Objectives:We aimed to evaluate the effect of percutaneous atrial septal defect(ASD)closure in children using right heart indices and serum galectin-3(Gal-3),as potential biomarkers of right heart ***:This case–control prospective study included 40 children with ASD and 25 control *** echocardiographic evaluation was performed before the procedure,as well as 24 h,1 month,and 6 months after *** Gal-3 was measured before,and 1 month after the ***:Serum Gal-3 concentration,right atrial(RA)dimensions,right ventricular(RV)dimensions,indexed RA area,and right index of myocardial performance(RImP)were significantly increased in children with ASD compared with control subjects while tricuspid annular plane systolic excursion(TAPSE)was significantly *** months after closure,RA,and RV dimensions significantly decreased and RVfunction improved(RImP decreased and TAPSE increased).Gal-3 oncentration significantly decreased 1 month after ASD closure,but it did not reach normal range compared with control subjects.A positive correlation between Gal-3 and age at closure,RA area,RV dimensions,and RImP was observed.A positive correlation was observed between the decrease in Gal-3 concentration and the decrease in RA area and RV dimensions 1 month after ASD closure.A significant negative correlation was observed between TAPSE and Gal-3 concentration before and after ***:Percutaneous ASD closure can improve right-sided indices and decrease serum Gal-3 ***-3 can be used as a sensitive biomarker of right heart remodeling,with a decrease in Gal-3 concentration suggesting reversal of maladaptive remodeling.

Introduction:Toxic epidermal necrolysis(TEN)is a medical emergency that most commonly occurs as an adverse effect of certain ***,we describe a case of a 41-year-old man with no comorbid illness who developed *** presentation:The patient had been prescribed ibuprofen for myalgia and developed skin lesions after the single *** lesions were erythematous papules and macules distributed all over the body after ibuprofen *** was diagnosed based on the patient’s clinical presentation and laboratory *** was treated with intravenous dexamethasone,intravenous immunoglobulin,and *** dressing changes and skin care was done with saline,chlorhexidine,and liquid *** patient was intubated and tracheostomized,and he gradually improved and ***,he developed septicemia in the intensive care unit and was treated ***:The management of TEN includes cessation of the causative cause,multidisciplinary intensive care unit(ICU)care,prevention and early detection of sepsis,fluid and electrolyte balance,adequate analgesia and temperature control,proper organ support,aggressive nutritional management,and good psychological *** pharmacological therapy for TEN includes corticosteroids,intravenous immunoglobulin,and *** key elements of management are aseptic care and proper dressing of the ***:TEN is associated with high mortality if not managed in a systemic and protocolized way.

Helicobacter pylori and other Helicobacter species DNA in human bile samples from patients with various hepato-biliary diseases

AIm： To investigate the presence of Helicobacter species by nested PCR of 16S rRNA genes followed by the presence of Flelicobacter pylori （H pylori） 16S rRNA, ureA, cagA genes in bile obtained at endoscopic retrograde cholangio-pancreatography （ERCP） from 60 Indian subjects. mETHODS： Sixty bile samples were obtained from patients diagnosed with various hepato-biliary diseases and control subjects at ERCP. PCR analysis was carried out using primers for Helicobacter genus 16S rRNA gene and H pylori （16S rRNA, urea and cagA） genes. Gastric H pylori status was also assessed from biopsies obtained at endoscopy from patients with various hepatobiliary diseases and controls. The control group mainly consisted of subjects with gastric disorders. Sequencing analysis was performed to confirm that PCR products with 16S rRNA and cagA primers were derived from H pylori. RESULTS No Helicobacters were grown in culture from the bile samples. Helicobacter DNA was detected in bile of 96.7% and 6.6% of groups Ⅰ and Ⅱ respectively. Ten from group Ⅰ were positive for 16S rRNA and ureA and 9 were positive for cagA gene. In contrast of the 2 from the control, 1 amplified with 16S rRNA, ureA and cagA primers used. The sequences of the 16S rRNA genes and cagA were 99% similar to Helicobacter pylori. CONCLUSION： Helicobacters are associated with the pathogenesis of various hepato-biliary disorders.

Synthesis, Spectroscopic and Antimicrobial Investigations of Scandium(Ⅲ) Complexes with Four Kinds of Sulfa Drugs

Herein,this article was focused on the synthesis and discussed the spectroscopic characterizations of four new scandium(Ⅲ)sulfa-drug *** nomenclature and symbols of these drugs were sulfadimidine(sulp-1),sulfanilamide(sulp-2),sulfamethoxazole(sulp-3)and sulfadiazine(sulp-4).The microanalytical and spectroscopic analyses which utilized in this study were micro-analyses,magnetic,FT-IR,UV-Vis *** mid infrared spectra deduced that the four sulfa-drug chelates acts as a bidentate chelates with scandium(Ⅲ)ion via two nitrogen atoms of-NH2-Ar and-NH-SO2 ***,the FTIR spectra of Sc3+complexes referred to the existed of new medium weak bands in the range of 500~400 cm^-1 due to stretching vibration bands ofν(m-N).The elemental analysis technique confirmed the 1∶2 stoichiometry between Sc3+ions and sulp ligand with molecular formula[Sc(sulp)2(Cl)2]·*** room temperature,the results of magnetic measurements for the Sc(Ⅲ)complexes indicated that all of the synthesized complexes have a diamagnetic character with octahedral *** electronic spectra of the free sulfa-drug ligands shows band at 275 and 310 nm which are intraligand charge transfer *** electronic sbsorption spectra of the Sc3+complexes were recorded using DmSO *** spectra of complexes display bands within 275~388 nm,which attributed toπ-π*,n-π*and charge-transfer m-LCT electronic transitions,which strongly favors the octahedral geometry around Sc(Ⅲ)metal ions.1HNmR spectra of complexes referred to the downfield proton shifts of the-NH2 and NHSO2 groups,which supported the place of *** half maximal inhibitory concentration(IC50)of the ScⅢcomplexes was assessed against the human hepato cellular carcinoma(HepG-2)tumor cell line.

Synthesis and Spectroscopic Investigations of Four New Y(Ⅲ), Ge(Ⅳ), W(Ⅵ) and Si(Ⅳ) Penicillin Antibiotic Drug Complexes

A four new penicillinate complexes were prepared through the chemical interactio ns of penicillin potassium salt (Pin) with YCl 3, GeCl 4, WCl 6 and SiCl 4 m etal ions. These metal complexes were characterized using spectroscopic techniqu es (e.g. 1H-NmR, infrared, electronic UV-Vis) as well as elemental, cond uctivity, and magnetic measurements. The molar conductance values were highly, s howing their electrolytic nature. The magnetic and electronic study strongly rec ommends the octahedral geometry of all penicillinate complexes. A monomeric stru ctures of Pin complexes are proposed with octahedral coordinated metals ions. Th e metal ions are coordinated toward Pin as tridentate ligand through the amide a nd β-lactam carbonyls and a carboxylate group from penicillin. The in vitro antimicrobial activity of all the complexes, at concentrations in μg·mL^-1 , was screened against four bacterial pathogens, namely, Bacillus subtilis , Escherichia coli, Pseudomonas aeruginosa , and Staphylococcus aureus , and two kinds of fungi Aspergillus flavus and Candida albicans showed better activi ty compared to parent drug and control drug. The anti-cancer inhibition of the tungsten(Ⅵ) complex was assessed against the human hepato cellular carcinoma (H epG-2) tumor cell line with IC50 value is 646 μg·mL^-1 .

Hepatoprotective activity of Sapindus mukorossi and Rheum emodi extracts: In vitro and in vivo studies

AIm: To study the hepatoprotective capacity of Sapindus mukorossi (S. mukorossi) and Rheum emodi (R. emodi) extracts in CCl4 treated male rats. mETHODS: The dried powder of S. mukorossi and R. emodi was extracted successively with petroleum ether, benzene, chloroform, and ethanol and concentrated in vacuum. Primary rat hepatocyte monolayer cultures were used for in vitro studies. In vivo, the hepatoprotective capacity of the extract of the fruit pericarp of S. mukorossi and the rhizomes of R. emodi was analyzed in liver injured CCl4-treated male rats. RESULTS: In vitro: primary hepatocytes monolayer cultures were treated with CCl4 and extracts of S. mukorossi & R. emodi. A protective activity could be demonstrated in the CCl4 damaged primary monolayer culture. In vivo : extracts of the fruit pericarp of S. mukorossi (2.5 mg/mL) and rhizomes of R. emodi (3.0 mg/mL) were found to have protective properties in rats with CCl4 induced liver damage as judged from serum marker enzyme activities. CONCLUSION: The extracts of S. mukorossi and R. emodi do have a protective capacity both in vitro on primary hepatocytes cultures and in in vivo in a rat model of CCl4 mediated liver injury.

BACKGROUND Hepatobiliary manifestations occur in ulcerative colitis(UC)*** effect of laparoscopic restorative proctocolectomy(LRP)with ileal pouch anal anastomosis(IPAA)on hepatobiliary manifestations is *** To evaluate hepatobiliary changes after two-stages elective laparoscopic restorative proctocolectomy for patients with *** Between June 2013 and June 2018,167 patients with hepatobiliary symptoms underwent two-stage elective LRP for UC in a prospective observational *** with UC and having at least one hepatobiliary manifestation who underwent LRP with IPAA were included in the *** patients were followed up for four years to assess the outcomes of hepatobiliary *** The patients'mean age was 36±8 years,and males predominated(67.1%).The most common hepatobiliary diagnostic method was liver biopsy(85.6%),followed by magnetic resonance cholangiopancreatography(63.5%),Antineutrophil cytoplasmic antibodies(62.5%),abdominal ultrasonography(35.9%),and Endoscopic retrograde cholangiopancreatography(6%).The most common hepatobiliary symptom was Primary sclerosing cholangitis(PSC)(62.3%),followed by fatty liver(16.8%)and gallbladder stone(10.2%).66.4%of patients showed a stable course after *** or regressive courses occurred in 16.8%of *** was 6%,and recurrence or progression of symptoms required surgery for 15%.most PSC patients(87.5%)had a stable course,and only 12.5%became ***-thirds(64.3%)of fatty liver patients showed a regressive course,while one-third(35.7%)showed a stable *** rates were 98.8%,97%,95.8%,and 94%at 12 mo,24 mo,36 mo,and at the end of the *** In patients with UC who had LRP,there is a positive impact on hepatobiliary *** caused an improvement in PSC and fatty liver *** most prevalent unchanged course was PSC,while the most common improvement was fatty liver disease.

Ovarian vein thrombosis (OVT) is a rare thrombotic condition commonly attributed to the puerperium. The complications of OVT can be significant, and diagnosis relies on a high clinical index of suspicion. It can occur with lower quadrant pain that may mimic a surgical abdomen, especially in the setting of recent pregnancy, abdominal surgery, inflammatory disease, or malignancy. Diagnosis can be made with confidence using ultrasound, computed tomography, or magnetic resonance imaging. management of OVT is particularly important in postpartum patients, with anticoagulation therapy being the treatment of choice.
