Alternative Exposure Parameters and Post Process Noise Reduction Expect Considerable Dose Reduction in Single Mammography Views—Initial Experience on Mastectomy Specimens

Objective: To experimentally indicate a lower limit of dose in mammographic imaging yielding adequate image quality for complementary diagnostic views, by evaluation of image series with different exposure parameters and additional image processing on mastectomy specimens with diverse pathology. Methods: Image series were obtained on seven specimens with different target-filter combinations at different exposure values. Three experienced radiologists assessed the lowest acceptable dose level per specimen using a relative grading technique. With the standard image as reference, fibroglandular tissue and pathological structures, including microcalcifications, were evaluated. Subsequently, a series of pixel binning processes was tested and subjectively assessed on the selected images. Results: The lowest dose level at which image quality was acceptable, and achieved with a W/Ag target-filter combination at 32 kV and 4 mAs. These images can be acquired with 10.4% to 22.4% of the average glandular dose in standard images. Post process pixel binning added to the interpretability of such low dose images. Conclusion: This specimen study suggests that dose level of mammography images might be reduced substantially by general application of a W/Ag spectrum, particularly when combined with post process noise reduction. Future studies should focus on the feasibility of this technique in clinical mammography.

Explainable artificial intelligence models for mineral prospectivity mapping

Mineral prospectivity mapping(MPM) is designed to reduce the exploration search space by combining and analyzing geological prospecting big data. Such geological big data are too large and complex for humans to effectively handle and interpret. Artificial intelligence(AI) algorithms, which are powerful tools for mining nonlinear mineralization patterns in big data obtained from mineral exploration, have demonstrated excellent performance in MPM. However, AI-driven MPM faces several challenges, including difficult interpretability, poor generalizability, and physical inconsistencies. In this study, based on previous studies, we devised a novel workflow that aims to constructing more transparent and explainable artificial intelligence(XAI) models for MPM by embedding domain knowledge throughout the AI-driven MPM, from input data to model design and model output. This newly proposed approach provides strong geological and conceptual leads that guide the entire AI-driven MPM model training process, thereby improving model interpretability and performance. Overall, the development of XAI models for MPM is capable of embedding prior and expert knowledge throughout the modeling process, presenting a valuable and promising area for future research designed to improve MPM.

Sexual and reproductive health(SRH) services are crucial for women especially during disasters, to reduce maternal mortality and morbidity from miscarriages, unsafe abortions, and post-abortion complications. This study explored the SRH interventions provided during disaster response. A systematic review was conducted to identify what menstrual regulation(MR), safe abortion(SA), and post-abortion care(PAC) approaches/interventions exist to promote resilience in the health system in disaster settings; what intervention components were most eff ective; and challenges and opportunities to meeting SRH rights. Five electronic databases were searched, resulting in 4194 records. Following the screening process, seven publications were included. The intervention-related information in each publication was assessed based on availability,accessibility, acceptability, and quality. Two SRH approaches/interventions were found. The eff ectiveness of intervention components could not be conducted due to the limited number of relevant studies. Challenges were found at facility and community levels, and opportunities included overcoming them, making MR, SA, and PAC integral to the mitigation phase,and policy change to overcome barriers related to unaff ordability and inaccessibility. Recommendations are provided to encourage research and policy towards improving neglected SRH in disaster settings to realize Sustainable Development Goal 3 and the Global Strategy and Sendai Framework's priority to promote disaster-resilient health systems.

Introduction: The improvement of survival in patients with cancer and the expansion of therapeutic options have led to the emergence of a new profile of cardiotoxicity, specifically associated with antimitotic agents. Our study aimed to assess the incidence of chemotherapy-induced myocardial toxicity in patients with cancer. Patients and Methods: We conducted a looking-forward longitudinal cohort study including all patients admitted to the Cardiology departments of Aristide le Dantec Hospital and Dalal Jamm national Hospital centre for apre-chemotherapy check-up. The included patients did not undergo any pre-existing cardiopathy. Results: Over a period of two years ranging from January 2019 to December 2021, a total of 37 patients were included in the study. Notably, there was a female predominance (92%) with an average age of 49.7 years ± 13.69. breast cancer accounted for 70% of the neoplasms. Laboratory findings revealed moderate anemia in 19 patients (51%). At inclusion, the left ventricle (LV) was of normal size (LV diastole at 44.46 ± 4.97 mm). The systolic function of the left ventricle was normal in all patients, with an average ejection fraction (EF) of 63.1% ± 5.80 and a mean global longitudinal strain (GLS) of −20.4% ± 2.58. The most commonly used agents were anthracyclines. During follow-up, 3 patients (8.1%) developed clinical symptoms of left heart failure, and LV dysfunction on echocardiography was observed in 5 (13.5%) patients, with a significant decrease in EF Conclusion: The incidence of cardiac toxicity is not negligible, hence the importance of early screening. Strain imaging is an essential tool that should be performed as part of the assessment before chemotherapy and re-evaluated during treatment.

An evaluation index is a prerequisite for the scientific evaluation of a public meteorological *** paper aims to explore a technical method for determining and screening evaluation *** on public satisfaction survey data obtained in Wafangdian,China in 2010,this study investigates the suitability of fuzzy clustering analysis method in establishing an evaluation *** quantitative analysis of multilayer fuzzy clustering of various evaluation indicators,correlation analysis indicates that if the results of clustering were identical for two evaluation indicators in the same sub-evaluation layer,then one indicator could be removed,or the two indicators *** evaluation indicators in different sub-evaluation layers,although clustering reveals attribute correlations,these indicators may not be substituted for one *** of the applicability of the fuzzy clustering method shows that it plays a certain role in the establishment and correction of an evaluation index.

目的比较胶片摄影与数字摄影在乳腺癌筛查中对于临床相关癌症的检出率。材料与方法研究经过地区医学伦理委员会批准,不需要知情同意书。在荷兰每两年1次的筛查项目中,全国广泛使用数字乳腺摄影之前,有3个筛查区进行了数字乳腺摄影。最初的筛查检查中,每侧乳腺进行内外侧斜位和头尾位摄影。而在随后的检查中,常规进行内外侧斜位摄影。如果有需要才进行头尾位摄影。将最初和随后的乳腺胶片摄影和数字摄影的筛查结果,包括放射学与病理学特点进行比较。结果 2003—2007年间共进行1198493例筛查,18896例进行了复查(胶片摄影:最初的筛查为2.6%,随后的检查为1.3%;数字摄影:最初的筛查为4.4%,随后的检查为2.1%;两者P<0.001)。6410例妇女诊断为乳腺癌(胶片摄影:最初的检查每1000名妇女的检出率为5.6,随后的检出率为5.2;数字摄影:最初的检查中每1000名妇女的检出率为6.8,随后的检出率6.1;P=0.02和P<0.001)。不论哪次筛查,数字乳腺摄影均可发现更多的导管原位癌。在随后的检查中,可发现更多的浸润癌,特别是伴有微钙化者(P=0.047)。两种成像方法中发现的导管原位癌和浸润癌的组织病理学分化等级是相似的。但是,在随后的检查中数字乳腺摄影能检出更多高级别的导管原位癌(P=0.013)。结论在对人群的乳腺筛查项目中,乳腺数字摄影对于导管原位癌和浸润癌的检测显著优于胶片摄影。采用数字乳腺摄影进行筛查,低级别导管原位癌的检出并没有增加的迹象,表明有可能存在过度诊断。相反的,数字乳腺摄影似乎增加了高级别导管原位癌的检测。
