1 activation in the human body typically promotes the release of neurotransmitters,controls pain and memory learning,and regulates metabolism and the cardiovascular system.Clinically,CB1 is a direct drug target for drug addiction,neurodegenerative diseases,pain,epilepsy,and obesity.Unlike the exclusive expression of CB1 in the nervous system,CB2 is mainly distributed in peripheral immune cells.Selective CB2 agonists would have therapeutic potential in the treatment of inflammation and pain and avoid side effects caused by currently used clinical drugs.Although significant progress has been made in developing agonists toward CB receptors,efficient clinical drugs targeting CB receptors remain lacking due to their complex signaling mechanisms.The recent structural elucidation of CB receptors has greatly aided our understanding of the activation and signal transduction mechanisms of CB receptors.

A comprehensive neuromonitoring approach in a large animal model of cardiac arrest

Background:Anoxic brain injuries represent the main determinant of poor outcome after cardiac arrest(CA).Large animal models have been described to investigate new treatments during CA and post-resuscitation phase,but a detailed model that includes extensive neuromonitoring is lacking.Method:Before an electrically-induced 10-minute CA and resuscitation,46 adult pigs underwent neurosurgery for placement of a multifunctional probe(intracranial pressure or ICP,tissue oxygen tension or PbtO_(2) and cerebral temperature)and a bolt-based technique for the placement and securing of a regional blood flow probe and two sEEG electrodes;two modified cerebral microdialysis(CMD)probes were also inserted in the frontal lobes and accidental misplacement was prevented using a perforated head support.Result:42 animals underwent the CA procedure and 41 achieved the return of spontaneous circulation(ROSC).In 4 cases(8.6%)an adverse event took place during preparation,but only in two cases(4.3%)this was related to the neurosurgery.In 6 animals(13.3%)the minor complications that occurred resolved after probe repositioning.Conclusion:Herein we provide a detailed comprehensive neuromonitoring approach in a large animal model of CA that might help future research.

HMGB1 from Astrocytes Promotes EAE by Influencing the Immune Cell Infiltration-Associated Functions of BMECs in Mice

High mobility group box 1(HMGB1)has been reported to play an important role in experimental autoimmune encephalomyelitis(EAE).Astrocytes are important components of neurovascular units and tightly appose the endothelial cells of microvessels by their perivascular endfeet and directly regulate the functions of the blood-brain barrier.Astrocytes express more HMGB1 during EAE while the exact roles of astrocytic HMGB1 in EAE have not been well elucidated.Here,using conditional-knockout mice,we found that astrocytic HMGB1 depletion decreased morbidity,delayed the onset time,and reduced the disease score and demyelination of EAE.Meanwhile,there were fewer immune cells,especially pathogenic T cells infiltration in the central nervous system of astrocytic HMGB1 conditional-knockout EAE mice,accompanied by up-regulated expression of the tight-junction protein Claudin5 and down-regulated expression of the cell adhesion molecules ICAM1 and VCAM1 in vivo.In vitro,HMGB1 released from astrocytes decreased Claudin5 while increased ICAM1 and VCAM1 expressed by brain microvascular endothelial cells(BMECs)through TLR4 or RAGE.Taken together,our results demonstrate that HMGB1 derived from astrocytes aggravates EAE by directly influencing the immune cell infiltration-associated functions of BMECs.

Molecular chaperones in stroke-induced immunosuppression

Stroke-induced immunosuppression is a process that leads to peripheral suppression of the immune system after a stroke and belongs to the central nervous system injury-induced immunosuppressive syndrome.Stroke-induced immunosuppression leads to increased susceptibility to post-stroke infections,such as urinary tract infections and stroke-associated pneumonia,worsening prognosis.Molecular chaperones are a large class of proteins that are able to maintain proteostasis by directing the folding of nascent polypeptide chains,refolding misfolded proteins,and targeting misfolded proteins for degradation.Various molecular chaperones have been shown to play roles in stroke-induced immunosuppression by modulating the activity of other molecular chaperones,cochaperones,and their associated pathways.This review summarizes the role of molecular chaperones in stroke-induced immunosuppression and discusses new approaches to restore host immune defense after stroke.
