Prevalence of celiac disease in adult patients with refractory functional dyspepsia:Value of routine duodenal biopsy

AIM: To investigate the prevalence of celiac disease (CD) in adult patients referred to an open access gas-troenterology clinic in the south of Italy and submitted to esophago-gastro-duodenoscopy (EGD) for evaluation of refractory functional ***: Seven hundred and twenty six consecutive dyspeptic patients (282 male, 444 female; mean age 39.6 years, range 18-75 years) with unexplained prolonged dyspepsia were prospectively enrolled. Duo-denal biopsies were taken and processed by standard staining. Histological evaluation was carried out according to the Marsh-Oberhuber ***: The endoscopic findings were: normal in 61.2%, peptic lesions in 20.5%, malignancies in 0.5%, miscellaneous in 16.7%. CD was endoscopically diagnosed in 8 patients (1.1%), histologically in 15 patients (2%). The endoscopic features alone showed a sensitivity of 34.8% and specificity of 100%, with a positive predictive value (PPV) of 100% and a negative predictive value (NPP) of 97.9%.CONCLUSION: This prospective study showed that CD has a high prevalence (1:48) in adult dyspeptic patients and suggests the routine use of duodenal biopsy in this type of patient undergoing EGD.

The main aim of antineoplastic treatment is to maximize patient benefit by augmenting the drug accumulation within affected organs and tissues,thus incrementing drug effects and,at the same time,reducing the damage of non-involved tissues to cytotoxic *** stromal cells(MSC)represent a group of undifferentiated multipotent cells presenting wide self-renewal features and the capacity to differentiate into an assortment of mesenchymal family *** the last year,they have been proposed as natural carriers for the selective release of antitumor drugs to malignant cll,s thus optimizing cytotoxic action on cancer cll,while significantly reducing adverse side efect on healthy *** chemotherapeutic drug loading and delivery is an encouraging new area of cell therapy for several tumors,especially for those with unsatisfactory prognosis and limited treatment options *** some experim ental models have been sucesfuly developed,phase I dinical studies are needed to confirm this potential application of cell therapy,in particular in the case of primary and secondary lung cancers.

Appropriateness of Amikacin Dose Prescription, Monitoring and Safety during Hospitalization as an Impact of Clinical Pharmacologist Intervention, in the Israeli Regional Hospital

Background: Use of inappropriate amikacin dose is one of the most important factors in inducing toxicity, prolonged hospitalization as well as in increasing patient’s mortality. Objective: The aims of this study are the analysis of amikacin dose, serum level and the examination of the effectiveness of the clinical pharmacologist (CP) therapeutic drug monitoring (TDM) intervention to guarantee the safety of amikacin use. Methods: This is a one-year retrospective observational chart review study, which evaluates amikacin dose, serum drug level, development of adverse effects in patients on amikacin with or without CP TDM consultation. Results: Amikacin was prescribed for 393 complex patients, with median age 83. Amikacin group (AG) included 140 (32%) courses with CP consultation (AG1) and 292 (68%) courses without CP consultation (AG2). The distribution of most study characteristics in both groups was similar including amikacin dose (9-10 mg/kg/day), renal failure (14%) and mortality (12%). Acceptance for CP consultation was in 46% of amikacin courses and dose changes were done in 63% after CP intervention. Prolonged antibiotic course (4.6 ± 1.5 vs 3.8 ± 1.6 days, p < 0.0001) and the patient’s hemodynamic instability (15% vs 7%, p = 0.01) were more frequent in the AG1 compared to the AG2. There was a strong association between CP consultation and prolonged hospitalization (p = 0.005), while no association between it and amikacin adverse effects, renal failure or mortality. Conclusions: There was no trend to reducing amikacin toxicity, days of hospitaliza tion or mortality in patients with CP consultation. CP TDM intervention was more in the management of complicated clinical situations. However, it is necessary to optimize it.

Background:This study aimed to determine the effect of different carbohydrate(CHO)doses on exercise capacity in patients with McArdle disease—the paradigm of“exercise intolerance”,characterized by complete muscle glycogen unavailability—and to determine whether higher exogenous glucose levels affect metabolic responses at the McArdle muscle cell(in vitro)***:Patients with McArdle disease(n=8)and healthy controls(n=9)underwent a 12-min submaximal cycling constant-load bout followed by a maximal ramp test 15 min after ingesting a non-caloric *** a randomized,double-blinded,cross-over design,patients repeated the tests after consuming either 75 g or 150 g of CHO(glucose:fructose=2:1).Cardiorespiratory,biochemical,perceptual,and electromyographic(EMG)variables were ***,glucose uptake and lactate appearance were studied in vitro in wild-type and McArdle mouse myotubes cultured with increasing glucose concentrations(0.35,1.00,4.50,and 10.00 g/L).Results:Compared with controls,patients showed the“classical”second-wind phenomenon(after prior disproportionate tachycardia,myalgia,and excess electromyographic activity during submaximal exercise,all p<0.05)and an impaired endurance exercise capacity(-51%ventilatory threshold and55%peak power output,both p<0.001).Regardless of the CHO dose(p<0.05 for both doses compared with the placebo),CHO intake increased blood glucose and lactate levels,decreased fat oxidation rates,and attenuated the second wind in the ***,only the higher dose increased ventilatory threshold(+27%,p=0.010)and peak power output(+18%,p=0.007).In vitro analyses revealed no differences in lactate levels across glucose concentrations in wild-type myotubes,whereas a doseresponse effect was observed in McArdle ***:CHO intake exerts beneficial effects on exercise capacity in McArdle disease,a condition associated with total muscle glycogen *** of these benefits are dose dependent.

Understanding the Link: Emotional Attention in Italian Families and Children’s Social Development

Background Emotional attention refers to the capacity to recognize and properly respond to one’s and others’ emotional states. On another note, family is a primary source of socialization that influences the development of various social skills. In another line, adolescence is a complex stage that has been associated with emotional difficulties that could be related to competences such as prosociability and inclusion. It is inferred that through the family context and the attention that is processed, a series of competencies are transmitted to the youngsters, but this relationship is still unclear. For this reason, the study’s purpose was to examine the relationship between the emotional attention of mothers and fathers and the social and prosocial competencies of their children. Methods To accomplish this, a cross-sectional study and ex post facto design were conducted. The Trait-Meta Mood Scale (TMMS-24)-to assess emotional attention-, the Strengths and Difficulties Questionnaire (SDQ)-to determine the prosociality dimension-, and the Test on Passing Developmental Tasks in Adolescence (TCS-A)-to measure social inclusion-were administered. The participants were N = 228 families composed of n = 114 mothers/fathers (67.5% females) and n = 114 daughters/sons (38.5% females) selected through non-probabilistic sampling using a convenience sample in the Bari region of Italy. A descriptive and inferential analysis-Student’s test and Pearson’s correlation-was performed with the statistic package SPSS version 28 (significance level p < 0.05). Results The findings showed that the families possessed an adequate background in emotional attention and that their daughters and sons were also generally well-adjusted in social inclusion and prosociality. It also found certain signs that the prosocial competence of the children could be related to the behavior of their mothers and fathers. 以宁夏盐池县荒漠草原为研究对象,设置3个放牧强度:围栏封育(NG,no grazing)、中度放牧(MG,moderate grazing)、重度放牧(HG,heavy grazing),运用主成分分析与路径分析对草地质量进行综合评价,比较不同放牧条件下草地植被和土壤理化性质的差异,以期为退化草地生态系统恢复与重建提供理论指导。结果表明:NG处理下草地植被生物量较MG和HG处理分别提高10.78%和74.85%,植被高度与盖度显著提高31.21%~131.27%和26.03%~93.29%。NG处理的土壤机械稳性团聚体以粒径>0.25 mm的大团聚体为主,HG处理以0.25 mm的大团聚体占比、表层土壤水稳性团聚体的平均质量直径和几何平均直径大小依次为:NG>MG>HG;NG处理下的土壤供肥潜力、土壤供肥强度和土壤综合质量显著高于HG。综上所述,围栏封育下的草场植被、土壤结构和肥力恢复效果更好,对土壤的综合质量有明显的改善作用,是适合的草地生态恢复措施。
