Relationship between Glucocorticoid-induced Osteoporosis and Vitamine D Receptor Genotypes

By means of polym erase chain reaction- restriction fragment length polymorphism (PCR- RFL P) assay,the association between vitamine D receptor (VDR) genotypes and bone min- eral density (BMD) in the patients receiving long- term glucocorticoid therapy was *** clinical data and blood of71patients with rheumatosis who received long- term glucocorticoid ther- apy were *** was m easured by dual- energy X- ray *** gene frag- ment(about185 bp) was amplified by PCR from the extracted genomic DNA,then digested with restriction endonuclease Bsm *** genotypes were evaluated based on the fragment length fol- lowing endonuclease digestion and the association between genotypes and BMD or Z- score values was *** the 71cases,the detected genotypes were Bb and bb with the distribution frequency being 11.3% and 88.7% *** distribution frequency of the alleles was in agreement with the Hardy- Weinberg *** was no significant difference between the two genotypes in age,gender,body m ass index(BMI) ,disease duration,disease types,time of glucocorticoid administration and cumulative dosage(P>0 .0 5 ) .Osteoporosis rate of the patients with Bb or bb genotype was37.5 % and33.3% respectively,with the difference being notsignif- icant (χ2 =0 .0 5 ,P=0 .8) .The BMD and Z- score values at lumbar spine and femur in two geno- types were not similar,but the difference had no significant (P>0 .0 5 ) .The distribution frequen- cy of bb type of VDR genotypes in Han populations of China was m ore prevalent,followed by Bb and bb types in *** the patients receiving long- term glucocorticoid therapy,there was no sig- nificant difference in BMD between Bb and bb *** data suggest that the VDR geno- types may not be m eans of identifying patients at greater risk of glucocorticoid- induced osteoporo- sis,which await to be further confirmed by a large sample size.

To study the effect of paroxetine on depressive symptom accompanying somatic disease and the value of platelet 5-HT concentration in the diagnosis of depression, 30 patients with depressive symptom were treated with paroxetine. All patients were evaluated on Zung and HAMD scale and assayed of platelet 5-HT concentration before and after treatment. It was found that patients had a lower level of platelet 5-HT concentration than healthy people (P0.05). It was suggested that paroxetine was a good antidepressant and platelet 5-HT concentration was useful in the screening of depression.

Based on elongation and shear microstrains between the doublet particles, we present the dynamic scaling equations of plane wave propagation in a cubic–tetrahedral assembly (lattice type H4) in accordance with the formalism of doublet mechanics (DM). The relations between the micro-moduli of DM constitutions and the macro-parameters of the Cosserat continuum model are obtained by a mapping process and a non-scale approximation of the DM model. With multi-scale characteristics, the frequency dispersion curves from DM reflect the effect of granule inhomogeneity and the anisotropic nature of the micro-structure on the wave propagation in the H4 medium.

Avascular necrosis (AVN), also termed as osteonecrosis or aseptic necrosis, is a process caused by direct and indirect damage to the vascular supply of the involved bone.1-5 Clinical course of the disease is usually progressive and causes significant pain and limitation of movement. Trauma causes AVN by the obvious direct interruption of the vascular supply, but there are a variety of underlying systemic disorders associated with the development of AVN via indirect vascular compromise. The known risk factors include corticosteroid use, alcohol abuse, smoking, sickle cell anemia, coagulopathies, systemic lupus erythematosus, hypercholesterolemia, Gaucher ' s disease, chronic pancreatitis, and hyper-triglyceridemia.3 It is very difficult to define reasons of AVN in individual patient, because most of the patients had multiple risk factors for AVN. On the other hand, approximately 10%-20% of AVN cases have no known risk factors and are classified as idiopathic AVN.

Objective: To gain further understanding of the antiphospholipid syndrome(APS) . Methods : Analysing clinical and laboratory data on ten cases of APS. Results: Thrombocytopenia appeared in all *** thrombi of limbs appeared in five eases and neurological abnormalities in two cases. Renal impairments were found in three cases. One case manifested left renal venous thrombi and the other two cases thrombotic microangiopathy. Budd-Chiari syndrome was found in one case. One of the ten cases was catastrophic APS(CAPS) presented as acute diffuse swelling, cyanosis, pain, ischemia and necrosis in fingers and limbs,recurrem shock, ascites, hepatic and respiratory dysfunction. Anticoagulams and corticosteroids could be effective for dealing with APS. It was critical to treat catastrophic APS with anticoagulants or plasmapheresis as early as possible. Conclusions: APS shows variable manifestations for good prognosis, but catastrophic APS has fatal risk. The main treatment for APS is the use of anticoagulams and immunosuppressives.

AIM:Primary biliary cirrhosis (PBC) is a chronic, cholestatic disease of autoimmune etiology,the histology of which shows a destruction of the intrahepatic bile duct and portal inflammation. Ursodeoxycholic acid (UDCA) is now used as a first-line drug for asymptomatic PBC (aPBC) because it is reported that UDCA decreases mortality and prolongs the time of liver ***, only 20-30% of patients respond fully to ***,lipoprotein-lowering agents have been found to be effective for *** aim of this study was to examine the safety and efficacy of fenofibrate, a member of the fibrate class of hypolipidemic and anti-inflammatory agent via peroxysome proliferatory-activated receptor α,in patients with ***:Fenofibrate was administered for twelve weeks in nine patients with aPBC who failed to respond to *** was used along with fenofibrate during the *** data from aPBC patients were analyzed to assess the biochemical effect of fenofibrate during the ***: The serum levels of alkaline phosphatase (ALP)(285±114.8IU/L) and immunoglobulin M (IgM) (255.8±85.9mg/dl) significantly decreased to 186.9±76.2IU/L and 192.9±67.5mg/dL respectively, after fenofibrate treatment in patients with aPBC (P<0.05). Moreover,the titer of antimitochondrial antibody (AMA) also decreased in 4 of 9 patients with aPBC. No adverse reactions were observed in any ***:Fenofibrate appears to be significantly effective in treating patients with aPBC who respond incompletely to UDCA *** the mechanism of fenofibrate on aPBC has not yet been fully clarified,combination therapy using fenofibrate and UDCA might be related to the anti-immunological effects, such as the suppression of AMA production as well as its antiinflammatory effect.
