Efficacy of antimicrobials in preventing resistance in solid organ transplant recipients:A systematic review of clinical trials

Background:Patients with inflammatory bowel disease(IBD)are at increased risk of herpes zoster(HZ).We evaluated the incidence of HZ in ulcerative colitis(UC)and Crohn’s disease(CD)patients and compared this with HZ incidence in a non-IBD ***:We conducted a retrospective cohort study(GSK study identifier:VEO-000043)of adults aged18 years with UC and CD and without IBD who were identified from claims recorded in a US healthcare database between October 2015 and February *** HZ incidence rates/1,000 person-years(PY)were calculated,and comparisons of HZ incidence rates between UC or CD and non-IBD cohorts were made using adjusted generalized linear ***:The study population comprised a total of 29,928 UC,25,959 CD,and 11,839,329 non-IBD *** overall HZ incidence rates were 13.64/1,000 PY(UC),15.94/1,000 PY(CD),and 7.95/1,000 PY(non-IBD).UC and CD patients had increased HZ incidence rates,with adjusted incidence rate ratios of 1.35(95%confidence interval[CI],1.26–1.44)and 1.66(95%CI,1.56–1.77),respectively,compared with non-IBD *** analysis indicated increased relative rates of HZ in progressively younger age strata in the UC and CD patients compared with non-IBD *** incidence rates were higher in UC and CD patients who had previously received thiopurines or methotrexate,TNF-inhibitors,or corticosteroids than in UC and CD patients who did not take those ***:UC and CD patients had increased HZ incidence rates compared with patients without IBD,demonstrating the importance of HZ prevention in IBD patients.

Pro-resolving lipid mediator reduces amyloid-β42–induced gene expression in human monocyte–derived microglia

Specialized pro-resolving lipid mediators including maresin 1 mediate resolution but the levels of these are reduced in Alzheimer's disease brain, suggesting that they constitute a novel target for the treatment of Alzheimer's disease to prevent/stop inflammation and combat disease pathology. Therefore, it is important to clarify whether they counteract the expression of genes and proteins induced by amyloid-β. With this objective, we analyzed the relevance of human monocyte–derived microglia for in vitro modeling of neuroinflammation and its resolution in the context of Alzheimer's disease and investigated the pro-resolving bioactivity of maresin 1 on amyloid-β42–induced Alzheimer's disease–like inflammation. Analysis of RNA-sequencing data and secreted proteins in supernatants from the monocyte-derived microglia showed that the monocyte-derived microglia resembled Alzheimer's disease–like neuroinflammation in human brain microglia after incubation with amyloid-β42. Maresin 1 restored homeostasis by down-regulating inflammatory pathway related gene expression induced by amyloid-β42 in monocyte-derived microglia, protection of maresin 1 against the effects of amyloid-β42 is mediated by a re-balancing of inflammatory transcriptional networks in which modulation of gene transcription in the nuclear factor-kappa B pathway plays a major part. We pinpointed molecular targets that are associated with both neuroinflammation in Alzheimer's disease and therapeutic targets by maresin 1. In conclusion, monocyte-derived microglia represent a relevant in vitro microglial model for studies on Alzheimer's disease-like inflammation and drug response for individual patients. Maresin 1 ameliorates amyloid-β42–induced changes in several genes of importance in Alzheimer's disease, highlighting its potential as a therapeutic target for Alzheimer's disease.

Objectives: The purpose of this study is to analyse the practical implementation of regional and na-tional policies through the Protocol of care of Children and Adolescents in School (2010) in Extremadura Region (Spain), and to compare its contents with the international standards of diabetes care at school defined by American Diabetes Association and International Diabetes Federation. The measures not only affect the security and diabetes care, but also inclusion and the right to health. Methods: A documental comparative analysis between the local and international standards about diabetes care in school setting is carried out. This analysis is framed in a larger project focused on the study of health promoting school and diabetes education, in which perceptions of children and adolescents with diabetes, their parents and school staff were studied. Results: The Protocol of care of Children and Adolescents in School (2010) contains some international recommendations about the care of T1DM at school, but in other cases the measures are non-specific. The distribution of responsibilities for care at school is unclear and no monitoring and evaluation indicators are defined. Some elements are identified to be implemented in the tool to favour the security, management of T1DM care and wellbeing. In general, these elements refer to school plan for diabetes care, school organization and teachers, and school community training. Conclusion: It is required to develop specific policies and decisive action to ensure the right to health of children with diabetes and the full application of international standards for diabetes care at school.

Therapeutic applications of collagenase(metalloproteases): A review

Non-invasive therapeutic methods have recently been used in medical sciences. Enzymes have shown high activity at very low concentrations in laboratories and pharmaceutical,enabling them to play crucial roles in different biological phenomena related to living organism, especially human medicine. Recently, using the therapeutic methods based on non-invasive approaches has been emphasized in medical society. researchers have focused on producing medicines and tools reducing invasive procedures in *** are proteins which catalyze chemical processes and break the peptide bonds in collagen. Collagen may be generated more than the required amount or produced in unsuitable sites or may not degrade after a certain time. In such cases, using an injectable collagenase or its ointment can be helpful in collagen degradation. In both in vitro and in vivo tests, it has been revealed that collagenases have several therapeutic properties in wound healing, burns, nipple pain and some diseases including intervertebral disc herniation, keloid, cellulite, lipoma among others. This review describes the therapeutic application of collagenase in medical sciences and the process for its production using novel methods, paving the way for more effective and safe applications of collagenases.
