Systemic lupus erythematosus(SLE),a chronic autoimmune disease,is marked by impaired immune tolerance and heightened activity in both innate and adaptive immune *** of SLE include elevated type I interferons and autoantibody production,though the exact causes of SLE remain elusive despite advances in research *** to SLE research is understanding its pathogenesis and developing effective diagnostic and therapeutic *** studies have highlighted a significant link between mitochondrial abnormalities and SLE's development and *** dysfunction plays a key role in SLE pathogenesis through various mechanisms such as mitochondrial DNA mutations,oxidative stress,immune metabolic reprogramming,and cellular *** factors collectively contribute to the loss of self-tolerance,increased autoantibody production,and impaired clearance of immune complexes,leading to their *** triggers sustained inflammatory responses and damages multiple *** review focuses on the diagnostic and therapeutic approaches related to mitochondrial abnormalities in *** mitochondrial pathways presents a promising strategy for treating SLE,potentially improving prognosis and quality of life for those *** research should further investigate the role of mitochondria in the onset and progression of SLE,providing new avenues for understanding and managing this complex disease.

Background: The impact of sex on the clinical manifestations of rheumatoid arthritis (RA) were diversely reported in the literature. The Chinese Registry of rhEumatoiD arthrITis provides a platform for the investigation of this issue in Chinese ***: Demographic and clinical parameters were collected from all enrolled patients with RA and from patients with early RA (disease duration ≤6 months). The differences in data regarding disease activity, comorbidities, and medications for RA were compared between men and women. The proportions of patients who achieved remission and low disease activity were compared at enrollment and during 3-, 6-, and 12-month follow-up ***: A total of 11,564 patients were enrolled, 83.6% of whom were female. In all the enrolled patients and patients with early RA, C-reactive protein (CRP, 12.0vs. 6.7 mg/L), pain visual analogue scale (4.8vs. 4.5), patient’s and physician’s global assessment (4.9vs. 4.5 and 4.9vs. 4.5), 28-joint disease activity score using DAS28-CRP (4.3vs. 4.0) simplified disease activity index (21.9vs. 19.9), and clinical disease activity index (19.3vs. 18.0) were significantly higher in men than in women. Additionally, the swollen joint count/tender joint count and DAS28 using erythrocyte sedimentation rate were higher in male patients than in female patients with early RA. More female patients with early RA reached the treatment target at baseline than male patients (23.4%vs. 18.2%, assessed by CDAI). At 3 months, 6 months, and 12 months, the proportion of remission and treatment target achievement was similar in both sexes. Coronary artery disease (CAD) and stroke were more frequent in men than in ***: In Chinese patients with RA, men were found to have more active disease, as well as more cases of CAD and stroke. Therefore, sex should be carefully considered during the personalization of RA treatment.

To the Editor,Lupus nephritis(LN),a renal symptom of systemic lupus erythematosus(SLE),frequently causes severe organ damage in SLE patients.1 There is no consensus on the definition of refractory LN,which has been suggested to occur when remission is not achieved after 3–12 months of combined glucocorticoids and immunosuppressive therapy.2 Podocytic infolding glomerulopathy(PIG),a rare type of glomerular morphological alteration,is primarily characterized by the presence of microspheres,microtubular structures,or a combination of the two,which are associated with podocyte infolding into the glomerular basement membrane(GBM).3,4 The exact pathogenesis of PIG remains unclear,but it may be associated with autoimmune diseases like SLE and Sjögren's *** occurrence of refractory LN complicated by PIG is rare and poses significant challenges in ***,we endeavor to provide the patient with benefits by employing a novel treatment,Telitacicept,which has demonstrated the potential to achieve complete renal remission in such cases.

目的探讨系统性红斑狼疮(简称狼疮)TC小鼠肠道微生物菌群结构与抗ds DNA抗体之间的关系。方法 ELISA法检测狼疮TC小鼠血清抗ds DNA抗体,然后分别收集ds DNA阳性组和阴性组的小鼠粪便,利用Illumina Hi Seq 2500高通量测序,进行粪便样本16S测序,比较两组之间的肠道微生物菌群结构与抗ds DNA抗体水平之间的关系。结果通过ELISA检测抗体结果和小鼠粪便高通量测序结果显示,ds DNA阳性组TC小鼠的物种群落多样性显著低于ds DNA阴性组;同时,两组TC小鼠肠道微生物分别在门水平Chloroflexi(绿弯菌门);纲水平Betaproteobacteria(β变形菌纲)、Deltaproteobacteria(δ变形菌纲)和Ktedonobacteria(纤线杆菌纲);目水平Burkholderiales(伯克氏菌目)、Desulfovibrionales(脱硫弧菌目)和Ktedonobacterales(纤线杆菌目);科水平Alcaligenaceae(产碱菌科)和Desulfovibrionaceae(脱硫弧菌科);属水平Parasutterella(副萨特氏菌属)和Desulfovibrio(脱硫弧菌属)上差异有显著性(P<0.05)。结论狼疮TC小鼠肠道微生物菌群结构与抗ds DNA抗体高度相关,本研究为进一步阐明肠道微生物菌群与系统性红斑狼疮发病机制的关系提供依据。

目的通过对老龄系统性红斑狼疮小鼠(TC)皮肤症状的观察和鉴定,探讨其是否可以作为一种研究皮肤型狼疮的小鼠模型。方法观察TC小鼠出现皮肤症状的年龄,对比同龄小鼠与C57BL/6小鼠的皮肤症状,ELISA法检测小鼠血清抗dsDNA抗体及抗ANA抗体,HE染色检测小鼠皮肤病理学症状。结果 2/3的TC小鼠出现毛发脱失、皮肤溃烂等症状,其病变最早发生于40周龄,皮肤病变的狼疮小鼠血清抗dsDNA抗体和抗ANA抗体滴度明显高于C57BL/6小鼠(P<0.05);HE染色结果显示狼疮小鼠角质层缺失,上皮层连续性中断,真皮层大量淋巴细胞浸润。结论狼疮小鼠在40周后会出现皮肤病变,显示该小鼠可作为一种研究慢性皮肤型狼疮的疾病模型。

Background:The onset and clinical presentation of systemic lupus erythematosus(SLE)are *** studies have investigated the distinctions in clinical characteristics and treatment preferences in male and female SLE patients in the initial *** study aimed to improve the understanding of Chinese SLE patients by characterizing the different sexes of SLE patients in the inception ***:Based on the initial patient cohort established by the Chinese SLE Treatment and Research Group,a total of 8713 patients(795 men and 7918 women)with newly diagnosed SLE were enrolled between April 2009 and March *** these,2900 patients(347 men and 2553 women)were eligible for lupus nephritis(LN).A cross-sectional analysis of the baseline demographic characteristics,clinical manifestations,laboratory parameters,organ damage,initial treatment regimens,and renal pathology classification was performed according to ***:In the SLE group,as compared to female patients,male patients had a later age of onset(male ***:37.0±15.8 years vs.35.1±13.7 years,P=0.006)and a higher SLE International Collaborative Clinic/American College of Rheumatology damage index score(male ***:0.47±1.13 vs.0.34±0.81,P=0.015),LN(male ***:43.6%vs.32.2%,P<0.001),fever(male ***:18.0%vs.14.6%,P=0.010),thrombocytopenia(male ***:21.4%vs.18.5%,P=0.050),serositis(male ***:14.7%vs.11.7%,P=0.013),renal damage(male ***:11.1%vs.7.4%,P<0.001),and treatment with cyclophosphamide(CYC)(P<0.001).The frequency of leukopenia(male ***:20.5%vs.25.4%,P=0.002)and arthritis(male ***:22.0%vs.29.9%,P<0.001)was less in male patients with *** LN,no differences were observed in disease duration,SLE Disease Activity Index score,renal biopsy pathological typing,or 24-h urine protein quantification among the *** comparisons with female patients with LN,male patients had later onset ages(P=0.026),high serum creatinine(P<0.001),higher end-stage renal failure

目的观察羟氯喹对系统性红斑狼疮小鼠(TC小鼠)外周免疫器官(脾)淋巴细胞分布的影响。方法将30周龄的雌性TC小鼠随机分配,实验组经灌胃给予羟氯喹治疗处理[10 mg/(kg·d)],对照组经灌胃给予生理盐水处理[10 mg/(kg·d)],共处理5周。用酶联免疫吸附试验(ELISA)检测小鼠外周血抗dsDNA抗体滴度水平,苏木精-伊红(HE)染色法检测肾组织病理学改变,流式细胞术检测小鼠脾淋巴细胞比例。结果与对照组相比,实验组小鼠肾组织病理学评分显著低于对照组(P0.05)。结论羟氯喹对狼疮小鼠外周免疫器官淋巴细胞的免疫应答具有调节作用并且可缓解TC小鼠肾组织病理损伤。

目的探讨水中抗阻训练对类风湿关节炎居家患者手足关节功能康复的影响。方法将风湿免疫科门诊就诊的类风湿关节炎患者71例根据就诊日期单双日分为观察组(n=35)和对照组(n=36)。对照组接受常规护理及关节功能训练;观察组在对照组基础上实施水中抗阻训练,每天训练30~60 min,每周至少训练3 d。比较两组干预前后双手握力均值、功能障碍、疼痛、疲劳评分及晨僵时间。结果训练后观察组双手握力均值显著高于对照组,功能障碍、疼痛、疲劳得分及晨僵时间显著低于对照组(P<0.05,P<0.01)。结论对类风湿关节炎居家患者实施水中抗阻训练,有利于手足关节功能康复,改善相关症状,提高身体功能和日常生活活动能力。
