We present a rare challenging case of metastatic non-small cell lung cancer with Epstein-Barr virus positivity that was also diagnosed with pulmonary tuberculosis at the same time. Palliative chemotherapy gemcitabine and carboplatin was started after two weeks of anti-tuberculosis treatment with the hopes that this period would be sufficient to keep acid fast bacilli non-viable to minimise risk of tuberculosis re-activation due to chemotherapy induced immunosuppression. She completed four cycles of chemotherapy and six months of anti-tuberculosis treatment with good results and minimal side effects. Two years later, there was disease recurrence in cervical and mediastinal lymph nodes which was treated with local treatment i.e. surgery and palliative radiotherapy. It has been two years since last radiotherapy and overall more than five years since diagnosis with no active disease at present. Given the complexity and rarity of this case, significant multidisciplinary team involvement, including oncologists and radiation oncologists, pulmonologists with special interest in tuberculosis and pathologists was necessary throughout.

Background：Musculoskeletal disorders（MSD） comprise a wide range of conditions,associated with an enormous pain and impaired mobility,and are affecting people＇s lives and *** of musculoskeletal disorders typically involves a multidisciplinary team *** findings have been found in previous studies evaluating the effectiveness of complementary therapies,though little attention has been paid to evaluating of the effectiveness of integrated packages of care combining conventional and complementary approaches for musculoskeletal conditions in a National Health Service（NHS） ***：To determine the feasibility of all aspects of a pragmatic observational study designed：（1） to evaluate the effectiveness and cost effectiveness of integrated treatments for MSDs in an integrated NHS hospital in the UK;（2） to determine the acceptability of the study design and research process to patients;（3） to explore patients＇ expectation and experience of receiving integrated ***：This is an observational feasibility study,with 1-year recruitment and 1-year follow-up,conducted in Royal London Hospital for Integrated Medicine,University College London Hospital Trust,*** eligible patients with MSDs newly referred to the hospital were included in the *** are integrated packages of care（conventional and complementary） as currently provided in the ***-36 Health Survey,short form Brief Pain Inventory,Visual Analogue Scale,and modified Client Service Receipt Inventory will be assessed at 4/5 time ***-structured interview/focus group will be carried out before treatment,and 1 year after commence of ***：We intend to conduct a pragmatic observational study of integrated medical treatment of MSDs at a public sector *** will inform the design of a future trial including recruitment,retention,suitability of the outcome measures and patients experiences.

Geochemical Characterizations of Source and Depositional Environment of Hydrocarbons in the Lake Albert Rift Basin

Despite the upsurge in hydrocarbon exploration in the Lake Albert Rift Basin(LARB)over the past three decades,systematic characterization of hydrocarbon compositions remains lacking,leading to uncertainties in source rock and oil generation *** characterized crude oil compositions and oil sand samples in the northern and southern subbasins of *** relative abundance of normal and branched linear alkanes,hopanes,steranes,and aromatic hydrocarbon suggest that northern and southern hydrocarbons were deposited in anoxic to suboxic lacustrine environments and share similar biological source compositions(i.e.,a mixture of plants and aquatic algae and bacteria).Relative to southern samples,northern samples show more negative δ^(13)C values for oils,saturates and aromatics,indicating longer migration paths,and exhibit higher MPI-1,DNR-1 and 4-/1-MDBT ratios,indicating higher *** the two possible sets of source rocks(upper Miocene and Jurassic strata),the positive δ^(13)C values of saturated hydrocarbons(average=-20.5‰)suggest that the upper Miocene lacustrine shale is the most likely *** index(<5% in our samples)does not exclude either source rock possibility,and C_(28)/C_(29) regular sterane(average=0.63)may be biased by high terrestrial inputs in a lacustrine ***,our data show that northern and southern oils originate from the same source rocks but different oil *** the similar geochemical characteristics of southern and northern oils,previous exploration successes in the northern subbasin likely suggest similar potential in the southern sector,while other elements influencing exploration success must be also evaluated.

Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise ***:A systematic search for relevant studies was performed in 3 *** controlled trials investigating the effects of exercise training on at least one of the following exerkines were included:adiponectin,apelin,brain-derived neurotrophic factor,fetuin-A,fibroblast growth factor-21,follistatin,ghrelin,interleukin(IL)-6,IL-8,IL-10,IL-15,IL-18,leptin,myostatin,omentin,resistin,retinol-binding protein 4,tumor necrosis factor-α,and ***:Forty randomized controlled trials were selected for data extraction(n=2160).Exercise training induces changes in adiponectin,fetuin-A,fibroblast growth factor-21,IL-6,IL-10,leptin,resistin,and tumor necrosis factor-a levels but has no significant effects on apelin,IL-18,and ghrelin compared to *** exercise training favored large and positive changes in pooled exerkines(i.e.,an overall effect size calculated from several exerkine s)(Hedge’s g=1.02,95%confidence interval(95%CI):0.76-1.28),which in turn were related to changes in glycated hemoglobin(mean difference(MD)=-0.81%,95%CI:-0.95%to-0.67%),fasting glucose(MD=-23.43 mg/dL,95%CI:-30.07 mg/dL to-16.80 mg/dL),waist circumference(MD=-3.04 cm,95%CI:-4.02 cm to-2.07 cm),and body mass(MD=-1.93 kg,95%CI:-2.00 kg to-1.86 kg).Slightly stronger effects were observed with aerobic,resistance,or high-intensity interval protocols at moderate-to vigorous-intensity and with programs longer than 24 weeks that comprise at least 3 sessions per week and more than 60 min per ***:Exercise training represents an anti-inflammatory therapy and metabolism-improving strategy with minimal side effects for patients with type 2 diabetes mellitus.

The spatial-temporal development of Cenozoic reactivation along Mesozoic suture zones in the Tibetan Plateau are first-order parameters needed for assessing models of plateau *** Ganzi-Litang suture,in the eastern Tibetan Plateau,developed in the Late Triassic because of subduction and closure along the eastern branch of the Paleo-Tethys *** the city of Litang,the Ganzi-Litang suture is defined by a melange sequence with faultbound,synorogenic nonmarine strata along the western and eastern flanks,suggesting post-Triassic structural *** present detrital zircon U-Pb geochronology and field data to determine the timing and style of reactivation along the Ganzi-Litang suture,as well as sedimentary provenance of nonmarine strata.A reverse fault placing melange rock on top of nonmarine strata along the eastern flank of the Ganzi-Litang suture indicates a contractional deformational regime during *** clast counts indicate a local sediment source of recycled Ganzi-Litang suture and Yidun terrane *** zircons indicate a localized provenance consisting of recycled material from Triassic Yidun Arc plutons and Triassic Yidun Group turbidite rock.A weighted mean average of Cenozoic zircon grains(n=10) establishes a maximum depositional age of41.5 ± 1.2 Ma for nonmarine strata in the Ganzi-Litang *** interpret the maximum depositional age of the nonmarine strata to represent the upper-limit for structural reactivation along the Ganzi-Litang suture while undeformed Neogene strata in the suture zone represent the lower-limit;thereby bracketing structural reactivation from ca.42-25 *** results provide enhanced spatial-temporal resolution for Cenozoic deformation in the eastern Tibetan Plateau.

1997年发生在新疆伽师的强震群迄今仍在持续活动,从1997年1月21日震群发生至1997年10月18日,共发生5级以上地震15次.震群发生前,该地区几乎没有地震活动.根据伽师震群地震的精确定位结果及震源机制解,我们设定该震群的发震构造是一组NNW(北北西)向的雁形右旋走滑断层和一组NE(北东)向雁形正倾滑断层.并以此作为模拟该区域理论地震活动的力学模型,开展震群区域理论地震活动的模拟研究.模拟结果证实了断层间的相互作用或应力传递能产生震群活动的猜测,并对伽师震群中为何部分地震的震源机制是正断层进行了合理解释.进一步推测伽师位于地壳厚度的陡变带也可能是引起该地区地震活动丛集发生的原因.

Use of second generation contrast-enhanced ultrasound in the assessment of focal liver lesions

Ultrasound (US) is often the first imaging modality employed in patients with suspected focal liver lesions. The role of US in the characterisation of focal liver lesions has been transformed with the introduction of specific contrast media and the development of specialized imaging techniques. Ultrasound now can fully characterise the enhancement pattern of hepatic lesions, similar to that achieved with contrast enhanced multiphasic computed tomography (CT) and magnetic resonance imaging (MRI). US contrast agents are safe, well-tolerated and have very few contraindications. Furthermore, real-time evaluation of the vascularity of focal liver lesions has become possible with the use of the newer microbubble contrast agents. This article reviews the enhancement pattern of the most frequent liver lesions seen, using the second generation US contrast media. The common pitfalls for each type of lesion are discussed. The recent developments in US contrast media and specific imaging techniques have been a major advance and this technique, in view of the intrinsic advantages of US, will undoubtedly gain popularity in the years to come.

In a recent study in Nature,Junqueira et *** evidence that severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infects human monocytes and lung macrophages.1 The infection is antibody-mediated via Fc-receptors(FcγRs),and activates NLRP3 and AIM2 inflammasomes,which in turn induce pyroptosis and halt viral *** monocytes secrete the proinflammatory cytokines interleukin-1β(IL-1β)and IL-18,which contribute to systemic inflammation and severe coronavirus disease 19(COVID-19)(Fig.1).
