Objective: To investigate the effects of a herb complex extract(HCE) prepared from Cornus officinalis Sieb. Et Zucc., Eriobotrya japonica Lindley, and olive leaves on immune response of mouse spleen NK cells in vitro and in vivo analysis. Methods: The activity of natural killer(NK) cells was measured in splenocytes and YAC-1 cells. Mice were immunosuppressed using cyclophosphamide(5 mg/kg body weight). Three different doses of HCE(200, 400, and 800 mg/kg body weight) and red ginseng extract(800 mg/kg body weight) which was used as standard immunomodulatory herb were administered orally for 4 weeks. The body weight, dietary, water intake, organs(liver, thymus, and spleen) weight, completed blood count, and cytokines(tumor necrosis factor alpha, interferon gamma, and interleukin-2) production was measured. Results: At the maximum concentration of HCE, the activity of NK cells was increased by 48.5%. HCE increased liver, spleen, and thymus weights without altering numbers of white blood cells, lymphocytes, and neutrophils in a cyclophosphamide-induced immunosuppression rat model. However, HCE recovered the inhibited cytokine expression; HCE(800 mg/kg) increased cytokines levels. The results indicate the immune enhancement potential of this HCE. Conclusion: The HCE enhances immunity by increasing NK cell activity, regulating cytokine levels, and maintaining spleen weight. Therefore, it may be used as a potential immunity enhancer.

Effect of different fire strengths on the smoke distribution in the subway station is investigated. Shin-Gum-Ho station （line #5） in Seoui is selected as a case study for variation of CO （carbon monoxide） distribution caused by the fire in the platform. The ventilation in the station is set to be an air supply mod in the lobby and an air exhaustion mod in the platform. One-side main tunnel ventilation （7,000 m3/min） is applied to operate in the tunnel. The fire is assumed to break out in the middle of train parked in the platform tunnel. Two kinds of fire strength are used. One is 10 MW and the other is 20 MW. Ventilation diffusers in the station are modeled as 317 square shapes ＆ four rectangular shapes in the lobby and platform. The total of 7.5 million grids is generated and whole domain is divided to 22 blocks for parallel computation. Large eddy simulation method is applied to solve the momentum equation. The behavior of CO is calculated according to different fire strengths and compared with each other.

AIM:To systematically assess the association between diabetes and incidence of gastric ***:We searched MedLine (PubMed),EMBASE,and the Cochrane Library without any limitations with respect to publication date or language,we also searched the references of qualifying *** studies and cohort studies comparing the risk of gastric cancer between diabetic patients and control subjects were *** excluded studies reporting only standardized incidence ratios without control groups and those that investigated only mortality but not *** studies met our criteria,and the qualities of these studies were assessed using theNewcastle-Ottawa Quality Assessment *** performed a meta-analysis of pre-existing diabetes and gastric cancer incidence using the DerSimonian-Laird method for *** subgroup analyses,we separated the studies by study type,region,sex and method to determine confounding factors and *** also conducted subgroup analyses to examine the effects of smoking,Helicobacter pylori (***) infection,and cancer *** bias was evaluated using Begg's ***:A random-effects model meta-analysis showed an increased gastric cancer risk in diabetic patients [relative risk (RR)=1.19;95%CI:1.08-1.31].Subgroup analyses indicated that this result persisted in cohort studies (RR=1.20;95%CI:1.08-1.34),in studies on populations of both Western (RR=1.18;95%CI:1.03-1.36) and Eastern countries (RR=1.19;95%CI:1.02-1.38),in a female subgroup (RR=1.24;95%CI:1.01-1.52),and in highly qualified studies (RR=1.17;95%CI:1.05-1.31).Moreover,these results persisted when the analysis was confined to studies adjusted for well-known gastric cancer risk factors such as smoking (RR=1.17;95%CI:1.01-1.34) and *** infection (RR=2.35;95%CI:1.24-4.46).CONCLUSION:Pre-existing diabetes mellitus may increase the risk of gastric cancer by approximately 19%.This effect seems to be unrelated to geographical region.

AIM： To explore the relation between B-cell-specific Moloney murine leukemia virus insertion site 1 （Bmi-1） expression and the clinicopathological features of gastric carcinoma （GC）.METHODS： Immunohistochemistry was used to detect the expression of Bmi-1 and ki-67. Doublelabeling staining was used to display the distribution of Bcl-2^＋/ki-67 cells in 162 cases of GC and its matched normal mucosa and precancerous ***： The positive rate of Bmi-1 expression in GC（52.5%） was significantly higher than that in normal gastric mucosa （21.6%, X^2 = 33.088, P 〈 0.05）. The Bmi-1 expression in GC was closely related with the Lauren＇s and Borrmann＇s classification and clinicalstage （X^2 = 4.400, 6.122 and 11.190, respectively, P〈 0.05）. The expression of ki-67 was related to the Borrmann＇s classification （X^2 = 13.380, P 〈 0.05）.Bcl-2 expression was correlated with the Lauren＇s classification （Z2 = 4.725, P 〈 0.05）, and the Bmi-1 expression both in GC （rk = 0.157, P 〈 0.05） and inintestinal metaplasia （rk = 0.270, P 〈 0.05）.CONCLUSION： Abnormal Bmi-1 expression in GCmay be involved in cell proliferation, apoptosis andcancerization. This marker can objectively indicate theclinicopathological characteristics of GC.

BACKGROUND: Although perioperative portal vein (PV) stent implantation is an effective treatment for steno-occlusive disease in adult living donor liver transplantation (LDLT) re-cipients, we experienced high incidence of biliary anastomotic strictures (BAS) after PV stenting. In this study, we sought to clarify the relation between BAS and PV stenting and to suggest the possible mechanism of BAS and measures to reduce its in-cidence. METHODS: We retrospectively analyzed 44 LDLT recipients who underwent PV stent implantation across the line of PV anastomosis regardless of the location of steno-occlusion (stent group) and their matched controls (non-stented LDLT recipients, n=131). RESULTS: The incidence of BAS was higher in patients in the stent group than that in the control group (43.2% vs 17.6%, P=0.001). Cumulative 6-month and 1-, 2- and 5-year BAS rates were 31.8%, 34.1%, 41.4% and 43.2%, respectively, in the stent group and 13.0%, 13.8%, 16.1% and 17.8%, respectively, in the control group (P=0.001). Multivariate analysis revealed that PV stenting was an independent risk factor for BAS. CONCLUSIONS: Although PV stent implantation is a reliable treatment modality for steno-occlusive PV in adult LDLT recipients, innovative methods to prevent the PV stent from crossing the line of PV anastomosis may be necessary to reduce the incidence of postoperative BAS.

AIM:To determine the effects of curcumin,(-)-epigallocatechin-3-gallate (EGCG),lovastatin,and their combinations on inhibition of esophageal ***:Esophageal cancer TE-8 and SKGT-4 cell lines were subjected to cell viability methyl thiazolyl tetrazolium and tumor cell invasion assays in vitro and tumor formation and growth in nude mouse xenografts with or without curcumin,EGCG and lovastatin *** expression was detected using immunohistochemistry and Western blotting in tumor cell lines,tumor xenografts and human esophageal cancer tissues,***:These drugs individually or in combinations significantly reduced the viability and invasion capacity of esophageal cancer cells in ***,these three agents reduced the expression of phosphorylated extracellular-signal-regulated kinases (Erk1/2),c-Jun and cyclooxygenase-2 (COX-2),but activated caspase 3 in esophageal cancer *** nude mouse xenograft assay showed that EGCG and the combinations of curcumin,EGCG and lovastatin suppressed esophageal cancer cell growth and reduced the expression of ki67,phosphorylated Erk1/2 and *** expression of phosphorylated Erk1/2 and COX-2 in esophageal cancer tissue specimens was also analyzed using *** data demonstrated that 77 of 156 (49.4%) tumors expressed phosphorylated Erk1/2 and that 121 of 156 (77.6%) esophageal cancers expressed COX-2 *** particular,phosphorylated Erk1/2 was expressed in 23 of 50 (46%) cases of esophageal squamous cell carcinoma (SCC) and in 54 of 106 (50.9%) cases of adenocarcinoma,while COX-2 was expressed in 39 of 50 (78%) esophageal SCC and in 82 of 106 (77.4%) esophageal ***:The combinations of curcumin,EGCG and lovastatin were able to suppress esophageal cancer cell growth in vitro and in nude mouse xenografts,these drugs also inhibited phosphorylated Erk1/2,c-Jun and COX-2 expression.
