Submucosal injection of platelet-rich plasma in endoscopic resection of large sessile lesions

AIM To prospectively evaluate the efficacy of submucosal injection of platelet-rich plasma(PRP) on endoscopic resection of large sessile lesions.METHODS Eleven patients were submitted to endoscopic mucosal resection(EMR) with prior injection of PRP, obtained at the time of endoscopy. Patients were followed during 1 mo. The incidence of adverse events(delayed bleeding or perforation) and the percentage of mucosal healing(MHR) after 4 wk were registered. RESULTS EMR was performed in 11 lesions(46.4 mm ± 4 mm, range 40-70 mm). Delayed bleeding or perforation was not observed in any patient. Mean ulcerated area atbaseline was 22.7 cm^2 ± 11.7 cm^2 whereas at week 4 were 2.9 cm^2 ± 1.5 cm^2. Patients treated with PRP showed a very high MHR after 4 wk(87.5%). CONCLUSION PRP is an easy-to-obtain solution with proven and favourable biological activities that could be used in advanced endoscopic resection.

Endoscopic retrograde cholangiopancreatography (ERCP) is an important tool for the diagnosis and treatment of the hepatobiliary system. The use of fluoroscopy to aid ERCP places both the patient and the endoscopy staff at risk of radiation-induced injury. Radiation dose to patients during ERCP depends on many factors, and the endoscopist cannot control some variables, such as patient size, procedure type, or fluoroscopic equipment used. Previous reports have demonstrated a linear relationship between radiation dose and fluoroscopy duration. When fluoroscopy is used to assist ERCP, the shortest fluoroscopy time possible is recommended. Pulsed fluoroscopy and monitoring the length of fluoroscopy have been suggested for an overall reduction in both radiation exposure and fluoroscopy times. Fluoroscopy time is shorter when ERCP is performed by an endoscopist who has many years experience of performing ERCP and carried out a large number of ERCPs in the preceding year. In general, radiation exposure is greater during therapeutic ERCP than during diagnostic ERCP. Factors associated with prolonged fluoroscopy have been delineated recently, but these have not been validated.

To avoid soil degradation,adapt to climate change and comply with the Sustainable Development goals 2030(UN General Assembly),establishing the quality/fertility of the agricultural soils of the Mediterranean region and evaluate how these evolve with time is mandatory.This enables adequate management practices to be implemented.The Mediterranean calcareous region has received little attention in this sense,in spite its soil particularities.So,24 different representative calcareous agricultural soils of the Mediterranean region,including the main management strategies of agricultural soils(rainfed and irrigated),were sampled from the island of Mallorca;and their physical,chemical and biological properties determined.The values obtained for most of the soil characteristics allowed to establish an initial approach to the reference values for the type of calcareous agricultural soils considered,and to clearly distinguish the effect of rainfed or irrigation management practices on soils properties and dynamics.Some enzyme activities were not stimulated by the irrigation conditions assayed or they were only in dry conditions,suggesting optimum enzyme activities could be obtained when alternating dry and humid soil conditions.Soil organic carbon,calcium carbonate and active lime revealed of significant importance in the collection of soils.Finally,the results obtained clearly indicate the heterogeneity of the region and its implications on the different soil characteristics.Therefore,this study could serve as a starting point to adequately establish the quality(fertility)for Mediterranean calcareous agricultural soils and their reference values by conducting further research in this region and including more type of soils.

AIM:To describe colon anatomy with colonoscopy and computed tomography(CT) to develop a rat model for future studies of therapeutic colonoscopy.METHODS:Eighteen male Sprague-Dawley rats,on average 400-420 g,underwent total colonoscopy,CT and histological examination.Colonoscopy was performed after bowel preparation with a baby upper gastrointestinal endoscopy with an outer diameter of 6.7 mm.CT obtained a 3D image of total colon after a rectal enema with radiological contrast.Macroscopic and microscopic examinations were examined with a conventional technique(hematoxylin and eosin).Colonic wall thickness,length and diameter measurements were taken from the anus,3,7,14 and 20 cm from the anal margin.RESULTS:The median colonoscope depth was 24 cm(range 20-28 cm).Endoscopic and tomographic study of colon morphology showed an easy access with tubular morphology in the entire left colon(proximal left colon and rectum).Transverse colon was unapparent on colonoscopy.Right colon,proximal to the splenic flexure,was the largest part of the colon and assumed saccular morphology with tangential trabecula.Radiological measurements of the colonic length and diameter substantiate a subdivision of the right colon into two parts,the cecum and distal right colon.In addition,histological measurement of the colonic wall thickness confirmed a progressive decrease from rectum to cecum.The muscular layer was thinner in the proximal left colon.CONCLUSION:The combination of colonoscopy,tomography and histology leads to a better characterization of the entire colon.These data are important for deciding when to perform endoscopic resections or when to induce perforations to apply endoscopic treatments.

In this paper we characterize the phase portraits of the Leslie-Gower model for competition among species.We give the complete description of their phase portraits in the Poincarédisc(i.e.,in the compactification of R^(2) adding the circle S1 of the infinity)modulo topological equivalence.It is well-known that the equilibrium point of the Leslie-Gower model in the interior of the positive quadrant is a global attractor in this open quadrant,and in this paper we characterize where the orbits attracted by this equilibrium born.

One challenge in the engineering of biological systems is to be able to recognise the cellular stress states of bacterial hosts,as these stress states can lead to suboptimal growth and lower yields of target products.To enable the design of genetic circuits for reporting or mitigating the stress states,it is important to identify a relatively reduced set of gene biomarkers that can reliably indicate relevant cellular growth states in bacteria.Recent advances in high-throughput omics technologies have enhanced the identification of molecular biomarkers specific states in bacteria,motivating computational methods that can identify robust biomarkers for experimental characterisation and verification.Focused on identifying gene expression biomarkers to sense various stress states in Bacillus subtilis,this study aimed to design a knowledge integration strategy for the selection of a robust biomarker panel that generalises on external datasets and experiments.We developed a recommendation system that ranks the candidate biomarker panels based on complementary information from machine learning model,gene regulatory network and co-expression network.We identified a recommended biomarker panel showing high stress sensing power for a variety of conditions both in the dataset used for biomarker identification(mean f1-score achieved at 0.99),as well as in a range of independent datasets(mean f1-score achieved at 0.98).We discovered a significant correlation between stress sensing power and evaluation metrics such as the number of associated regulators in a B.subtilis gene regulatory network(GRN)and the number of associated modules in a B.subtilis co-expression network(CEN).GRNs and CENs provide information relevant to the diversity of biological processes encoded by biomarker genes.We demonstrate that quantitatively relating meaningful evaluation metrics with stress sensing power has the potential for recognising biomarkers that show better sensitivity and robustness to an extended set of stress conditions and enable a more
