The purpose of this study was to determine the relationship between insulin resistance, obesity and serum prostate-specific antigen （PSA） levels in healthy men with serum PSA level below 4 ng mL-1. The men included in the study cohort were 11 827 healthy male employees of the Korea Hydro and Nuclear Power Co., LTD who had undergone medical checkups including fasting glucose, fasting insulin and serum PSA between January 2003 and December 2008. Insulin resistance was calculated by homeostasis model assessment （HOMA [fasting glucose × fasting insulin]/22.5） and quantitative insulin sensitivity check index （QUICK/; 1/[log （fasting insulin）＋ log （fasting glucose）]）. Age-adjusted body mass index （BMI） was significantly increased according to increasing quartile of insulin resistance as determined by HOMA and QUICKI, respectively, in analysis of variance （ANOVA） test and Duncan＇s multiple comparison test （P 〈 0.001）, but age-adjusted serum PSA concentration was significantly decreased according to increasing quartile of insulin resistance as determined by HOMA and QUICK/（P 〈 0.001）. Age, BMI, insulin resistance by HOMA or QUICK/were significantly independent variables to serum PSA level in a multivariate linear regression analysis （P 〈 0.001）. Insulin resistance was a significant independent variable to serum PSA level along with BMI. Insulin resistance and BMI were negatively correlated with serum PSA level in healthy men. Insulin resistance was positively correlated with BMI.

Objective:Traditionally,gallbladder is routinely removed during left hepatectomy even if there is no gallbladder ***,adverse consequence after cholecystectomy,though rare,still occa-sionally *** study aims to evaluate the feasibility of gallbladder preservation during robotic left ***:All consecutive robotic left hepatectomy cases between December 2010 and January 2022 in Prince of Wales Hospital,the Chinese University of Hong Kong were retrieved from a prospectively collected *** gallbladder was preserved by moving the liver transection line just away from the gallbladder *** were divided into two groups:gallbladder preservation(GBP)and non-gallbladder preservation(ngBP).Operative results and long-term outcomes were compared between these two ***:There were 11 cases in the GBP group and 25 cases in the ngBP *** two groups were comparable in terms of the patient demographics and disease *** was no operative *** was no difference between the two groups in operative time(GBP 270 min *** 332 min,p=0.132),blood loss(GBP 50 mL *** 150 mL,p=0.115)or complication rate(GBP 27.3%*** 24.0%,p>0.999).There was also no difference in 5-year overall *** the GBP group,no patient developed specific symptoms or complications related to the preserved ***-up ultrasound or computed tomography revealed a normal appearance of the preserved gallbladders except in one patient who developed a 3-mm gallbladder *** the other hand,one(4%)patient in the ngBP group developed troublesome diarrhoea after ***:Gallbladder preservation is safe and feasible during robotic left *** preserved gallbladder does not lead to any symptoms,while postcholecystectomy diarrhoea can be avoided.

OBJECTIVE Pain sensations in the periphery are carried by primary sensory neurons whose cell bodies lay in the dorsal root ganglia(DRG).The present study focuses on examining the interactions between DRG neurons and glial cells in vitro in relation to their roles as detectors of tissue *** Acutely dissociated DRG cells were isolated from adult SD rats,and plated on poly-DL-ornithine and laminin-coated tissue culture *** in 3 cell groups were compared over time in culture: mixed DRG cells(45%neurons on plating),neuron-enriched cells(70%neurons on plating) and glial cells(99%glial cells on plating).Cell proliferation was determined by counting cells,protein assay and MTT assay.[H]cAMP production in the presence of specific receptor agonists and antagonists was assayed to detect Gs-coupled *** acidic fibrillary protein(GFAP),glutamine synthase(GS) and Toll-like receptor 4(TLR4) mRNA and protein expression were determined by real time PCR and immunocyto-chemistry, *** The presence of Gs-coupledβAR,CGRP,EP and IP receptors were identified in glial cell preparations although only CGRP receptors have been reported on satellite glial cells in ***,IP receptor-stimulated adenylyl cyclase activity in glial cells was significantly inhibited by the presence of DRG *** neurons did not overtly affect glial cell proliferation or the expression of GFAP(classical markers of glial cell activation),but appeared to inhibit the expression of TLR4 and prevent the loss of GS in glial *** Time-dependent changes occur in DRG glial cells in vitro with regard to the expression of TLR4,Gs-coupled receptors and glial cell markers,and all these changes are influenced by the presence of DRG neurons. Whether similar neuron-glial interactions occur in vivo remains to be determined.

目的:探讨辅助依赖性腺病毒载体(Helper-dependent adenoviral vector,Hd-Adv)在中枢神经系统,尤其在帕金森病(PD)中的应用前景。方法:首先在Cre／loxP系统中构建带有标记基因LacZ的辅助依赖性腺病毒Hd-Adv-LacZ,经感染体外培养的细胞证实其表达。然后将Hd-Adv-LacZ或PBS立体定向注射入大鼠的纹状体区,分别于术后3、7、21、30、60 和90天,用X-gal检测LacZ基因的表达,用免疫组化方法检测脑内免疫反应。结果:注射后3天在纹状体和黑质区即出现 LacZ基因的表达,并可维持至少90天,脑内未见明显的免疫炎症反应。结论:由Hd-Adv介导的LacZ基因在体内可安全、长期的表达,并可逆行转运到黑质区。这一结果为Hd-Adv作为基因治疗载体治疗PD提供了重要的实验依据。

Serum prostate-specific antigen value adjusted for non-cancerous prostate tissue volume in patients undergoing radical prostatectomy： a new predictor of biochemical recurrence in localized or locally advanced prostate cancer

The aim of this study was to investigate the significance of serum prostate-specific antigen （PSA） value adjusted for total tumor volume （PSA/tumor volume） and serum PSA value adjusted for non-cancerous prostate tissue volume （NCPV）（PSA/NCPV） as a predictor of pathological findings and clinical outcome after radical prostatectomy. Clinical and pathological data of 407 patients （median age： 66.5 years; range： 41.8--85.7 years） were reviewed retrospectively. The median follow-up period was 18. I months （range： 1.0- 107.8 months）. Biochemical recurrence was defined as detectable PSA levels （greater than 0.2 ng ml-1） and the time of biochemical recurrence was taken to be the first time PSA became detectable. In the multivariate model, PSA/NCPV was an independent predictor of extracapsular extension and positive surgical margin （P〈O.05）, but PSA/tumor volume was not. Kaplan-Meier curves revealed that PSA/NCPV correlated with biochemical recurrence-free survival （P〈O.O01; log-rank test） but PSA/tumor volume did not （P=0.275; log-rank test）. PSA/NCPV was also a significant independent prognostic factor for biochemical recurrence-free survival on multivariate Cox proportional hazard analysis （P=0.004, relative risk=2.42）. Our findings suggest that PSA/NCPV is associated independently with extracapsular extension and surgical margin status and may be an independent prognostic variable of PSA recurrence after radical prostatectomy.

Epitaxially grown Ⅲ-nitride alloys are tightly bonded materials with mixed covalent-ionic bonds. This tight bonding presents tremendous challenges in developing Ⅲ-nitride membranes, even though semiconductor membranes can provide numerous advantages by removing thick, inflexible, and costly substrates. Herein, cavities with various sizes were introduced by overgrowing target layers, such as undoped GaN and green LEDs, on nanoporous templates prepared by electrochemical etching of n-type GaN. The large primary interfacial toughness was effectively reduced according to the design of the cavity density, and the overgrown target layers were then conveniently exfoliated by engineering tensile-stressed Ni layers. The resulting Ⅲ-nitride membranes maintained high crystal quality even after exfoliation due to the use of GaN-based nanoporous templates with the same lattice constant. The microcavity-assisted crack propagation process developed for the current Ⅲ-nitride membranes forms a universal process for developing various kinds of largescale and high-quality semiconductor membranes.
