Introduction: The efficacy of chemotherapy in bacteraemia caused by non-typhoidal Salmonella (NTS) is compromised by antibiotic resistance. Objective: This study was undertaken to describe the mechanism of resistance among clinical NTS isolates. Materials & Methodology: Thirty of NTS were isolated from blood (n = 19), stool (n = 10) and bronchioalveolar lavage (BAL;n = 1) respectively. These isolates were tested for susceptibility testing by disc diffusion method against ampicillin, gentamicin, tetracycline, co-trimoxazole, nalidixic acid, ciprofloxacin and ceftriaxone. Epsilometer tests (E-test) for nalidixic acid and ciprofloxacin were performed for nalidixic acid resistant isolates by disc diffusion method. DNA sequencing was carried out on six of the nalidixic acid resistant Salmonella Enteritidis isolates to identify mutations within quinolones resistance determining regions (QRDR) of gyrA, gyrB, parC and parE genes. Results: Resistance rates of NTS isolates from blood, stool, and BAL were respectively 37%, 20% and 0% for ampicillin, 79%, 40% and 0% for tetracycline, 32%, 40% and 0% for co-trimoxazole, 37%, 10% and 100% for nalidixic acid. Eight isolates were resistant to nalidixic acid and had exhibited reduced susceptibility towards ciprofloxacin by E-test. Mutation within QRDR was detected in gyrA gene (n = 6;Asp 47 → His [3], Asp 51 → Asn [1], Asp 73 → Gly [1], and Gly 48 → Asp [1]) and double mutation was detected in parE gene (n = 3;Gly 48 → Asp [3], Glu 82 → Ser [3]). Out of six isolates, three isolates were found to have both gyrA and parE gene mutations. Conclusions: There was no mutation observed in gyrB and parC gene. Mutation in gyrA gene was sufficient to induce decreased susceptibility to ciprofloxacin. Variation in amino acid sequences are novel, while detection of other gene mutation was uncommon.

Arthroscopic Assessment of Intra-Articular Injury in Patients with Acute Unstable Malleolar Fracture

The application of crude protease of Aspergillus oryzae to suppress the hatchability of Pomacea canaliculata eggs is highlighted. The enzyme was produced through solid state fermentation(SSF) using cassava, which is cheap and abundantly available. It was found that the enzyme suppressed the hatchability of P. canaliculata eggs as much as 30% and 20% at 20.8U/ml enzyme for 3 and 9 days old eggs respectively. The ANOVA result revealed that the hatchability was significantly affected by both protease concentration as well as egg’s age. The LC for 3 and 9 days old eggs were 3.29 U/ml and 3.54 U/ml, respectively;while LC for 3 and 9 days old eggs were 19.11 U/ml and 20.09 U/ml respectively. It is proposed that the crude protease propagated from SSF using locally available and cheap material should be considered as an alternative biopesticide to suppress the hatchability of P. canaliculata eggs.

Introduction: Purpose to study prevalence of the intraarticular chondral lesion in the malleolar fracture by using ankle arthroscopy to fully understand the severity and complexity of the injury. Methods: Cross sectional study of 32 patients diagnosed with ankle fracture and undergone open reduction and internal fixation with arthroscopic assessment performed stimultaneously. The mechanism of injury, patterns of injury and intraarticular chondral injury were documented. Results: Mean age was 38 years (SD = 14.1, range 18 - 68 years). Eighteen were female and 14 were male. Fifteen involved syndesmostic distruption, 22 had Danis-Weber B injury and 16 had supination external rotation (SER). Ten (31.2%) had positive intraoperative cartilage injury. Significant correlation between the Lauge-Hansen classifications with positive findings with 6 had SER, 2 had pronation adduction and 2 had pronation external rotation. Conclusion: The prevalence of chondral injury in ankle fracture was quite high and may leads to poor outcome. Arthroscopy procedure allow surgeon to assess intraarticular surface and reduction of the ankle fracture which prompt further intervention that may improve the clinical outcomes and prognosis of the patients.

Morphine is a schedule II-controlled substance that used to allow the diminution of intra-operative, post-operative orchronic pain. However, its usage is limited due to addiction and overdose liabilities. Morphine was observed to cause tolerance,dependence and withdrawal in human. Justification: to date lack of scientific evidence of morphine addiction was carried out byusing specific single human neuroblastoma cell-line (SK-N-SH). Therefore, this study was performed to establish the morphineaddiction model in this cell line. The cells were exposed to morphine for 24 hrs before treatment with methadone, as ananti-withdrawal drug for subsequence 24 hours. The cytosolic fraction of the cell was used in different objectives including receptoraffinity, withdrawal properties, endocytic machinery, desensitisation or internalisation and cellular adaptation. The result shows thatmorphine and methadone bind to the μ-opioid receptor. The morphine-treated cells were observed to increase the expression ofaddiction markers, have a low rate of the endocytic machinery, cause desensitisation of receptor and reduce cellular *** changes by morphine were normalised by the treatment of methadone. As a whole, it is postulated that neuroblastoma cell line,SK-N-SH, can be used as an in-vitro model to demonstrate morphine addiction before animal and human testing.

Catalytic hydrogenation of CO2 into methanol and dimethyl ether was carried out over hybrid catalysts consisting of methanol-synthesis catalyst and zeolite. The methanol-synthesis catalyst, Cu/ZnO/Al2O3, was prepared by a co-precipitation method. Then it was physically mixed with HZSM-5 zeolite at weight ratios of 2：1, 1：1 and 1：2. The CO2 hydrogenation reaction was conducted in a fixed-bed microreactor at 250℃ and 40 bar in pre-mixed H2/CO2 feed with H2：CO2 molar ratios of 3：1 and 7：1. Products detected include methanol, dimethyl ether, carbon monoxide and water. Conversion of CO2 and yield of oxygenated products were influenced by the weight ratio of Cu/ZnO/Al2O3：HZSM-5 in the hybrid system and also the feed ratio. The Cu/ZnO/Al2O3： HZSM-5 hybrid at 1：1 resulted in methanol yield of 22.0% and was found to be an efficient hybrid catalyst for the CO2 hydrogenation reaction.

AIM:To study the role of needle knife assisted ampullary biopsy in the diagnosis of periampullary ***:In this study the authors retrospectively analyzed clinical records of patients with periampullary tumors diagnosed by ampullary biopsy taken after needle knife papillotomy in whom surface ampullary biopsies were non ***:Between January 2008 and December 2010,38 patients with periampullary tumors were seen by us and initial side viewing endoscopy with surface biopsy from the papilla was positive for malignancy in 25 *** patients with a negative surface biopsy for malignancy underwent a repeat ampullary biopsy following needle knife *** were 8(61.5%)males and 5(38.5%)*** most common presenting symptom was jaundice(100%),followed by fever(46.2%),melena(38.5%),abdominal pain(30.8%)and weight loss(30.8%).All the patients had hyperbilirubinemia with a mean ± SD serum bilirubin of(11.2 ± 1.9)mg/dL(normal value 70.5 IU/L)in all of them with a median of 1200 IU/L(inter quartile range 274-3500).Side viewing endoscopy showed a bulky papilla in all of *** tissue was obtained in all of the 13 patients for histological evaluation;12 of the 13 patients were reported to have adenocarcinoma while one patient had *** were no complications from the needle knife papillotomy in any of the ***:Needle knife assisted ampullary biopsy appears to be a safe and effective diagnostic modality for periampullary carcinoma.
