Nephronophthisis-related ciliopathies(NPHP-RC)represent one of the most common causes of chronic kidney disease in the first three decades of life and are characterized by a broad genetic and clinical heterogeneity.1 To date,more than 90 genes have been identified that cause autosomalrecessive NPHP-RC if mutated,accounting for up to 60%of cases.1 Among these,homozygous deletions of NPHP1 are the most common *** genes localize to primary cilia,basal bodies,or the centrosome and lead to a primary ciliary disruption if mutated,thereby causing a broad phenotypical spectrum.1 Patients that suffer from NPHP-RC can either have an isolated renal phenotype,such as polyuria,polydipsia,decreased urinary concentration ability,and secondary enuresis due to loss of tubular function,or present with extrarenal symptoms including retinal degeneration,cerebellar vermis hypoplasia,or hepatic *** are often diagnosed in early adolescence,reaching end-stage renal disease before 25 years of age,but early onset and rapidly progressive forms of NPHP-RC also exist.1 Renal ultrasound indicates kidneys of normal or reduced renal length with increased echogenicity and corticomedullary cysts.

Objective In this study,we aimed to compare long-term physical and mental health outcome between SARS-CoV-2 infected and uninfected household members to differentiate between infection-related and pandemic-related outcomes after about two and a half years of the ***,possible differences in the outcome of adults and children and young people(CYP)were of *** In a cross-sectional study design,we compared the long-term physical and mental health outcome of between infected and uninfected as well as between adult and CYP(household members).Setting The FamilyCoviDD19 study—a serology study in households—was initially conducted to evaluate virus transmission in a close contact setting focusing on households with children and adolescents in *** least 1year after initial infection in the respective households,a follow-up took place in which the prevalence and type of possible long-term consequences were surveyed on the basis of self-reported information on physical and mental *** In this study,a total of 533 household members of 146 families participated and responded to our survey,including 296(55.5%)adults and 237(44.5%)*** The difference in frequency of reported symptoms between infected and uninfected individuals was very moderate,suggesting that the vast majority of reported symptoms were not attributable to a previous SARS-CoV-2 ***,regardless of age and infection status,this study showed overall high rates of self-reported symptoms with CYP having fewer long-term sequelae than adults oneyear after ***,over 50%of those reporting symptoms were not affected in their daily life,with CYPs reporting an even lower percentage compared with *** CYP are at reduced risk not only to develop symptomatic infection or severe disease courses(previous analyses)but also to develop infection-associated long-term sequelae(this study).Independent of infection CYP reported high rates of

目的通过标准病例测评数字化症状评估程序在疾病覆盖、诊断准确性以及安全分诊方面的应用情况。方法2020年3月8日至8月11日筛选符合纳入标准的8款中文版症状评估应用程序。将200个标准病例的症状分别输入各症状评估程序,并做应答,直到应用程序给出诊断建议、紧急性建议,或无法继续。分析各应用程序所能给出诊断的疾病比例、诊断的准确率以及安全分诊的比例。所有病例的创建及诊断标准、分诊标准,症状输入及应答,与标准诊断的比较、裁决,均由分别独立的专业组完成。结果不同症状的评估应用程序表现不同。针对研究中的标准病例,可给出建议的应用程序中,覆盖疾病比例为29.0%(58/200)~99.5%(199/200),前3位疾病的诊断准确率为8.5%(17/200)~61.5%(123/200),安全分诊比例为84.8%(167/197)~99.5%(198/199)。结论数字化症状评估程序应用的疾病覆盖、诊断准确性及安全性分诊表现差异较大。良好表现的症状评估应用程序,可作为辅助医生的工具,但即使表现良好的症状评估程序,仍有开展真实诊疗环境中有效性研究的必要。

Formation of claudin-10 based tight junctions(TJs)is paramount to paracellular Na+transport in multiple *** variants in CLDN10 have been linked to HELIX syndrome,a salt-losing tubulopathy with altered handling of divalent cations accompanied by dysfunctional salivary,sweat,and lacrimal ***,we investigate molecular basis and phenotypic consequences of a newly identified homozygous CLDN10 variant that translates into a single amino acid substitution within the fourth transmembrane helix of *** addition to hypohidrosis(H),electrolyte(E)imbalance with impaired urine concentrating ability,and hypolacrimia(L),phenotypic findings include altered salivary electrolyte composition and amelogenesis imperfecta but neither ichthyosis(I)nor xerostomia(X).Employing cellular TJ reconstitution assays,we demonstrate perturbation of cis-and trans-interactions between mutant claudin-10 *** of reconstituted TJ strands show disturbed continuity and reduced abundance in the mutant ***,both major isoforms,claudin-10a and claudin-10b,are differentially affected with claudin-10b showing more severe molecular ***,expression of the mutant in renal epithelial cells with endogenous TJs results in wild-type-like ion selectivity and conductivity,indicating that aberrant claudin-10 is generally capable of forming functional paracellular ***,mutant proteins prove pathogenic by compromising claudin-10 TJ strand *** ex vivo investigations indicate their insertion into TJs to occur in a tissue-specific manner.

Background:Classical pterional appoach for temporal surgeries may cause atrophy and dysfunction of temporalis,injury to the facial nerve,and unnecessary cortical *** an alternative to the classical pterional approach for such surgeries,we hereby describe an mini-temporal approach which reduces these risks and proven to be practical in neurological *** and methods:In the mini-temporal incision design,the frontal end of the incision never surpassed the hairline at the level of temporal line,and a one-layer skin-galea-muscle flap was detached from the cranium,effectively avoiding the injuries of facial *** surgical bone window was completely located underneath the temporalis muscle,allowing it to be completely repositioned ***:We demonstrated the application of mini-temporal approach in a variety of temporal region tumors,which can be applied to complete successful resective surgeries while effectively reducing injuries to extra-temporal cortex,temporalis,and facial *** were no postoperative complications related to extra-temporal cortical damage,atrophy of temporalis,or injury to the facial ***:The mini-temporal approach can effectively shorten the time of craniotomy and closure,decrease the size of bony removal,increase the restoration of temporalis during closure,and lower the chance of facial nerve ***,it improves cosmetic outcomes and reduces the risk of unintentional extra-temporal cortical injury,which fully embodies the minimally invasive principle in neurosurgery.
