Minimally Invasive Surgical Approach for Spinal Canal Tumors—Technique Description and Experience from a Reference Center

Background: Spinal canal tumors are difficult to diagnose and treat. The traditional surgical approaches for attempting a complete excision of these lesions frequently involve big incisions and tissue dissection with high risk of postop instability and cerebrospinal fluid leakage. Also, there is a risk of neurological worsening, sometimes irreversible. Methods: We present our experience in a patient series with spinal canal tumors and describe the surgical approach with minimally invasive techniques (MIS). All of them were performed by the Neurosurgery team of the Hospital Universitario San Ignacio during the period of 2011-2016. Results: We reviewed forty patients with spinal canal tumors surgically treated with MIS techniques. 15 patients (37.5%) had Meningioma diagnosis (complete resection in 11 (73.3%), subtotal in 3 (20%) and biopsy in one patient), 10 patients (25%) with Schwannomas reached complete resection in 70% and subtotal in 30%. 5 patients had spinal cord metastasis, with complete resection in 4 patients (80%) and subtotal in 1 (20%). Other included ependymoma, astrocytoma, and miscellaneous. No patient has had cerebrospinal fluid leakage and no postoperative fusion has been required. Conclusions: The minimally invasive approach allowed complete tumors removal in a high number of patients and good postoperative results. These findings are similar and in some cases, better than the reported with traditional techniques. This MIS technique provides encouraging results. It requires a wide learning curve and a high degree of surgical experience.

AIM To noninvasively investigate tumor cellularity measured using diffusion-weighted magnetic resonance imaging(DW-MRI) and glucose metabolism measured by 18 Flabeled fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG-PET/CT) during radiation therapy(RT) for human papillomavirus negative(HPV-) head and neck squamous cell carcinoma(HNSCC).METHODS In this prospective study, 6 HPV- HNSCC patients underwent a total of 34 multimodality imaging examinations(DW-MRI at 1.5 T Philips MRI scanner [(n = 24) pre-, during-(2-3 wk), and post-treatment(Tx), and 18F-FDG PET/CT pre- and post-Tx(n = 10)]. All patients received RT. Monoexponential modeling of the DW-MRI data yielded the imaging metric apparent diffusion coefficient(ADC) and the mean of standardized uptake value(SUV) was measured from 18F-FDG PET uptake. All patients had a clinical follow-up as the standard of care and survival status was documented at 1 *** There was a strong negative correlation between the mean of pretreatment ADC(ρ =-0.67, P = 0.01) and the pretreatment 18F-FDG PET SUV. The percentage(%) change in delta(?) ADC for primary tumors and neck nodal metastases between pre- and Wk2-3 Tx were as follows: 75.4% and 61.6%, respectively, for the patient with no evidence of disease, 27.5% and 32.7%, respectively, for those patients who were alive with disease, and 26.9% and 7.31%, respectively, for those who were dead with *** These results are preliminary in nature and are indicative, and not definitive, trends rendered by the imaging metrics due to the small sample size of HPV- HNSCC patients in a Meixoeiro Hospital of Vigo Experience.

阐述了一种新型的可直接而快速分离红葡萄酒中酚类化合物(苯甲酸、黄烷-3-醇、肉桂酸、黄酮醇、花色素)的反相高效液相色谱法。采用的色谱柱可用于低pH环境且能高流速运行,在均含有0.2%三氟乙酸的水和乙腈两相中进行梯度洗脱。为了提高选择性,每种化合物都在其最大吸光度处被检测,从而建立其精密度、线性和灵敏度。尽管这种方法不能检测酒中所有组分,但适用于主要成分的测定和对特殊类别酚类化合物总量的定量分析。

Measurement invariance of maternal ratings of ADHD symptoms across clinic referred children’s with and without ADHD

The study examined the measurement invariance (configural,metric,scalar,and error variances) and factor mean scores equivalencies of a modified version of the Strengths and Weaknesses of ADHDSymptoms and Normal Behavior Scale (SWAN-M) across ratings provided by mothers of clinic-referred children and adolescents,diagnosed with (N = 666) and without (N = 202) ADHD. Confirmatory factor analysis (CFA) of these ratings provided support for the bi-factor model of ADHD [orthogonal general and specific factors for inattention (IA) and hyperactivity/impulsivity (HI) symptoms]. Multiple-group confirmatory factor analysis (CFA) of the bi-factor model supported full measurement invariance. Findings also showed that for latent mean scores,the ADHD group had higher scores than the non-ADHD group for the ADHD general and IA specific factors. The findings indicate that observed scores (based on maternal ratings of the SWAN-M) are comparable,as they have the same measurement properties. The theoretical,psychometric and clinical implications of the findings are discussed.

＜正＞ Multiple alphal-adrenoceptors seem to contract rat tail artery. AlphalA- predominates and either alpha1B-,alpha1D-,or alpha1L-is the second population. We characterized alphal-adrenoceptors with selective agonists/antagonists in tail artery of young Wistar rat. Tail artery was exposed to A61603 （N-[5-（4.5- dihy-dro- 1H- imidazol - 2 - yl） - 2 - hydroxy - 5. 6. 7. 8 - tetrahydronaphthalen- 1 - yl] methanesulfonamide） or to phenylephrine （alphalA- and alphal- agonists ）, and to prazosin （alphal- antagonist）, the alphal A-antagonists 5 - methy-lurapidil,RS 100329 （5-methyl - 3 - [3 - [4 - [2 - （2,2,2,-trifluoroethoxy） phenyl] - 1-piperazinyl] propyl] - 2,4-（1H）-pyrimidinedione）,RS 17053 （N- [2（2-cyclopropylmethoxy） ethyl] - 5-chloro-a, a-dimethyl - 1H-indole - 3-ethylamine）, and the alpha1D-antagonist BMY 7378 （8-[2-[4-（2-methoxyphenyl） - 1-piperazinyl] ethyl] - 8 - azaspiro[4.5] decane - 7,9 -dione） . A61603 showed 100 fold higher affinity over PHE. Prazosin,RS100329,5-MU,and RS17053 displaced agonists with high affinity indicating alphal A-adrenoceptors while PHE activated alphal B-adrenoceptors since BMY 7378 had low affinity.
