Background:Triphala extract is a well known medicinal herbal formula which is usually prescribed by Thai traditional doctors to adjust the physiological functions of the *** studies have reported that Triphala has antioxidant,anti-inflammatory,antihypercholesterolemia and anticancer *** this herbal recipe is commonly used in Thailand,its human safety,especially in the oral form,has not been *** therefore conducted a clinical trial(Phase I).Objective:This study evaluated the safety of administering the aqueous extract of Triphala to healthy volunteers at 2500 mg/***,setting,participants and interventions:An open-label,single-arm trial was conducted at Chulabhorn International College of medicine,Thammasat University,Pathum Thani,Thailand,between July 2017 and July *** study enrolled 10 male and 10 female healthy volunteers;all were given Triphala(water extract;five capsules of 500 mg each)orally,once a day,at bedtime,for four consecutive *** outcome measures:Signs and symptoms,physical examinations,hematology and blood chemistry were assessed at the beginning of the trial and every week thereafter,for four consecutive *** finishing the trial,on day 28,all volunteers were invited to a follow-up session on day 35 to evaluate the safety of the herbal recipe using the same ***:At the oral dose of 2500 mg/d,Triphala had no serious adverse effects in healthy ***,it was found to have significantly improved the volunteers’high-density lipoprotein cholesterol(HDL-C)levels on day 35 and also reduced their blood sugar levels on days 14 and ***:We conclude that aqueous extract of Triphala is safe for healthy volunteers and that it elevates HDL-C levels and lowers blood *** clinical study should investigate its effects on HDL-C and blood sugar levels among the dyslipidemic and prediabetic *** registration:This trial was registered in the Thai clinical Trial Regis

Chinese medicine Education and Its Challenges in the United States

Over the past 4 decades Chinese medicine （CM） has comeincreasingly into the spotlight in the United States as the clinical effectiveness ofCM has been not only empirically well-tested over a long period of time but alsoproven by recent scientific research. It has proven cost effectiveness, safety, andis authorized for natural and holistic approaches. In consideration, CM is one of theunderutilized health care professions in the United States with a promising ***, CM faces many challenges in its education and system, its niche in thehealth care system as an independent profession, legal and ethical issues. This paperdiscusses the confronting issues in the United States： present education, standardsof CM education with shifting first professional degree level, new delivery systemsof CM education. Development of new research models, training of evidence-basedpractice, and implementation of integrative medicine into CM education also are the key issues in the currentCM profession. This paper also discusses opportunities for the CM profession going beyond the current status,especially with a focus on fusion medicine.

Compelling shreds of evidence derived from both clinical and experimental research have demonstrated the crucial contribution of receptor for advanced glycation end products(RAGE)axis activation in the development of neoplasms,including gastric cancer(GC).This new actor in tumor biology plays an important role in the onset of a crucial and long-lasting inflammatory milieu,not only by supporting phenotypic changes favoring growth and dissemination of tumor cells,but also by functioning as a pattern-recognition receptor in the inflammatory response to Helicobacter pylori *** the present review,we aim to highlight how the overexpression and activation of the RAGE axis contributes to the proliferation and survival of GC cells as and their acquisition of more invasive phenotypes that promote dissemination and ***,the contribution of some single nucleotide polymorphisms in the RAGE gene as susceptibility or poor prognosis factors is also discussed.

Gastric juice acidity in upper gastrointestinal diseases

AIM: To search the independent factors determining gastric juice acidity and to investigate the acidity of gastric juices in various benign and malignant upper gastrointestinal diseases. METHODS: Fasting gastric juice acidity of 165 healthysubjects and 346 patients with esophageal ulcer (n = 21), gastric ulcer (n = 136), duodenal ulcer (n = 100) or gastric cancer (n = 89) were measured and compared. Additionally, gastric specimens were taken from the antrum and body for rapid urease test and histological examination. RESULTS: Multivariate analysis revealed that bile stain of gastric juice, high acute inflammatory score of the corpus, and atrophy of the corpus were independent risk factors for the development of gastric hypoacidity with odds ratios of 3.1 (95% CI: 1.3-7.3), 3.1 (95% CI: 1.2-7.9) and 3.5 (95% CI: 1.3-9.2). Esophageal ulcer and duodenal ulcer patients had a lower pH level (1.9 and 2.1 vs 2.9, both P < 0.05) of gastric juices than healthy subjects. In contrast, gastric ulcer and gastric cancer patients had a higher pH level (3.4 and 6.6 vs 2.9, both P < 0.001) than healthy controls. Hypoacidity existed in 22%, 5%, 29%, 5% and 88% of healthy subjects, esophageal ulcer, gastric ulcer, duodenal ulcer and gastric cancer patients, respectively. CONCLUSION: Bile reflux, atrophy and dense neutrophil infiltrate of the corpus are three independent factors determining the acidity of gastric juice.

Type 2 diabetes mellitus patients have a markedly higher risk of developing *** multiple factors contribute to this predisposition,one of these involves the increased secretion of amylin,or islet amyloid polypeptide,that accompanies the pathophysiology of type 2 diabetes *** amyloid polypeptide accumulation has undoubtedly been implicated in various forms of dementia,including Alzheimer’s disease and vascular dementia,but the exact mechanisms underlying islet amyloid polypeptide’s causative role in dementia are *** this review,we have summarized the literature supporting the various mechanisms by which islet amyloid polypeptide accumulation may cause neuronal damage,ultimately leading to the clinical symptoms of *** discuss the evidence for islet amyloid polypeptide deposition in the brain,islet amyloid polypeptide interaction with other amyloids implicated in neurodegeneration,neuroinflammation caused by islet amyloid polypeptide deposition,vascular damage induced by islet amyloid polypeptide accumulation,and islet amyloid polypeptide-induced *** are very few therapies approved for the treatment of dementia,and of these,clinical responses have been controversial at ***,investigating new,targetable pathways is vital for identifying novel therapeutic strategies for treating *** such,we conclude this review by discussing islet amyloid polypeptide accumulation as a potential therapeutic target not only in treating type 2 diabetes mellitus but as a future target in treating or even preventing dementia associated with type 2 diabetes mellitus.

Emergency physician's perception of cultural and linguistic barriers in immigrant care:results of a multiple-choice questionnaire in a large Italian urban emergency department

BACKGROUND: A poor communication with immigrants can lead to inappropriate use of healthcare services, greater risk of misdiagnosis, and lower compliance with treatment. As precise information about communication between emergency physicians(EPs) and immigrants is lacking, we analyzed difficulties in communicating with immigrants in the emergency department(ED) and their possible associations with demographic data, geographical origin and clinical ***: In an ED with approximately 85 000 visits per year, a multiple-choice questionnaire was given to the EPs 4 months after discharge of each immigrant in ***: Linguistic comprehension was optimal or partial in the majority of patients. Signifi cant barriers were noted in nearly one fourth of patients, for only half of them compatriots who were able to translate. Linguistic barriers were mainly found in older and sicker patients; they were also frequently seen in patients coming from western Africa and southern Europe. Non-linguistic barriers were perceived by EPs in a minority of patients, more frequently in the elderly and frequent attenders. Factors independently associated with a poor f inal comprehension led to linguistic barriers, non-linguistic obstacles, the absence of intermediaries, and the presence of patient's fear and hostility. The latter probably is a consequence, not the cause, of a poor ***: Linguistic and non-linguistic barriers, although quite infrequent, are the main factors that compromise communication with immigrants in the ED, with negative effects especially on elderly and more seriously ill patients as well as on physician satisfaction and appropriateness in using services.

The gastrointestinal(GI) barrier serves a critical role in survival and overall health of animals and humans. Several layers of barrier defense mechanisms are provided by the epithelial, immune and enteric nervous systems. Together they act in concert to control normal gut functions(e.g., digestion,absorption, secretion, immunity, etc.) whereas at the same time provide a barrier from the hostile conditions in the luminal environment. Breakdown of these critical GI functions is a central pathophysiological mechanism in the most serious GI disorders in pigs. This review will focus on the development and functional properties of the GI barrier in pigs and how common early life production stressors, such as weaning, can alter immediate and long-term barrier function and disease *** stress-related pathophysiological mechanisms responsible for driving GI barrier dysfunction induced by weaning and the implications to animal health and performance will be discussed.

Effectiveness of Huachansu injection combined with chemotherapy for treatment of gastric cancer in China: a systematic review and Meta-analysis

OBJECTIVE:to evaluate the effectiveness and safety of Huachansu(HCS)injection plus chemotherapy in the treatment of gastric ***:A thorough and systematic retrieval of randomized controlled trials(RCTs)concerning HCS injection for treating gastric cancer was conducted in several electronic databases from inception to May 10,*** quality of the RCTs was assessed by the Cochrane risk of bias *** the data about objective remission rate,performance status,adverse drug reactions(ADRs)and other outcomes were extracted and analyzed by Review Manager 5.3 and Stata 13.0 ***:A total of 14 RCTs with 976 participants were involved in the current *** results suggested that HCS injection combined with chemotherapy was associated with better effects than receiving conventional chemotherapy alone in respect of improving the objective response rate[RR=1.18,95%CI(1.03,1.37),Z=2.32,P=0.02],and performance status[RR=1.84,95%CI(1.43,2.36),Z=4.74,P<0.00001].In addition,HCS injection combined with chemotherapy could relieve pain for patients with gastric ***:This Meta-analysis revealed that HCS injection plus chemotherapy might more effective than chemotherapy in treating gastric ***,more large-scale and rigorously designed RCTs should be performed to validate this finding.
