Endoscopic stent therapy in patients with chronic pancreatitis:A 5-year follow-up study

AIM:This study analyzed clinical long-term outcomes after endoscopic therapy,including the incidence and treatment of relapse. METHODS:This study included 19 consecutive patients(12 male,7 female,median age 54 years)with obstructive chronic pancreatitis who were admitted to the 2nd Medical Department of the Technical University of *** patients presented severe chronic pancreatitis(stageⅢ°)according to the Cambridge *** majority of the patients suffered intermittent pain attacks.6 of 19 patients had strictures of the pancreatic duct;13 of 19 patients had strictures and *** first endoscopic retrograde pancreatography(ERP)included an endoscopic sphincterotomy, dilatation of the pancreatic duct,and stent *** first control ERP was performed 4 wk after the initial intervention,and the subsequent control ERP was performed after 3 mo to re-evaluate the clinical and morphological *** follow-up was performed annually to document the course of pain and the management of *** course of pain was assessed by a pain scale from 0 to *** date and choice of the therapeutic procedure were documented in case of relapse. Bones and Crohn’s:No benefit of adding sodium fluoride or ibandronate to calcium and vitamin D

AIM: To compare the effect of calcium and cholecalciferol alone and along with additional sodium fluoride or ibandronate on bone mineral density (BMD) and fractures in patients with Crohn's disease (CD). METHODS: Patients (n =148) with reduced BMD (T-score < -1) were randomized to receive cholecalciferol (1000 IU) and calcium citrate (800 mg) daily alone(group A, n = 32) or along with additional sodium fluoride (25 mg bid) (group B, n = 62) or additional ibandronate (1 mg iv/3-monthly) (group C, n = 54). Dual energy X-ray absorptiometry of the lumbar spine (L1-L4) and proximal right femur and X-rays of the spine were performed at baseline and after 1.0, 2.25 and 3.5 years. Fracture-assessment included visual reading of X-rays and quantitative morphometry of vertebral bodies (T4-L4).RESULTS: One hundred and twenty three (83.1%) patients completed the first year for intention-to-treat (ITT) analysis. Ninety two (62.2%) patients completed the second year and 71 (47.8%) the third year available for per-protocol (PP) analysis. With a significant increase in T-score of the lumbar spine by +0.28 ± 0.35 [95% conf idence interval (CI): 0.162-0.460, P < 0.01], +0.33 ± 0.49 (95% CI: 0.109-0.558, P < 0.01), +0.43 ± 0.47 (95% CI: 0.147-0.708, P < 0.01) in group A, +0.22 ± 0.33 (95% CI: 0.125-0.321, P < 0.01); +0.47 ± 0.60 (95% CI: 0.262-0.676, P < 0.01), +0.51 ± 0.44 (95% CI: 0.338-0.682, P < 0.01) in group B and +0.22 ± 0.38 (95% CI: 0.111-0.329, P < 0.01), +0.36 ± 0.53 (95% CI: 0.147-0.578, P < 0.01), +0.41 ± 0.48 (95% CI: 0.238-0.576, P < 0.01) in group C, respectively, during the 1.0, 2.25 and 3.5 year periods (PP analysis), no treatment regimen was superior in any in- or between-group analyses. Prevalence of cholelithiasis in patients with chronic inflammatory bowel disease

AIM： To investigate the effect of chronic inflammatory bowel disease （CIBD） specific risk factors for cholecystolithiasis, as duration and involvement pattern of the disease and prior surgery in patients with Crohn＇s disease （CD） and ulcerative colitis （UC）. METHODS： A total of 222 patients with CD （135 females, 87 males, average age, 35.8±11.8 years; range 17-81 years） and 88 patients with UC （39 females, 49 males; average age, 37.2±13.6 years; range 16-81 years） underwent clinical and ultrasound examinations. Besides age, sex and degree of obesity, patients＇ CIBD specific parameters, including duration and extent of disease and prior operations were documented and evaluated statistically using logistic regression. RESLTLTS： The overall prevalence of gallbladder stone disease in patients with CD was 13% （n = 30）. Only age could be shown to be an independent risk factor （P = 0.014）. Compared to a collective representative for the general population in the same geographic region, the prevalence of cholecystolithiasis was higher in all corresponding age groups. Patients with UC showed an overall prevalence of gallbladder stone disease of only 4.6%. CONCLUSION：Only age but not disease-specific factors such as duration and extent of disease, and prior surgery are independent risk factors for the development of cholecystolithiasis in patients with CIBD.

Approximately 950 individual sequences of nonribosomally biosynthesised peptides are produced by the genus Trichoderma/Hypocreathat belong to a perpetually growing class of mostly linear antibiotic oligopeptides,which are rich in the non-proteinogenic α-aminoisobutyric acid(Aib).Thus,they are comprehensively named ***,peptaibiotics represent ca.80% of the total inventory of secondarymetabolites currently known from Trichoderma/*** unique membrane-modifying bioactivity results from amphipathicity and helicity,thus making them ideal candidates in assisting both colonisation and defence of the natural habitats by their fungal *** this,reports on the in vivo-detection of peptaibiotics have scarcely been published in the *** order to evaluate the significance of peptaibiotic production for a broader range of potential producers,we screened nine specimens belonging to seven hitherto uninvestigated fungicolous or saprotrophic Trichoderma/Hypocrea species by liquid chromatography coupled to electrospray high resolution mass *** of peptaibiotics found were independently confirmed by analysing the peptaibiome of pure agar cultures obtained by single-ascospore isolation from the *** the nine species examined,five were screened positive for peptaibiotics.A total of 78 peptaibiotics were sequenced,56(=72%)of which are ***,dihydroxyphenylalaninol and O-prenylated tyrosinol,two C-terminal residues,which have not been reported for peptaibiotics before,were found as well as new and recurrent sequences carrying the recently described tyrosinol residue at their *** majority of peptaibiotics sequenced are 18-or 19-residue *** homologies with‘classical representatives’of subfamily 1(SF1)-peptaibiotics argue for the formation of transmembrane ion channels,which are prone to facilitate the producer capture and defence of its substratum.

With the growing importance of the black soldier fly(Hermetia illucens)for both sustainable food production and waste management as well as for science,a great demand of understanding its immune system ***,we present the first description of the circulating larval hemocytes with special emphasis on uptake of microorganisms and distinguishing hemocyte *** histological,zymographic,and cytometric methods and with a set of hemocyte binding lectins and antibodies,the hemocytes of *** are identified as plasmatocytes,crystal cells,and putative *** hemocyte counts(THC)are determined,and methods for THC determination are *** 1100 hemocytes per microliter hemolymph are present in naive animals,while hemocyte density decreases dramatically shortly after wounding,indicating a role of hemocytes in response to wounding(and immune response in general).The determination of the relative abundance of each hemocyte type(differential hemocyte count,DHC)revealed that plasmatocytes are highly abundant,whereas prohemocytes and crystal cells make up only a small percentage of the circulating *** are not only the most abundant but also the professional phagocytes in *** rapidly engulf and take up bacteria both in vivo and in vitro,indicating a very potent cellular defense against invading *** bioparticles such as yeasts are also removed from circulation by phagocytosis,but slower than *** is the first analysis of the potent cellular immune response in the black soldier fly,and a first toolbox that helps to identify hemocyte(types)is presented.

Lack of correlation between Treg quantification assays in inflammatory bowel disease patients

AIM:To compare the number of regulatory T-cells( Tregs) measured by flow cytometry with those obtained using a real-time quantitative PCR(q PCR) method in patients suffering from inflammatory bowel disease(IBD).METHODS:Tregs percentages obtained by both flow cytometry and q PCR methods in 35 adult IBD patients,18 out of them with Crohn′s disease(CD)and 17 with ulcerative colitis(UC)were compared to each other as well as to scores on two IBD activity questionnaires using the Harvey Bradshaw Index(HBI)for CD patients and the Simple Colitis Clinical Activity Index(SCCAI)for UC *** Treg percentages by flow cytometry were defined as CD4+CD25highCD127lowFOXP3+cells in peripheral blood mononuclear cells,whereas the Treg percentages by q PCR method were determined as FOXP3 promoter demethylation in genomic ***:We found an average of 1.56%±0.78%Tregs by using flow cytometry,compared to 1.07%±0.53%Tregs by using q PCR in adult IBD *** were no significant correlations between either the percentages of Tregs measured by flow cytometry or q PCR and the HBI or SCCAI questionnaire scores in CD or UC patients,*** addition,there was no correlation between Treg percentages measured by q PCR and those measured by flow cytometry(r=-0.06,P=0.73;Spearman Rho).These data suggest that,either Treg-related immune function or the clinical scores in these IBD patients did not accurately reflect actual disease *** the cause(s)for these differences are more clearly defined,the resultssuggest caution in interpreting studies of Tregs in various inflammatory ***:The two methods did not produce equivalent measures of the percentage of total Tregs in the IBD patients studied which is consistent with the conclusion that Tregs subtypes are not equally detected by these two assays.

Ultrathin coating of particles in fluidized bed using submicron droplet aerosol

In this work,we demonstrate that particles can be coated in a fluidized bed w让h coating solution provided by a novel aerosol *** droplets are smaller than 1 pim in volume-based diameter,hence they are very much smaller than droplets in conventional spray fluidized bed processes(around 40μm).A proof-of-principle experiment with 30%aqueous coating solution of sodium benzoate and γ-Al2O3 core particles in 150 mm fluidized bed fed with droplet aerosol supplied from the chamber side is *** simultaneously coat and dry the particles,inlet of fluidization air was at 50℃ .Moreover,Monte Carlo simulations of coating with small aerosol and large spray droplets were *** to dramatically smaller building blocks,ultrathin particle coating of high-resolution(very small layer thickness)can be attained with the new aerosol process,with the potential of even going *** coverage of particles is reached substantially faster than in the conv entional process,so that material dema nd is much lower and sensitive materials can be processed in short reside nee *** yield of around 30%was much higher than expected,that is considered to be technically viable and may be enhanced by the recycling of entrained solids or better equipment design.

Ray tracing Particle Image Velocimetry(RT-PIV)is an optical technique for high resolution velocity measurements in challenging optical systems,such as transparent packed beds,that uses ray tracing to correct for distortions introduced by transparent geometries in the light *** ray tracing based correction is a post processing step applied to the raw PIV particle images before classical PIV *** this study,RT-PIV is performed in the top layer of a body centred cubic(bcc)sphere packing with gaseous flow,where optical access is obtained by the use of transparent N-BK7 glass balls with a diameter of d=40 ***-PIV introduces new experimental and numerical challenges,for example a limited field of view,illumination difficulties,a very large required depth of field and high sensitivity to geometric parameters used in the ray tracing *** challenges and their implications are the main scope and discussed in the present ***,the validation of the ray tracing reconstruction step is presented and examples for the obtained corrected vector fields in a packed bed are *** results show the strength of the method in reconstructing velocity fields behind transparent spheres that would not have been accessible by optical measurement techniques without the ray tracing correction.
