Long-term result of endoscopic Histoacryl （N-butyl-2-cyanoacrylate）injection for treatment of gastric varices

AIM:To evaluate the long-term efficacy and safety of endoscopic obliteration with Histoacryl  for treatment of gastric variceal bleeding and ***:Between January 1994 and March 2010 at SoonChunHyang University Hospital,a total of 127 patients with gastric varices received Histoacryl  injections *** hundred patients underwent endoscopic Histoacryl  injections because of variceal bleeding,the other 27 patients received such injections as a prophylactic ***:According to Sarin classification,56 patients were GOV1,61 patients were GOV2 and 10 patients were *** of the varices were large(F2 or F3,111 patients).The average volume of Histoacryl  per each session was 1.7±1.3 cc and mean number of sessions was 1.3±0.6.(1 session-98 patients,2 sessions-25 patients,≥3 sessions-4 patients).Twenty-seven patients with high risk of bleeding(large or fundal or RCS+or Child C) received Histoacryl  injection as a primary prophylactic *** these patients,hepatitis B virus was the major etiology of cirrhosis,25 patients showed GOV1 or 2(92.6%)and F2 or F3 accounted for 88.9%(n=24).The rate of initial hemostasis was 98.4%and recurrent bleeding within one year occurred in 18.1%of *** hemostasis during episodes of rebleeding was achieved in 73.9%of *** survival was 50 mo (95%CI 30.5-69.5).Major complications occurred in 4 patients(3.1%).The rebleeding rate in patients with hepatocellular carcinoma or GOV2 was higher than in those with other *** of the 27 subjects who were treated prophylactically experienced treatment-related *** survival rates of the 127 patients at 6 mo,1,3,and 5 years were 92.1%,84.2%,64.2%,and 45.3%,*** 6 mo cumulative survival rate of the 27 patients treated prophylactically was 75%.CONCLUSION:Histoacryl  injection therapy is an effective treatment for gastric varices and also an effective prophylactic treatment of gastric varices which

Energy crops play a vital role in meeting future energy and chemical demands while addressing climate ***,the idealization of lowcarbon workflows and careful consideration of cost-benefit equations are crucial for their more sustainable ***,we propose tobacco as a promising energy crop because of its exceptional water solubility,mainly attributed to a high proportion of water-soluble carbohydrates and nitrogen,less lignocellulose,and the presence of *** then designed a strategy that maximizes biomass conversion into bio-based products while minimizing energy and material *** autoclaving tobacco leaves in water,we obtained a nutrient-rich medium capable of supporting the growth of microorganisms and the production of bioproducts without the need for extensive pretreatment,hydrolysis,or additional ***,cultivating tobacco on barren lands can generate sufficient biomass to produce approximately 573 billion gallons of ethanol per *** approach also leads to a reduction of greenhouse gas emissions by approximately 76%compared to traditional corn stover during biorefinery ***,our study presents a novel and direct strategy that could significantly contribute to the goal of reducing carbon emissions and global sustainable development compared to traditional methods.

延世大学(韩国)牙科学院附属医院是一所大型私立医院,拥有200名牙科医师和256套牙科座椅。它以8个专科和日均1100个门诊病人而闻名于全国。我们医院已经拥有诸如医嘱通信系统(OCS)以及图像存档和通信系统(PACS),并且今年我们正计划实施电子病案(EMR)系统。实施一个高质量的电子病案系统,有四个要求:技术(硬件和软件),完善的临床术语分类(CTS),用户的满意和电子病案同旧纸张病案的有效衔接。本文试图描绘牙科医院电子病案中的临床术语数据库。我们收集了315份主诉(cc),594个诊断和1040个操作术语。然后利用医学系统命名法-临床术语类中的概念标识和描述标识对它们进行分析和一一对应。当牙科医师在电子病案系统里输入医学数据时,这些系统化的医学术语数据便被自动地存进数据库中及被日后使用。如果系统自动地为这些汇集的术语数据分类。就能为形形色色的用户检索临床数据提供可能性。本专业论文阐述了牙科医院电子病案中的术语数据的收集和分类过程。我们使用了国际疾病分类一第10版(ICD-10)和国际疾病分类-9-临床修订版(ICD-9-CM)来编码疾病和治疗过程,并且使用医学系统命名法-临床术语类(SNOMED-CT)来系统化地安排牙科临床术语。

Objective To examine the relationship of single-nucleotide polymorphisms(SNPs)in candidate genes with the lipid responses to *** Chinese patients were treated with simvastatin 40 mg daily for at least 6 weeks.20 SNPs in 11 genes were *** 95 patients age(mean±SD) 57.5±10.6 years completed the *** Adiponectin 45T>G polymorphism was significantly re- lated to absolute reductions in total cholesterol(TC)and LDL-cholesterol with a trend(P=0.053)for percentage reductions in TC(TT:TG:GG=-38.4%:-35.6%:-32.6%).Similar findings were seen with LDL-Receptor(LDLR)SNPs(2052T>C and 1866C>T)with absolute reductions in TC and LDL-cholesterol significantly *** Breast Cancer Resistance Protein(ABCG2)421C>A poly- morphism was related(Pcc:CA:AA=+0.50%:-5.73%:- 11.41%).Conclusions Adiponectin,LDLR and ABCG2 SNPs had some influence on the lipid responses to simvastatin.

Efficacy of Simple Assessment System of Oocyte Maturity in IVF-ET Cycles

Objective To establish and evaluate the efficacy of the simple assessment system of oocyte maturity. Methods A total of 251 couples were enrolled in this study and female patients were divided into two groups. In group Ⅰ, oocytes were inseminated at 6 h after ovum pick-up. In group Ⅱ, oocyte maturity was rapidly categorized by simple assessment system. Mature oocytes were inseminated at 3-4 h after ovum pick-up when oocyte-corona complexes （Occ） exhibited clear thick ring-like halo （RLH） and expanded cumulus cells （cc） or 5-6 h when Occ exhibited RLH and a few clumped and/or dark cc, respectively. Intermediate oocytes were inseminated at 7-8 h when RLH was not found around the Occ and cc were compacted and clumped and/or dark. Results Normal fertilization rate was higher in group Ⅱ （76.5%） than that in group Ⅰ （58.0%）（P〈0.001）. However, abnormal fertilization rate was higher in group Ⅰ（11.3%） than that in group 11 （3.6%）（P〈0.001）. The cleavage （82.6% vs 90.0%）, chemical pregnancy （4. 8% vs 3.9%）, twin pregnancy （6. 7% vs 3.9%） and implantation rate （8.4% vs 10.6%） had no statistically differences between group I and Ⅱ. Rate of clinical and singleton pregnancy was higher in groupⅡ （35.3% and 31.4%） than those in group Ⅰ （24.8% and 18.2%）（P〈0.05）. Conclusion This simple assessment system is useful and effective for evaluation and categorization of the oocyte maturity with better reproductive outcomes.

BACKGROUND Although cytoreductive surgery(CRS)and hyperthermic intraperitoneal chemotherapy(HIPEC)offer the potential for long-term survival in peritoneal carcinomatosis,outcomes following CRS/HIPEC vary *** To identify the clinical factors associated with progression-free survival(PFS)after complete CRS/HIPEC in patients with colorectal/high-grade appendiceal,ovarian,and gastric *** We retrospectively evaluated the risk of recurrence within 1 year after CRS/HIPEC and its impact on overall survival(OS)in patients recruited between 2015 and *** regression models were used to assess the prognostic factors for the risk of recurrence within 1 ***–Meier survival curves and Cox proportional hazards models were used to evaluate the association between recurrence and *** Of the 80 enrolled patients,39 had an unfavorable PFS(cc-0)or length of CRS≤6 h had a favorable PFS[odds ratio(OR)=0.141,P=0.004;and OR=0.361,P=0.027,respectively].In multiple logistic regression,achieving cc-0 was the strongest prognostic factor for a favorable PFS(OR=0.131,P=0.005).A peritoneal cancer index score>12 was associated with a lower rate of achieving cc-0(P=0.027).The favorable PFS group had a significantly longer OS(median 81.7 mo vs 17.0 mo,Pcc-0 was associated with a lower early recurrence rate and improved long-term *** study underscores the importance of selecting appropriate candidates for CRS/HIPEC to manage peritoneal carcinomatosis.

AIM:To examined the efficacy and safety of treatment with boceprevir,PEGylated-interferon and ribavirin(PR)in hepatitis C virus genotype 1(HCVGT1) PR treatmentfailures in ***:The Boceprevir Named-Patient Program provided boceprevir to HCVGT1 PR *** physicians were invited to contribute data from their patients:baseline characteristics,ontreatment responses,sustained virological response at week 12(SVR12),and safety were collected and *** analysis was performed to determine predictors of ***:150 patients were enrolled from Australia,Malaysia,Singapore and Thailand(Asians = 86,Caucasians = 63).Overall SVR12 was 61%(Asians= 59.3%,Caucasians = 63.5%).SVR12 was higher in relapsers(78%) compared with non-responders(34%).On-treatment responses predicted SVR,with undetectable HCVRNA at week 4,8 and 12 leading to SVR12 s of 100%,87%,and 82%respectively,and detectable HCVRNA at week 4,8 and 12,leading to SVR12 s of 58%,22%and 6%*** patients were similar to Caucasian patients with regards to on-treatment *** with cirrhosis(n= 69) also behaved in the same manner with regards to on-treatment *** with the IL28 B cc genotype(80%) had higher SVRs than those with the CT/TT(56%) genotype(P = 0.010).Multivariate analysis showed that TW8 and TW12 responses were independent predictors of *** adverse events occurred in 18.6%:sepsis(2%),decompensation(2.7%) and blood transfusion(14%).Discontinuations occurred in 30.7%,with 18.6%fulfilling stopping ***:Boceprevir can be used successfully in PR treatment failures with a SVR12 > 80%if they have good on-treatment responses;however,discontinuations occurred in 30%because of virological failure or adverse events.
