Dendritic cells(DCs)are the master regulators of the immune response and engage in constant cross-talk with other immune cells to initiate an appropriate immune *** outcome of cross-talk with various immune cells towards DC functions has been shown to be dependent on the particular subset of cells and their activation *** we show that for human B cells to induce maturation of DCs with an enhanced T-cell stimulatory capacity,B-cell receptor(BCR)signaling is sufficient for memory B-cells,whereas additional CD40 signaling is required for naïve B cells to induce complete activation of DCs.

Basophils are rare *** representing only~0.5%of all leukocytes,basophils have several important physiological functions.1,2 Although basophils lack the classic features of professional antigen-presenting cells,3–7 through the secretion of cytokines,they orient the immune response by polarizing Th2 differentiation and supporting B-cell differentiation and class *** are also critical for mediating protection against helminth infection.1,2,8,9 Basophils receive activation signals from diverse *** is well recognized that cytokines such as IL-3,granulocyte–macrophage colony-stimulating factor(GM-CSF),thymic stromal lymphopoietin(TSLP)and IL-33;various toll-like receptor ligands;allergen-bound IgE provide activation signals to basophils and induce the release of inflammatory mediators.10–15 In addition,several reports have also demonstrated the existence of anti-IgE autoantibodies that possess the capacity to induce basophil activation in patients with chronic spontaneous urticaria(CSU),atopic or non-atopic asthma or autoimmune disease.16–21 However,isolation and functional exploration of such anti-IgE IgG autoantibodies from either healthy donors or patients have not been attempted *** using a pooled normal IgG preparation from healthy donors,specifically intravenous immunoglobulin G(IvIG)22 that represents the complete IgG repertoire of a normal individual,we attempted to address this outstanding question in the field.

CD4＋FoxP3＋ regulatory T cells （Tregs） are immunosuppressive cells that are critical for immune tolerance. Several studies have demonstrated that one of the anti-inflammatory mechanisms of action of intravenous immunoglobulin （Wig） involves the expansion of Tregs. Recently, we demonstrated that IvIgmediated Treg expansion involves the cyclooxygenase-2 （COX-2）-dependent induction of prostaglandin E2 （PGE2） in human dendritic cells （DCs）. However, the validity of these findings in autoimmune patients is lacking.

AIM: To determine real life clinical outcomes in poorly responsive and treatment-nave neovascular age-related macular degeneration(nv AMD) patients using bimonthly fixed dosing aflibercept ***: This was a retrospective study of 165 eyes with nv AMD started on aflibercept at Southampton Eye Unit between June 2013 and June 2014. Patients were either switched from pro re nata(PRN) ranibizumab/bevacizumab due to poor response(107 eyes), or treatment- nave( 58 eyes). Patients initially received 3-monthly intravitreal aflibercept injections followed by 2-monthly fixed doses. Clinic visits were scheduled at month 0, 4, 10 and 12. Mean change in best-corrected visual acuity(BCvA) and central retinal thickness(CRT)from baseline were assessed using the Wilcoxon signedrank test. The proportion of patients maintaining BCvA(vA change at month 12 was +3.29 and +4.67 letters in the switched and nave aflibercept groups respectively(P vA was maintained in 95.3% of switched and 96.6% of nave patients. CRT at month 12 showed a decrease of -6.16 μm in the switched group and -35.36 μm in the nave group(P viously treated with ranibizumab/bevacizumab had on average received 7.4 ranibizumab/bevacizumab injections over 12.6 mo, attending 10 clinic visits. The fixed dosing aflibercept regimen required an average of 7.1 injections(nave group), 7.5 injections(switched group) and 4 clinic visits per ***: Fixed bimonthly aflibercept is effective in both treatment-nave and poorly responsive nv AMD patients. Adopting a fixed dosing regimen can reduce patient burden without compromising on outcomes.

Patterns of medication use for the treatment of menopausal symptoms before and after the women’s health initiative;implications for decision-making practices of women and women’s health professionals

Background: The Women’s Health Initiative (WHI) published findings in 2002 that changed the perception of the use of hormone replacement therapy (HRT) for the reduction of cardiovascular risks. Menopausal women using HRT for the relief of vasomotor symptoms were advised to use the lowest possible dose of HRT over the shortest possible duration. Objective: This study sought to examine patterns of HRT use for the treatment of menopausal symptoms before and after the WHI among women at least 40 years of age. Methods: A retrospective analysis was performed on a total of 1367 women in the pre-WHI group and 6467 women in the post-WHI group using the U.S. General Electric (GE) Centricity electronic medical record database. Menopause diagnosis was assessed using ICD-9 codes. Clinical characteristics and medication use were assessed for women with at least 3 years of enrollment (1 year baseline, 2 years follow-up). Results: The proportion of women in the post-WHI group that initiated HRT was significantly less than that of women in the pre-WHI group (31.3% vs. 56.9%, respectively;p vs. 7.2% post- WHI cohort, p v. 22.3% post-WHI cohort, p v. 15.8% post-WHI cohort, p vasomotor symptoms and women’s health professionals demonstrates that despite HRT precautions, women continue to exhibit a need for HRT use. This study’s findings suggest that women seeking treatment for menopausal symptom relief and women’s health professionals continue to work together to find the appropriate balance between therapy use and adherence to therapy use guidelines.
