Neoadjuvant chemoradiotherapy for locally advanced gastric cancer with bulky lymph node metastasis:Five case reports

BACKGROUND Neoadjuvant chemoradiotherapy(NACRT)has not been accepted as a general therapy for gastric cancer because of its localized effect and toxicity for radiosensitive ***,if radiation therapy could compensate for the limited or inadequate treatment choices available for elderly patients and/or those at high risk,the available therapeutic options for advanced gastric cancer might *** this perspective,we present our experiences of five patients with advanced gastric cancer in whom we used NACRT therapy with interesting *** SUMMARY We admitted five patients with clinical Stage III gastric cancer and bulky lymph node metastasis or adjacent organ invasion at the time of diagnosis.A total of 50 Gy of preoperative intensity modulated radiation therapy was delivered to the patients in doses of 2.0 Gy/d,together with a regimen of concomitant chemotherapy comprising two courses of oral tegafur/gimeracil/oteracil(S-1;65 mg/m2 per day)for three consecutive weeks followed by two weeks of rest,starting at the same time as *** patients underwent no residual tumor resection and a pathological complete response of the primary tumors was achieved in two *** incidence of hematological toxicity was low,although the digestive toxicities of anorexia and diarrhea developed in three of the five patients,necessitating termination of radiation therapy at 30 Gy and S-1 at three ***,even 30 Gy of irradiation and half the dose of S-1 resulted in sufficient downstaging,indicating that even a reduced amount of NACRT could confer considerable *** Slightly reduced NACRT might be useful and safe for patients with locally advanced gastric cancer.

Endoplasmic reticulum stress,autophagy,neuroinflammation,and sigma 1 receptors as contributors to depression and its treatment

The etiological factors contributing to depression and other neuropsychiatric disorders are largely undefined. Endoplasmic reticulum stress pathways and autophagy are well-defined mechanisms that play critical functions in recognizing and resolving cellular stress and are possible targets for the pathophysiology and treatment of psychiatric and neurologic illnesses. An increasing number of studies indicate the involvement of endoplasmic reticulum stress and autophagy in the control of neuroinflammation, a contributing factor to multiple neuropsychiatric illnesses. Initial inflammatory triggers induce endoplasmic reticulum stress, leading to neuroinflammatory responses. Subsequently, induction of autophagy by neurosteroids and other signaling pathways that converge on autophagy induction are thought to participate in resolving neuroinflammation. The aim of this review is to summarize our current understanding of the molecular mechanisms governing the induction of endoplasmic reticulum stress, autophagy, and neuroinflammation in the central nervous system. Studies focused on innate immune factors, including neurosteroids with anti-inflammatory roles will be reviewed. In the context of depression, animal models that led to our current understanding of molecular mechanisms underlying depression will be highlighted, including the roles of sigma 1 receptors and pharmacological agents that dampen endoplasmic reticulum stress and associated neuroinflammation.

Glyphosate as a direct or indirect activator of pro-inflammatory signaling and cognitive impairment

Glyphosate-based herbicides are widely used around the world, making it likely that most humans have significant exposure. Because of habitual exposure, there are concerns about toxicity including neurotoxicity that could result in neurological, psychiatric, or cognitive impairment. We recently found that a single injection of glyphosate inhibits long-term potentiation, a cellular model of learning and memory, in rat hippocampal slices dissected 1 day after injection, indicating that glyphosate-based herbicides can alter cognitive function. Glyphosate-based herbicides could adversely affect cognitive function either indirectly and/or directly. Indirectly, glyphosate could affect gut microbiota, and if dysbiosis results in endotoxemia(leaky gut), infiltrated bacterial by-products such as lipopolysaccharides could activate pro-inflammatory cascades. Glyphosate can also directly trigger pro-inflammatory cascades. Indeed, we observed that acute glyphosate exposure inhibits long-term potentiation in rat hippocampal slices. Interestingly, direct inhibition of long-term potentiation by glyphosate appears to be similar to that of lipopolysaccharides. There are several possible measures to control dysbiosis and neuroinflammation caused by glyphosate. Dietary intake of polyphenols, such as quercetin, which overcome the inhibitory effect of glyphosate on long-term potentiation, could be one effective strategy. The aim of this narrative review is to discuss possible mechanisms underlying neurotoxicity following glyphosate exposure as a means to identify potential treatments.

Characteristics of Small Pancreatic Carcinoma: Neural Invasion is Associated with Tumor Recurrence

Background: Pancreatic carcinoma has a poor prognosis. It is important to focus on early detection and treatment for improvement of the prognosis. Understanding the characteristics of TS1 pancreatic carcinoma (≤2 cm) could improve the treatment outcome of pancreatic carcinoma. Methods: Among 444 patients with conventional pancreatic carcinoma who underwent surgery at our facility, the study targeted 65 (14.6%) with a histopathological diagnosis of TS1 pancreatic carcinoma. We examined 65 cases of TS1 pancreatic carcinoma in reason for hospital visit, examination findings, histopathological findings, and treatment outcomes. Results: The detection rate of TS1 pancreatic carcinoma by ultrasonography was 83.1% for visualization of the tumor mass and 96.9% for main pancreatic duct dilatation. The corresponding rates for endoscopic ultrasound were 92.9% and 100%. With regard to postoperative outcome, 43.1% of patients had a recurrence;the site of recurrence was local in 24.6% and hepatic in 9.2%. On multivariate analysis, intrapancreatic neural invasion was an independent risk factor for recurrence (odds ratio, 6.333;95% confidence interval, 1.834 - 21.872;p = 0.004). Conclusions: To screen for TS1 pancreatic carcinoma, the study first examined for main pancreatic duct dilatation by ultrasonography and then conducted a detailed examination with endoscopic ultrasound. Patients with pancreatic neural invasion require careful attention for local recurrence.

A comparison of the efficacy of dutasteride on reducing lower urinary tract symptoms among patients with small versus large benign prostatic hyperplasia

Science and technology innovation has always been present in Asia,but its application in disaster risk reduction(DRR) has been *** Asia,globally significant hotspots of disasters and economic development have emerged in which the application of science and technology in DRR has become an essential requirement for informed decision *** has supported establishment and implementation of major international initiatives in DRR,including the Hyogo Framework for Action *** more recent Sendai Framework for DRR 2015-2030 recognizes the importance of science and technology in all of its priority action areas,and subsequent global and regional forums and conferences have reconfirmed science and technology's *** perceive and monitor the progress of science and technology in DRR,a qualitative assessment of different countries is made using three major attributes:(1) science-based decision making;(2) investment in science and technology;and(3) the intensity of science's link to the *** assessment exercise points out several strengths and weaknesses in science and technology application;the method can be employed to develop future multistakeholder and multidisciplinary science and technology plans at the country *** implement regional and national activities,a set of 15 recommendations is put forward,which will strengthen the collective regional "science voice" in DRR.

Fabrication of a model specimen for understanding micro-galvanic corrosion at the boundary ofα-Mg andβ-Mg17Al12

A model specimen with a single boundary of theα/βphase simulating Mg-Al alloys was successfully fabricated by spark plasma sintering.A small electrode area ofαphase orβphase was prepared using the model specimen,and the OCPs(open-circuit potentials)of each phase and a small electrode area containing theα/βphase boundary in 0.1 M NaCl at pH 8.0 were compared:theβphase exhibited a higher potential,and theαphase showed a lower *** OCP of the small area containing theα/βphase boundary was the intermediate value of these *** a small area containingα/βphase boundary,discoloration and gas bubbles were observed on theαphase,but no bubble generation was detected on theβ*** gas bubbles were initially generated on theαphase near theβphase,but as the discoloration(corrosion)of theαphase approached theβphase,the bubbles were generated on theβ*** micro-galvanic corrosion of theαandβphases,theβphase did not always function as the preferred ***αphase partially corroded(or discolored)and became the anodes,so that the surrounding areas were most likely to be the *** corroded areas(anodes)in theαphase approached theβphase,theβphase would become *** addition to the micro-galvanic corrosion mechanism,the role of Al in corrosion resistance at theα/βphase boundary was determined by surface analysis.

The purpose of this paper is to study the dynamic behavior of softground including a porous layer by considering porosity change. Inorder to take the porosity change into account, the concept of thevolume fraction, which has been proposed in continuum mechanics, isintroduced. The constitutive equations presented by Bowen are appliedto the analysis of the porous media. 黑泷正昭曾撰文批评大村假说,认为《德意志意识形态》手稿不是“马克思口述、恩格斯笔录”的产物,本文尝试对此批评展开反批评。黑泷指出,他仔细检讨了笔者对假说的证明过程,发现笔者并未能成功证明此假说。但是黑泷对其中两大关键性证据的批评无法令人信服。第一大证据在于,手稿中出现了许多即时异文,这在恩格斯单独执笔的手稿中较少发现,而在马克思单独执笔的手稿中却屡见不鲜。事实上,该手稿中即时异文的出现频率是《1844年经济学哲学手稿》的两倍。为什么会发生这样的情况,黑泷对此并未加以考察。第二大证据在于,手稿中出现了许多德文同音异义词的混淆错误,笔者给出了6处例证。黑泷认为其中4处不应视为对同音异义词的替换更正,黑泷的这一看法要么以假定一份原本并不存在的手稿存在为前提,要么以否认恩格斯替换更正同音异义词这一事实为前提。因而笔者无法承认黑泷的批评是基于学术检讨做出的。

Evaluation of precipitation behavior of zirconium molybdate hydrate

In the dissolution step of spent nuclear fuel, there is a world-concern problem that zirconium molybdate hydrate precipitates as a byproduct, and accumulates in some reprocessing equipments. In order to prevent this accumulation, we have developed a new method based on the controlled reaction crystallization of zirconium molybdate hydrate （ZMH） in the reprocessing solution, followed by solid liquid separation. In order to measure the particle size of ZMH, batch crystallization experiments were conducted by varying nitric acid concentration and operating temperature. In result, almost all particle sizes scatter around 1μm on average, despite the higher concentration of nitric aid and operating temperature, and then small particles grow up as an aggregate sticking to the crystallizer. Moreover, polymorph and color changing were observed by varying the concentration of nitric acid and reaction time. These results suggest that crystal color and adhesiveness are closely related to the particle size of ZMH. And the control of nitric acid concentration and small particle growth would be the useful technique to prevent the ZMH sticking.
