Type 1 diabetes mellitus is heterogeneous in many facets. The patients suffered from type 1 diabetes present several levels of islet function as well as variable number and type of islet-specific autoantibodies. This study was to investigate prevalence and heterogeneity of the islet autoantibodies and clinical phenotypes of type 1 diabetes mellitus; and also discussed the process of islet failure and its risk factors in Chinese type 1 diabetic patients. A total of 1,291 type 1 diabetic patients were enrolled in this study. Demographic information was collected. Laboratory tests including mixed-meal tolerance test, human leukocyte antigen alleles, hemoglobin A1 c, lipids, thyroid function and islet autoantibodies were conducted. The frequency of islet-specific autoantibody in newly diagnosed T1 DM patients(duration shorter than half year) was 73% in East China. According to binary logistic regressions, autoantibody positivity, longer duration and lower Body Mass Index were the risk factors of islet failure. As the disease developed, autoantibodies against glutamic acid decarboxylase declined as well as the other two autoantibodies against zinc transporter 8 and islet antigen 2. The decrease of autoantibodies was positively correlated with aggressive beta cell destruction. Autoantibodies can facilitate the identification of classic T1 DM from other subtypes and predict the progression of islet failure. As there were obvious heterogeneity in autoantibodies and clinical manifestation in different phenotypes of the disease, we should take more factors into consideration when identifying type 1 diabetes mellitus.

蝙蝠是许多人畜共患病病毒的储备宿主。但是关于个体蝙蝠内病毒的多样性和丰度的了解还较少,因此它们之间病毒共感染和溢出的频率也不清楚。我们用无偏的元转录组学方法,对中国云南省采样的149只个体蝙蝠中携带的哺乳动物相关病毒进行了表征。结果显示研究的动物中病毒的高频共感染(即蝙蝠个体同时感染多种病毒物种)和溢出,这可能进一步促进病毒的重组和再组合。

Background Off-label application of drug-eluting stents （DES） during percutaneous coronary intervention （PCI） was not uncommon in daily practice, however DES in treating Chinese patients with complex lesion subset was under-investigated. The primary objective of the FIREMAN registry was to evaluate the long term efficacy and safety of the Firebird sirolimus-eluting stent （SES） in treating patients with complex coronary lesions. Here we report the mid-term of one-year clinical outcomes and eight-month angiographic follow-up results of FIREMAN *** The FIREMAN registry was a prospective multi-center registry, which included 1029 consecutive patients undergoing PCI with Firebird SES implantation between September 2006 and July 2007 in 45 centers in China. The clinical follow-up was designed to be performed at 1, 6, 12, 18, 24, 30 and 36 months post index procedure, and non-mandatory angiographic follow-up at 8 months was planned. One hundred percent site monitoring was *** Long lesions （59.2%）, multi-vessel disease （50.4%）, and small vessel disease （31.6%） were mostly found in angiography. Major adverse cardiac events （MACE） occurred in 51 （5.1%） patients at 1 year clinical follow-up,including cardiac mortality in 6 （0.6%）, non-fatal myocardial infarction in 11 （1.1%）, and target lesion revascularization in 36 （3.5%） of the patients. Definite and probable stent thrombosis （ST） by academic Research Consortium （ARC） definition occurred in 12 （1.36%） patients at one-year clinical follow-up. Soy isoflavones are natural tyrosine kinase inhibitors closely associated with decreased morbidity and mortality of various *** activation of tyrosine kinases such as ERBB2 is the mechanism by which cholecystitis transforms into gallbladder cancer(GBC),therefore,it is important to investigate the relationship between long-term exposure to soy isoflavones and the occurrence and progression of *** case-control study(n=85 pairs)found that the high level of plasma soy isoflavoneDgenistein(GEN)was associated with a lower risk of gallbladder cancer(≥326.00 ng/m L compared to≤19.30ng/m L,crude odds ratio 0.15,95%CI 0.04–0.59;P for trend=0.016),and that the level of GEN exposure negatively correlated with Ki67 expression in GBC tissue(n=85).Consistent with these results,the proliferation of GBC cells was inhibited in the long-term exposure models of GEN in vitro and in *** long-term exposure to GEN reduced the tyrosine kinase activity of ERBB2 and impaired the function of the PTK6-AKT-GSK3βaxis,leading to downregulation of the MCM complex in GBC *** summary,long-term exposure to GEN associated with soy products intake might play a certain role in preventing GBC and even inhibiting the proliferation of GBC cells.

Recently, fragile topology [1–11] and its important role in the properties of twisted bilayer graphene [8], non-Abelian topology[9], flat bands [10] and related emergent phases have attracted much attention, and led to discoveries that connect band topology with correlated electron states [11,12] such as superconductivity[12]. Interestingly, the band topology can impose constraints on the superconducting order parameters as revealed by recent studies [12]. Fragile topological phases are characterized by nontrivial signatures in either the Wilson loops [3,6] or the band representations [2,3,13] but can be connected with some obstructed atomic insulator phases, which are trivial insulators with charge centers away from atoms, via adding some trivial bands [2]. It has been shown that fragile topological insulators do not have robust edge states, imposing challenges on their experimental signatures. Later,it was proposed that local gauge flux insertion [4] or twisted boundary conditions [14,15] can be a useful tool to probe the fragile topology. However, neither local gauge flux insertion nor the twisted boundary condition is feasible in genuine condensed matter experimental conditions.

The goal of achieving carbon neutrality in the next 30-40 years is approaching worldwide consensus and requires coordinated efforts to combat the increasing threat of climate *** main sets of actions have been proposed to address this grand *** is to reduce anthropogenic CO2emissions to the atmosphere,and the other is to increase carbon sinks or negative emissions,i.e.,removing CO2from the *** we advocate eco-engineering approaches for ocean negative carbon emission(ONCE),aiming to enhance carbon sinks in the marine *** international program is being established to promote coordinated efforts in developing ONCE-relevant strategies and methodologies,taking into consideration ecological/biogeochemical processes and mechanisms related to different forms of carbon(inorganic/organic,biotic/abiotic,particulate/dissolved) for *** focus on marine ecosystem-based approaches and pay special attention to mechanisms that require transformative research,including those elucidating interactions between the biological pump(BP),the microbial carbon pump(MCP),and microbially induced carbonate precipitation(MICP).Eutrophic estuaries,hypoxic and anoxic waters,coral reef ecosystems,as well as aquaculture areas are particularly considered in the context of efforts to increase their capacity as carbon *** approaches are thus expected to be beneficial for both carbon sequestration and alleviation of environmental stresses.

BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

Aim:The study aimed to investigate the short-term outcomes of hospitalized patients with chronic liver disease(CLDs)and assess the prognostic impact of predisposition and precipitants,which currently remains ***:The study included 3970 hospitalized patients with CLDs from two prospective longitudinal multicenter studies(NCT02457637 and NCT03641872)conducted in highly endemic hepatitis B virus(HBV)*** risk analysis was used to evaluate the effect of predispositions,including the etiology and severity of CLDs and precipitants;on sequential 28,90,and 365-day liver transplantation(LT)-free ***:Among all enrolled patients,76.8%of adverse outcomes(including death and LT)within one year occurred within 90 *** with alcoholic etiology,the association of HBV etiology with poorer outcomes was remarkably on the 28th day(hazard ratio[HR],1.81;95%confidence interval[CI],1.07-3.06;p=0.026);however,and dimin-ished or became insignificant at 90 days and 365 *** increased the adjusted risk for 365-day(HR,1.50;CI,1.13-1.99;p=0.004)LT-free mortality when compared with *** patients with cirrhosis,prior decompensation(PD)independently increased the adjusted risk of 365-day LT-free mortality by 1.25-fold(p=0.021);however,it did not increase the risk for 90-day *** the category nor the number of precipitants influenced the adjusted risk of 28 or 90-day LT-free ***:The 90-day outcome should be considered a significant endpoint for evaluating the short-term prognosis of hospitalized patients with *** factors,other than etiology,mainly affected the delayed(365-day)*** effective therapy for CLD etiology,especially antiviral treatments for HBV,and post-discharge long-term surveillance monitoring in cirrhotic patients undergoing PD are suggested to enhance disease management and reduce mortality.
