Is convenience really king? Comparative evaluation of catastrophic costs due to tuberculosis in the public and private healthcare sectors of Viet Nam: a longitudinal patient cost study

Background In Viet Nam, tuberculosis (tb) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with tb may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among tb-affected households in the public and private *** Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for tb and 60 tb patients treated by the National tb Program (NTP) in Ha Noi, Hai Phong and ho chi minh city, Viet Nam. Using a local adaptation of the Who tb patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate *** The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). however, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;Phorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;Ptb-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with tb in Viet Nam face similarly high risk of catastrophic costs whether treated in the

Knowledge and attitude toward COVID-19 among healthcare workers at District 2 hospital, ho chi minh city

Objective:To assess the knowledge and attitude toward coronavirus disease-2019(COVID-19)among healthcare workers at District 2 hospital in ho chi minh city(HCMC).Methods:A cross-sectional study was performed between January 2020 and February 2020 at District 2 hospital.A systematic random sampling strategy was carried out and the data was collected through a self-administered questionnaire of the knowledge and attitude of healthcare workers regarding *** analysis was reported to describe the demographic,mean knowledge and attitude score of healthcare *** statistics including t-test,ANOVA and Spearman’s correlation were used to evaluate the relationship between study ***:A total of 327 eligible healthcare workers had a mean score of knowledge and attitude of 8.17±1.3(range 4-10)and 1.86±0.43(range 1-5),*** showed good knowledge and a positive ***,approximately two thirds of the participants knew the mode of transmission,the isolation period and treatment(67.0%,65.8%,and 58.4%,respectively),and 82.3%and 79.8%,respectively,held positive attitude regarding the risk of personal and family members getting *** was a negative correlation between knowledge scores and attitude scores(r=-0.21,P<0.001).Additionally,healthcare workerspredominately used social media to inform themselves about COVID-19(91.1%).Conclusions:The majority of healthcare workers had good knowledge and positive attitude toward ***,the level of some knowledge and attitude lower than that expected for their position level towards the *** education interventions and campaigns are required for healthcare workers.

背景:越南胡志明市目的:确定国家结核病规程(NTP)中病人的初始不合作程度、原因以及初始不合作后的求医行为和在其它医疗机构接受治疗的情况。方法:对2000年在NTP中被诊断为涂阳肺结核但未在NTP登记治疗的病人进行问卷调查。结果:初始不合作率为8.3％;79％的不合作者与NTP的工作程序或治疗策略有关,17.5％则是由于病人对结核病以及/或治疗必要性的知识不足所致。65％的病人在初始不合作后到其它医疗机构接受了抗结核治疗,其中74％在全私人机构接受治疗。结论:初始不合作主要是由于一些病人对于NTP的工作程序和/或治疗策略的误解而产生的。在初始不合作后大多数病人在私人诊所接受治疗。NTP需要增进病人对治疗策略的理解,提供亲切的服务使更多病人得到治疗,减少病人在私人诊所接受不标准治疗带来的风险。

地点:越南胡志明市目的:确定促进结核病控制公立 — 私立结合项目 (PPM)中私人肺病专家的病人治疗转归情况。方法:对由 PPM 项目中私人肺病专家治疗,和在国家结核病规划(NTP)机构中治疗的病人队列进行 12 个月的随访。以治疗卡中的信息为基础,确定病人管理质量和治疗转归。作为补充措施,还开展了小组访谈。种不同的治疗方案,根本就没有采用直接观察下的治疗,利用痰涂片镜检进行治疗效果评价以及健康教育都不够。总治疗成功率为60%,丢失率为37%,比 NTP 机构明显为差。结论:PPM 项目采用了培训、督导、标准化转诊和信息系统以及经济激励机制的综合措施,但私人医生仍然不能获得良好的治疗转归。其原因可能包括对私人医生(PPs)和病人的问卷调查以及私人医生未进行药物成本补偿以及缺乏实施细则。

背景:越南胡志明市。目的:描述药剂师对向国家结核病控制规划(NTP)转诊可疑结核病人的态度,并且评价一种新型转诊系统的可行性。方法:1)对150家私人药房进行问卷调查;2)对16名药剂师进行定性调查;3)对新转诊单的使用进行21个月的监测。结果:72％的药剂师认为本简化转诊单便于使用。药剂师提出了多种与将可疑结核病人转诊到NTP有关的风险;病人可能流向另一家药房(61％的应答者);如果顾客未患结核病他们就会对药剂师提出批评(42％):由于顾客未去NTP就诊因此延误治疗(41％)。58名(39％)合格药剂师至少转诊过一个病人。至少转诊了373例可疑病人,149例接受了检查,10例患结核病。结论:让药剂师转诊可疑结核病人是可行的。由于对病例发现有重要影响,因此认识到药剂师所担心的问题,并且努力改变病人乖供方两方面对药剂师作用的期望非常重要。

目的:评价越南年轻成年人的结核病登记率增加是否造成结核病在大众人群中传播的增加。方法:使用1996—2005年间县/区级结核病防治机构常规报告的数据计算出新涂阳结核登记率的趋势。对6～9岁儿童进行结核菌素调查,使用其结果计算结核年感染危险率的趋势。结果:自1996年至2006年间,15～24岁年龄组结核病登记率的年增长率为4.3%,且城市地区的年增长率(6.7%)比农村地区(1.7%)更高。城市地区的结核年感染危险率从1992年的2.4%下降到1998年的1.2%和2005年的0.9%;摘要农村地区的结核年感染危险率从1991年的0.7%上升到1997年的1.2%,之后下降到2006年的0.9%。结论:在胡志明省,年轻成年人结核病登记率升高,但同时儿童结核年感染危险率下降。这说明年轻成年人的结核病登记率变化趋势反映了在这一年龄段从结核菌感染发展成为结核病患者的比例升高了,以及/或在这一年龄段的结核病传播增加了,但不能说结核病在全人群中的传播增加了。

背景:越南胡志明市。目的:描述诊断延误;比较私人和公共卫生保健提供者所采用的诊断程序和转诊常规;检查接触各种不同类型行医者与结核病诊断延误危险性之间的联系。设计:在国家结核病规程中新病人的横断面调查。对以前接触过的卫生保健人员所采用的寻医和诊断程序的回顾性评价。结果:首次症状出现后4周,81%的病人出门看病。首次接触卫生保健机构后4周,有47%诊断为结核病。私人医生比公共卫生保健人员使用X线检查,痰涂片和转诊明显少。首次就到私人药房、私人医生或公立医院就诊的病人比首次就去国家结核病规程部门就诊的病人更有可能发生提供者长时间延误。结论:胡志明市结核病诊断延误更多地是由于卫生保健人员没有能力检出结核病,而不是卫生保健服务未得到充分利用。诊断程序需要完善,转诊工作需要加强,尤其在私人提供者中。
