Mono-Therapy versus Poly-Therapy: Ten Years Indian Experience on Various Seizure Disorders

Objectives: To find out prescription patterns and seizure freedom with different mono-, duo- and poly-therapies used in various seizure disorders in Indian setting. Material and Methods: Subjects with seizures, not responding to first antiepileptic drug, were evaluated prospectively for a period of 6 months. Patients on various dual antiepileptic drugs (AEDs) prescriptions were prospectively analyzed for 1) the pattern and frequency of different duo-therapies;2) etiological profile of patients in duo-therapy prescription;and 3) frequency of seizure freedom on various duo-therapy groups. Results: Among 2542 patients, 293 (11.5%) lost in follow-up and thus, 2249 (88.5%) were followed. 1324 (58.9%) had seizure freedom on mono-therapy, 532 (23.7%) required duo-therapy and only 45 (2%) were better controlled on poly-therapy. Among the subjects, who were on mono-therapy, Carbamazepine/Oxcarbazepine was the most commonly prescribed in 1285 (50.55%) patients as first drug followed by Valproate compound and Phenytoin. The most common duo-therapy used was combination of Valproate with Lamotregine, followed by Phenytoin and Phenobarbitone. Other mono-therapy and combinations are given in this paper. Conclusions: Duo-therapy was required and found to be effective in 23.7% of Indian patients with epilepsy. Selection of appropriate two drug combination is based on individual approach and overall clinical profile of patient.

This paper proposes a method to design multichannel cosine modulated filter bank for image compression using multiobjective optimization technique. The design problem is a combination of stopband residual energy, least square error of the overall transfer function of the filter bank, coding gain with dc leakage free condition as constraint. The proposed algorithm uses Non-dominated Sorting Genetic Algorithm (NSGA) to minimize the mutually contradictory objective function by minimizing filter tap weights of prototype filter. The algorithm solves this problem by searching solutions that achieve the best compromise between the different objectives criteria. The performance of this algorithm is evaluated in terms of coding gain and peak signal to noise ratio (PSNR). Simulation results on different images are included to illustrate the effectiveness of the proposed algorithm for image compression application.

The foramen magnum (FM), being a vital landmark at the base of the skull, includes interests to many fields of medicine. Many authors haverecounted the usefulness of the FM in gender determination. The emphasis of this study is to evaluate the shape and dimensions of the FM among males and females and to establish its role in sexual dimorphism using cone-beam computed tomography (CBCT). Furthermore, to evaluate the area and FM index (FMI) among males and females and finally to evaluate the differences in FM measurements if any in twodifferent regions of an Indian population. One hundred and ten CBCT scans (55 males and 55 females;age range, 20–80 years) were selectedfor this study. The sagittal diameter, transverse diameter, area, FMI, and circumference of FM were measured, and data were subjected to discriminant analysis for the evaluation of sexual dimorphism. The area of FM was the best discriminant parameter which was used to study the sexual dimorphism with an overall accuracy of 90.9%. It can be concluded that the reconstructed CBCT image provides valuable measurements for the FM and could be used for gender determination.

Commonly transition metal nano particle are synthesized by physical, chemical or electrochemical methods. In the present work colloidal iridium nanoparticles were synthesized by chemical oxidation method with different surfactants like poly vinyl pyrrolidone (PVP), poly vinyl alcohol (PVA) and poly oxyethylene lauryl ether (POLE). It was found that shape and size of Ir-nano particles resulted were related to kind of capping agent (surfactant) used. The characterization of the synthesized nano particle has been carried out by UV-vis, X-ray diffraction (XRD), FT-IR, scanning electron microscopy (SEM) and transmission electron microscopic (TEM) techniques. UV-vis and FT-IR confirm the oxidation of IrCl3 into IrO2 while XRD confirms the amorphous nature of the iridium nanoparticles synthesized. The morphology and size of the particle were confirmed by TEM. The average particle size determined by Scherrer equation was about 4.12 nm to 4.23 nm with PVP, 2.74 to 3.36 nm with PVA and 20.41 to 42.25 nm with POLE. Poly oxyethylene lauryl ether particles were not further analyzed because of their large size and less stability. Further particle size was confirmed with TEM, which was 4.5 nm with PVP and 7.0 nm with PVA. The particles are spherical with no agglomeration tendency.

Does endoscopic sclerotherapy in filarial chyluria affect renal function and morphology? A prospective study using dimercaptosuccinic acid renal scan

Objective:To look for change in relative renal function and document renal scarring following endoscopic renal pelvic instillation sclerotherapy(RPIS)in patients with chyluria by dimercaptosuccinic acid(DMSA)renal ***:A prospective study was performed between November 2015 and September *** patients with biochemically documented chyluria who underwent RPIS using either 1%-silver nitrate or 0.1%-povidine iodine were *** received either 3-,6-or *** renal scan was performed before and 2e3 months after ***:Of the 34 patients,22 were *** age was 41.08±16.64 years(range,15-70 years).Thirty-two patients(94.1%)responded to therapy while two did not respond even after *** follow-up was 8.94±3.70 *** mean relative renal function(preinstillation)of normal kidney was 50.76%±3.55%while that of affected renal unit(side of instillation)was 49.20%±3.44%(range,43.0%-61.0%).After instillation therapy,the mean relative renal function of normal side was 52.26%±3.57%while that of affected renal unit was 47.50%±3.56%(range,41.0%-54.0%).The relative renal function did not change>5%from the baseline value in any patient except one(in which the differential function increased paradoxically by 12%).Two patients developed renal scar in post-instillation renal ***:Endoscopic sclerotherapy in chyluria is safe and *** relative renal function does not deteriorate by more than 5%.There is a small risk of development of renal *** studies involving larger number of patients are needed to answer this dilemma.

REBa_(2)Cu_(3)O_(7−δ)(REBCO,RE=rare earth)tapes doped with 5%and 15%Zr have been scaled up to lengths more than 40 m in a pilot‐scale advanced metal organic chemical vapor deposition(A‐MOCVD)*** precursor compositions used for the long tapes were guided by a study of the influence of(Ba+dopant)/Cu content on the critical current density(Jc)of 5 and 15 mol.%Hf‐and Zr‐added tapes at 4.2 K and 13 *** 40‐mlong tapes exhibited a critical current(Ic)over 4,000 A/12 mm at 4.2 K and 13 T as well as over 1,400 A/12 mm at 20 K and 20 *** critical current densities of a 40‐m‐long tape doped with 5%Zr at 4.2 K measured at the National High Magnetic Field Laboratory(NHMFL)were>10 MAcm^(−2)and>5 MAcm^(−2)at 14 T and 30 T,respectively,which are over three times those of commercial REBCO *** infield Jc of 5%Zradded 40‐m‐long tapes was similar to those of previously‐reported high‐performance short samples made with 15%Zr or *** results demonstrate the excellent potential of A‐MOCVD for manufacturing high Ic REBCO tapes for use in ultrahigh‐field magnet applications.

Objective:To assess efficacy of albendazole against Ascaris(A.)lumbricoides among school-children at three sites of Rajahmundry,Sundergarh and ***:Sites were selected based on soil-transmitted helminths prevalence,different sociocultural-geographical characteristics and operational feasibility.9-12 Years old children from 14 schools were *** study was conducted in two rounds:baseline&follow up *** eligible children found positive for *** at baseline were administered supervised single tablet albendazole 400 mg under the National Deworming Day *** post-treatment follow-up survey,eligible children who provided second stool sample were included in the final *** samples were examined using WHO recommended Kato-Katz *** was estimated based on cure rate(CR)and Fecal Egg Count Reduction Rate(FECRR)among children found positive in baseline and follow-up ***:328 Out of 625(52.5%)were found positive for *** in the baseline survey and 85 out of 178 samples(47.8%)during post-treatment follow-up ***,the estimated CR was 52.2%.FECRR(95%CI)for albendazole was 72.6%(79.2%-66.0%).The FECRR was highest in East Godaveri[89.7%(96.8%-82.6%)],followed by Sundergarh[86.4%(95.3%-77.5%)]and Wayanad[69%(81.7%-56.4%)].Conclusions:Our study confirmed overall reduction in FEC although with varying albendazole efficacy for *** infection at three different locations in ***,these finding may be assessed in context of ongoing MDA under Filaria control program and further studies by in vivo as well as in vitro methods are required to reach to a conclusion on possible resistance if any.

Beaconless Position-based Routing with Guaranteed Delivery for Wireless Ad hoc and Sensor Networks

Existing position-based routing algorithms, where packets are forwarded in the geographic direction of the destination, normally require that the forwarding node should know the positions of all neighbors in its transmission range. This information on direct neighbors is gained by observing beacon messages that each node sends out periodically. Several beaconless greedy routing schemes have been proposed recently. However, none of the existing beaconless schemes guarantee the delivery of packets. Moreover, they incur communication overhead by sending excessive control messages or by broadcasting data packets. In this paper, we describe how existing localized position based routing schemes that guarantee delivery can be made beaconless, while preserving the same routes. In our guaranteed delivery beaconless routing scheme, the next hop is selected through the use of control RTS/CTS messages and biased timeouts. In greedy mode, the neighbor closest to destination responds first. In recovery mode, nodes closer to the source will select shorter timeouts, so that other neighbors, overhearing CTS packets, can eliminate their own CTS packets if they realize that their link to the source is not part of Gabriel graph. Nodes also cancel their packets after receiving data message sent by source to the selected neighbor. We analyze the behavior of our scheme on our simulation environment assuming ideal MAC, following GOAFR+ and GFG routing schemes. Our results demonstrate low communication overhead in addition to guaranteed delivery.

Silica nanoparticles have been one of the most promising nanosystems for biomedical applications due to their facile surface chemistry and non-toxic nature. However, it is still challenging to effectively deliver them into tumor sites and noninvasively visualize their in vivo biodistribution with excellent sensitivity and accuracy for effective cancer diagnosis. In this study, we design a yolk/shell-structured silica nanosystem ^(64) Cu-NOTAQD@HMSN-PEG-TRC105, which can be employed for tumor vasculature targeting and dual-modality PET/optical imaging, leading to superior targeting specificity, excellentimaging capability and more reliable diagnostic *** combining vasculature targeting, pH-sensitive drug delivery, and dual-modality imaging into a single platform,as-designed yolk/shell-structured silica nanosystems may be employed for the future image-guided tumor-targeted drug delivery, to further enable cancer theranostics.
