Objective we sought to assess the utilization and strategy of intracardiac echocardiography(ICE) during noncoronary Cardiac Interventions,to evaluate the feasibility and accuracy guided with and without the help of ICE. Methods A consecutive series of 46 patients were selected from the cardiology department of the first affiliated hospital in Kunming Medical College,who underwent transcatheter closure of interatrial communications,ablations of supraventricular tachycardia and implantations of pacemakers through jugular with ICE(groupl:23patients,12men,llwomen,45.78±21.54 years) and without ICE(group2: 23patients,12men,11 women,49.22±24.79years).We measured the fluoroscopy time and the total procedure time,compared the difficulty of the procedure,locations of the ablation catheters and the complications between the two groups,assessed the feasibility,safety of using ICE for the guidance of noncoronary cardiac interventions.A level of statistical significance was set to P23 patients with the ICE guidance in group land the another 23 without ICE were completed the whole procedure,and none sever complements or damage of cardiac structure was observed,②During the procedure for closure of ASD,the diameter measured with ICE and TEE were correlated with each *** predict diameter of ASD with ICE was trended to be larger(by 2～4mm) than the measurement.③During the ablation procedure,ICE could provide imaging e of the movement and position of the sheaths and catheters,guide the transseptal puncture,monitor the damage to adjacent structure,improve the whole procedure.④During the implant procedure,ICE could provide the visualization images of the movement of the cathodes,embedment of screw-in electrodes.⑤The total fluoroscopy time in patients guided with ICE were shorter than those guided with TEE:(13.87±5.63) min versus(25.09±10.32) min. Glycine blunts transplantative liver ischemia-reperfusion injury by downregulating interleukin 1 receptor associated kinase-4

Aim:To determine whether glycine could downregulate interleukin 1 receptorassociated kinase-4(IRAK-4)expression to interfere with lipopolysaccharides(LPS)signal transduction and blunt transplantative liver ischemia-reperfusioninjury(I/RI).Methods:SD rats were randomly divided into two groups:donoranimals of the glycine group(n=40)were given glycine(1.5 mL;300 mmol/L,iv)1h before harvest,and the control group were treated with 1.5 mL physiologicalsaline(n=40).Orthotropic liver transplantation was then performed according tothe Kamada *** animals in each group were followed up for 7 d aftersurgery to assess *** remaining animals in each group were dividedinto 3 subgroups(n=10)at 1h,2h and 6 h after portal vein *** ofLPS,serum aspartate aminotransferase(AST),alanine aminotransferase(ALT)and total bilirubin in portal circulation,as well as IRAK-4 and TNF-α expression,NF-κB transcriptional activity and morphological study of liver tissues ***:Reperfusion resulted in a significant elevation of LPS concen-trations in each group persisting to the end of our ***,glycine,whichled to improved survival rate and liver function,significantly alleviated liver pa-renchyma cell damage by downregulating 1RAK-4,TNF-α expression and NF-κBtranscriptional activity compared with the control ***:Glycinecan attenuate hepatic I/RI by downregulating IRAK-4 to interfere with LPS signaltransduction.

AIM: To evaluate the relationship between the expression of lipopolysaccharides (LPS) binding protein (LBP) and CD14 mRNA and the severity of liver injury in alcohol-fed rats. METHODS: Twenty Wistar rats were divided into two groups:ethanol-fed group (group E) and control group (group C). Group E was fed with ethanol(5-12 g x kg(-1) x d(-1)) and group C received dextrose instead of ethanol. Rats of the two groups were sacrificed at 4 weeks and 8 weeks. Levels of endotoxin and alanine transaminase (ALT) in blood were measured, and liver pathology was observed under light and electronic microscopy. Expressions of LBP and CD14 mRNA in liver tissues were determined by RT-PCR analysis. RESULTS: Plasma endotoxin levels were increased more significantly in group E(129+/-21) ng x L(-1) and (187+/-35) ng x L(-1) at 4 and 8 wk than in control rats(48+/-9) ng x L(-1) and (53+/-11) ng x L(-1), respectively (P250) nkat x L(-1) and (2450+/-367) nkat x L(-1) in Group E. The values were increased more dramatically in ethanol-fed rats than in Group C after 4 and 8 weeks. In liver section from ethanol-fed rats, there were marked pathological changes (steatosis, cell infiltration and necrosis). In ethanol-fed rats, ethanol administration led to a significant increase in LBP and CD14 mRNA levels compared with the control group (P<0.05). CONCLUSION: Ethanol administration led to a significant increase in endotoxin levels in serum and LBP and CD14 mRNA expressions in liver tissues. The increase of LBP and CD14 mRNA expression might wake the liver more sensitive to endotoxin and liver injury.

Cancer invasion and metastasis, involving a variety of pathological processes andcytophysiological changes,contribute to the high mortality of lung *** type 1 insulin-like growthfactor receptor (IGF-1R),associated with cancer progression and invasion,is a potential anti-invasion andanti-metastasis target in lung *** inhibit the invasive properties of lung cancer cells,we successfullydown-regulated IGF-1R gene expression in A549 human lung cancer cells by small interfering RNA (siRNA)technology,and evaluated its effects on invasion-related gene expression,tumor cell in vitro invasion,andmetastasis in xenograft nude mice.A549 cells transfected with a plasmid expressing hairpin siRNA forIGF-1R showed a significantly decreased IGF-1R expression at the mRNA level as well as the *** biological assays,transfected A549 cells showed a significant reduction of cell-matrix adhesion,migration and *** with these results,we found that down-regulation of IGR-1Rconcomitantly accompanied by a large reduction in invasion-related gene expressions,including MMP-2,MMP-9,u-PA,and IGF-1R specific downstream *** tail vein injections of plasmid expressinghairpin siRNA for IGF- 1R significantly inhibited the formation of lung metastases in nude *** resultsshowed the therapeutic potential of siRNA as a method for gene therapy in inhibiting lung cancer invasionand metastasis.

Chiral metabolism of propafenone in rat hepatic microsomes treated with two inducers

AIM: To study the influence of inducers of drug metabolism enzyme, beta-naphthoflavone (BNF) and dexamethasone (DEX), on the stereoselective metabolism of propafenone in the rat hepatic microsomes. METHODS: Phase I metabolism of propafenone was studied using the microsomes induced by BNF and DEX and the non-induced microsome was used as the control. The enzymatic kinetics parameters of propafenone enantiomers were calculated by regress analysis of Eadie-Hofstee Plots. Propafenone enantiomer concentrations were assayed by a chiral HPLC. RESULTS: The metabolite of propafenone, N-desalkylpropafenone, was found after incubation of propafenone with the rat hepatic microsomes induced by BNF and DEX. In these two groups, the stereoselectivity favoring R(-) isomer was observed in metabolism at low substrate concentrations of racemic propafenone, but lost the stereoselectivity at high substrate concentrations. However, in control group, no stereoselectivity was observed. The enzyme kinetic parameters were: (1) K(m). Control group: R(-) 83+/-6, S(+) 94+/-7; BNF group: R(-) 105+/-6, S(+)128+/-14; DEX group: R(-) 86+/-11, S(+) 118+/-16; (2)V(max). Control group: R(-) 0.75+/-0.16, S(+) 0.72+/-0.07; BNF group: R(-) 1.04+/-0.15, S(+)1.07+/-14; DEX group: R(-) 0.93+/-0.06, S(+) 1.04+/-0.09; (3)Cl(int). Control group: R(-) 8.9+/-1.1, S(+) 7.6+/-0.7; BNF group: R(-) 9.9+/-0.9, S(+)8.3+/-0.7; DEX group: R(-) 10.9+/-0.8, S(+) 8.9+/-0.9. The enantiomeric differences in K(m) and Cl(int) were both significant, but not in V(max), in BNF and DEX group. Whereas enantiomeric differences in three parameters were all insignificant in control group. Furthermore, K(m) and V(max) were both significantly less than those in BNF or DEX group. In the rat liver microsome induced by DEX, nimodipine (NDP) decreased the stereoselectivity in propafenone metabolism at low substrate concentration.
