pseudoexfoliation(pEX) syndrome is a well-recognized late-onset disease caused by a generalized fibrillopathy. It is linked to a broad spectrum of ocular complications including glaucoma and perioperative problems during cataract surgery. Apart from the long-known intraocular manifestations, pEX deposits have been found in a variety of extraocular locations and they appear to represent a systemic process associated with increased cardiovascular and cerebrovascular morbidity. However, as published results are inconsistent, the clinical significance of the extraocular pEX deposits remains controversial. Identification of pEX deposits in the heart and the vessel wall, epidemiologic studies, as well as, similarities in pathogenetic mechanisms have led to the hypothesis of a possible relation between fibrillar material and cardiovascular disease. Recent studies suggest that pEX syndrome is frequently linked to impaired heart and blood vessels function. Systemic and ocular blood flow changes, altered parasympathetic vascular control and baroreflex sensitivity, increased vascular resistance and decreased blood flow velocity, arterial endothelial dysfunction, high levels of plasma homocysteine and arterial hypertension have all been demonstrated in pEX subjects. Common features in the pathogenesis of both atherosclerosis and pEX, like oxidative stress and inflammation and a possible higher frequency of abdominal aorta aneurysm in pEX patients, could imply that these grey-white deposits and cardiovascular disorders are related or reflect different manifestations of the same process.

Overview of the Association Between Non-Alcoholic Fatty Liver Disease and Hypertension

Non-alcoholic fatty liver disease (NAFLD) is the most common chronic liver disease, and its prevalence is rising. NAFLD is closely associated with metabolic syndrome, with both conditions sharing common clinical characteristics such as obesity, insulin resistance, type 2 diabetes mellitus, hypertension, and hypertriglyceridemia. Several observational studies have evaluated the relationship between NAFLD and hypertension, with the overall evidence suggesting a bidirectional relationship. It is hypothesized that activation of the sympathetic nervous and renin-angiotensin systems, observed in NAFLD with or without insulin resistance promotes the development of hypertension. In patients with hypertension, activation of these systems can lead to hepatic fibrosis and progressive inflammation through increased oxidative stress and activation of hepatic stellate cells and Kupffer cells. The present review examines the pathophysiologic and clinical evidence supporting the bidirectional association between NAFLD and hypertension.

Treatment of Helicobacter pylori infection:Meeting the challenge of antimicrobial resistance

Treatment of Helicobacter pylori (H. pylori) infection is paramount for the management of prevalent gastrointestinal disorders including peptic ulcer disease and gastric cancer. Due to the wide increase in prevalence of H. pylori resistance to antibiotics, clarithromycin-based triple therapies are not any more suitable for unconditional empiric use, and should not be recommended, unless local resistance to this antibiotic is low (proposed to overcome the issue of increasing clarithromycin resistance, and some of them are already implemented in clinical practice. These comprise: (1) adoption of novel, more effective, empirical treatments: bismuth quadruple, sequential, non-bismuth quadruple (concomitant), dual-concomitant (hybrid), and levofloxacin-based regimens, the latter mainly designated as second-line/rescue options; (2) perspectives for a susceptibility-guided (tailored) therapeutic approach based on culture-free molecular testing methods; and (3) adjunct use of probiotics to improve eradication rates. The present article is aimed to provide a comprehensive overview of current and emerging strategies in the treatment of H. pylori infection, focusing on the challenge of antimicrobial resistance.

Fault Tolerance Limits and Input Stimulus Selection Using an Implemented FpGA-Based Testing System

In this paper, the selection of fault tolerance limits and input stimulus using an implemented adaptive FpGA-based testing system based on a method utilizing wavelet transformation of the current waveforms is presented. The testing scheme is innovative because it offers the ability of applying different input stimulus signals with respect to the requirements of the examined circuit. Moreover, the method used is simple, offers a single-point test measurement solution and may easily be adapted to test various other analog and mixed-signal systems. Experimental results are presented showing the advantages of the proposed testing scheme.

Scleromyxedema is a rare dermatologic disorder,characterized by erythematous or yellowish lichenoid waxy *** manifestations are rare but well-recognized.A 51-year-old woman,diagnosed with scleromyxedema,was admitted to the hospital with status epilepticus,caused by brain lesions,as disclosed in a brain magnetic resonance imaging(MRI).The patient was treated with anticonvulsants and corticosteroids and gradually recovered fully.A complete remission of the lesions was shown in a follow-up brain *** cases with scleromyxedema and the presence of neurological manifestations,we need to pay attention to central nervous system involvement,especially when combined with brain MRI lesions,and treat the patient appropriately.

Identifying mechanisms and pathways involved in gene–environment interplay and phenotypic plasticity is a long-standing *** is highly desirable to establish an integrated framework with an environmental dimension for complex trait dissection and prediction.A critical step is to identify an environmental index that is both biologically relevant and estimable for new *** extensive field-observed complex traits,environmental profiles,and genome-wide single nucleotide polymorphisms for three major crops(maize,wheat,and oat),we demonstrated that identifying such an environmental index(i.e.,a combination of environmental parameter and growth window)enables genome-wide association studies and genomic selection of complex traits to be conducted with an explicit environmental ***,genes identified for two reaction-norm parameters(i.e.,intercept and slope)derived from flowering time values along the environmental index were less colocalized for a diverse maize panel than for wheat and oat breeding panels,agreeing with the different diversity levels and genetic constitutions of the *** addition,we showcased the usefulness of this framework for systematically forecasting the performance of diverse germplasm panels in new *** general framework and the companion CERIS-JGRA analytical package should facilitate biologically informed dissection of complex traits,enhanced performance prediction in breeding for future climates,and coordinated efforts to enrich our understanding of mechanisms underlying phenotypic variation.

A Comparison between Three-Dimensional Conformal Radiotherapy, Intensity-Modulated Radiotherapy, and Volumetric-Modulated Arc Therapy Techniques for Stereotactic Body Radiotherapy of Lung Tumors

Introduction: The aim of this study is to dosimetrically compare 3D CRT, IMRT, and VMAT techniques that employ coplanar and non-coplanar beams for the SBRT of lung tumors. Methods and Materials: Nine (n = 9) consecutive SBRT lung patients with ten tumor sites who were previously treated at our institution were selected for this study. Six (n = 6) treatment plans were created for each pTV: 1 coplanar and 1 non-coplanar 3D CRT, IMRT, and VMAT such that 98% of pTV received 100% of prescription dose, Rx of 50 Gy in 5 fractions. The data collected from each plan included the conformity index, R50%, and homogeneity index of the target as well as the volume of normal tissue irradiated by 5 Gy, 25 Gy isodose lines, V5 and V20 of total lung, and maximum dose to organs at risk. Beam angles and arc lengths were chosen in order to achieve the lowest total lung V5, V20 and R50% values. Results: According to the observed data, the mean total lung V5 and V20 values were lowest for the non-coplanar VMAT plans, but were not statistically different from the other planning techniques. Conformity values were similar for the IMRT and VMAT plans, and significantly lower than the 3D CRT plans. R50% values were lowest for the VMAT plans and significantly lower than both IMRT and 3D CRT plans in both coplanar and non-coplanar beam arrangements. However, dose homogeneity in the pTV is significantly higher in the IMRT coplanar plans than the corresponding 3D CRT and SmartArc (SA) coplanar plans. Coplanar VMAT plans were able to produce a significant 35.5% reduction in the maximum cord dose than coplanar 3D CRT plans
