BACKGROUND Anti-tumor necrosis factor agents were the first biologic therapy approved for the management of Crohn's disease(CD).Heart failure(HF)is a rare but potential adverse effect of these *** objective of this report is to describe a patient with CD who developed HF after the use of *** SUMMARY A 50-year-old woman with a history of hypertension and diabetes presented with abdominal pain,diarrhea,and weight *** and enterotomography showed ulcerations,areas of stenosis and dilation in the terminal ileum,and thickening of the intestinal *** patient underwent ileocolectomy and the surgical specimen confirmed the diagnosis of stenosing *** patient started infliximab and azathioprine treatment to prevent post-surgical *** 6 mo after initiating infliximab therapy,the patient complained of dyspnea,orthopnea,and paroxysmal nocturnal dyspnea that gradually *** revealed biventricular dysfunction,moderate cardiac insufficiency,an ejection fraction of 36%,and moderate pericardial effusion,consistent with *** cardiac disease was considered an infliximab adverse effect and the drug was *** patient received treatment with diuretics for HF and showed improvement of symptoms and cardiac ***,the patient is using anti-interleukin for CD and is *** This reported case supports the need to investigate risk factors for HF in inflammatory bowel disease patients and to consider the risk-benefit of introducing infliximab therapy in such patients presenting with HF risk factors.

Protein/carbohydrate interactions through specific protein families termed lectin control essential biological processes. Galectins, a family of animal lectins defined by shared amino acid sequence with affinity for β-galactosides, appear to be functionally polyvalent in a wide range of biological activity. Recent studies have identified immunoregulatory roles of galectins in intestinal inflammatory disorders. Galectin-1 and galectin -2 contribute to the suppression of intestinal inflammation by the induction of apoptosis of activated T cells, whereas galectin-4 is involved in the exacerbation of this inflammation by specifically stimulating intestinal CD4+ T cells to produce IL-6. We review how different members of the galectins provide inhibitory or stimulatory signals to control intestinal immune response under intestinal inflammation.

Correlation of the Rutgeerts score and recurrence of Crohn’s disease in patients with end ileostomy

Sleep disorders have become a global issue,and discovering their causes and consequences are the focus of many research *** estimated 70 million Americans suffer from some form of sleep *** sleep disorders have been shown to cause neurocognitive impairment such as decreased cognitive ability,slower response times and performance *** research suggests that individuals with sleep abnormalities are also at greater risk of serious adverse health,economic consequences,and most importantly increased all-cause *** research studies support the associations among sleep,immune function and ***,we review the current research linking sleep,immune function,and gastrointestinal diseases and discuss the interdependent relationship between sleep and these gastrointestinal *** physiologic processes including immune system and inflammatory cytokines help regulate the *** inflammatory cytokines such as tumor necrosis factor,interleukin-1(IL-1),and IL-6 have been shown to be a significant contributor of sleep *** the other hand,sleep disturbances such as sleep deprivation have been shown to up regulate these inflammatory *** in these cytokine levels have been demonstrated in certain gastrointestinal diseases such as inflammatory bowel disease,gastro-esophageal reflux,liver disorders and colorectal *** turn,abnormal sleep brought on by these diseases is shown to contribute to the severity of these same gastrointestinal *** of these relationships will allow gastroenterologists a great opportunity to enhance the care of their patients.

Emerging complexity and new roles for the RIG-I-like receptors in innate antiviral immunity

Innate immunity is critical for the control of virus infection and operates to restrict viral susceptibility and direct antiviral immunity for protection from acute or chronic viral-associated diseases including cancer. RIG-I like receptors(RLRs) are cytosolic RNA helicases that function as pathogen recognition receptors to detect RNA pathogen associated molecular patterns(PAMPs) of virus infection. The RLRs include RIG-I, MDA5, and LGP2. They function to recognize and bind to PAMP motifs within viral RNA in a process that directs the RLR to trigger downstream signaling cascades that induce innate immunity that controls viral replication and spread. Products of RLR signaling also serve to modulate the adaptive immune response to infection. Recent studies have additionally connected RLRs to signaling cascades that impart inflammatory and apoptotic responses to virus infection. Viral evasion of RLR signaling supports viral outgrowth and pathogenesis, including the onset of viral-associated cancer.

BACKGROUND Patients with lipopolysaccharide(LPS)-responsive beige-like anchor protein(LRBA)deficiency have a variety of clinical symptoms,but there is no apparent genotype–phenotype correlation,and patients carrying the same mutations may have different ***,it is not easy for doctors to make a decision regarding hematopoietic stem cell transplantation(HSCT)for LRBA-deficient *** hypothesized that there may be a protein–phenotype correlation to indicate HSCT for LRBA-deficient *** To report on three Chinese LRBA-deficient patients and determine the correlation between residual protein expression and disease *** Clinical data of three Chinese LRBA-deficient patients were collected,and protein levels were detected by Western blot *** addition,LRBA mutation information of another 83 previously reported patients was *** All the major clinical findings indicated enteropathy,but patients 1 and 3 presented with more severe symptoms than patient *** and histology indicated nonspecific colitis for patients 1 and 3 but Crohn's disease-like colitis for patient *** heterozygous mutations in LRBA were found in patient 1,and homozygous mutations in LRBA were found in patient 2 and patient *** patient 2 responded well to traditional immunosuppressive *** expression of the LRBA protein in patients 1 and 3 was very low,but in patient 2,a more than 0.5-fold in expression of the LRBA protein was found compared to that in the *** HSCT,patient 1 had increased LRBA protein *** summarized the genetic information of 86 patients,and the mutations in patients 1 and 3 were novel *** We described three Chinese LRBA-deficient patients,two of whom carried novel *** patients had no genotype-phenotype correlations,but their residual LRBA protein expression might be associated with disease outcome and could be an indicator for HSCT.
