contact burn injuries account for a considerable proportion of admissions that frequently require debridement. Such debridements of these multiple open wounds might benefit from the application of negative pressure wound therapy (NPWT) for the removal of proinflammatory mediators and to promote granulation tissue with macrostrain and microstrain. Having four or more Vacuum Assist closure (V.A.c.) Ulta devices connected to the same patient for adequate wound coverage is cumbersome in the management of the patient’s wound care and tethers the patient to the bed. The V.A.c. RX-4 is a multichannel device that can deliver NPWT with a smaller footprint. In addition, the V.A.c. RX-4 has a weight of 16 lbs. versus one V.A.c Ulta which is 7.4 lbs. Therefore, collectively, four V.A.c. Ultas would equal 29.6 lbs. or almost double the weight of a single V.A.c. RX-4. Use of the V.A.c. RX-4 by healthcare providers can mean greater mobility for the patient and easier transport between patient destinations within the hospital. This case report demonstrates the utility of the V.A.c. RX-4 for open and freshly debrided, large soft tissue wounds in a burn patient.

Background:Forests perform various important ecosystem functions that contribute to ecosystem *** many parts of the world,forest management has shifted from a focus on timber production to multi-purpose forestry,combining timber production with the supply of other forest ecosystem ***,it is unclear which forest types provide which ecosystem services and to what extent forests primarily managed for timber already supply multiple ecosystem *** on a comprehensive dataset collected across 150 forest plots in three regions differing in management intensity and species composition,we develop models to predict the potential supply of 13 ecosystem *** use those models to assess the level of multifunctionality of managed forests at the national level using national forest inventory ***:Looking at the potential supply of ecosystem services,we found trade-offs(*** both bark beetle control or dung decomposition and both productivity or soil carbon stocks)as well as synergies(*** temperature regulation,carbon storage and culturally interesting plants)across the 53 most dominant forest types in *** single forest type provided all ecosystem services *** ecosystem services showed comparable levels across forest types(*** or richness of saprotrophs),while others varied strongly,depending on forest structural attributes(*** availability or cover of edible plants)or tree species composition(*** nitrification activity).Variability in potential supply of ecosystem services was only to a lesser extent driven by environmental ***,the geographic variation in ecosystem function supply across Germany was closely linked with the distribution of main tree ***:Our results show that forest multifunctionality is limited to subsets of ecosystem *** importance of tree species composition highlights that a lack of multifunctionality at the stand lev

Over 11% of all pregnancies in the US result in preterm birth, greatly contributing to perinatal morbidity and mortality （Goldenberg and Rouse, 1998）. Preterm birth etiologies remain largely unknown, and effective prevention methods have yet to be developed. The use of biofluid （e.g., serum or urine） for the analysis of the naturally occurring peptidome （MW 〈 4000） as a source of biomarkers has been reported for different diseases （Villanueva et al., 2006; Ling et al., 2010a, 2010b, 2010c, 2011）. Mass spectrometry-based profiling of naturally occurring peptides can provide an extensive in- ventory of serum peptides derived from either high-abundant endogenous circulating proteins or cell and tissue proteins （Liotta and Petricoin, 2006）.

The Mesoamerican Ministers of Health have set 2020 as the target for malaria elimination to be achieved in the region. Imported malaria cases are a potential threat to countries attempting elimination or working to prevent resurgence. We report the first imported Plasmodium ovale infection with molecular confirmation in central America, which occurred in a Guatemalan soldier that had been deployed in Africa. The obstacles for its diagnosis using the standard microscopy technique and the need to improve its detection are discussed.

The etiology and pathogenesis of pulmonary fibrosis is poorly understood. We and others reported that M-cSF/cSF-1, M-cSF-R and downstream AKT activation plays an important role in lung fibrosis in mice models and in IPF patients. To understand potential molecular pathways used by M-cSF-R activation to direct lung fibrosis, we used a novel transgenic mouse model that expresses a constitutively-active form of AKT, myristoylated AKT (Myr-Akt), driven by the c-fms (M-cSF-R) promoter. We were particularly interested in the basal immune state of the lungs of these Myr-Akt mice to assess M-cSF-R-related priming for lung fibrosis. In support of a priming effect, macrophages isolated from the lungs of unchallenged Myr-Akt mice displayed an M2-tropism, enhanced co-expression of M-cSF-R and α-SMA, reduced autophagy reflected by reduced expression of the key autophagy genes Beclin-1, MAP1-Lc3a(Lc3a), and MAP1-Lc3b(Lc3b), and increased p62/STSQM1 expression compared with littermate WT mice. Furthermore, Myr-Akt mice had more basal circulating fibrocytes than WT mice. Lastly, upon bleomycin challenge, Myr-Akt mice showed enhanced collagen deposition, increased F4/80+ and cD45+ cells, reduced autophagy genes Beclin-1, Lc3a, and Lc3b expression, and a shorter life-span than WT littermates. These data provide support that M-cSF-R/AKT activation may have a priming effect which can predispose lung tissue to pulmonary fibrosis.

Blocking transforming growth factor(TGF)β1 signal transduction has been a central strategy for scar reduction;however,this approach appears to be minimally ***,we show that fibromodulin(FMOD),a 59-kD small leucine-rich proteoglycan critical for normal collagen fibrillogenesis,significantly reduces scar formation while simultaneously increasing scar strength in both adult rodent models and porcine wounds,which simulate human cutaneous scar ***,FMOD uncouples promigration/contraction cellular signals from pro-fibrotic signaling by selectively enhancing SMAD3-mediated signal transduction,while reducing AP-1-mediated TGFβ1 auto-induction and fibrotic extracellular matrix ***,FMOD accelerates TGFβ1-responsive adult fibroblast migration,myofibroblast conversion,and ***,our findings strongly indicate that,by delicately orchestrating TGFβ1 activities rather than indiscriminately blocking TGFβ1,FMOD elicits fetal-like cellular and molecular phenotypes in adult dermal fibroblasts in vitro and adult cutaneous wounds in vivo,which is a unique response of living system undescribed *** together,this study illuminates the signal modulating activities of FMOD beyond its structural support functions,and highlights the potential for FMOD-based therapies to be used in cutaneous wound repair.
