AIM: To describe a population of outpatients in China infected by hepatitis B virus (HBV) and/or hepatitis C virus (HCV), and assess their current management status. METHODS: A multicenter, cross-sectional study of HBV- and/or HCV-infected patients was conducted from August to November, 2011 in western China. Patients >= 18 years of age with HBV and/or HCV infections who visited outpatient departments at 10 hospitals were evaluated, whether treated or not. Data were collected on the day of visit from medical records and patient interviews. RESULTS: A total 4010 outpatients were analyzed, including 2562 HBV- infected and 1406 HCV-infected and 42 HBV/HCV co-infected patients. The median duration of documented infection was 7.5 years in HBV- infected and 1.8 years in HCV-infected patients. Cirrhosis was the most frequent hepatic complication (12.2%), appearing in one-third of patients within 3 years prior to or at diagnosis. The HCV genotype was determined in only 10% of HCV-infected patients. Biopsy data were only available for 54 patients (1.3%). Antiviral medications had been received by 58.2% of patients with HBV infection and 66.6% with HCV infection. Nucleos(t) ide analogs were the major antiviral medications prescribed for HBV- infected patients (most commonly adefovir dipivoxil and lamivudine). Ribavirin + pegylated interferon was prescribed for two-thirds of HCV-infected patients. In the previous 12 mo, around one-fifth patients had been hospitalized due to HBV or HCV infection. CONCLUSION: This observational, real-life study has identified some gaps between clinical practice and guideline recommendations in China. To achieve better health outcomes, several improvements, such as disease monitoring and optimizing antiviral regimens, should be made to improve disease management. Acute pathological changes of facial nucleus and expressions of postsynaptic density protein-95 following facial nerve injury of varying severity A semi-quantitative analysis

BACKGROUND： Previous studies have demonstrated that postsynaptic density protein-95 （PSD-95） is widely distributed in the central nervous system and is related to the development of the CNS and sensory signal transmission as well as acute or chronic nerve cell death following ischemic brain injury. OBJECTIVE： To semi-quantitatively determine the pathological changes of apoptotic facial neurons and the expression of PSD-95 in the facial nucleus following facial nerve injury of varying extents using immunohistochemical staining methods. DESIGN, TIME AND SETTING： Randomized, controlled animal experiments were performed in the Ultrasonic Institute of the second affiliated hospital of chongqing university of medical Sciences from September to December 2007. MATERIALS： Sixty-five healthy, adult, Sprague-Dawley （SD） rats, both male and female, were used for this study. Rabbit anti-rat PSD-95 polyclonal antibody was purchased from Beijing Biosynthesis Biotechnology Co., Ltd. METHODS： SD rats were randomly assigned into a control group with five rats and three injured groups with 20 rats per group. Exposure, clamp and cut for bilateral facial nerve trunks were performed in the rats of the injury groups, and no injury was inflicted on the rats of the control group. MAIN OUTCOME MEASURES; The brainstems of all the rats were excised on days 1, 3, 7, and 14 post injury, and then the facial nuclei were stained with hematoxylin-eosin to observe any pathological changes due to apoptosis in facial neurons. PSD-95 expression in facial nuclei was detected by immunohistochemistry and the number of PSD-95 positive cells was counted under a light microscope. RESULTS： The expression of PSD-95 in the facial nucleus and morphology of the facial neuron within the exposure group had no obvious changes at various points in time tested （P 〉 0.05）. Colonic pouch confers better bowel function and similar postoperative outcomes compared to straight anastomosis for low rectal cancer

Role of international normalized ratio in nonpulmonary sepsis screening:An observational study

Currently,there is a lack of sepsis screening tools that can be widely used *** sepsis can be of sufficient concern to physicians due to their noticeable symptoms,which usually rely less on screening *** To investigate the efficiency of the international normalized ratio(INR)for the early rapid recognition of adult nonpulmonary infectious *** This is a prospective observational study.A total of 108 sepsis patients and 106 nonsepsis patients were enrolled according to relevant inclusion and exclusion *** used clinical indicators,such as white blood cell,neutrophil count,lymphocyte count,neutrophil-lymphocyte count ratio(NLCR),platelets(PLT),prothrombin time,INR,activated partial thromboplastin time,and quick Sequential“Sepsis-related”Organ Failure Assessment(qSOFA)scores were recorded within 24 h after *** diagnostic performances of these clinical indicators were analyzed and compared through multivariate logistic regression analysis,Spearman correlation,and receiver operating characteristic curve *** INR value of the sepsis group was significantly higher than that of the nonsepsis *** has superior diagnostic efficacy for sepsis,with an area under the curve value of 0.918,when those preexisting diseases which significantly affect coagulation function were *** diagnostic efficacy of the INR was more significant than that of NLCR,PLT,and qSOFA(P<0.05).Moreover,INR levels of 1.17,1.20,and 1.22 could be used to categorize the relative risk of nonpulmonary infections sepsis into three categories:low,medium and high risk,*** The INR is a promising and easily available biomarker for diagnosis,and it can be used as one of the indicators for early screening of adult nonpulmonary infectious *** its value is higher than the optimal cutoff value(1.22),high vigilance is required for adult nonpulmonary infectious sepsis.

Transcriptomic profiling of peroxisome-related genes reveals a novel prognostic signature in hepatocellular carcinoma

Emerging evidence suggests that peroxisomes play a role in the regulation of tumorigenesis and cancer ***,the prognostic value of peroxisome-related genes has been rarely *** study aimed to establish a peroxisome-related gene signature for overall survival(OS)prediction in patients with hepatocellular carcinoma(HCC).First,univariate Cox regression analysis was employed to identify prognostic peroxisome-related genes in The Cancer Genome Atlas liver cancer cohort,and least absolute shrinkage and selection operator Cox regression analysis was used to construct a 10-gene *** risk score based on the signature was positively correlated with poor prognosis(HR=4.501,95%CI=3.021–6.705,P=1.39e−13).second,multivariate Cox regression incorporating additional characteristics revealed that the signature was an independent ***-dependent ROC curves demonstrated good performance of the signature in predicting the OS of HCC *** prognostic performance was validated using International Cancer Genome Consortium HCC cohort *** set enrichment analysis revealed that the signature-related alterations in biological processes mainly involved peroxisomal ***,we developed a nomogram model based on the gene signature and TNM stage,which showed a superior prognostic power(C-index=0.702).Thus,our study revealed a novel peroxisome-related gene signature that may help improve personalized OS prediction in HCC patients.

Glycine blunts transplantative liver ischemia-reperfusion injury by downregulating interleukin 1 receptor associated kinase-4

Aim:To determine whether glycine could downregulate interleukin 1 receptorassociated kinase-4(IRAK-4)expression to interfere with lipopolysaccharides(LPS)signal transduction and blunt transplantative liver ischemia-reperfusioninjury(I/RI).Methods:SD rats were randomly divided into two groups:donoranimals of the glycine group(n=40)were given glycine(1.5 mL;300 mmol/L,iv)1h before harvest,and the control group were treated with 1.5 mL physiologicalsaline(n=40).Orthotropic liver transplantation was then performed according tothe Kamada *** animals in each group were followed up for 7 d aftersurgery to assess *** remaining animals in each group were dividedinto 3 subgroups(n=10)at 1h,2h and 6 h after portal vein *** ofLPS,serum aspartate aminotransferase(AST),alanine aminotransferase(ALT)and total bilirubin in portal circulation,as well as IRAK-4 and TNF-α expression,NF-κB transcriptional activity and morphological study of liver tissues ***:Reperfusion resulted in a significant elevation of LPS concen-trations in each group persisting to the end of our ***,glycine,whichled to improved survival rate and liver function,significantly alleviated liver pa-renchyma cell damage by downregulating 1RAK-4,TNF-α expression and NF-κBtranscriptional activity compared with the control ***:Glycinecan attenuate hepatic I/RI by downregulating IRAK-4 to interfere with LPS signaltransduction.

Identification of blood-brain barrier function following subarachnoid hemorrhage in rats at different stages

BACKGROUND： Recent studies have indicated that blood-brain barrier （BBB） disruption following subarachnoid hemorrhage （SAH） significantly correlates with the development of brain injury and poor prognosis of patients subjected to SAH. OBJECTIVE： To investigate both functional and structural changes related to BBB in various phases after SAH in rats through quantitative and qualitative methods. DESIGN, TIME AND SETTING： This experiment, a completely randomized design and controlled experiment, was performed at the Department of Neurosurgery, the second affiliated hospital of chongqing university of medical Sciences from June 2006 to March 2007. MATERIALS： A total of 128 female, healthy, Sprague-Dawley rats were selected for this study. Main reagents and instruments： Evans Blue dye （Sigma Company, USA）, fluorescence spectrophotometer （Shimadzu Company, Japan）, and transmission electron microscope （Olympus Company, Japan）. METHODS： The included 128 rats were randomly divided into two groups： sham-operated group （n = 16） and SAH group （n = 112）. Rats in the SAH group were divided into seven subgroups： 6, 12, 24, 36, 48, 60, and 72 hours after SAH （16 rats for each time point）. Experimental SAH was induced by blood injection into the pre-chiasmatic cistern （300 μ L）. The sham-operated group received an equivalent volume of normal saline solution （300 μ L） injected into the subarachnoid space. MAIN OUTCOME MEASURES： Brain tissue water content was determined by the wet-dry method. BBB permeability in the cerebral cortex was determined by Evans Blue dye and fluorescent spectrophotometer. The ultrastructural changes in BBB were observed with transmission electron microscope. RESULTS： Compared with the sham-operated group, SAH induced a significant increase in brain water content between 24 and 60 hours （F = 888.32, P 〈 0.05）. Brain water content increased to a maximum by 36 hours after SAH, normalizing by 72 hours. Evans Blue content in the cerebral cortex of SAH group rats
