Patterns of medication use for the treatment of menopausal symptoms before and after the women’s health initiative;implications for decision-making practices of women and women’s health professionals

Background: The Women’s Health Initiative (WHI) published findings in 2002 that changed the perception of the use of hormone replacement therapy (HRT) for the reduction of cardiovascular risks. Menopausal women using HRT for the relief of vasomotor symptoms were advised to use the lowest possible dose of HRT over the shortest possible duration. Objective: This study sought to examine patterns of HRT use for the treatment of menopausal symptoms before and after the WHI among women at least 40 years of age. Methods: A retrospective analysis was performed on a total of 1367 women in the pre-WHI group and 6467 women in the post-WHI group using the U.S. General Electric (GE) Centricity electronic medical record database. Menopause diagnosis was assessed using ICD-9 codes. Clinical characteristics and medication use were assessed for women with at least 3 years of enrollment (1 year baseline, 2 years follow-up). Results: The proportion of women in the post-WHI group that initiated HRT was significantly less than that of women in the pre-WHI group (31.3% vs. 56.9%, respectively;p < 0.001). Combination HRT use declined significantly (21.9% pre-WHI cohort vs. 7.2% post- WHI cohort, p < 0.001) among increases in non-HRT use, namely SSRIs (15.2% pre-WHI cohort v. 22.3% post-WHI cohort, p < 0.001) and tranquilizers (9.5% pre-WHI cohort v. 15.8% post-WHI cohort, p < 0.001). Conclusion: The results of the WHI 2002 publication made an impression on the perception of HRT’s role in the relief of menopausal symptoms. Decision-making on the part of women seeking treatment for vasomotor symptoms and women’s health professionals demonstrates that despite HRT precautions, women continue to exhibit a need for HRT use. This study’s findings suggest that women seeking treatment for menopausal symptom relief and women’s health professionals continue to work together to find the appropriate balance between therapy use and adherence to therapy use guidelines.

基于CiteSpace的突发性聋伴耳鸣文献计量学分析及思考

目的探讨突发性聋(突聋)伴耳鸣的发生率、检查方案选择及治疗,为临床建立完善的突聋新指南提供参考。方法运用CiteSpace软件分析及数据挖掘,对计算机检索出的2011—2021年中国知网、万方以及Web of Science数据库收录的有关突聋伴耳鸣相关辅助检查和治疗的文章进行分析总结。结果本研究共检索随机对照研究207篇,其中中文121篇,英文86篇;最终纳入中文文献74篇,英文文献16篇。74篇中文文献中,伴随症状为耳鸣的64篇(86.5%),头晕/眩晕58篇(78.4%),耳闷25篇(33.8%),头痛10篇(13.5%),失眠4篇(5.4%),头晕耳鸣混杂4篇(5.4%),呕吐2篇(2.7%);16篇英文文献中伴随症状为耳鸣的15篇(93.8%),眩晕12篇(75.0%),耳闷1篇(6.3%),各种症状混杂1篇(6.3%)。64篇提及耳鸣的中文文献中,仅9篇提到了耳鸣匹配检查;1篇提到耳鸣伴随症状的治疗为声治疗及心理辅导。4种不同类型突聋伴耳鸣发生率由低到高依次为:低中频82.4%,中高频90.7%,全聋型92.4%,平坦型92.8%。结论耳鸣是突聋最常见的伴随症状,耳鸣匹配检查是有效评估手段。在建立科学、完善、全面、系统的突聋诊疗体系或指南的同时应重视耳鸣症状及其不良心理反应的诊疗,减少突聋康复后期耳鸣的发生率。

The Role of Exportin-5 in MicroRNA Biogenesis and Cancer

MicroRNAs （miRNAs） are conserved small non-coding RNAs that play an important role in the regulation of gene expression and participate in a variety of biological processes. The biogenesis of miRNAs is tightly controlled at multiple steps, such as transcription of miRNA genes, processing by Drosha and Dicer, and transportation of precursor miRNAs （pre-miRNAs） from the nucleus to the cytoplasm by exportin-5 （XPO5）. Given the critical role of nuclear export of premiRNAs in miRNA biogenesis, any alterations of XPO5, resulting from either genetic mutation, epigenetic change, abnormal expression level or posttranslational modification, could affect miRNA expression and thus have profound effects on tumorigenesis. Importantly, XPO5 phosphorylation by ERK kinase and its cis/trans isomerization by the prolyl isomerase Pinl impair XPO5＇s nucleo-to-cytoplasmic transport ability of pre-miRNAs, leading to downregulation of mature miRNAs in hepatocellular carcinoma. In this review, we focus on how XPO5 transports pre-miRNAs in the cells and summarize the dysregnlation of XPO5 in human tumors.

云南金沙矿业磷矿为了延长尾矿库服务年限和治理采空区,亟需开展磷矿充填试验研究,为充填系统工艺设计提供基础数据。通过对矿山充填材料的现场取样测试,分析了磷矿尾砂的物化性质参数。根据充填料浆流动性能、泌水率及充填配比试验,综合确定了充填浓度、灰砂比等影响因素对充填体力学性能的影响规律。基于扫描电镜-能谱仪(SEM-EDS)微观测试手段,分析了充填体强度增长机制,水泥水化生成纤维状C—S—H凝胶与细尾砂颗粒搭接形成整体,并填充试件内部孔隙,是充填体强度增长的主要原因。研究成果可为确定国内类似磷矿矿山尾砂胶结充填提供参考。

Adsorption and desorption of Cu2＋ on paddy soil aggregates pretreated with different levels of phosphate

Interactions between anions and cations are important for understanding the behaviors of chemical pollutants and their potential risks in the *** we prepared soil aggregates of a yellow paddy soil from the Taihu Lake region,and investigated the effects of phosphate（P） pretreatment on adsorption-desorption of Cu2＋ of soil aggregates,free iron oxyhydrates-removed soil aggregates,goethite,and kaolinite with batch adsorption *** results showed that Cu2＋ adsorption was reduced on the aggregates pretreated with low concentrations of P,and promoted with high concentrations of P,showing a V-shaped *** with the untreated aggregates,the adsorption capacity of Cu2＋ was reduced when P application rates were lower than 260,220,130 and110 mg/kg for coarse,clay,silt and fine sand fractions,*** the contrary,the adsorption capacity of Cu2＋ was higher on P-pretreated soil aggregates than on the control ones when P application rates were greater than those ***,the desorption of Cu2＋ was enhanced at low levels of P,but suppressed at high levels of P,displaying an inverted V-shaped change over P *** Cu2＋ adsorption by the aggregate particles with and without P pretreatments was well described by the Freundlich *** results were obtained on P-pretreated ***,such P effects on Cu2＋adsorption-desorption were not observed on kaolinite and free iron oxyhydrates-removed soil *** present results indicate that goethite is one of the main soil substances responsible for the P-induced promotion and inhibition of Cu2＋ adsorption.

Effect of Yiqihuoxue prescription on myocardial energy metabolism after myocardial infarction via cross talk of liver kinase B1-depen-dent Notch1 and adenosine 5'-monophosphate-activated protein kinase

OBJECTIVE: To investigate the effect of Yiqihuoxue prescription(YQHX) from Traditional Chinese Medicine(TCM) on myocardial glucose and lipid metabolism after myocardial infarction via the cross talk between the liver kinase B1(LKB1)-dependent Notch1 and adenosine 5'-monophosphate(AMP)-activated protein kinase(AMPK). YQHX was prepared with substances with properties that benefit, to activate blood circulation based on the TCM ***: Animal models of myocardial infarction were established by ligating Sprague Dawley rats' left anterior descending coronary arteries. The animals were randomly divided into a myocardial infarction(MI) group, a YQHX group, a perindopril group, a γ-secretase inhibitor, Notch signal inhibitor(DAPT) group, a DAPT+YQHX group and a sham group. The related drugs were administered on the second day after operation, and changes in the relevant indexes were examined on weeks 1 and *** in cardiac structure and function were examined by echocardiography. The glucose and free fatty acids(FFA) were examined by ELISA. The expression of Notch, LKB1 and AMPK m RNA was examined by a real-time fluorescence quantitative method. The expression of glucose transporter 4(GLUT4), and the expression of total acetyl-Co A carboxylase(ACC) and its phosphorylation were examined by western ***: Compared with the sham group, the expression of Notch, LKB1 and AMPK m RNA in the MI group was lower. Compared with the MI group, the expression of these m RNAs in the YQHX and perindopril groups was higher, and their expression in the DAPT group was lower. At all time points, the protein expression of GLUT4 and p ACC decreased in the MI group. On week 1, the expression of p ACC protein was higher. In the DAPT group, the expression of p ACC protein decreased. Compared with the YQHX group, the expression of p ACC protein in the DAPT + YQHX group was lower.
