The Effectiveness of Diamel in the Treatment of Type 2 Diabetic Patients Receiving Insulin Therapy: A Randomized, Controlled Clinical Trial

Aims/Introduction: Effectiveness of Diamel? on the biochemical variables: fasting blood glucose, postprandial glucose, cholesterol, triglycerides, and glycosylated haemoglobin [HbA1c] in type-2 diabetic patients receiving insulin terapy. Diamel is a natural product composed of trace elements: amino acids, vitamins, cranberry extract, and lettuce extract that have been activated by means of a magnetization process. Materials and methods: Randomized, doubleblind, comparator-controlled clinical trial. A central randomization centre used computer generated tables to allocate treatments. 116 patients suffering from type-2 diabetes mellitus treated with insulin at the Diabetes Care Centre in Pinar del Rio (Cuba), were recruited and randomly. The subjects were separated into two groups: Group A (n = 59), who were administered Diamel and Group B (n = 57) using placebo. The clinical and biochemical variables were assessed for 24 weeks. Results: Two subjects were excluded from the Diamel treatment group during the clinical trial. From the statistical point of view, a significant drop in the levels of the baseline glucose, postprandial glucose, glycated haemoglobin, and triglycerides was observed in the group taking Diamel (Group A). There was observed an increase in insulin requirements and in waist circumference in the subjects of Group B There was not reported any severe or serious adverse reactions during this clinical trial. Conclusion: Diamel (nutritional supplement) together with the administration of insulin in type-2 diabetic patients is useful to optimize the biochemical variables (baseline and postprandial glucose, Hb A1C, plus triglycerides), as well as to prevent the increase of insulin requirements at medium-term.

Morphometrics confirms the conspecific between Blaptica dubia(Serville) and B. interior Hebard(Blattodea:Blaberidae)

Blaptica dubia(Serville, 1838) and B. interior Hebard, 1921 were considered to be cryptic species. The characteristics originally proposed for their identification are not *** the relationship between the two species should be re-considered. As a comparative approach and to analyze intraspecific variability, morphological and morphometric methods were performed. Traditional and new characters such as wing shape and pronotum contour were utilized. We found significant differences between both species(p <0.05) in the shape of the wing, but there was an overlap of values. We did not find differences in the body measurements or in the shape and size of the pronotum between the species. The variability gradient does not allow an accurate identification of species neither with the male genitalia or the pronotal *** on the obtained results, B. interior Hebard 1921 is considered to be a junior synonym of B. dubia(Serville, 1838).

Importance of the Pharmaceutical Council in the Dispensation of Laxants and Probiotics

The goal of pharmaceutical care is to improve the patient’s quality of life, and it must do so by maximising its effectiveness and reducing the risks of the treatment. Therapeutic protocols are an important tool for encouraging the correct use of drugs. The aims of this work are： to analyse the type of patients who visit the pharmacies in the study; to determine the importance of pharmaceutical advice in dispensing; and to identify the most commonly used strains of probiotics and laxatives. A transversal descriptive and observational study was conducted with 500 patients in five pharmacies. The population taking part in the study is predominantly female, aged over 61, of Spanish nationality, actively employed and with middle or higher educational studies. Over 90% of the patients followed the advice of the pharmacist over the advice of their doctors. The most widely used probiotics were Lactobacillus acidophillus and Saccharomyces boulardii for gastrointestinal protection and treatment of diarrhoea. The most commonly used laxatives are bulk-forming and contact laxatives. The pharmacist’s opinion is increasingly important in the treatment of pathologies that may be considered as minor. Therapeutic protocols are an aid to correct dispensing.

Tribological behavior of ammonium-based protic ionic liquid as lubricant additive

In this study, the tribological behavior of an ammonium-based protic ionic liquid(PIL) as an additive in a base mineral oil(MO) is investigated on a steel–steel contact at room temperature and 100℃. Tri-[bis(2-hydroxyethylammonium)] citrate(DCi) was synthesized in a simple and low-cost way, and the ionic structure of DCi was confirmed by proton nuclear magnetic resonance(^(1)H NMR). The stability measurement of 1 wt% DCi to a MO was investigated, and the lubricating ability and anti-wear properties of DCi as an additive in MO were also examined using a custom-designed reciprocating ball-on-flat tribometer. Optical microscope and profilometry were used to obtain the worn morphology of the steel disks. Scanning electron microscopy(SEM) and energy dispersive X-ray spectroscopy(EDS) were carried out to investigate the wear mechanism and to analyze the surface interactions between the rubbing components. When 1 wt% DCi is added into the base MO, frictional performance is improved at both temperatures studied with a friction reduction of 29.0% and 35.5%, respectively. Moreover, the addition of 1 wt% DCi to MO reduced the wear volume 59.4% compared to the use of MO. An oxygen-richened tribolayer is confirmed by EDS on the disk surface when DCi was used as additive under 100℃.

Loss of follow-up or reinfections hinder the expectations of hepatitis C eradicationdespite the existence of highly effective treatments. Moreover, the elimination ofthe infection does not imply the reversion of those chronic alterations derivedfrom the previous infection by hepatitis C virus (HCV). This review analyzes therisk factors associated with loss to follow-up in diagnosis or treatment, and thepossibility of reinfection. Likewise, it assesses the residual alterations induced bychronic HCV infection considering the liver alterations (inflammation, fibrosis,risk of decompensation, hepatocellular carcinoma, liver transplantation) and, onthe other hand, the comorbidities and extrahepatic manifestations (cryoglobulinemia,non-Hodgkin lymphoma, peripheral insulin resistance, and lipid, boneand cognitive alterations). Peculiarities present in subjects coinfected with humanimmunodeficiency virus are analyzed in each section.

“Bad regenerators” die after spinal cord injury:insights from lampreys

In mammalian species, including humans, spinal cord in- jury （SCI） leads to permanent disability. A major cause of disability after SCI is the failure of axotomized descending axons to regenerate across the trauma zone and to reconnect to they appropriate targets distal to the site of injury. Cur- renfly, major research efforts are devoted to find new ways of promoting the regrowth of damaged descending axons. However, activation of axonal regrowth will depend on the survival of the axotomized descending brain neurons.

Biliary stone causing afferent loop syndrome and pancreatitis

We report the case of an 84-year-old female who had a partial gastrectomy with Billroth-Ⅱ anastomosis 24 years ago for a benign peptic ulcer who now presented an acute pancreatitis secondary to an afferent loop syndrome. The syndrome was caused by a gallstone that migrated through a cholecystoenteric fistula. This is the first description in the literature of a biliary stone causing afferent loop syndrome.
