Limbic encephalitis represents a cluster of autoimmune disorders,with inflammation in the medial temporal lobe characterised by subacute onset of neuropsychiatric symptoms such as anxiety,affective symptoms,psychosis,short-term memory impairment as well as faciobrachial and grand mal *** aim to present a case of a 53-year-old man with positive anti-voltage-gated potassium channel(VGKC)complex antibodies who initially presented with symptoms of psychotic *** weeks post-psychiatric symptomatology,he presented with neurological symptoms such as faciobrachial jerking and tonic-clonic *** patient had no previous psychiatric history and was initially treated with psychotropic *** experience emphasises the fact that limbic encephalitis is not easy to identify as most patients initially present with psychiatric symptomatology than neurological ***,immunological and laboratory testing takes a rather long time to determine the *** is more,few psychiatrists consider autoimmune nature of the neuropsychiatric ***,it is important to consider autoimmune encephalitis in patients with new-onset psychosis or mania who also present with neurological symptoms at some stage of their *** indicators of autoimmune encephalitis include neurological symptoms such as facial twitching,seizures,confusion and cognitive decline.

Background Intimate partner violence(IPV) and sexual violence(SV) occur commonly and are a violation of basic human rights. There are limited studies to date that examine the impact of IPV, SV and mental health outcomes in *** The objective of the present study was to estimate the prevalence of intimate partner physical violence and IPV in Thai women and the association between these forms of violence and psychiatric *** The present study used data from a national cross-sectional, population-based, household design survey. This study analysed data from 3009 female respondents above the age of 18 who were interviewed in person using the World Mental Health-Composite International Diagnostic Interview V.3.0(WMH-CIDI 3.0). We estimated the lifetime and the 12-month period prevalence of IPV and SV, the lifetime and the past 12-month period correlation of IPV/SV with psychiatric disorders and the OR for psychiatric disorders associated with these types of *** There was only 5.2 % of the weighted sample that reported experiencing some form of violence, including reported rates of intimate partner physical violence of 3.5% and IPV of 2.0%. Women who had experienced IPV have a lifetime prevalence for common psychiatric disorders of 28.9%, for suicidal behaviours of 12.2% and for substance use disorders of 8.8%. Women who had experienced SV have a lifetime diagnoses for common psychiatric disorders of 21.4%, for suicidal behaviours of 16.5% and for substance use disorders of 19.4%. There was a statistically significant association between IPV/SV and being diagnosed with a psychiatric disorder during the past year and also during one's *** The association between IPV/SV and psychiatric disorders is significant; therefore, performing a formal assessment for a history of violence in psychiatric patients is often beneficial in refining the diagnosis and treatment.

Addiction specialist's role in liver transplantation procedures for alcoholic liver disease

Although liver transplantation(LT) is performed increasingly for patients with end-stage alcoholic liver disease(ALD), the topic remains controversial. Traditionally, the role of an addiction specialist focused on the screening and identification of patients with a high risk on relapse in heavy alcohol use. These patients were in many cases subsequently excluded from a further LT ***, awareness is growing that not only screening of patients but also offering treatment, helping patients regain and maintain abstinence is essential, opening up a broader role for the addiction specialist(team)within the whole of the transplant procedure. Within this context, high-risk assessment is proposed to be an indication of increasing addiction treatment intensity,instead of being an exclusion criterion. In this review we present an overview regarding the state of the art on alcohol relapse assessment and treatment in patients with alcohol use disorders, both with and without ***, treatment and monitoring is suggested as central roles for the addiction specialist(team) integrated within transplant centers.

Complex posttraumatic stress disorder: The need to consolidate a distinct clinical syndrome or to reevaluate features of psychiatric disorders following interpersona trauma?

Complex posttraumatic stress disorder(Complex PTSD)has been recently proposed as a distinct clinical entity in the WHO International Classification of Diseases,11^(th) version, due to be published, two decades after its first initiation. It is described as an enhanced version of the current definition of PTSD, with clinical features of PTSD plus three additional clusters of symptoms namely emotional dysregulation, negative self-cognitions and interpersonal hardship, thus resembling the clinical features commonly encountered in borderline personality disorder(BPD). Complex PTSD is related to complex trauma which is defined by its threatening and entrapping context, generally interpersonal in nature. In this manuscript, we review the current findings related to traumatic events predisposing the above-mentioned disorders as well as the biological correlates surrounding them, along with their clinical features. Furthermore, we suggest that besides the present distinct clinical diagnoses(PTSD; Complex PTSD; BPD), there is a cluster of these comorbid disorders, that follow a continuum of trauma and biological severity on a spectrum of common or similar clinical features and should be treated as such. More studies are needed to confirm or reject this hypothesis, particularly in clinical terms and how they correlate to clinical entities' biological background, endorsing a shift from the phenomenologically only classification of psychiatric disorders towards a more biologically validated classification.

Large-scale epidemiological studies have found that hyperhomocysteinemia is a powerful, independent risk factor for Alzheimer＇s disease. Trillium tschonoskii maxim is a traditional Chinese medicine that is used to promote memory. However, scientific understanding of its mechanism of action is limited. This report studied the potential neuroprotective effects of Trillium tschonoskii maxim extract against homocysteine-induced cognitive deficits. Rats were intravenously injected with homocysteine（400 μg/kg） for 14 days to induce a model of Alzheimer＇s disease. These rats were then intragastrically treated with Trillium tschonoskii maxim extract（0.125 or 0.25 g/kg） for 7 consecutive days. Open field test and Morris water maze test were conducted to measure spontaneous activity and learning and memory abilities. Western blot assay was used to detect the levels of Tau protein and other factors involved in Tau phosphorylation in the hippocampus. Immunohistochemical staining was used to examine Tau protein in the hippocampus. Golgi staining was applied to measure hippocampal dendritic spines. Our results demonstrated that homocysteine produced learning and memory deficits and increased levels of Tau phosphorylation, and diminished the activity of catalytic protein phosphatase 2A. The total number of hippocampal dendritic spines was also decreased. Trillium tschonoskii maxim extract treatment reversed the homocysteine-induced changes. The above results suggest that Trillium tschonoskii maxim extract can lessen homocysteine-induced abnormal Tau phosphorylation and improve cognitive deterioration such as that present in Alzheimer＇s disease.

Rethinking hospital psychiatry in Italy in light of COVID-19 experience

BACKGROUND Italy retains a distinctive organization of mental health services according to a community-based model of care with a multidisciplinary team serving a welldefined catchment area under the coordination of the local department of mental *** coronavirus disease 2019(COVID-19)pandemic is forcing Italian mental health services to develop new organizational strategies at all levels of care in order to face the associated *** To explore factors associated with changes in psychiatric admissions to an inpatient psychiatric unit located in Lombardia Region,*** All hospital admissions(n=44)were recorded to an inpatient psychiatric unit during a three month national lockdown in Italy in 2020 and compared with those occurring over the same time period in 2019(n=71).For each admission,a 20-item checklist was completed to identify factors leading to *** analyses were performed using Statistical Package for Social Sciences for Windows,release ***-square test(or Fisher’s exact test)and Mann-Whitney U-test were applied,where *** Hospital admissions dropped by 38%during the COVID-19 *** significant differences were found in demographics,clinical variables associated with hospital admissions and length of stay between 2019 and *** with 2019,a significantly greater proportion of hospital admissions in 2020 were related to difficulties in organizing care programs outside the hospital(chi-square=4.91,df 1,one-way P=0.035)and in patients’family contexts(chi-square=3.71,df 1,one-way P=0.049).On the other hand,logistic and communication difficulties pertaining to residential facilities and programs were significantly more common in 2019 than in 2020(chi-square=4.38,df 1,one-way P=0.032).CONCLUSION Admissions to the inpatient psychiatric unit dropped significantly during the COVID-19 pandemic in 2020,with difficulties in organizing care programs outside the hospital and in patients’family contexts

In the Turmoil of the Cybercriminal Triad, Occult Practices and Toxicomania: The Case of a Young Ivorian Cyber Crook

Far from the financial gains of cyber crooks, the informed observer will see in cyber crookery a complex field of psychopathological expression where addictions and ritualistic practices intermingle. We thus carried out a qualitative observational study, focusing on a young Ivorian cyber-escroc, hard drug user and follower of mystical practices taken in charge by a multidisciplinary team during a resocialization course for children in conflict with the law from November to December 2022. The aim was to describe the difficulties experienced by a young Ivorian cyber crook in escaping the clutches of occult practices, internet addictions and psychoactive substances encountered in the field of cyber crookery. KA is confronted with the problem of cybercrime associated with occult practices and drug addiction. He is a heroin addict and secondary internet addict, undergoing de facto detoxification and presenting risky consumption behaviors. His fear is fuelled by the sudden death of his childhood friend, who introduced him to cyber-scams and occult practices, and the impossibility of returning his “Agbaladai” fetish when he can no longer honor the weekly sacrifices. Our results show an interweaving of psychopathological entities, namely cyberaddiction and addiction to psychoactive substances, against a backdrop of mystico-cultural practices and interpretations.

Determinants of internet addiction among medical students in Casablanca: a cross-sectional study

Objective:This study aimed to estimate the prevalence and determinants of Internet addiction among medical students at the Faculty of Medicine and Pharmacy of Casablanca,***:This was a cross-sectional study conducted among students at the Faculty of Medicine and Pharmacy in Casablanca between October and March *** online questionnaire was administered to students to collect data and internet addiction was assessed by the Young questionnaire.A score threshold≥50 was adopted to define *** and multivariate logistic regression analyses were used to identify factors associated with internet ***:Out of a total of 4093 FMPC students enrolled in the 2020-2021 academic year,506 agreed to participate in this study,including 303 females and 203 *** mean addiction score assessed on the Young scale was(49.08±16.11).The prevalence of Internet addiction was 44.5%(225/506,95% CI:40% to 49%).Multiple regression analysis showed that being older than 20 years(OR=0.17,95% CI:0.40 to 0.64),being female(OR=1.70,95% CI:1.04 to 2.78),being in the dissertation year(6th year)(OR=5.17,95% CI:2.23 to 11.44),having a history of psychiatric consultation(OR=2.64,95% CI:1.34 to 5.21),having divorced parents(OR=2.64,95% CI:1.05 to 5.87),use of sleeping medication(OR=2.9,95% CI:1.05 to 3.70),sleep disorders(OR=2.06,95% CI:1.25 to 3.79),sleep deprivation(OR=2.26,95% CI:1.39 to 3.65),excessive daytime sleepiness(OR=5.39,95% CI:2.19 to 13.24),anxiety disorders(OR=1.47,95% CI:1.18 to 2.30),duration of internet connection(>4 h)(OR=11.43,95% CI:4.85 to 27.66),and having frequent conflicts with parents(OR=2.37,95% CI:1.49 to 3.79)and friends(OR=0.26,95% CI:0.11 to 0.65)were independently associated with internet ***:The prevalence of Internet addiction among medical students in Casablanca remains *** action on the determinants would be of great value in prevention.
