Caval replacement with parietal peritoneum tube graft for septic thrombophlebitis after hepatectomy: A case report

BACKGROUND Caval vein thrombosis after hepatectomy is rare, although it increases mortality and morbidity. The evolution of this thrombosis into a septic thrombophlebitis responsible for persistent septicaemia after a hepatectomy has not been reported to date in the literature. We here report the management of a 54-year-old woman operated for a peripheral cholangiocarcinoma who developed a suppurated thrombophlebitis of the vena cava following a hepatectomy.CASE SUMMARY This patient was operated by left lobectomy extended to segment V with bile duct resection and Roux-en-Y hepaticojejunostomy. After the surgery, she developed Streptococcus anginosus, Escherichia coli, and Enterococcus faecium bacteraemias, as well as Candida albicans fungemia. A computed tomography scan revealed a bilioma which was percutaneously drained. Despite adequate antibiotic therapy,the patient's condition remained septic. A diagnosis of septic thrombophlebitis of the vena cava was made on post-operative day 25. The patient was then operated again for a surgical thrombectomy and complete caval reconstruction with a parietal peritoneum tube graft. Use of the peritoneum as a vascular graft is an inexpensive technique, it is readily and rapidly available, and it allows caval replacement in a septic area. Septic thrombophlebitis of the vena cava after hepatectomy has not been described previously and it warrants being added to the spectrum of potential complications of this procedure.CONCLUSION Septic thrombophlebitis of the vena cava was successfully treated with antibiotic and anticoagulation treatments, prompt surgical thrombectomy and caval reconstruction.

Immunohistochemical Analysis of the Acid Secretion Potency in Gastric parietal Cells

Gastric parietal cells are important in acid secretion, but it is unclear which cells throughout the gastric gland have the highest secretion potency. Here, we used immunohistochemical methods with anti-H+, K+-ATPase, phosphoryl ezrin and CD44 antibodies to study the distribution of gastric acid secretion activity. Stomach tissues from freely fed and starved rats were cryofixed for light microscopy or fixed by high-pressure freezing for electron microscopy. parietal cells from freely fed animals corresponded to the active secretion phase and to the inactive resting phase from starved rats. Anti-H+, K+-ATPase and anti-phosphoryl ezrin labeling were observed on the membrane of the intracellular canaliculi and the tubulovesicle from freely fed rats, while cells from starved animals showed weak labeling with anti-phosphoryl ezrin antibody staining. Morphometrical analysis at the electron microscopic level was performed on active and inactive acid secretory phases between the upper and base regions of the gland. H+, K+-ATPase and CD44 were distributed on both sites of the microvillous and tubulovesicle membrane in the same cells, but phosphoryl ezrin localized predominantly on the microvillous membrane in active cells of the glandular neck and upper base. Therefore, the highest secreting potency appeared to be in cells of the glandular neck and upper base.

Brain activation induced by different strengths of hand grasp: a functional magnetic resonance imaging study

Mirror neuron system can be activated by observation and execution of an action.It has an important function of action understanding.We investigated brain activations in humans by observing the strength of a hand grasp using functional magnetic resonance imaging.Twenty right-handed healthy individuals,consisting of 10 males and 10 females,aged 22.40 ± 2.04 years,were recruited into this study from September to November 2017 via posters.Light hand grasp task video showed a hand lightly grasping and releasing a ball repeatedly.Powerful hand grasp task video showed a hand tightly grasping and releasing a ball repeatedly.Functional magnetic resonance imaging block design paradigm comprised five stimulation blocks alternating with five baseline blocks.Stimulation blocks were presented with two stimulus tasks,consisting of a light grasp and a powerful grasp.Region of interest was defined around the inferior parietal lobule,inferior frontal gyrus,and superior temporal sulcus which have been called mirror neuron system.The inferior parietal lobule,fusiform,postcentral,occipital,temporal,and frontal gyri were activated during light and powerful grasp tasks.The BOLD signal response of a powerful grasp was stronger than that of a light grasp.These results suggest that brain activation of the inferior parietal lobule,which is the core brain region of the mirror neuron system,was stronger in the powerful grasp task than in the light grasp task.We believe that our results might be helpful for instructing rehabilitation of brain injury.This study was approved by the Institutional Review Board of Daegu Oriental Hospital of Daegu Haany University on September 8,2017 (approval No.DHUMC-D-17020-PRO-01).

Renal progenitors: Roles in kidney disease and regeneration

Kidney disease is a devastating condition that affects millions of people worldwide, and its prevalence predicted to significantly increase. The kidney is complex organ encompassing many diverse cell type organized in a elaborate tissue architecture, makin regeneration a challenging feat. In recent years, there ha been a surge in the field of stem cell research to develo regenerative therapies for various organ systems. Here we review some recent progressions in characterizing th role of renal progenitors in development, regeneration and kidney disease in mammals. We also discuss how the zebrafish provides a unique experimental anima model that can provide a greater molecular and genet understanding of renal progenitors, which may contribut to the development of potential regenerative therapies fo human renal afflictions.

Spatial orientation during eyes closed versus open in the dark: Are they the same?

We hypothesized that keeping one’s balance with eyes open in the dark is different and more difficult than eyes closed because the brain continues to process visual inputs in the dark when the eyes are open. On the other hand, when the eyes are closed, the visual system does not signal incongruent information with which the brain must compare the other sensory systems. A variety of cognitive (subtracting backwards by seven as quickly and accurately as possible) and support surface (fixed versus sway-referenced) conditions were used to probe the neural mechanisms underlying the sensory organization processes in healthy young adults. Peak-to-peak anteroposterior sway performance revealed two dissociated components of the treatment effects. The first component came from the visuospatial factor. Balance control during eye closure and eyes open in the dark were found to be similar but poorer than baseline condition (eyes open under typical lighting). The second component was the effect of task difficulty in which balance control in the sway-referenced condition was worse compared to fixed support during eye closure or eyes open in the dark. Analyses of the cognitive performance also revealed different underlying neural mechanisms of the experimental conditions. Subtraction speed under the fixed support surface condition was similar among all the conditions but was faster with eyes closed during the sway-referenced support surface condition. Accuracy was not affected among the visual and surface conditions. We conclude that sensory processing load with eyes closed is lower than eyes open in the dark, thereby allowing cognitive performance to proceed more efficiently. Performing a difficult subtraction task with eyes closed may afford a decrease in dual-task interference since similar brain areas, particularly the parietal region, are involved in both tasks. The results are discussed with reference to clinical application and spatial disorientation in aviation.

Ethanol Effects on Cortical EEG Correlation and Sexual Behavior in Male Rats

Ethanol affects sexual behavior drastically, as much as its execution as well as its motivation. Ethanol is a central nervous system depressant and the prefrontal cortex (PFC) is one of the cortical areas most sensitive to its effects. This prefrontal area participates in the modulation of the cognitive control, and furthermore, by mean of connections with the parietal cortex (PC) plays an important role in the processing of sexually relevant stimuli. Considering that it has been reported that the ethanol affects the proper functioning of these cortical areas, particularly decreasing the degree of electroencephalographic (EEG) coupling or correlation between them, the aim of this study was to characterize the effect of a low dose of ethanol on EEG correlation, as well determining if this effect is associated with impaired sexual behavior in male rats. Nine male rats unilaterally implanted were simultaneously recorded the EEG from the PFC and PC to calculate the degree of EEG synchronization (correlation, r) between both cortices during the first 40 minutes post administration of 0.75 g/kg of ethanol. At the end of the EEG recording, the males were submitted to copulatory behavior with a receptive female rat. The ethanol administration was associated with a decreased intrahemispheric correlation of the fast bands between PFC and PC, as well as with an increased mount and intromission latency. This data showed that ethanol in moderate doses decreased the coupling degree between cortices. It is likely that this functional disconnection between the PFC and PC after the ethanol administration could be associated with the difficulty in discriminating incentive value of the sexual stimuli so that decreased the sexual motivation but not the performance of the copulatory parameters in the male rat.

Evaluating the Signal Processing Capacities of Post-Mortem Cerebral Cortical Tissue by Artificial Phototransduction of Dynamic Visual Stimuli

The signal processing function of human cerebral cortical tissues is determined by the regional cytoarchitectures distributed throughout the brain. Based upon this assumption, we pursued the hypothesis that residual microstructure within the primary and associative visual cortices of a fixed, post-mortem whole human brain would process electrical signals differentially. To this end, we designed and engineered a very simple brain-photocell interface. Photostimuli, presented as either periodic flashes or as dynamic visual images, were transduced by photocells attached to the optic nerve of a post-mortem human brain specimen. The novel approach revealed that microvolt fluctuations within the primary and associative visual cortices could be discriminated. Simple light-dark discrimination was noted for the primary visual area (BA17) whereas within the right occipito-parietal cortices of the dorsal visual stream (BA19, BA7), spectral power of microvolt fluctuations could discriminate moving visual stimuli from those which were non-moving. Discriminant analysis classified movement represented within the right parietal lobe with 80% success. Together, the results suggest that artificially generated electrical signals are processed differentially by alternative cortical regions in the post-mortem brain.

Histamine 3 receptor activation mediates inhibition of acid secretion during Helicobacter-induced gastritis

AIM:To test the hypothesis that histamine 3 receptor (H3R)activation during Helicobacter infection inhibits gastric acid secretion in vivo and in vitro.METHODS:Helicobacter felis(H.felis)infected and uninfected C57Bl/6 mice were infused with either PBS or the H3 receptor antagonist thioperamide(THIO)for 12 wk.After treatment,mice were analyzed for morphological changes and gastric acid content.Total RNA was prepared from the stomachs of each group and analyzed for changes in somatostatin and gastrin mRNA abundance by real time-polymerase chain reaction(RTPCR).Location of H3 receptors in the stomach was analyzed by co-localization using antibodies specific for the H3 receptor and parietal cell marker H + ,K + -ATPase βsubunit. RESULTS:Inflammation and parietal cell atrophy was observed after 12 wk of H.felis infection.Interestingly, treatment with the H3R antagonist thioperamide(THIO) prior to and during infection prevented H.felis-induced inflammation and atrophy.Compared to the uninfected controls,infected mice also had significantly decreased gastric acid.After eradication of H.felis with THIO treatment,gastric acidity was restored.Compared to the control mice,somatostatin mRNA abundance was decreased while gastrin gene expression was elevated during infection.Despite elevated gastric acid levels, after eradication of H.felis with THIO,somatostatin mRNA was elevated whereas gastrin mRNA was suppressed.Immunofluorescence revealed the presence of H3 receptors on the parietal cells,somatostatin-secreting D-cells as well as the inflammatory cells. CONCLUSION:This study shows that during H.felis infection,gastric acidity is suppressed as a consequence of an inhibitory effect on the parietal cell by H3R activation.The stimulation of gastric mucosal H3Rs increases gastrin expression and release by inhibiting release of somatostatin.
