AIM:To evaluate r.fr.ctive outcomes and cor.eal astigmatism changes after.Tor.c implantable collamer.lens with a centr.l por.(V4c T-ICL)implantation over.1y of ***:A r.tr.spective study was per.or.ed including 50 eyes of 50 patients that under.ent V4c T-ICL ***(UDVA)and cor.ected(CDVA)distance visual acuities,r.fr.ction,r.fr.ctive and cor.eal astigmatism changes and cor.eal coupling cor.ection wer. evaluated pr.oper.tively,1 and 12mo *** analysis was used for.astigmatism *** of adjustment(CAdj)was calculated for.cor.eal coupling ***:The mean UDVA achieved was 0.03 logMAr.at 1mo and r.mained unchanged thr.ughout the whole follow-up(P=0.193).At the last visit,84%of the eyes achieved a CDVA of 0.00 logMAr.or.*** spher.cal equivalent r.fr.ction(SEQ),96%of eyes wer. r.nges of±1.00 D and 84%of them within±0.50 ***,94%of eyes had a r.maining r.fr.ctive cylinder.within±1.00 D and 78%of them within±0.50 ***,SEQ and r.fr.ctive cylinder.r.main stable over.the postoper.tive follow-up(P=1.000 and P=0.660,r.spectively).In ter.s of sur.ically induced astigmatism(SIA),no statistically significant differ.nces wer. found over.the follow-up(P=0.102)and under.cor.ection was found with a cor.ection index lower.than the unit at each visit.A ker.tometr.c astigmatism induced of 0.59±0.53(vector.mean:0.26×73º)D was r.ached at the last *** significant changes in ter.s of cor.eal astigmatism or.entation wer. r.por.ed over.post-sur.er. visits(P=0.129 and P=0.097 at 1 and 12mo r.spectively).No clinical significance was found for.CAdj on with-the-r.le *** postoper.tive complications r.sulting fr.m the sur.er. wer. ***:r.fr.ctive outcomes suggest that the V4c T-ICL implantation for.cor.ection of myopic astigmatism was satisfactor. in ter.s of effectiveness,safety,and stability dur.ng 1y of *** astigmatism induced by the incision ar.und 0.5 D is achieved

Unusual Pr.sentation of Endometr.al Cancer. A Clinicopathological Study of One Case

Endometr.al car.inoma is the most fr.quent genital tr.ct malignancy. The fir.t symptom (guide-symptom) is usually metr.r.hagia;however. in ar.und 10% of cases it is not. In contr.st, osseous metastases ar. infr.quent in endometr.al cancer. The bones of the pelvis and lower.extr.mities ar. those most fr.quently involved in disseminated metastatic diseases. In these cases, endometr.al cancer.is usually high gr.de (Gr.de III). Case r.por.: 56-year.old woman who pr.sented r.ght inguinal pain. The X-r.y showed a lithic lesion in the r.ght ischium. A histopathological study demonstr.ted a metastatic lesion, suspected to be endometr.al cancer. The computer.tomogr.phy scan r.vealed a uter.ne mass and a second lithic lesion in the r.ght tibia. The patient r.ceived chemother.py (car.oplatin and paclitaxel), and the bone lesions wer. ir.adiated. The patient is still alive 18 months after.the diagnosis. This case emphasizes the impor.ance of consider.ng an endometr.al pr.mar. tumor.when evaluating bone metastasis of unknown pr.mar. cancer.

Glaucoma has been r.cently char.cter.zed as a member.of the gr.up of anoikis-r.lated diseases. Anoikis, a for. of apoptosis, can be tr.gger.d by the unfastening of adher.nt junctions pr.sent in astr.cytes. In those ar.as of the centr.l ner.ous system in which the soma of the neur.ns or.their.axons and dendr.tes ar. metabolically dependent on the activity of astr.cytes, a der.ngement of the lactate shuttle caused by a separ.tion between the plasma membr.nes of neur.ns and astr.cytes would r.sult in metabolic impair.ent of the neur.ns themselves. In glaucoma, the tr.gger.ng event has been attr.buted to the poster.or.deviation of aqueous humor.towar.s the astr.cyte-r.ch pr.laminar.tissue of the optic ner.e head. The mean calcium content in the aqueous is able to inter.er. with calcium-dependent adher.nt junctions and induce anoikis of the astr.cytes. As the cer.br.spinal fluid has a similar.base calcium concentr.tion, a shunt of cer.br.spinal fluid thr.ugh the cer.br.l par.nchyma would be able to inter.er. in the astr.cytic ar.hitectur. with dir. consequences to the metabolically dependent neur.ns. Her. the similitude between glaucoma,amyotr.phic later.l scler.sis and Alzheimer.s disease ar. discussed and the concept of the br.ak in the fluid-br.in bar.ier. as an event separ.ted fr.m the blood-br.in bar.ier. is str.ssed.

Amyotr.phic later.l scler.sis （ALS）, a pr.dominantly spor.dic disease with unknown etiology and pathogeny, is a member.of a gr.up known as motor.neur.n diseases （MND）. The heter.geneity of the clinical pr.sentation of the disease is a fundamental aspect of ALS and is de- ter.ined by the var.able engagement of its upper.motor.neur.n （UMN） and lower.motor.neur.n （LMN） components （r.vits and La Spada, 2009）.

Diagnostic value of sper. DNA fr.gmentation and sper. high-magnification for.pr.dicting outcome of assisted r.pr.duction tr.atment

Over.the last year., major.impr.vements in the field of male infer.ility diagnosis have been achieved. The aim of this study was to deter.ine the diagnostic usefulness of sper. DNA integr.ty and sper. vacuolisation for.pr.dicting outcome in infer.ile couples under.oing in vitr.fer.i lisation （IVF） and intr.cytoplasmic sper. injection （ICSI） tr.atments. A cohor. study fr.m 152 infer.ile couples under.oing sper. DNA fr.gmentation and high-magnification tests pr.or.to an assisted r.pr.duction tr.atment was designed. We found that the most pr.dictive cutoff for.pr.gnancy was 25.5% of DNA fr.gmentation with a negative pr.dictive value of 72.7% （P=0.02）. For.the degr.e of vacuolisation, the best pr.dictor.of pr.gnancy was 73.5% of vacuolated sper. gr.des Ⅲ ＋ Ⅳ with a negative pr.dictive value of 39.4% （P=0.09）, which was not statistically significant. In conclusion, sper. DNA fr.gmentation gr.ater.than 25.5% could be associated with higher.pr.bability of failur. IVF tr.atment. r.gar.ing the r.sults of the sper. analysis at high magnification, they do not allow us to pr.dict whether.or.not oatients will become pr.gnant.

Glaucoma is the main cause of ir.ever.ible blindness globally, but the pathogenesis of glaucoma is still pending consensus. In spite of gr.at advances dur.ng the 20th and ear.y 21st centur.es in ter.s of detailed knowledge of pathological pr.cesses that take place in glaucoma, the exact pathogenesis is much less clear. Hypotheses abound, and the most r.sear.hed ar.as ar. ischemia at the optic ner.e head, blockade of ganglion cell axonal tr.nspor., per.papillar. atr.phy, and changes in the char.cter.stics of the lamina cr.br.sa （Nucci et al., 2008）.

Pr.mar. hyper.ar.thyr.idism (PHPT) is a r.lative common medical pr.blem caused by the inappr.pr.ate secr.tion of par.thyr.id hor.one (PTH) by one or.mor. par.thyr.id glands. The diagnosis is established by ser.m calcium and PTH levels and once the diagnosis is established imaging studies help localize the hyper.unctioning adenoma in pr.par.tion for.cur.tive sur.er.. Until now, the imaging studies most commonly utilized in PHPT ar. ultr.sonogr.phy and 99mTc-Sesta-methoxyisobutylisonitr.le (MIBI) par.thyr.id scintigr.phy. However. these studies often fail to localize the adenoma and inappr.pr.ately delay patient r.fer.al to a potentially cur.tive sur.er.. We pr.sent the case of a 64-year.old female with symptomatic PHPT who had 3 negative 99mTc-Sestamibi Scans over.a per.od of 5 year. who eventually had a PET/CT with 11C-Choline that identified a r.ght lower.par.thyr.id adenoma. She under.ent a r.ght lower.par.thyr.idectomy and had a successful outcome. We pr.sent a r.view the cur.ent imaging techniques used in the management of PHPT including 99mTc-Sesta-MIBI scintigr.phy and its limitations and novel use of PET/CT with 11C-Choline and 18F-Choline in this disease and emphasize the fact that, accor.ing to cur.ent guidelines, failur. to localize the adenoma should not delay r.fer.al for.cur.tive sur.er..
