18F-FDG pet/CT was used for evaluation of spleen infiltration in patients with B-cell non-Hodgkin lymphoma(NHL).Five patients with histological diagnosed B-cell NHL underwent1 8F-FDG pet/CT *** integrated pet/CT image,spleen infiltration was considered when pet images revealed a discrete margin of solid splenic masses or diffuse lesion of spleen with higher maximum standardized uptake value(SUV) greater than those of normal liver *** images demonstrating a positive splenic index(>480 mL)or focal hypodensities were classified as positive for spleen *** the patients underwent systemic ***1 8F-FDG pet and CT results were compared with final *** patients had spleen infiltration originating from B-cell NHL at final *** diagnoses,which were confirmed by clinical and CT(n=3) or1 8F-FDG pet/CT(n=1) follow-up in 4 patients and biopsy of 1 *** integrated pet/CT image,1 8F-FDG pet was true-positive in all 5 patients with spleen *** was true-positive in 4 of the 5 patients with spleen infiltration and false-negative in 1 patients (spleen infiltration without morphology changes).The accuracies of1 8F-FDG pet and CT for evaluating the spleen infiltration were 100% and 80% at staging,*** preliminary results suggested metabolic imaging of1 8F-FDG pet/CT may be helpful in the diagnosis of spleen infiltration in B-cell NHL *** patients may benefit from1 8F-FDG pet/CT in diagnosis,when spleen infiltration without morphology changes,which may not be diagnosed exactly by conventional image.

In this paper, 18F-FDG pet/CT data of 19 malignant SINs (small intestinal neoplasms) were consecutively reviewed. Nnine patients accepted pet/CT scan for preoperative diagnosis and staging, while ten patients presented follow-up after treatment and restaging. The results were correlated with abdomen enhanced CT and surgical pathological findings. Abdominal pain and weight loss were the most common findings. About 16% SINs located in the duodenum, 52% in the jejunum and 32% in the ileum. Lymphoma was the most frequent neoplasms. pet/CT revealed residual or recurrent malignant SINs in 5 patients who had negative or non-definite findings by abdomen CT and demonstrated extra-abdomen metastasis in 3 patients. Clinical decisions of treatment were changed for 6 patients after pet/CT examinations. The 18F-FDG pet/CT were better than CT in accuracy, negative predictive value and positive predictive value (89.5% vs 68.4%, 100% vs 66.7%, and 81.8% vs 69.2%, respectively). Whole body 18F-FDG pet/CT may be an effective molecular imaging method for staging and restaging of malignant SINs.

Spontaneous rupture of hepatocellular carcinoma(HCC) is a life-threatening complication and its prognosis is significantly poor because of the high recurrence rate after initial hepatectomy. Resection of isolated extrahepatic metastasis of HCC has been advocated to obtain a possibility of long-term survival. However, it is a challenge for clinicians to detect implantation metastasis of spontaneously ruptured HCC. Accurate re-staging plays the most important role in making a decision on isolated metastasis resection. 18F-fluorodeoxyglucose(18F-FDG) positron emission tomography/computed tomography(pet/CT) is useful in detecting intraabdominal implantation metastasis from a variety of malignancies and shows superior accuracy to conventional imaging modalities in determining the location of metastasis. We present one patient with a new isolated pelvic implantation metastasis detected by 18F-FDG pet/CT and pathologically confirmed by pet/CT-guided percutaneous biopsy, who had a history of resection of spontaneously ruptured HCC two years ago. The patient's condition was stable at the 6-mo follow-up after resection of the isolated pelvic metastasis.

In this paper,eleven consecutive hepatocellular carcinoma(HCC) patients with an embolus in the hepatic vein(HV),inferior vena cava(IVC)and right atrium(RA) were studied with 18F-FDG pet/CT and contrast enhanced *** correlated with final diagnosis,18F-FDG pet/CT and contrast enhanced CT was positive in 11 patients(100%),7 patients(63.6%),*** accuracy of pet/CT and contrast-enhanced CT were 100%,63.6%.Three cases with secondary blood thrombi in the distant IVC,which were confirmed by pet/CT,cannot be identified by contrast enhanced *** average survival was 3 months(range,1-12mo).The 12 months survival rate was 9.1%.Our results suggest that highly metabolic tumor thrombus in the HV,IVC and RA may be depicted on 18F-FDG pet/CT in HCC *** may discriminate between malignant and secondary benign blood thrombi.

A Practical Process for the Synthesis of Translocator Protein 18kDa Imidazopyridine Ligand

In this article, an effective feasible method to synthesize translocator protein 18kDa （TSPO） imidazopyridine ligand 7 is discussed. A new procedure for the synthesis of the key intermediate 17 has been developed in four steps （condensation, diazo reaction, hydrolysis, amidation） from 2-aminopyridine 18. The overall yield was increased from 18% to 31.6%. One of the key steps is using activated copper powder as a catalyst in several cycles. Finally, reduction of 17 with Zn dust completed the synthesis of TSPO imidazopyridine ligand 7 in 86% yield.

Due to relatively high uptake of glucose in the brain cortex, the use of FDG pet imaging is greatly limited in brain tumor imaging, especially for low-grade gliomas and some metastatic tumours. More and more tracers with higher specificity were developed lately for brain tumor imaging. There are 3 main types of non-FDG pet tracers: amino acid tracers, choline tracers and nucleic acid tracers. These tracers are now widely applied in many aspects of brain tumor imaging. This article summarized the general use of non-FDG pet in different aspects of brain tumor imaging.

AIM To investigate the relationship between glucose metabolism and glypican-3(GPc3)expression in hepatocellular carcinoma(Hcc).METHODSImmunohistochemical staining of pathological samples for GPc3 and glucose transporter 1(GLUT1),and whole-body ^(18)F-FDG pet/c T for measuring tumour glucose uptake were performed in 55 newly diagnosed Hcc *** maximum standard uptake value(s UVmax)and tumour-to-non-tumourous liver uptake(T/NT)ratio were used to quantify ^(18)F-FDG *** vitro ^(18)F-FDG uptake assay of GPc3-expressing Hep G2 and non-GPc3-expressing RH7777 cel ls was used to examine the effect of GPc3 in cellular glucose *** relationships between GPc3 expression and ^(18)F-FDG uptake,GLUT1 expression,tumour differentiation,and other clinical indicators were analysed using spearman rank correlation,univariateand multiple logistic regression *** GPc3 expression was observed in 67.3%of Hcc patients,including 75.0%of those with well or moderately differentiated Hcc and 36.4%of those with poorly differentiated *** was an inverse relationship between GPc3 expression and s UVmax(Spearman correlation coefficient=-0.281,P=0.038)and a positive relationship between GLUT1 expression and sU Vmax(Spearman correlation coefficient=0.681,P0.05).Further multivariate analysis revealed that only the T/N ratio was significantly correlated with GPC3 expression in patients with Hcc(P<0.05).In vitro assay revealed that the uptake of ^(18)F-FDG in GPc3-expressing HepG2 cells was significantly lower than that of non-GPc3-expressing RH7777 cells(t=-20.352,P<0.001).CONCLUSIONThe pres

Fibroblast activation protein (FAP) is among the most popular targets in nuclear medicine imaging and cancer theranostics. Several small-molecule moieties (FAPI-04, FAPI-46, etc.) are used for developing FAP-targeted theranostic agents. Nonetheless, the circulation time of FAP inhibitors is relatively short, resulting in rapid clearance via kidneys, low tumor uptake, and associated unsatisfactory treatment efficacy. To address the existing drawbacks, we engineered 3 peptides named FD1, FD2, and FD3 with different circulation times through solid-phase peptide synthesis. All the 3 reported peptides bind to human and murine FAP with single-digit nanomolar affinity measured by surface plasmon resonance. The diagnostic and therapeutic potential of the agents labeled with 68Ga and 177Lu was assessed in several tumor models exhibiting different levels of FAP expression. While radiolabeled FD1 was rapidly excreted from kidneys, radiolabeled FD2/FD3 have significantly prolonged circulation, increased tumor uptake, and decreased kidney accumulation. Our findings indicated that [68Ga]Ga-DOTA-FD1 positron emission tomography (pet) effectively detected FAP dynamics, whereas [177Lu]Lu-DOTA-FD2 and [177Lu]Lu-DOTA-FD3 exhibited remarkable therapeutic efficacy in FAP-overexpressing tumor models, including pancreatic cancer cell models characterized by abundant stroma. Moreover, a pilot translational investigation demonstrated that [68Ga]Ga-DOTA-FD1 had the capability to identify both primary and metastatic tumors with precision and distinction. In summary, we developed [68Ga]Ga-DOTA-FD1 for same-day pet imaging of FAP dynamics and [177Lu]Lu-DOTA-FD2 and [177Lu]Lu-DOTA-FD3 for effective radioligand therapy of FAP-overexpressing tumors.

AIM:To evaluate the value of 18F-fluorodeoxyglucose positron emission tomography/computed tomography(18F-FDG pet/CT) in the restaging of resected rectal ***:From January 2007 to Sep 2008,21 patients who had undergone curative surgery resection for rectal carcinoma with suspicious relapse in conventional imaging or clinical findings were retrospectively enrolled in our *** patients underwent 28 pet/CT scans(two patients had two scans,one patient had three and one had four scans).Locoregional recurrences and/or distant metastases were confirmed by histological analysis or clinical and imaging ***:Final diagnosis was confirmed by histopathological diagnosis in 12 patients(57.1) and by clinical and imaging follow-up in nine patients(42.9).Eight patients had extrapelvic metastases with no evidence of pelvic *** patients had both pelvic recurrence and extrapelvic metastases,and two patients had pelvic recurrence only.18F-FDG pet/CT was negative in two patients and positive in 19 patients.18F-FDG pet/CT was true positive in 17 patients and false positive in *** accuracy of 18F-FDG pet/CT was 90.5,negative predictive value was 100,and positive predictive value was *** patients with perirectal recurrence underwent 18F-FDG pet/CT image guided tissue core biopsy.18F-FDG pet/CT also guided surgical resection of pulmonary metastases in three patients and monitored the response to salvage chemotherapy and/or radiotherapy in four ***:18F-FDG pet/CT is useful for evaluating suspicious locoregional recurrence and distant metastases in the restaging of rectal cancer after curative resection.
