AIM:To analyze the changing trends in penetrating keratoplasty(PKP)***:This retrospective study included all patients with PKP between 2006 and *** were classified using histological *** groups were as the following:pseudophakic or aphakic bullous keratopathy,regraft,acute necrotizing and ulcerative keratitis,keratoconus,Fuchs'dystrophy,corneal dystrophy other than Fuchs',corneal scar,other diagnoses and failed endothelial keratoplasty ***,two different time-periods(2006-2012 and 2013-2017)were ***:Totally 1721 histological analyses of 1214 patients were available for *** diagnoses were pseudophakic or aphakic bullous keratopathy in 487(28.3%),regraft in 443(25.7%),acute necrotizing and ulcerative keratitis in 313(18.2%),corneal scar in 153(8.9%),keratoconus in 140(8.1%).Fuchs'dystrophy in 61(3.5%),corneal dystrophy other than Fuchs'in 46(2.7%),other diagnoses in 44(2.6%)and failed endothelial keratoplasty graft in 34(2.0%)*** the first to the second analysed time-period,incidence of acute necrotizing and ulcerative keratitis,corneal scar,Fuchs'dystrophy increased(P<0.032 for all)and incidence of keratoconus significantly decreased(P=0.015).CONCLUSION:Pseudophakic or aphakic bullous keratopathy is the leading indication for PKP,followed by regraft and acute necrotizing and ulcerative keratitis.

Intrusion-related gold deposits(IRGDs)occur in the Eastern Desert(ED)of Egypt within magmatic districts that are exploited for tungsten and tin *** and intrusion-related rare metal deposits(IRRMDs)are almost invariably linked with the late to post collisional Younger Granites(YGs)that have three successive phases(Ⅰ,ⅡandⅢ).At~635–630 Ma,the ED underwent a transition in deformation style from compressional to extensional and a switch from subduction with crustal thickening to delamination with crustal *** transition was concurrent with the emplacement of a short magmatic pulse(~635–630 Ma)that represents a transition between orogenic gold deposits and IRGDs.K-rich calc alkaline granites(phaseⅠandⅡof the YGs)hosting IRGDs like the Hangalia deposit were emplaced during the time span 630–610 *** magmatism began at 610 Ma,coexisting with the K-rich calc-alkaline magmatism over the 610–590 Ma time span,where the Fawakhir(598±3 Ma)and Um Had(596±2 Ma)granites that host the IRGDs were *** time,the alkaline magmatism became more alkaline giving rise to phaseⅢof the YGs that hosts IRRMDs.A distinct metallogenic epoch comprising both IRGDs and IRRMDs,was undergoing extreme growth at~600 Ma.

Rapid thick strip casting （RTSC） by Anton Hulek, Inventmetall, is an innovative concept for the production of hot strips with a final as-cast thickness of about 25 mm before rolling. The innovation of the mechanism consists in a vertical mould performing a caterpillar motion. This moving mould has an unconventional parallelogram-shaped cross-section. The conventional rectangular shape is formed in the shaping machine, which is placed straight below the mould. Further elements of the technology are state-of-the-art. For the investigation of this new casting system theoretical calculations were complemented with practical experiments. The investigation focused mainly on two key aspects： the characteristics of the mould and the shaping process. For the practical analysis a static mould with three pairs of elements in laboratory scale was developed and commissioned by the Dept. of Ferrous Metallurgy @ RWTH Aachen University. The shaping experiments were carried out in model scale with two different materials and in variable boundary conditions. The results of these experiments delivered important mechanical as well as thermal informations about the casting system.

Background:Extended-spectrum beta-lactamase (ESBL)-producing Gram-negative bacteria are highly dangerous to *** our Neonatal Intensive Care Unit (NICU),the presence of these bacteria became so threatening in 2011 that immediate intervention was ***:This study was conducted during a nearly two-year period consisting of three phases:retrospective (9 months),educational (3 months) and prospective (9 months).Based on retrospective data analysis,a complex management plan was devised involving the introduction of the INSURE protocol,changes to the antibiotic regimen,microbiological screening at short intervals,progressive feeding,a safer bathing protocol,staff hand hygiene training and continuous monitoring of the number of newly infected and newly colonized *** these intervals,a total of 355 patients were ***:Both ESBL-producing Enterobacter cloaceae and Klebsiella pneumoniae were found (in both patients and environmental samples).In the prospective period a significant reduction could be seen in the average number of both colonized (26/167 patients;P=0.029) and infected (3/167 patients;P=0.033) patients compared to data from the retrospective period regarding colonized (72/188 patients) and infected (9/188 patients) *** was a decrease in the average number of patientdays (from 343.72 to 292.44 days per months),though this difference is not significant (P=0.058).During the prospective period,indirect hand hygiene compliance showed a significant increase (from the previous 26.02 to 33.6 hand hygiene procedures per patient per hospital day,P<0.001).Conclusion:Colonizations and infections were rolled back successfully in a multi-step effort that required an interdisciplinary approach.

AIM: To compare the incidence of intraoperative complications during primary phacoemulsification(phaco) surgery between resident surgeons(residents) and staff surgeons(specialists) and to objectively determine the difficulty of stages in phaco surgery. METHODS: This retrospective study included cases of phaco cataract surgery performed between January and December 2019. There were no exclusion criteria. For each patient, demographics, clinical history, case complexity, type of surgeon, and operative details were reviewed. Primary outcomes included intraoperative complication rates and the objective measure of difficulty in the steps of the surgery performed by residents and ***: A total of 3272 cases were included;7.4%(n=241) of cases were performed by residents. The overall complication rate was 5.4%(n=177). The intraoperative complication rate was significantly higher(P<0.001) in residents(n=33, 13.7%) than in specialists(n=144, 4.8%). The most frequent complications were posterior capsule tear(n=85, 2.6%), anterior capsule tear(n=50, 1.53%), zonular fiber loss(n=45, 1.38%), and dropped nucleus(n=15, 0.46%). Objectively, the most difficult steps during surgery were phaco in 66(60.0%), capsulorhexis in 21(19.1%), irrigation/aspiration in 13(11.8%), hydrodissection in 9(8.2%), and intraocular lens(IOL) implantation in 1(0.9%) case. CONCLUSION: Intraoperative complication rates are higher in residents than in specialists. The order of objective difficulty in phaco surgery steps is in line with the subjective findings of other surveys, revealing that the most challenging parts of phaco surgery are phaco and capsulorhexis.

Crohn＇s disease （CD） is a chronic relapsing and remitting autoinflammatory disorder of the gastrointestinal tract that has many intestinal and extraintestinal complications. The purpose of treatment is long-term remission, reduction of complications, and improvement of patients＇ quality of life. In many cases, this can be quite challenging and it is necessary to have a well thought out management strategy. We present the case of a 38-year-old woman with fistulizing CD that manifested as diffuse abdominal pain and bloody diarrhea accompanied by arthralgia. In addition, there were ulcerative lesions surrounded by cutaneous inflammation and erythema on her extremities, indicative of pyoderma gangrenosum. The patient was treated with high doses of parenteral methylprednisolone without any improvement and was started on adalimumab. A positive response to adalimumab therapy was observed： after 2 mo of therapy, the ulcerative skin lesion healed completely and the enterogastric fistula was closed affcer 5 mo adalimumab treatment. Adalimumab might be a suitable initial as well as maintenance therapy in patients with complicated CD.

Reactive oxygen species （ROS） are continuously generated during aerobic metabolism and at moderate level. They play a role in redox signaling, but in significant concentration they cause oxidative damage and neurodegeneration. Because of the enhanced sensitivity of brain to ROS, it is especially important to maintain the normal redox state in different types of neuron cells. In last decade it became clear that regular exercise beneficially affects brain function, and can play an important preventive and therapeutic role in stroke, Alzheimer, and Parkinson diseases. The effects of exercise appear to be very complex and could include neurogenesis via neurotrophic factors, increased capillariszation, decreased oxidative damage, and increased proteolyfic degradation by proteasome and neprilysin. Data from our and other laboratories indicate that exercise-induced modulation of ROS levels plays a role in the protein content and expression of brain-derived neurotrophic factor, tyrosinerelated kinase B （TrkB）, and cAMP response element binding protein, resulting in better function and increased neurogenesis. Therefore, it appears that exercise-induced modulation of the redox state is an important means, by which exercise benefits brain function, increases the resistance against oxidative stress, facilitates recovery from oxidative stress, and attenuates age-associated decline in cognition.
