Extracellular vesicles(EVs)have been identified as promising biomarkers for the noninvasive diagnosis of various ***,challenges in separating EVs from soluble proteins have resulted in variable EV recovery rates and low ***,we report a high-yield(>90%)and rapid(pping(FLOAT).The FLOAT approach utilizes an acoustofluidic droplet centrifuge to rotate and controllably heat liquid *** adding a thermoresponsive polymer flocculant,nanoparticles as small as 20 nm can be rapidly and selectively concentrated at the center of the *** demonstrate the ability of FLOAT to separate urinary EVs from the highly abundant Tamm-Horsfall protein,addressing a significant obstacle in the development of EV-based liquid *** to its high-yield nature,FLOAT reduces biofluid starting volume requirements by a factor of 100(from 20 mL to 200µL),demonstrating its promising potential in point-of-care diagnostics.

A rapid,precise method for identifying waterborne pathogens is critically needed for effective disinfection and better ***,conventional methods,such as culture-based counting,generally suffer from slow detection times and low ***,we developed a rapid detection method for tracing waterborne pathogens by an innovative optofluidic platform,a plasmonic bacteria on a nanoporous mirror,that allows effective hydrodynamic cell trapping,enrichment of pathogens,and optical signal *** designed and simulated the integrated optofluidic platform to maximize the enrichment of the bacteria and to align bacteria on the nanopores and plasmonic mirror via hydrodynamic cell *** nanoparticles are self-assembled to form antenna arrays on the surface of bacteria,such as Escherichia coli and pseudomonas aeruginosa,by replacing citrate with hydroxylamine hydrochloride in order to amplify the signal of the plasmonic optical *** to the synergistic contributions of focused light via the nanopore geometry,self-assembled nanoplasmonic optical antennas on the surface of bacteria,and plasmonic mirror,we obtain a sensitivity of detecting *** as low as 102 cells/ml via surface-enhanced Raman *** believe that our label-free strategy via an integrated optofluidic platform will pave the way for the rapid,precise identification of various pathogens.

The ability to capture the chemical signatures of biomolecules(i.e.,electron-transfer dynamics)in living cells will provide an entirely new perspective on biology and *** can be accomplished using nanoscale optical antennas that can collect,resonate and focus light from outside the cell and emit molecular ***,we describe biologically inspired nanoscale optical antennas that utilize the unique topologies of plant viruses(and thus,are called gold plant viruses)for molecular fingerprint *** electromagnetic calculations for these gold viruses indicate that capsid morphologies permit high amplification of optical scattering energy compared to a smooth *** experimental measurements of various gold viruses based on four different plant viruses,we observe highly enhanced optical cross-sections and the modulation of the resonance wavelength depending on the viral ***,in label-free molecular imaging,we successfully obtain higher sensitivity(by a factor of up to 10^(6))than can be achieved using similar-sized *** virtue of the inherent functionalities of capsids and the plasmonic characteristics of the gold layer,a gold virus-based antenna will enable cellular targeting,imaging and drug delivery.

Nucleic acid amplification and quantification via polymerase chain reaction(pCR)is one of the most sensitive and powerful tools for clinical laboratories,precision medicine,personalized medicine,agricultural science,forensic science and environmental *** multiplex pCR,characterized by low power consumption,compact size and simple operation,is ideal for timely diagnosis at the point-of-care(pOC).Although several fast/ultrafast pCR methods have been proposed,the use of a simple and robust pCR thermal cycler remains challenging for pOC ***,we present an ultrafast photonic pCR method using plasmonic photothermal light-to-heat conversion via photon–electron–phonon *** demonstrate an efficient photonic heat converter using a thin gold(Au)film due to its plasmon-assisted high optical absorption(approximately 65%at 450 nm,the peak wavelength of heat source light-emitting diodes(LEDs)).The plasmon-excited Au film is capable of rapidly heating the surrounding solution to over 150℃ within 3 *** this method,ultrafast thermal cycling(30 cycles;heating and cooling rate of 12.7960.93℃ s^(-1) and 6.660.29℃ s^(-1),respectively)from 55℃(temperature of annealing)to 95℃(temperature of denaturation)is accomplished within 5 *** photonic pCR thermal cycles,we demonstrate here successful nucleic acid(λ-DNA)*** simple,robust and low cost approach to ultrafast pCR using an efficient photonic-based heating procedure could be generally integrated into a variety of devices or procedures,including on-chip thermal lysis and heating for isothermal amplifications.

Tissue-specific Tregs in cancer metastasis:opportunities for precision immunotherapy

Decades of advancements in immuno-oncology have enabled the development of current immunotherapies,which provide longterm treatment responses in certain metastatic cancer ***,cures remain infrequent,and most patients ultimately succumb to treatment-refractory metastatic *** insights suggest that tumors at certain organ sites exhibit distinctive response patterns to immunotherapy and can even reduce antitumor immunity within anatomically distant tumors,suggesting the activation of tissue-specific immune tolerogenic mechanisms in some cases of therapy *** immune cells known as regulatory T cells(Tregs)are present within all tissues in the body and coordinate the suppression of excessive immune activation to curb autoimmunity and maintain immune *** the high volume of research on Tregs,the findings have failed to reconcile tissue-specific Treg functions in organs,such as tolerance,tissue repair,and regeneration,with their suppression of local and systemic tumor immunity in the context of immunotherapy *** improve the understanding of how the tissue-specific functions of Tregs impact cancer immunotherapy,we review the specialized role of Tregs in clinically common and challenging organ sites of cancer metastasis,highlight research that describes Treg impacts on tissue-specific and systemic immune regulation in the context of immunotherapy,and summarize ongoing work reporting clinically feasible strategies that combine the specific targeting of Tregs with systemic cancer *** knowledge of Tregs in the framework of their tissue-specific biology and clinical sites of organ metastasis will enable more precise targeting of immunotherapy and have profound implications for treating patients with metastatic cancer.

To evaluate patient preferences for sildenafil citrate or tadalafil （pDE-5 inhibitors available for the treatment of erectile dysfunction [ED]） and assess potential reasons for these preferences. Methods： This open-label study was conducted on Korean men taking sildenafil, at least 6 weeks prior to study entry, for ED. Following screening, patients continued sildenafil treatment for 4 weeks, then after a 1-week washout period, switched to tadalafil for 8 weeks. patients then continued with their treatment of choice during an extension phase. psychosocial factors （time concern, spontaneity, sexual self-confidence） were evaluated using psychological and Interpersonal Relation- ship Scales （pAIRS）, while timing of dose to sexual attempt patterns were assessed from patient diaries. Results： The present study enrolled 160 Korean men （mean age 55 years） with prior median sildenafil use of 585 days. During the extension phase, 73.7% of patients elected to take tadalafil, whereas 26.3% chose sildenafil （p 〈 0.001）. After switching from sildenafil to tadalafil, mean pAIRS time concern scores decreased from 2.54 to 2.42 （p = 0.002）, with no statistically significant differences observed between the sildenafil and tadalafil assessment phases in sexual spontaneity and self-confidence scores. Sexual attempts made 〉 4 h to 〈 36 h post-dose occurred in 4.5% of patients during the sildenafil assessment phase compared with 17.5% during the tadalafil assessment phase. Conclusion： After experiencing both sildenafil and tadalafil, the majority of patients exhibited a preference for tadalafil. This preference might be influenced by psychosocial factors, such as decreased time concerns, and a broader window of opportunity available for sexual activity.

Evaluation of a point of care ultrasound curriculum for Indonesian physicians taught by fi rst-year medical students

BACKGROUND: The purpose of this study was to assess the short-term efficacy of a 4-week ultrasound curriculum taught by American first-year medical students to general practitioners working in public health care clinics, or puskesmas, in Bandung, ***: We performed a prospective, observational study of Indonesian health care practitioners from public clinics in Bandung, Indonesia. These practitioners were enrolled in a 4-week ultrasound training course taught by first-year American medical students. A total of six sessions were held comprising of 38 ultrasound milestones. A pre-course and post-course written exam and practical exam was taken by each ***: We enrolled 41 clinicians in the course. The average pre-course exam score was 35.2% with a 2.4% pass rate, whereas the average post-course exam score was 82.0% with a 92.7% pass rate. The average practical score at the completion of the course was 83.2%(SD=0.145) with 82.9% of the class passing(score above 75.0%).CONCLUSION: Our data suggests that first-year medical students can effectively teach ultrasound to physicians in Indonesia using a 4-week intensive ultrasound training course. Future studies are needed to determine the amount of training required for proficiency and to evaluate the physicians' perceptions of the student-instructors' depth of knowledge and skill in point of care ultrasound.

Comparison of ultrasound-measured properties of the common carotid artery to tobacco smoke exposure in a cohort of Indonesian patients

BACKGROUND: The purpose of this study was to use point-of-care ultrasound(pOCUS) to investigate the relationship between tobacco smoke exposure and the characteristics of the common carotid artery(CCA). The effect of both primary and secondary smoking on CCA properties was ***: We performed a prospective cross-sectional study across 20 primary care clinics in Bandung, West Java, Indonesia in July 2016. point of care ultrasound was performed on a convenience sample of Indonesian patients presenting to clinic. The CCA wall stiffness and carotid intima-media thickness(CIMT) were measured during diastole and systole. These measurements were correlated with smoke exposure and cardiovascular ***: We enrolled 663 patients in the study, with 426 patients enrolled in the smoking category and 237 patients enrolled in the second-hand smoke category. There was an overall positive correlation with the measured lifestyle factors and the ultrasound-measured variables in the group of individuals who smoked. For all variables, age seemed to contribute the most out of all of the lifestyle factors for the positive changes in CIMT and CCA wall ***: Our data yielded correlations between CCA properties and cardiovascular risk, as well as between CIMT and arterial stiffness. We were also able to demonstrate an increase in thickness of the CIMT in patients who have been exposed by tobacco through the use of ultrasound. Further large scale studies comparing patients with multiple cardiac risk factors need to be performed to confi rm the utility of ultrasound fi ndings of cardiovascular disease and stroke.
