Preeclampsia is a progressive,multi-system disorder of pregnancy associated with morbidity and mortality on both the mother and the ***,research is directed at identifying early biomarkers of preeclampsia in order to predict its *** is important because it helps understand the pathophysiology of the disease,and thus,promises new treatment *** a clear understanding of the pathogenesis of PE remains elusive,the currently most accepted theory suggests a two-stage *** first stage results in inadequate remodeling of the spiral arteries and leads to the second stage,whereby the clinical features of the syndrome are *** this review,we summarize the modalities that have been studies so far to predict *** use of uterine artery Doppler and several other biomarkers such as vitamin D,soluble fms-like tyrosine kinase 1/placental growth factor(sFLT1/PIGF)ratio,soluble endoglin,and a subset of T-lymphocytes has shown promising *** are still at early stages in this advance,and no clear recommendations have been made about their clinical use to *** studies are still needed to improve screening strategies and evaluate the cost-effectiveness of any intervention.

This survey paper provides a review and perspective on intermediate and advanced reinforcement learning(RL)techniques in process industries. It offers a holistic approach by covering all levels of the process control hierarchy. The survey paper presents a comprehensive overview of RL algorithms,including fundamental concepts like Markov decision processes and different approaches to RL, such as value-based, policy-based, and actor-critic methods, while also discussing the relationship between classical control and RL. It further reviews the wide-ranging applications of RL in process industries, such as soft sensors, low-level control, high-level control, distributed process control, fault detection and fault tolerant control, optimization,planning, scheduling, and supply chain. The survey paper discusses the limitations and advantages, trends and new applications, and opportunities and future prospects for RL in process industries. Moreover, it highlights the need for a holistic approach in complex systems due to the growing importance of digitalization in the process industries.

Color fading caused by a decrease in anthocyanin accumulation during the post-flowering stage significantly affects postharvest quality of ***,the underlying mechanism by which anthocyanin accumulation decreases during the post-flowering stage still unclear,which greatly restricts design of molecular breeding in ***,a chrysanthemum SG7 R2R3 MYB transcription factor(TF),CmMYB3-like,was identified to have a function in regulating anthocyanin biosynthesis during the post-flowering *** real time PCR(qRT-PCR)assays showed that the expression of CmMYB3-like was gradually downregulated when anthocyanin content increased during the flowering stage and was significantly upregulated during the post-flowering *** transformation of chrysanthemum and dual-luciferase assays in *** leaves showed that CmMYB3-like suppressed anthocyanin accumulation by inhibiting the transcription of CmCHS and CmANS directly and that of CmF3H ***,overexpression or suppression of CmMYB3-like did not affect the biosynthesis of flavones or *** transformation of chrysanthemum revealed that the overexpression of CmMYB3-like inhibited anthocyanin accumulation,but its suppression prevented the decrease in anthocyanin accumulation during the post-flowering *** results revealed a crucial role of CmMYB3-like in regulating the color of petals during the post-flowering stage and provided a target gene for molecular design breeding to improve the postharvest quality of chrysanthemum.

Hybrid IPOM: A Novel Technique for the Management of Incisional Hernia

Background and Aim: The incidence of incisional hernias has been reported to be around 15%. In the present scenario, a wide array of surgical procedures are available for their better management. In this study, we intend to share our experience with one novel technique, “Hybrid IPOM (Intraperitoneal onlay meshplasty)” as a management option for a selected cohort of patients. Methods: This prospective study was undertaken during January 2019 to July 2023 at King Abdullah medical city, Makkah. A total of 51 cases were selected for Hybrid IPOM repair as per inclusion criteria;medium sized (4 - 10 cm) hernia defects;uncomplicated hernias;age more than 18 years. The follow-up period of the patients varied from 6 months to 4 years. The operation commenced with open hernia dissection, mesh deployment into abdomen, defect closure and then conversion to laparoscopy for the posterior mesh placement. Results: A total of 51 cases were repaired successfully with this technique. 48 out of 51 cases were incisional hernias secondary to some primary procedure done either for hernias itself or some other intra-abdominal pathology. The three cases were primary hernias falling in medium to large category with unaesthetic overlying skin. The age range was 19 to 72 years. The mean (range) operative time was 135 (90 - 240) min, and the average blood loss was 70 ml. The mean (range) hospital stay was 3 (2 - 11) days. All patients returned to routine work within 2 - 3 weeks of surgery. The median follow-up was 15 (6 - 48) months. Of the 51 cases, 3 patients developed seroma (managed conservatively), 1 patient developed a large hematoma (needed evacuation), and 1 patient developed superficial wound infection (managed with antibiotics). Two patients had recurrences;one patient had previously failed multiple repairs, and the other developed a postoperative hematoma. None of our patients had an iatrogenic bowel injury. BACKGROUND Castleman's disease(CD)is a rare lymphoproliferative,emulating both benign and malignant *** diagnosis of CD is formulated upon the combination of clinical and laboratory criteria and ultimately confirmed by histopathological *** to its rarity,CD presents a challenge in treatment selection,with available options encompassing surgery,chemotherapy,and autologous stem cell ***,studies suggest that surgical resection of the lesion is the most effective treatment modality,especially for unicentric CD(UCD).CASE SUMMARY Here,we describe the case of a 25-year-old woman who presented with painless left thigh swelling for 10 *** had been following a low-fat diet to lose weight and had normal laboratory *** resonance imaging revealed a wellcircumscribed,demarcated cystic lesion located in the left inguinal region with eccentrically positioned signal void vascular structures,measuring 4.3 cm×3 cm×3.2 cm,likely of lymphoid *** patient underwent surgical resection,and the final histopathology showed a vascular proliferation and hyalinization of the vessel walls,along with atretic germinal centers traversed by penetrating vessels,consistent with *** patient was discharged home one day after the procedure in good condition,with a follow-up appointment scheduled in our outpatient *** Although surgical resection is the mainstay for UCD,a multidisciplinary approach is needed due the lack of specific diagnostic features and treatments.

The phenotypic diversity resulting from artificial or natural selection of sheep has made a significant contribution to human *** sheep are a local sheep breed unique to China with high reproductive rates and rapid *** selection signatures have been widely used to investigate the genetic mechanisms underlying phenotypic variation in ***,we conduct whole-genome sequencing of 207 Hu sheep and compare them with the wild ancestors of domestic sheep(Asiatic mouflon)to investigate the genetic characteristics and selection signatures of Hu *** on six signatures of selection approaches,we detect genomic regions containing genes related to reproduction(BMPR1B,BMP2,PGFS,CYP19,CAMK4,GGT5,and GNAQ),vision(ALDH1A2,SAG,and PDE6B),nervous system(NAV1),and immune response(GPR35,SH2B2,PIK3R3,and HRAS).Association analysis with a population of 1299 Hu sheep reveals that those missense mutations in the GPR35(GPR35 g.952651 A>G;GPR35 g.952496 C>T)and NAV1(NAV1 g.84216190 C>T;NAV1 g.84227412 G>A)genes are significantly associated(P<0.05)with immune and growth traits in Hu sheep,*** research offers unique insights into the selection characteristics of Hu sheep and facilitates further genetic improvement and molecular investigations.

Background and Objective: The outcome of locally advanced and metastatic urothelial carcinoma LA/mUC has improved over the past years with a plethora of new treatments and the approval of immune checkpoint inhibitors (ICIs), antibody-drug conjugates, and targeted agents, to identify locally advanced metastatic urothelial carcinoma’s current management practices in Lebanon and the implication of the ongoing economic crisis on the medical practice. Methods: An online questionnaire was used to survey ten Lebanese oncologists from six different hospitals, between July 5 and July 11, 2022, requesting information pertaining to their current clinical practice in the pharmacological treatment of locally advanced metastatic urothelial carcinoma. Key Findings: Cisplatin-based chemotherapy was the most frequently reported initial treatment of locally advanced metastatic urothelial carcinoma. Participants reported using immune checkpoint inhibitors in platinum-ineligible patients and those with PDL1 positive tumors. Also, they would not consider the concomitant use of immunotherapy and chemotherapy in the first-line setting. Participants believed that avelumab maintenance is effective in the absence of progression after first-line platinum-based chemotherapy;they would consider initiating it 2 - 10 weeks after completion of chemotherapy. Conclusions and Clinical Implications: After comparing with current international guidelines, this study shows that Lebanese oncologists follow international guidelines and have deep knowledge of recent clinical trials for the management of locally advanced metastatic urothelial carcinoma, regardless of economic crisis challenges.
