Comparison of QT Correction Methods in the Pediatric Population of a Community hospital: A Retrospective Study

Objective:Accurate measurement of QT interval,the ventricular action potential from depolarization to repolarization,is important for the early detection of Long QT syndrome.The most effective QT correction(QTc)formula has yet to be determined in the pediatric population,although it has intrinsically greater extremes in heart rate(HR)and is more susceptible to errors in measurement.The authors of this study compare six dif-ferent QTc methods(Bazett,Fridericia,Framingham,Hodges,Rautaharju,and a computer algorithm utilizing the Bazett formula)for consistency against variations in HR and RR interval.Methods:Descriptive Retrospective Study.We included participants from a pediatric cardiology practice of a community hospital who had an ECG performed in 2017.All participants were healthy patients with no past medical history and no regular med-ications.Results:ECGs from 95 participants from one month to 21 years of age(mean 9.7 years)were included with a mean HR of 91 beats per minute(bpm).The two-sample paired t-test or Wilcoxon signed-rank test assessed for any difference between QTc methods.A statistically signiﬁcant difference was observed between every combination of two QTc formulae.The Spearman’s rank correlation analysis explored the QTc/HR and QTc/RR relationships for each formula.Fridericia method was most independent of HR and RR with the lowest absolute value of correlation coefﬁcients.Bazett and Computer had moderate correlations,while Framingham and Rautaharju exhibited strong correlations.Correlations were positive for Bazett and Computer,reﬂecting results from prior studies demonstrating an over-correction of Bazett at higher HRs.In the linear QTc/HR regression analysis,Bazett had the slope closest to zero,although Computer,Hodges,and Fridericia had comparable values.Alternatively,Fridericia had the linear QTc/RR regression coefﬁcient closest to zero.The Bland-Altman method assessed for bias and the limits of agreement between correction formulae.Bazett and Computer exhibited good agreement with minimal

A widely used aortic valve bioprosthesis is susceptible to regurgitation between the sewing ring and the frame of the valve due to its relatively thin fabric coverage. In some cases this leak has been shown to resolve with administration of protamine, however, tension on this area from annular sutures placed in an asymmetric bicuspid valve annulus may exacerbate the defect.

Post-translational modifications(PTMs)play a critical role in bone remodeling,with phosphorylation and acetylation being particularly well characterized.Recently,succinylation,a relatively uncommon PTM on lysine,has received considerable research attention for its influence on several physiological and pathological processes and conditions.1 Several substrates involved in mitochondrial pathways have been validated as substrates for the desuccinylase sirtuin 5(Sirt5),a key regulator of succinylation.2 Bone is one of the most metabolically active organs,but the role of succinylation during bone remodeling is not well characterized.

Stem cell therapy:light in the tunnel for penetrating Crohn’s disease

Patients with Crohn’s disease frequently suffer from fistula resulting from adverse sequelae of persistent complicated active disease or surgical intervention.Fistula affects a patient’s quality of life and is directly associated with the need for surgical intervention.Diagnosis of fistula can be made through CT enterography,MR enterography,gastrograffin-based imaging,and transanal ultrasound.Treatment for fistula mainly consists of medication,endoscopic procedures,and surgery.There are emerging approaches under current investigation,such as stem cell therapy.The results showed a decent response in patients with perianal and rectovaginal fistula with minimal side effects.Further investigation is still needed for other internal fistula.

Carotid artery injury in endoscopic endonasal surgery: Risk factors, prevention, and management

Objective:Endoscopic approaches for sinus and skull base surgery are increasing in popularity.The objective of this narrative review is to characterize risk factors for internal carotid artery injury in endoscopic endonasal surgery(EES),highlight preventative measures,and illustrate key management principles.Data Sources:Comprehensive literature review.Methods:Relevant literature was reviewed using PubMed/MEDLINE.Results:Carotid artery injury in EES is rare,with most studies reporting an incidence below 0.1%.Anatomic aberrancies,wide dissection margins,as well as specific provider and hospital factors,may increase the risk of injury.Multidisciplinary teams,comprehensive preoperative imaging,patient risk assessment,and formal training in vascular emergencies may reduce the risk.Management protocols should emphasize proper visualization of the injury site,fluid replacement,rapid packing,angiography,and endovascular techniques to achieve hemostasis.Conclusions:While EES is a relatively safe procedure,carotid artery injury is a devastating complication that warrants full consideration in surgical planning.Important preventative measures include identifying patients with notable risk factors and obtaining preoperative imaging.Multidisciplinary teams and management protocols are ultimately necessary to reduce morbidity and mortality.

Venous thromboembolism in inflammatory bowel disease

Patients with inflammatory bowel disease(IBD)are at an increased risk for venous thromboembolism(VTE).VTE events carry significant morbidity and mortality,and have been associated with worse outcomes in patients with IBD.Studies have suggested that the hypercoagulable nature of the disease stems from a complex interplay of systems that include the coagulation cascade,natural coagulation inhibitors,fibrinolytic system,endothelium,immune system,and platelets.Additionally,clinical factors that increase the likelihood of a VTE event among IBD patients include older age(though some studies suggest younger patients have a higher relative risk of VTE,the incidence in this population is much lower as compared to the older IBD patient population),pregnancy,active disease,more extensive disease,hospitalization,the use of certain medications such as corticosteroids or tofacitinb,and IBD-related surgeries.Despite the increased risk of VTE among IBD patients and the safety of pharmacologic prophylaxis,adherence rates among hospitalized IBD patients appear to be low.Furthermore,recent data suggests that there is a population of high risk IBD patients who may benefit from post-discharge prophylaxis.This review will provide an overview of patient specific factors that affect VTE risk,elucidate reasons for lack of VTE prophylaxis among hospitalized IBD patients,and focus on recent data describing those at highest risk for recurrent VTE post-hospital discharge.

Evolving treatment strategies in Crohn’s disease

The natural history of Crohn’s disease(CD)dictates that patients re-quire long-term medical therapy for induction and maintenance to relieve their symptoms,improve health-related quality of life,pre-vent tissue damage,lessen the risk for neoplasia,and reduce hospi-talization and surgery.For the past quarter of a century,medical management of CD has evolved from corticosteroids for induction and immunomodulators formaintenance to antitumor necrosis fac-tor,anti-integrin,and anti-interleukin biological agents and,in the near future,small molecule agents,for both induction and mainte-nance.Despite advances inmedical therapy,themajority of patients with CDwould eventually require surgerywith incision and drainage of abscesses and fistulas,bowel resection and anastomosis,strictur-eplasty,or fecal diversion.Surgery will reset the clock for the disease course of CD and most patients require post-operative medical ther-apy to prevent disease recurrence.Colonoscopy plays a key role in disease monitoring and assessment of treatment response before and after surgery.Endoscopy also plays an important role in the treatment of CD-or its surgery-associated structural complications,such as primary or anastomotic strictures,abscesses,and fistulas.

Interventional inflammatory bowel disease:endoscopic therapy of complications of Crohn’s disease

Endoscopic therapy for inflammatory bowel diseases(IBD)or IBD surgery-associated complications or namely interventional IBD has become the main treatment modality for Crohn’s disease,bridging medical and surgical treatments.Currently,the main applications of interventional IBD are(i)strictures;(ii)fistulas and abscesses;(iii)bleeding lesions,bezoars,foreign bodies,and polyps;(iv)post-operative complications such as acute and chronic anastomotic leaks;and(v)colitis-associated neoplasia.The endoscopic treatment modalities include balloon dilation,stricturotomy,strictureplasty,fistulotomy,incision and drainage(of fistula and abscess),sinusotomy,septectomy,banding ligation,clipping,polypectomy,endoscopic mucosal resection,and endoscopic submucosal dissection.The field of interventional IBD is evolving with a better understanding of the underlying disease process,advances in endoscopic technology,and interest and proper training of next-generation IBD interventionalists.

Impact of coronavirus disease 2019 on semen parameters

Dear editor,Severe acute respiratory syndrome coronavirus-2(SARS-CoV-2),the virus responsible for the coronavirus disease 2019(COVID-19)pandemic,has been associated with hypogonadism in patients with acute infection[1].Angiotensin-converting enzyme 2(ACE2)receptor is regarded as the viral binding site that facilitates entry into host cells[2].Therefore,cells that express ACE2,such as spermatogonia,Leydig,and Sertoli cells,may be susceptible to SARS-CoV-2 infection[2].Testicular damage may possibly be caused directly by viral invasion or secondary to the generalized inflammatory response to COVID-19[3].Studies have demonstrated that viral shedding in the semen can take place during the recovery phase,even though SARS-CoV-2 is not present in the semen of all men with COVID-19[1,4].Herein,we describe the first case report of a male with longitudinal follow-up who experienced reversible cryptozoospermia after COVID-19.

The Chinese skeleton: insights into microstructure that help to explain the epidemiology of fracture

Osteoporotic fractures are a major public health problem worldwide, but incidence varies greatly across racial groups and geographic regions. Recent work suggests that the incidence of osteoporotic fracture is rising among Asian populations. Studies comparing areal bone mineral density and fracture across races generally indicate lower bone mineral density in Asian individuals including the Chinese, but this does not reflect their relatively low risk of non-vertebral fractures. In contrast, the Chinese have relatively high vertebral fracture rates similar to that of Caucasians. The paradoxically low risk for some types of fractures among the Chinese despite their low areal bone mineral density has been elucidated in part by recent advances in skeletal imaging. New techniques for assessing bone quality non-invasively demonstrate that the Chinese compensate for smaller bone size by differences in hip geometry and microstructural skeletal organization. Studies evaluating factors influencing racial differences in bone remodeling, as well as bone acquisition and loss, may further elucidate racial variation in bone microstructure. Advances in understanding the microstructure of the Chinese skeleton have not only helped to explain the epidemiology of fracture in the Chinese, but may also provide insight into the epidemiology of fracture in other races as well.
