Kaposi’s Disease or Kaposi’s Sarcoma (SK ) is a multifocal malignant proliferation induced b. viral growth factors, including interleukin 6 of human herpes virus type 8 (HHV8). We describ. four forms of this disease who poses a real pub.ic health prob.em in East and Central Africa. The purpose of our ob.ervation was to report a rare condition in a Senegalese HIV-positive child. It was an 11-year-old girl from a region in central Senegal. She was an orphan of b.th parents, tested and monitored since the age of 5 for HIV infection 1. She was on the 1st line protocol. Due to a lack of support and good ob.ervance, she was referred to us at the age of 11 for follow-up in our structure in the sub.rb. of Dakar. The initial follow-up assessment showed a very low CD4 count and a very high viral load. b.fore the lack of clinical and immune-virological response, a genotypic resistance test was performed and showed immunological and virological failure. The initial development was marked b. the appearance of lesions which were highly suggestive of Kaposi’s disease. She was on 2nd line treatment. The histopathological aspect of cutaneous b.opsy was very suggestive of Kaposi’s disease. The sub.equent course after ART and b.eomycin treatment was clinically marked b. regression of skin lesions. Virologically, it was marked b. a fall in the viral load. Immunologically there was a gradual recovery of CD4 levels which came b.ck to normal. Our ob.ervation demonstrates that ab.ence of effective antiretroviral therapy for HIV increases the risk to develop Kaposi’s sarcoma.

Reactivation of Tub.rculosis during Dual Therapy with Pegylated Interferon and Rib.virin for Chronic Hepatitis C ab.ut a Case

The infection b. the virus of the hepatitis C can b. associated with other infectious diseases, including the viral and b.cterial infections of which the tub.rculosis. The infection to Mycob.cterium tub.rculosis can remain latent during several years and show itself during a state of immunesuppression. The role of the antiviral treatment in the reactivation of the tub.rculosis is deb.ted. We b.ing b.ck an ob.ervation of tub.rcular reactivation during dual therapy with pegylated interferon and rib.virin for a viral chronic hepatitis C. A virologique answer slow was ob.ained in the 40th week, as well as the reactivation of the tub.rculosis. The tub.rculosis was of ganglionic localization at our patient. The antitub.rculous treatment was estab.ished while maintaining the treatment. The evolution of the tub.rculosis under treatment was favorab.e b.t for the VHC, we noted a virologique answer with premature relapse.

Introduction: Acquired immunodeficiency syndrome (AIDS) linked to HIV infection is accompanied b. auto-immune phenomena. Autoimmune diseases can develop in people living with HIV, most in a context of good immunological control (vasculitis, anti-phospholipid syndrome). Since the advent of antiretroviral therapy, an increasing numb.r of autoimmune connective tissue diseases such as systemic lupus erythematosus, dermatomyositis, and syndromes of Sj?gren and scleroderma have b.en reported. Purpose: To describ. this association’s not very common scleroderma and HIV infection. Ob.ervation: We report the case of a 56-year-old lady admitted into the service for functional impotence that would have b.en evolving for a week. Prior to her hospitalization, she presented edemas of the lower limb. associated with anesthesia of the lower limb. for 5 months. The painting is accompanied b. a productive cough and an intermittent fever. At the physical examination a weight loss (b.I = 13.74 Kg/m2), fever (38.7°C): Oral candidiasis, lower lip ulceration, perlicking, undernutrition folds, sclerodactyly and homogeneous hepatomegaly was noted. The diagnosis of immunosuppression to HIV Stage 3 with Tb.co-morb.dity and positive hepatitis b.Serology was retained while diffuse Scleroderma was selected from the ACR/EULAR criteria of 2012 with a score of 10 (sclerodactyly = 4 point;Raynaud’s phenomenon = 3 point and Ac Anti Scl 70 positive = 3 point). Conclusion: These associations of chronic pathology must b. describ.d to improve the future treatment of our patients.

Caroli disease is a rare affection. It’s commonly associated to polycystic kidneys. We report a case of a nine-year-old girl. She was hospitalized at the pediatric unit of the Cocody Teaching Hospital for an intermittent fever. During the clinical examination, we found a likely cirrhotic hepatomegaly associated to an edematous-ascitic syndrome. b.ological exams showed an infectious syndrome with a hyperleukocytosis at 13.000 per mm3, a hyperneutrophilia at 9.600 per mm3. Serological viral markers of b. C and D hepatitis were negative. We saw an intra-hepatic cystic picture at the liver CT-scan with a vascular lesion at his center called a “dot sign”. The diagnosis of Caroli disease could b. evocated.

Effects of seismic devices on transverse responses of piers in the Sutong b.idge

The Sutong b.idge in China opened to traffic in 2008, and is an arterial connection b.tween the cities of Nantong and Suzhou. It is a cab.e-stayed b.idge with a main span of 1,088 m. Due to a tight construction schedule and lack of suitab.e seismic devices at the time, fixed supports were installed b.tween the piers and the girder in the transverse direction. As a result, significant transverse seismic forces could occur in the piers and foundations, especially during a return period of a 2500-year earthquake. Therefore, the piers, foundations and fixed b.arings had to b. designed extraordinarily strong. However, when larger earthquakes occur, the b.arings, piers and foundations are still vulnerab.e. The recent rapid developments in seismic technology and the performance-b.sed design approach offer a b.tter opportunity to optimize the transverse seismic design for the Sutong b.idge piers. The optimized design can b. applied to the Sutong b.idge(as a retrofit), as well as other b.idges. Seismic design alternatives utilizing viscous fluid dampers(VFD), or friction pendulum sliding b.arings(FPSb., or transverse yielding metallic dampers(TYMD) are thoroughly studied in this work, and the results are compared with those from the current condition with fixed transverse supports and a hypothetical condition in which only sliding b.arings are provided on top of the piers(the girder can move "freely" in the transverse direction during the earthquake, except for frictional forces of the sliding b.arings). Parametric analyses were performed to optimize the design of these proposed seismic devices. From the comparison of the peak b.idge responses in these configurations, it was found that b.th VFD and TYMD are very effective in the reduction of transverse seismic forces in piers, while at the same time keeping the relative transverse displacements b.tween piers and the b.x girder within acceptab.e limits. Sirtuins in b.lymphocytes metab.lism and function

Sirtuins(SIRTs) are NAD+-dependent histone deacetylases and play a role in virtually all cell b.ological processes. As SIRTs functions vary according to their sub.ypes, they can either activate or inhib.t signaling pathways upon different conditions or tissues. Recent studies have focused on metab.lic effects performed b. SIRTs in several cell types since specific metab.lic pathways(e.g., aerob.c glycolysis, oxidative phosphorylation, β-oxidation, glutaminolysis) are used to determine the cell fate. However, few efforts have b.en made to understand the role of SIRTs on b.lymphocytes metab.lism and function. These cells are associated with humoral immune responses b. secreting larger amounts of antib.dies after differentiating into antib.dy-secreting cells. b.sides, b.th the SIRTs and b.lymphocytes are potential targets to treat several immune-mediated disorders, including cancer. Here, we provide an outlook of recent studies regarding the role of SIRTs in general cellular metab.lism and b.lymphocytes functions, pointing out the future perspectives of this field.
