Correlation of neutrophil to lymphocyte ratio to severity of coronary artery disease and in-hospital clinical outcomes in patients with acute coronary syndrome: A prospective observational study

Objective:To explore correlation of neutrophil-to-lymphocyte ratio(NLR)to severity of coronary artery disease(CAD)and in-hospital clinical outcomes in patients with acute coronary syndrome(ACS).Methods:In this prospective and observational study,we recruited 500 patients with ACS.For all the eligible patients,demographic details were collected,and laboratory parameters were evaluated.The CAD severity was evaluated in terms of the number of involved vessels.The NLR was calculated based on neutrophils and lymphocytes and the correlation of various risk factors and severity and outcome of CAD was performed.Results:77.2%of Patients was male,and 52%of the patients aged between 55-70 years.Based on the type of ACS,396 out of 500 patients had ST-elevation myocardial infarction.An ascending trend in the white blood cell levels and NLR value was noted as the severity of the ACS increased and the highest white blood cell levels and NLR was noted among classⅣpatients.The mean NLR value among the non-survivors were higher compared to the survivors(9.52±5.72 vs.4.76±2.36;P<0.01).Receiver operating curve showed that the cut-off NLR value was 5.76 with a sensitivity of 75.0%and a specificity of 77.3%.Conclusions:The NLR can be used as an independent prognostic marker in ACS.An elevated NLR value serves as a reliable predictor for short-term complications,notably in-hospital mortality.

The perforated stiffened panel is generally found as a sub-component of sophisticated structures.The fundamental purpose of this panel is to withstand against buckling under complicated loading and environmental conditions.Hence,an accurate knowledge of critical buckling behaviour of stiffened panels is very much essential for a reliable and lightweight structural design.In this paper,the focus is on quasi-laminated panels with different cutout shapes of various sizes and their responses to hygrothermal environments under nonlinearly varying edge loads and is compared with the locally stiffened panels.Towards this,the modelling of the panel and stiffener is done by adopting nine-noded heterosis plate elements and three noded beam elements respectively.The stiffener formulation is suitably modified in order to take the torsional effect also into consideration along with the effect of shear deformation.Initially,the plate and the stiffener elements are treated separately,and then the displacement compatibility is maintained between them by using the transformation matrix.For a given loading and geometric discontinuity,the stress distribution within the perforated panel is highly non-uniform in nature and hence a dynamic approach has been used to calculate buckling loads by adopting two sets of boundary conditions,one set for pre-buckling stress analysis and the second set for buckling analysis.Four different quasi-isotropic stacking sequences are deliberated in this work by varying different ply-orientation in each scheme.The study also addresses the effect of various parameters such as nonlinear loads,hygro-thermal loads,cutout size and shapes,position of cutout,stiffener parameters,stacking sequences,thickness of plate and boundary conditions.

Uterine fibroids are common in females of reproductive age and substantially affecting fertility and quality of life. Current management strategies mainly involve surgical interventions. For treatment, options available are surgical and non-surgical, but the mode of management leans on several factors, such as severity of symptoms, patient’s age, myoma characteristics, desire to preserve uterus and fertility. Alternative approach to surgery for the treatment of symptomatic females with uterine myomas has been recognized. Ulipristal acetate (UPA) has been the first selective progesterone-receptor modulator (SPRM) approved for the pre-operative and long-term management of uterine fibroids. There are evidences promoting an important role for progesterone pathways in the pathophysiology of uterine fibroids which supports the use of ulipristal acetate. The availability of alternative choices to surgical intervention is very necessary especially for those willing to preserve uterus and fertility. One of the alternatives is with ulipristal acetate, which has been proven to treat fibroid symptoms effectively.

Comparative evaluation of effect of injectable platelet-rich fibrin with collagen membrane compared with collagen membrane alone for gingival recession coverage

BACKGROUND Collagen membrane and platelet-rich fibrin(PRF)have emerged as vital biomaterials in the field of periodontal regeneration.Minimally invasive techniques are being preferred by most periodontists,as it is patient compliant with fewer post-surgical complications as compared to conventional surgical techniques.Thus,in this study we have evaluated the effect of injectable PRF(i-PRF)with collagen membrane compared with collagen membrane alone using vestibular incision subperiosteal tunnel access(VISTA)technique for gingival recession coverage.AIM To compare the efficacy of VISTA using collagen membrane with collagen membrane soaked in injectable PRF for gingival recession coverage.METHODS A split mouth randomized controlled clinical trial was designed;13 subjects having at least 2 teeth indicated for recession coverage were enrolled in this study.The sites were randomly assigned to control group(VISTA using collagen membrane alone)and the test group(VISTA using collagen membrane with i-PRF).The clinical parameters assessed were pocket depth,recession depth(RD),recession width(RW),relative attachment level,keratinised tissue width(KTW),keratinised tissue thickness(KTT),and percentage root coverage.RESULTS RD showed a statistically significant difference between the test group at 3 mo(0.5±0.513)and 6 mo(0.9±0.641)and the control group at 3 mo(0.95±0.51)and 6 mo(1.5±0.571),with P values of 0.008 and 0.04,respectively.RW also showed a statistically significant difference between the test group at 3 mo(1±1.026)and 6 mo(1.65±1.04)and the control group at 3 mo(1.85±0.875)and 6 mo(2.25±0.759),with P values of 0.008 and 0.001,respectively.Results for KTW showed statistically significant results between the test group at 1 mo(2.85±0.489),3 mo(3.5±0.513),and 6 mo(3.4±0.598)and the control group at 1 mo(2.45±0.605),3 mo(2.9±0.447),and 6 mo(2.75±0.444),with P values of 0.04,0.004,and 0.003,respectively.Results for KTT also showed statistically significant results between test group at 1 mo(2.69±0.233),3 mo(2.53±0.212),a

Microorganisms including bacteria,viruses,protozoa,and fungi living in the gastrointestinal tract are collectively known as the gut microbiota.Dysbiosis is the imbalance in microbial composition on or inside the body relative to healthy state.Altered Firmicutes to Bacteroidetes ratio and decreased abundance of Akkermansia muciniphila are the predominant gut dysbiosis associated with the pathogenesis of type 2 diabetes mellitus(T2DM)and metabolic syndrome.Pathophysiological mechanisms linking gut dysbiosis,and metabolic diseases and their complications include altered metabolism of short-chain fatty acids and bile acids,interaction with gut hormones,increased gut microbial metabolite trimethylamine-N-oxide,bacterial translocation/Leaky gut syndrome,and endotoxin production such as lipopolysaccharides.The association between the gut microbiota and glycemic agents,however,is much less understood and is the growing focus of research and conversation.Recent studies suggest that the gut microbiota and anti-diabetic medications are interdependent on each other,meaning that while anti-diabetic medications alter the gut microbiota,the gut microbiota also alters the efficacy of anti-diabetic medications.With increasing evidence regarding the significance of gut microbiota,it is imperative to review the role of gut microbiota in the pathogenesis of T2DM.This review also discusses the interaction between gut microbiota and the various medications used in the treatment of T2DM.

This article introduced the work of ethylcellulose based polymeric microsphere loaded with nifedipine for reduction in frequency of administration with low solubility in aqueous medium and high rate of absorption in the stomach. The non-aqueous polymeric suspension was put dropwise into an aqueous medium containing polyvinyl alcohol as a surfactant for the synthesis of microsphere by solvent evaporation. The microspheres were characterized by different techniques, namely, XRD, SEM, and NMR. The formation of microspheres was confirmed by SEM. XRD analysis revealed the semi-crystallinity nature of microspheres. The NMR study indicated the presence of hetero-aromatic nucleus in the microsphere.

Delftia acidovorans(earlier known as Comamonas acidovorans) is an aerobic,non-fermentative, Cram negative rod,classified in the Pseudomonas rRNA homology Group 111.Reports of isolation of the organism from serious infections like central venoug catheter associated bacleremia. corneal ulcers,otitis media exist.The microbiologists can identify this organism based on an orange indole reaction.This reaction demonstrates the organism's ability to produce anthranilic acid from tryptophan on addition of Kovac's reagent;which gives the media its characteristic "pumpkin orange" colour.Here we report the isolation of this organism from the Endotracheal tube aspirate of a 4 year old child.With the increasing use of invasive devices,it has become important to recognize these non fermentative gram negative bacilli as emerging source of infection even in immunocompetent individuals.

Sliding Wear Performance Evaluation of Red Mud (RM), RM + Fly Ash (FA) and RM + FA + Al Coatings on Mild Steel

The research paper focuses on evaluating the dry sliding wear behaviour of plasma sprayed coatings like red mud (RM), RM + 5% Fly Ash (FA) and RM + 5% FA + 5% Al on mild steel cylindrical shape substrate. Spraying was done at 10 kW operating power level. A conventional pin on disc wear tester is adopted for wear analysis. The wear test was performed at track diameter of 50 mm and sliding speed of 60 rpm (0.157 m/s) with normal load of 10 N. The duration of sliding varies from 54 minutes for pure red mud coating to a maximum value of 102 minutes for RM + 5% FA + 5% Al composite coating. A significant decrease in wear rate (in terms of mass loss) is observed by reinforcement of fly ash and aluminium to red mud, which might be due to improved interfacial properties.
