Epidemiological Patterns of AIDS in a Reference center from Catanduva,Sao Paulo State,Brazil

Objective: Hereby we present the epidemiological and clinical profile of HIV-infected population before and during the highly active antiretroviral therapy (HAART) era from a tertiary care hospital in the Southeastern region of Brazil. Methods: A retrospective, cross-sectional and descriptive study was carried out, this involved the analysis of the medi-cal records of patients diagnosed with HIV-1/AIDS admitted to Hospital Escola Emílio Carlos, located in the munici-pality of Catanduva, State of S?o Paulo, Brazil. Results: In both pre-HAART and HAART periods, HIV-1 infection was more prevalent in men. Heterosexuality and secondary education were associated with AIDS in the HAART period. Statistically significant association was only observed for co-infection with HIV-1/Hepatitis C in the pre-HAART era and the number of patients with opportunistic infection (OI) was lower in the HAART period. Among all OI it is worth mentioning pulmonary pneumocystosis, which despite being common in two periods, its occurrence was considerably greater in the pre-HAART era. Concerning the distribution of OIs according to the HIV-1 viral load and serial count of T CD4+ lymphocytes, a significant association was observed. The association between the number of deaths by OIs and death in the 1st year of diagnosis in the HAART treatment was significant. Conclusions: The clinical and epidemiological profile of a specialized HIV-1/AIDS center in Catanduva, Southeastern Brazilian region, is consistent with the epidemiology of AIDS in the country.

Background: To date, there is no effective medicine to treat coronavirus disease 2019 (COVID-19), and the antiviral efficacy of arbidol in the treatment for COVID-19 remained equivocal and controversial. The purpose of this study was to evaluate the efficacy and safety of arbidol tablets in the treatment of ***: This was a prospective, open-label, controlled and multicenter investigator-initiated trial involving adult patients with confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Patients were stratified 1:2 to either standard-of-care (SOC) or SOC plus arbidol tablets (oral administration of 200 mg per time, three times a day for 14 days). The primary endpoint was negative conversion of SARS-CoV-2 within the first week. The rates and 95% confidential intervals were calculated for each ***: A total of 99 patients with laboratory-confirmed SARS-CoV-2 infection were enrolled;66 were assigned to the SOC plus arbidol tablets group, and 33 to the SOC group. The negative conversion rate of SARS-CoV-2 within the first week in patients receiving arbidol tablets was significantly higher than that of the SOC group (70.3% [45/64]vs. 42.4% [14/33];difference of conversion rate 27.9%;95% confidence interval [CI], 7.7%-48.1%;P=0.008). Compared to those in the SOC group, patients receiving arbidol tablets had a shorter duration of clinical recovery (median 7.0 daysvs. 12.0 days;hazard ratio [HR]: 1.877, 95% CI: 1.151-3.060,P=0.006), symptom of fever (median 3.0 daysvs. 12.0 days;HR: 18.990, 95% CI: 5.350-67.410,P 0.05). The most common adverse event in the arbidol tablets group was the elevation of transaminase (5/200, 2.5%), and no one withdrew from the study due to adverse events or disease progression.C

目的分析内分泌代谢专科住院老年2型糖尿病患者低血糖特征。方法提取2014年1月~2015年12月内分泌科住院2型糖尿病1187例次患者共58 565次床边血糖监测系统(BGMS)数据,按患者年龄分为<45岁(128例次)、45~64岁(594例次)和≥65岁(465例次)3组,通过3组对比,分析老年患者住院期间低血糖发生率及时段分布特征。结果随年龄增加,低血糖发生风险逐渐增加,≥65岁组低血糖密度、患者占比和患者-住院日占比分别是<45岁组的2.38、2.57、2.54倍(0.95%vs 0.40%,28.17%vs 10.94%,4.48%vs 1.76%,P<0.001);凌晨是老年组低血糖高发时段,相比<45岁和45~64岁组,≥65岁组凌晨的低血糖密度分别是其2.44和1.40倍(2.66%vs 1.09%vs 1.90%,P<0.05),患者占比分别是其3.86和1.55倍(14.57%vs 3.77%vs 9.42%,P<0.02);≥65岁组反复低血糖(≥2次)患者占比是其他两组的5.70倍和1.41倍(13.33%vs 2.34%vs 9.43%,P<0.05)。结论 65岁及以上2型糖尿病患者低血糖风险显著高于非老年组,凌晨和反复低血糖情况最为突出。应针对老年糖尿病患者制订差异化血糖监测和管理策略,以提高医疗安全。

Background Trabeculectomy has become a mainstream treatment in intraocular pressure （lOP） reduction for primary angle-closure glaucoma （PACG）; combined trabeculectomy and cataract surgery was reported to reduce lOP and simultaneously improve vision for patients with PACG and coexisting cataract. This study was specialized to compare the efficacy and safety of combined phacotrabeculectomy with that of trabeculectomy only in the treatment of PACG with coexisting cataract. Methods This is a comparative case series study. Thirty-one patients （31 eyes） with PACG and coexisting cataract were enrolled. Of these, 17 underwent phacotrabeculectomy and 14 underwent trabeculectomy alone, lOP, filtering blebs, and complications were compared at the final follow-up. Complete success was defined as a final lOP less than 21 mmHg without lOP-lowering medication. Results After 10 months of postoperative follow-up, the phacotrabeculectomy and trabeculectomy groups showed no significant differences regarding lOP reduction （（20.59±7.94） vs. （24.85±14.39） mmHg, P=0.614）, complete success rate （88% vs. 71%, P=0.370）, formation rate of functioning blebs （65% （11/17） vs. 93% （13/14）, P=0.094）, and complications （41% （7/17） vs. 57% （8/14）, P=0.380）. lOP-lowering medication was not required for most of the patients in both groups. Additional surgery interventions, including anterior chamber reformation and phacoemulsification, were needed in the trabeculectomy group, whereas no surgery was needed postoperatively in the phacotrabeculectomy group. Conclusion Phacotrabeculectomy and trabeculectomy treatments exhibit similar lOP reduction, successful rates, and complications when it comes to treating PACG patients with coexisting cataract, although additional surgery intervention may be needed for a few cases with cataract and complications after trabeculectomy.

Rice blast is a devastating disease worldwide,threatening rice production and food *** blast fun-gus Magnaporthe oryzae invades the host via the appressorium,a specialized pressure-generating struc-ture that generates enormous turgor pressure to penetrate the host ***,owing to ongoing evolution of fungicide resistance,it is vitally important to identify new targets and ***,we show that Trs85,a subunit of the transport protein particle III complex,is essential for appressorium-mediated infection in *** explain how Trs85 regulates autophagy through Ypt1(a small guano-sine triphosphatase protein)in *** then identify a key conserved amphipathic a helix within Trs85 that is associated with pathogenicity of *** computer-aided screening,we identify a lead compound,SP-141,that affects autophagy and the Trs85–Ypt1 ***-141 demonstrates a substantial capacity to effectively inhibit infection caused by the rice blast fungus while also exhibiting wide-ranging potential as an antifungal agent with broad-spectrum *** together,our data show that Trs85 is a potential new target and that SP-141 has potential for the control of rice *** thus provide a novel strategy that may help in thefight against rice blast.

While the structures of plant primary metabolic pathways are generally well defined and highly conserved across species,those defining specialized metabolism are less well characterized and more highly variable across *** this study,we investigated polyphenolic metabolism in the lycopersicum complex by characterizing the underlying biosynthetic and decorative reactions that constitute the metabolic network of polyphenols across eight different species of *** this purpose,GC-MS-and LC-MS-based metabolomics of different tissues of Solatium lycopersicum and wild tomato species were carried out,in concert with the evaluation of cross-hybridized microarray data for MapMan-based transcriptomic analysis,and publicly available RNA-sequencing data for annotation of biosynthetic *** combined data were used to compile species-specific metabolic networks of polyphenolic metabolism,allowing the establishment of an entire pan-species biosynthetic framework as well as annotation of the functions of decoration enzymes involved in the formation of metabolic diversity of the flavonoid *** combined results are discussed in the context of the current understanding of tomato flavonol biosynthesis as well as a global view of metabolic shifts during fruit *** results provide an example as to how large-scale biology approaches can be used for the definition and refinement of large specialized metabolism pathways.

Objective:To study the effects of three-dimensional(3D)collagen scaffold on bone marrow cells to differentiate into ***:We cultured BMCs on the 3D collagen scaffolds and two-dimensional(2D)culture *** morphological characteristics of the differentiated DCs cultured on 2D and 3D collagen scaffold were imaged under optical microscope and Scanning Electron Microscopy(SEM).Next,we analyzed the expressions of CD11b,MHC-II and the co-stimulatory molecules CD40,CD80,CD86 and CD83 in i DCs and m DCs using flow cytometry on CD11c+*** then the phagocytosis ability of generated DCs,i DC-2D and i DC-3D were co-incubated with FITC–dextran and then the intensity of FITC taken up by the DCs was measured using flow cytometry on CD11c+*** addition,the T-cell proliferation was assessed using flow ***,the cytokines profiles of generated DCs were collected to measure IL-10,IL-12p70,and TNF-αby ELISA ***,we evaluated the suppressive effects of the generated DCs in vivo delay type hypersensitivity(DTH)in C57BL/6 ***:Our results showed that differentiated DCs derived from 3D collagen scaffold presented with high expression of CD11b and low expressions of CD11c,co-stimulator and MHC-II molecules compared with 2D ***,these cells possessed a weak antigen uptake ability and suppression of T-cell proliferation in ***,they displayed a potent immunoregulatory function to alleviate allo-delay type hypersensitivity(allo-DTH)in ***:The 3D culture provides a suitable niche in specialized DCs *** researches demonstrated that 3D collagen scaffold culture promotes the differentiation of BMCs into regulatory DCs,CD11bhi MHClo *** differentiated CD11bhi MHClo DCs possessed the capacity to negatively regulate immune response in vitro and in vivo,whereas a high level of IL-10 production.

In this paper, we present the first evidence of differences in the mitochondria-related gene expression profiles of adult articular cartilage derived from patients with Kashin-Beck disease and normal controls. The expression of 705 mitochondria-related genes was analyzed by mitochondria-related gene expression analysis and ingenuity pathways analysis. Mitochondria-related gene expression analysis identified 9 up-regulated genes in Kashin-Beck disease based on their average expression ratio. Three canonical pathways involved in oxidative phosphorylation, apoptosis signaling and pyruvate metabolism were identified, which indicate the involvement of mitochondrial dysfunction in the pathogenesis of Kashin-Beck disease.

Development of Nursing Protocol for Preventing Interruptions during Clinical Examinations and Treatments in the Early Days of Hospitalization for Acute Exacerbation of Chronic Heart Failure in Patients with Impaired Cognitive Function

The purpose of this study is to prepare a nursing protocol for preventing interruptions during clinical examinations and treatments performed in the early days of hospitalization for acute exacerbation of chronic heart failure in patients with impaired cognitive function. For the first stage of the research, we prepared a draft of the nursing protocol based on a basic survey. For the second stage, semi-structured interviews were conducted with 5 nurses specialized in chronic heart failure and 11 nurses in dementia care to ensure content validity of the draft protocol. For the third stage, we examined the possibility of clinical application of the revised version of the protocol draft prepared in the second stage of the study. For assessment items, significant points of nursing care, and specific nursing care practice in this revised version, 73 subjects (84.9%) considered effective for patients, in terms of prevention of interruptions during clinical examinations and treatments in the early days of hospitalization. All items and contents were considered useful by more than 60% of the nurses. Considering that the nurses working in the clinical setting reported 84.9% of usefulness, we concluded that this nursing protocol remained valid at a certain level. We consider that this nursing protocol will be useful especially for newly graduated/employed nurses as a procedure manual which can reduce their anxiety or stress caused by lack of knowledge or experiences.

Flexible bronchoscopy(FB)has become a standard of care for the triad of inspection,sampling,and treatment in critical care *** is an invaluable tool for diagnostic and therapeutic purposes in critically ill patients in intensive care unit(ICU).Less is known about its role outside the ICU,particularly in the intermediate care unit(IMCU),a specialized environment,where an intermediate grade of intensive care and monitoring between standard care unit and ICU is *** the IMCU,the leading indications for a diagnostic work-up are:To visualize airway system/obstructions,perform investigations to detect respiratory infections,and identify potential sources of *** main procedures for therapeutic purposes are secretion aspiration,mucus plug removal to solve atelectasis(total or lobar),and blood aspiration during *** decision to perform FB might depend on the balance between potential benefits and risks due to frailty of critically ill *** adverse events related to FB are relatively uncommon,but they may be due to lack of expertise or appropriate ***,nowadays,during dramatic recent coronavirus disease 2019(COVID-19)pandemic,the exact role of FB in COVID-19 patients admitted to IMCU has yet to be clearly ***,we provide a concise review on the role of FB in an IMCU setting,focusing on its indications,technical aspects and complications.
