BACKGROUND Advanced chronic kidney disease(CKD) is a common complication for people with type 1 and 2 diabetes and can often lead to glucose instability. Continuous glucose monitoring(CGM) helps users monitor and stabilize their glucose levels. To date, CGM and intermittent scanning CGM are only approved for people with diabetes but not for those with advanced *** To compare the performance of Dexcom G5 and FreeStyle Libre sensors in adults with type 1 or 2 diabetes and advanced *** This was a non-randomized clinical trial that took place in two outpatient clinics in western Sweden. All patients with type 1 or 2 diabetes and an estimated glomerular filtration rate(eGFR) of < 30 mL/min per 1.73 m^(2) were invited to participate. Forty patients(full analysis set = 33) carried the Dexcom G5 sensor for 7 d and FreeStyle Libre sensor for 14 d simultaneously. For referencing capillary blood glucose(SMBG) was measured with a high accuracy glucose meter(HemoCue®) during the study period. At the end of the study, all patients were asked to answer a questionnaire on their experience using the *** The mean age was 64.1(range 41-77) years, hemoglobin A1 c was 7.0% [standard deviation(SD) 3.2], and diabetes duration was 28.5(SD 14.7) years. A total of 27.5% of the study population was on hemodialysis and 22.5% on peritoneal dialysis. The mean absolute relative difference for Dexcom G5 vs SMBG was significantly lower than that for FreeStyle Libre vs SMBG [15.2%(SD 12.2) vs 20.9%(SD 8.6)], with a mean difference of 5.72 [95% confidence interval(CI): 2.11-9.32;P = 0.0036]. The mean absolute difference was also significantly lower for Dexcom G5 than for FreeStyle Libre, 1.21 mmol/L(SD 0.78) and 1.76 mmol/L(SD 0.78), with a mean diffrenec of 0.55(95%CI: 0.27-0.83;P = 0.0004).The mean difference(MD) was-0.107 mmol/L and-1.10 mmol/L(P = 0.0002), respectively. The parabrachial nucleus(PBN)integrates interoceptive and exteroceptive information to control various behavioral and physiological processes including breathing,emotion,and sleep/wake regulation through the neural circuits that connect to the forebrain and the ***,the precise identity and function of distinct PBN subpopulations are still largely ***,we leveraged molecular characterization,retrograde tracing,optogenetics,chemogenetics,and electrocortical recording approaches to identify a small subpopulation of neurotensin-expressing neurons in the PBN that largely project to the emotional control regions in the forebrain,rather than the *** activation induces freezing and anxiety-like behaviors,which in turn result in *** addition,optogenetic and chemogenetic manipulations of these neurons revealed their function in promoting wakefulness and maintaining sleep *** propose that these neurons comprise a PBN subpopulation with specific gene expression,connectivity,and function,which play essential roles in behavioral and physiological regulation.

Non-alcoholic fatty liver disease(NAFLD)has emerged as the most common liver disorder worldwide mainly attributed to the epidemic spread of obesity and type 2 diabetes *** it is considered a benign disease,NAFLD can progress to non-alcoholic steatohepatitis,liver cirrhosis and hepatocellular carcinoma(HCC).Most data regarding the epidemiology of NAFLD-related HCC are derived from cohort and population studies and show that its incidence is increasing as well as it is likely to emerge as the leading indication for liver transplantation,especially in the Western *** cirrhosis constitutes the main risk factor for HCC development,in patients with NAFLD,HCC can arise in the absence of cirrhosis,indicating specific carcinogenic molecular *** NAFLD as an underlying liver disease for HCC is often underdiagnosed due to lack of sufficient surveillance in this population,NAFLDHCC patients are at advanced HCC stage at the time of diagnosis making the management of those patients clinically challenging and affecting their prognostic *** this current review,we summarize the latest literature on the epidemiology,other than liver cirrhosis-pathogenesis,risk factors and prognosis of NAFLD-HCC ***,we emphasize the prevention of the development of NAFLD-associated HCC and we provide some insight into the open questions and issues regarding the appropriate surveillance policies for those patients.

As the most common internal modification of mRNA,Ne-methyladenosine(m^(6)A)and its regulators modulate gene expression and play critical roles in various biological and patholog-ical processes including *** was reported previously that m^(6)A methyltransferase(writer),methyltransferase-like 3(METTL3)adds m^(6)A in primary microRNAs(pri-miRNAs)and fa-cilitates its processing into precursor miRNAs(pre-miRNAs).However,it is unknown whether m^(6)A modification also plays a role in the maturation process of pre-miRNAs and(if so)whether such a function contributes to ***,we found that YTHDF2 is aberrantly overexpressed in acute myeloid leukemia(AML)patients,especially in relapsed patients,and plays an onco-genic role in ***,YTHDF2 promotes expression of miR-126-3p(also known as miR-126,as it is the main product of precursor miR-126(pre-miR-126)),a miRNA that was reported as an oncomiRNA in AML,through facilitating the processing of pre-miR-126 into mature ***,YTHDF2 recognizes m^(6)A modification in pre-miR-126 and recruits AGO2,a regulator of pre-miRNA processing,to promote the maturation of ***2 posi-tively and negatively correlates with miR-126 and miR-126's downstream target genes,respec-tively,in AML patients,and forced expression of miR-126 could largely rescue YTHDF2/Ythdf2 depletion-mediated suppression on AML cell growth/proliferation and leukemogenesis,indi-cating that miR-126 is a functionally important target of YTHDF2 in ***,our studies not only reveal a previously unappreciated YTHDF2/miR-126 axis in AML and highlight the ther-apeutic potential of targeting this axis for AML treatment,but also suggest that m^(6)A plays a role in pre-miRNA processing that contributes to tumorigenesis.

Although sympathetic blockade is clinically used to treat pain,the underlying mechanisms remain *** developed a localized microsympathectomy(mSYMPX),by cutting the grey rami entering the spinal nerves near the rodent lumbar dorsal root ganglia(DRG).In a chemotherapy-induced peripheral neuropathy model,mSYMPX attenuated pain behaviors via DRG macrophages and the anti-inflammatory actions of transforming growth factor-β(TGF-β)and its receptor TGF-β***,we examined the role of TGF-βin sympathetic-mediated radiculopathy produced by local inflammation of the DRG(LID).Mice showed mechanical hypersensitivity and transcriptional and protein upregulation of TGF-β1 and TGF-βR1 three days after *** prevented mechanical hypersensitivity and further upregulated Tgfb1 and *** delivery of TGF-β1 rapidly relieved the LID-induced mechanical hypersensitivity,and TGF-βR1 antagonists rapidly unmasked the mechanical hypersensitivity after LID+*** situ hybridization showed that Tgfb1 was largely expressed in DRG macrophages,and Tgfbr1 in *** suggest that TGF-βsignaling is a general underlying mechanism of local sympathetic blockade.

Aim:To investigate the effect of gabapentin on neural [neuron-specific enolase（NSE）] and glial markers [glial fibrillary acidic protein （GFAP） and S 100B] in differ-ent brain regions of diabetic ***:Diabetes was induced by a singleintraperitoneal injection of streptozotocine （50 mg/kg body weight）.Rats in onediabetic group received gabapentin (50 ***-1.d-1) and rats in the other diabeticgroup received vehicle only for 6 *** levels of GFAP,S100B,and NSE weredetermined by immunoblotting in the hippocampus,cortex,and *** （LPO as malondialdehyde+ 4-hydroxyalkenals） and glutathione （GSH）levels were also determined in the same brain ***:Total and degradedGFAP content and S100B protein expression in different areas of brain tissuessignificantly increased in diabetic rats compared to control ***,NSElevels were also significantly elevated in hyperglycemic *** addition,therewas a significant increase in LPO levels in the diabetic rat brain compared tocontrol rat *** with gabapentin prevented the upregulation ofGFAP,S 100B,and NSE in all brain regions of diabetic *** level of LPO wasreduced,but not completely halted,by treatment with ***:These results suggest that diabetes causes glial and neuronal injury,possibly asa result of elevated oxidative stress,and that gabapentin protects neurons andglial ***,we predict that gabapentin treatment will attenuate the hippo-campal and cortical neurodegeneration observed during diabetes mellitus in rats.
