Surgeon preferences in the treatment of thumb carpometacarpal osteoarthritis

BACKGROUND Primary thumb carpometacarpal(CmC)osteoarthritis is one of the most common conditions encountered by hand surgeons.Of the vast number of operations that have been proposed,none have demonstrated results significantly superior to trapeziectomy alone.AIm The purpose of our study was to determine why surgeons opt for their technique in treating CmC arthritis.mETHODS A cross-sectional survey of active members of the American Society for Surgery of the Hand was conducted to evaluate the reasons behind their preferred technique in the treatment of isolated thumb CmC arthritis.Surgeons were contacted by email once and provided with a link to a de-identified survey consisting of 5 treatment questions and 5 demographic questions.RESULTS Of 950 responses were received.40.5%of surgeons preferred trapeziectomy+ligament reconstruction tendon interposition(LRTI),followed by trapeziectomy+suspensionplasty(28.2%),suture button suspension(5.9%),trapeziectomy alone(4.6%),prosthetic arthroplasty(3.2%),arthrodesis(1.1%),and other(6.6%).Proponents of trapeziectomy+LRTI cited familiarity(73.2%),exposure during fellowship(48.8%)and less proximal migration(60%)to be the main reasons affecting their decision.Surgeons who preferred trapeziectomy+suspensionplasty most reported simplicity(74.9%),fewer complications(45.3%),less proximal migration(43.8%),and avoidance of autogenous tissue harvest(42.7%).Advocates of suture button suspension cited avoidance of autogenous tissue harvest(80.4%),shorter immobilization(76.8%),and quicker recovery(73.2%)with their technique.Advocates of trapeziectomy alone cited simplicity(97.7%),fewer complications(86.4%),and avoidance of autogenous tissue harvest(59.1%).In their comments,45%of surgeons choosing trapeziectomy alone cited evidence as an additional rationale.Advocates of prosthetic arthroplasty cited improved pinch strength(83.3%)and improved range of motion(63.3%),while those preferring arthrodesis cited better pinch strength(90%)and frequently in their comments,durability.Of the surgeon

Die filling is a critical stage during powder compaction,which can significantly affect the product quality and efficiency.In this paper,a forced feeder is introduced attempting to improve the filling performance of a lab-scale die filling system.The die filling process is analysed with a graphics processing units(GPU)enhanced discrete element method(DEm).Various stirrer designs are assessed for a wide range of process settings(i.e.,stirrer speed,filling speed)to explore their influence on the die filling performance of free-flowing powder.Numerical results show that die filing with the novel helical-ribbon(i.e.,type D)stirrer design exhibits the highest filling ratio,implying that it is the most robust stirrer design for the feeder configuration considered.Furthermore,die filling performance with the type D stirrer design is a function of the stirrer speed and the filling speed.A positive variation of filling ratio(ηf>0%)can be ensured over the whole range of filling speed by adjusting the stirrer speed(i.e.,increasing the stirrer speed).The approach used in this study can not only help understand how the stirrer design affects the die filling performance but also guide the optimization of feeder system and process settings.

随着记忆研究在近年的兴起,中文学界出现了“历史记忆”这一名称,含有将两者等同的意思。与之相对照,历史和记忆在西方学术中一直存在某种张力——史学在西方的诞生与对抗记忆的目的相关,之后历史和记忆长期处于一种分离的关系。以兰克为代表的近代科学史学,其理论前提是区别“过去”与“现在”,并从后者的立场考察、重构前者,而记忆则常常将两者混为一体。二战之后,兰克史学面临一系列挑战,记忆研究的兴盛便是其中之一。从莫里斯·阿布瓦赫写作《集体记忆》到皮埃尔·诺拉主编《记忆之场》,西方学者试图用记忆来补充和修正历史研究和书写的传统,改进和扩大历史学的方法和范围,促成其内容的更新和革新。但历史研究和记忆研究在近年的合流,仍没有改变两者之间的根本差异。

The majority of inherited retinal degenerative diseases and dry age-related macular degeneration are characterized by decay of the outer retina and photoreceptors,which leads to progressive loss of vision.The inner retina,including second-and third-order retinal neurons,also shows aberrant structural changes at all stages of degeneration.müller glia,the major glial cells maintain retinal homeostasis,activating and rearranging immediately in response to photoreceptor stress.These phenomena are collectively known as retinal remodeling and are anatomically well described,but their impact on visual function is less well characterized.Retinal remodeling has traditionally been considered a detrimental chain of events that decreases visual function.However,emerging evidence from functional assays suggests that remodeling could also be a part of a survival mechanism wherein the inner retina responds plastically to outer retinal degeneration.The visual system’s first synapses between the photoreceptors and bipolar cells undergo rewiring and functionally compensate to maintain normal signal output to the brain.Distinct classes of retinal ganglion cells remain even after the massive loss of photoreceptors.müller glia possess the regenerative potential for retinal recovery and possibly exert adaptive transcriptional changes in response to neuronal loss.These types of homeostatic changes could potentially explain the well-maintained visual function observed in patients with inherited retinal degenerative diseases who display prominent anatomic retinal pathology.This review will focus on our current understanding of retinal neuronal and müller glial adaptation for the potential preservation of retinal activity during photoreceptor degeneration.Targeting retinal self-compensatory responses could help generate universal strategies to delay sensory disease progression.

The aging population is an important issue around the world especially in developed countries.Although med-ical advances have substantially extended life span,the same cannot be said for the duration of health span.We are seeing in-creasing numbers of elderly people who are frail and/or have multiple chronic conditions;all of these can affect the quality of life of the elderly population as well as increase the burden on the healthcare system.Aging is mechanistically related to common medical conditions such as diabetes mellitus,ischemic heart disease,cognitive decline,and frailty.A recently accepted concept termed‘Accelerated Biological Aging’can be diagnosed when a person’s biological age—as measured by biomarkers of DNA methylation—is older than their corresponding chronological age.Taurine,a conditionally essential amino acid,has received much attention in the past few years.A substantial number of animal studies have provided a strong scientific foundation sug-gesting that this amino acid can improve cellular and metabolic health,including blood glucose control,so much that it has been labelled one of the‘longevity amino acids’.In this review article,we propose the rationale that an adequately powered random-ized-controlled-trial(RCT)is needed to confirm whether taurine can meaningfully improve metabolic and microbiome health,and biological age.This trial should incorporate certain elements in order to provide the much-needed evidence to guide doctors,and also the community at large,to determine whether this promising and inexpensive amino acid is useful in improving human metabolic health.

An intense laser pulse focused onto a plasma can excite nonlinear plasma waves.Under appropriate conditions,electrons from the background plasma are trapped in the plasma wave and accelerated to ultra-relativistic velocities.This scheme is called a laser wakefield accelerator.In this work,we present results from a laser wakefield acceleration experiment using a petawatt-class laser to excite the wakefields as well as nanoparticles to assist the injection of electrons into the accelerating phase of the wakefields.We find that a 10-cm-long,nanoparticle-assisted laser wakefield accelerator can generate 340 pC,10±1.86 GeV electron bunches with a 3.4 GeV rms convolved energy spread and a 0.9 mrad rms divergence.It can also produce bunches with lower energies in the 4–6 GeV range.

Prior research establishing that bone interacts in coordination with the bone marrow microenvironment(BmmE)to regulate hematopoietic homeostasis was largely based on analyses of individual bone-associated cell populations.Recent advances in intravital imaging has suggested that the expansion of hematopoietic stem cells(HSCs)and acute myeloid leukemia cells is restricted to bone marrow microdomains during a distinct stage of bone remodeling.These findings indicate that dynamic bone remodeling likely imposes additional heterogeneity within the BmmE to yield differential clonal responses.A holistic understanding of the role of bone remodeling in regulating the stem cell niche and how these interactions are altered in age-related hematological malignancies will be critical to the development of novel interventions.To advance this understanding,herein,we provide a synopsis of the cellular and molecular constituents that participate in bone turnover and their known connections to the hematopoietic compartment.Specifically,we elaborate on the coupling between bone remodeling and the BmmE in homeostasis and age-related hematological malignancies and after treatment with bone-targeting approaches.We then discuss unresolved questions and ambiguities that remain in the field.

A metastableβTi alloy was additively manufactured by laser powder bed fusion(LPBF).Tensile testing along the build direction of the as-LPBF material(LPBF-0°)revealed signifcant work softening immediately following yielding with no uniform deformation.By contrast,substantial work hardening and uniform elongation well over 10%were achieved perpendicular to the build direction(LPBF-90°).Similar effects were obtained in the build direction after super transus heat treatment(LPBF-0°+HT)although the strength was slightly lowered.In addition,the yield drop phenomenon observed in both orientations of the as-LPBF materials disappeared after HT.The enhanced work hardening ability,and thus ductility,can be attributed to increased interactions of slip bands/slip bands owing to additional{112}slip systems becoming operative in LPBF-0°+HT and LPBF-90°while LPBF-0°was dominated by{110}only.The other variations after HT may be related to the coarsening of grain structure and removal of specifc substructures in the as-LPBF microstructure.
