Background: Type 2 diabetes is a metabolic disease characterized by chronic hyperglycemia. The latter plays an essential role in inflammation by stimulating the production of pro-inflammatory cytokines or by increasing their secretion by an oxidative mechanism. These cytokines control the hepatic synthesis of an inflammatory protein called C-Reactive Protein (CRP). High or average values of CRP would have a predictive value of cardiovascular diseases and type 2 diabetes. The relationship between low CRP values and the risk of developing type 2 diabetes and its cardiovascular complications is not sufficiently studied. The hsCRP could serve as a predictive biomarker of risk of onset, follow-up and prognosis for type 2 diabetes and its cardiovascular complications. To answer this problem, we conducted this study, the aim of which was to study the predictive role of hsCRP in the risk of occurrence of type 2 diabetes and its cardiovascular complications. Materials and Methods: This is a prospective and cross-sectional case-control study involving 200 participants including 100 control women and 100 women with type 2 diabetes (mean age was respectively 49.89 years ± 8.26 & 51.92 years ± 7.18;p = 0.066). The interviews were conducted on the basis of a questionnaire. Physical examination collected biometric data and cardiovascular constants. The biochemical parameters such as hsCRP were analyzed by an automated Abbott device. Results: We noted that hsCRP was significantly higher in type 2 diabetic subjects compared to control subjects (p (rho = 0.40, p systolic blood pressure (rho = 0.30, p = 0.003), diastolic blood pressure (rho = 0.28, p = 0.006), total body fat (rho = 0.48, p Conclusion: hsCRP has a prognostic value in the evaluation of cardiovascular risk. Aim： To investigate the roles of liver X receptors （LXR） in the lipid composition and gene expression regulation in the murine caput epididymidis. LXR are nuclear receptors for oxysterols, molecules derived from cholesterol metabolism that are present in mammals as two isoforms： LXRα, which is more specifically expressed in lipid-metabolising tissues, such as liver, adipose and steroidogenic tissues, and macrophages, whereas LXRβ is ubiquitous. Their importance in reproductive physiology has been sustained by the fact that male mice in which the function of both LXR has been disrupted have fertility disturbances starting at the age of 5 months, leading to complete sterility by the age of 9 months. These defects are associated with epididymal epithelial degeneration in caput segments one and two, and with a sperm midpiece fragility, leading to the presence of isolated sperm heads and flagella when luminal contents are recovered from the cauda epididymidis. Methods： The lipid composition of the caput epididymidis of wild-type and LXR-deficient mice was assessed using oil red O staining on tissue cryosections and lipid extraction followed by high performance liquid chromatography or gas chromatography. Gene expression was checked by quantitative real time polymerase chain reaction. Results： Using LXR-deficient mice, we showed an alteration of the lipid composition of the caput epididymidis as well as a significantly decreased expression of the genes encoding SREBPlc, SCd1 and SCd2, involved in fatty acid metabolism. Conclusion： Altogether, these results show that LXR are important regulators of epididymal function, and play a critical role in the lipid maturation processes occurring during sperm epididymal maturation. （Asian J Androl 2007 July; 9： 574-582）

Lipid metabolism disorders would be among the components responsible for the risk of the onset of T2dM and its vascular complications. Apolipoprotein E plays an important role in lipid metabolism. We studied the involvement of the APOE gene in the onset of T2dM and its vascular complications. Clinical and biochemical parameters were assessed in each participant. APOE genotypes were identified by PCR-RFLP. Arterial stiffness was studied using a pOpmetre® which evaluates the pulse wave velocity (ft-PWV). Endothelial dysfunction was studied using an EndoPAT2000® which measures endothelium-dependent vasodilation (RHI). In control subjects, the ε3 allele was associated with an increase in fasting blood glucose (r = 2.36, p = 0.018), and a decrease in LdL cholesterol levels (r = −2.17, p = 0.03), and ε4 was associated with an increase in total cholesterol (r = 2.59, p = 0.01), LdL cholesterol (r = 2.84, p = 0.004), and No-HdL cholesterol (r = 2.74, p = 0.006). In type 2 diabetes subjects, the ε2 was associated with a decrease in diastolic blood pressure (r = −2.25, p = 0.02). The ε3 was associated with a decrease in ft-PWV (r = −2.26, p = 0.024) while the ε4 was associated with an increase in ft-PWV (r = 2.52, p = 0.012). Carrying the ε2ε3 genotype would have in 99% a limited risk of developing T2dM, and in event of T2dM, only 1 to 2% would have a significant risk of developing atherosclerosis, which would be severe in 17%. Of the ε2ε4 genotype, 93% had a limited or even possible risk of developing T2dM, the remaining 7% had a very high risk of developing T2dM. diabetics carrying ε2ε4 had in 7% very high risk of developing atherosclerosis. The latter had a 20% very high risk of being very severe. Biochemical parameters are useful in the diagnosis of many health abnormalities in athletes. Some studies suggest training well and eating a balanced diet to improve performance. The modification of biochemical parameters during endurance races has not yet been studied in many African countries. Our objective was to evaluate the dietary profile and the effects of competition on the biochemical parameters of Congolese endurance athletes. The method used was the questionnaire for the food profile and blood samples were taken to measure the biochemical parameters of the athletes. A total of 64 athletes, i.e. 20 girls and 44 boys divided into two groups (group 1 or experimental group (EG), 32 endurance athletes and group 2 or control group (CG), 32 walkers who participated in a cross-sectional study in Brazzaville. The subjects were respectively 26.16 ± 2.79 years old for the EG and 27.44 ± 3.34 years old for the CG. The results indicated that quality foods were difficult to access. The main course was more consumed compared to the starter and dessert (60% of girls and 63.63 of boys). However, biochemical parameters showed a significantly lower serum creatinine concentration in EG subjects compared to CG subjects (0.86 ± 0.06 mg/dl vs 1.04 ± 0.16 mg/dl;t = ?5.95;p 0.05). Similarly, Total Cholesterol was significantly low (1.66 ± 0.34 g/l vs 2.09 ± 0.50 g/l;t = ?3.99;p dL-C) were significantly elevated (0.98 ± 0.49 g/l vs 0.48 ± 0.18 g/l;t = 5.34;p data were normal but the food profile of the runners was unfavorable, inadequate and unbalanced. This is considered to be a performance limiting factor.

Impact of the professional exposure on the spermatic parameters and the results of ICSI at Tunisian unfertile couples

Introduction: The association of the professional exposure to the decline of the male reproductive function had been the subject of several studies but without arriving to one envisagement undeniable of its existence. The ICSI, technique of reference in severe male infertilities, is visavis unexplained failures whose environmental and professional exposure could be about its cause partly. In this work, we tried to highlight an association between certain factors of professional exposure and the deterioration of the spermatic parameters, then to evaluate the effect of such factors on the result of the ICSI. Materials and Methods: It is about a study of the exposed type—no exposed, realized through collaboration between the Unit of Medicine of the Reproduction and the Service of Medicine of Work of the CHU Farhat Hached of Sousse. This study was based on a questionnaire of which part of the items was collected medical files of the patients raising the general characteristics, the profile of exposure as well as the spermatic parameters and the biological assessment of ICSI. Results: Significant associations between the deterioration of sperm and the professional risk factors such as the exposure to the formaldehydes (p = 0.01) and heat (p = 0.001) were noted. The exposure to formaldehydes was associated at the higher risk of oli gozoospermy, of asthenozoospermy with a significant reduction in mobility (p = 0.05), whereas the talks with cement had a risk higher of teratozoospermy. In addition an exposure to heat was associated at the high risk of oligoteratozoospermy. The total rate of pregnancies after ICSI among 220 patients of our series was of 14.47%. The ICSI balance showed a reduction in total fertility rate (76.6% exposed vs 75.3% no exposed). Conclusion: In spite of the limited number of the patients, this study had shown a correlation between certain professional exposures of origin and the deterioration of sperm. Thyroid Function in Pregnant Women from a West-African Population

Thyroid dysfunction is frequent in pregnant women and is often associated with an increased risk of adverse maternal and fetal outcome. In the present work, thyroid function of pregnant women from Benin republic was studied. Two hundred forty (240) pregnant women, without thyroid disease history, have been included in the study. A blood sample was drawn for measurement of TSH, free T3 and free T4 serum levels. From the first to the third trimester, plasma levels of free T3 and free T4 decreased when plasma levels of TSH increased. Using recommendations of the 2011 American Thyroid Association (ATA) guidelines, thyroid dysfunction was observed in 24.17% of subjects. Hypothyroidism was present in 22.50% of subjects with 19.17% of subclinical hypothyroidism, 0.83% of overt hypothyroidism and 2.50% of hypothyroxinemia and hyperthyroidism was observed in 1.67% of subjects. An increase from 15.52% to 28.07% was observed in the frequency of hypothyroidism from the first to the third trimester of pregnancy. When an upper cut-off value of 4 mU/L was used for TSH, as recommended in the 2017 ATA guidelines, prevalence of thyroid disorders was 14.58% with 12.91% of hypothyroidism but no hypothyroidism was observed in women in the first trimester of pregnancy. A rise in hypothyroidism frequency was observed when pregnant women age increased. Hypothyroidism was very common in pregnant women in Benin. To allow accurate assessment of thyroid status in pregnant women in Benin, pregnancy specific range for plasma level of TSH and thyroid hormones should be established.

Life at high altitude is a challenge for thousands of animal species and millions of humans residing or visiting high altitude regions of the world. To try to understand the physiological mechanisms involved in the adaptation processes to high altitude hypoxia, it is convenient to start by defining what is 'extreme' from a biological point of view.

Introduction: Chlorine as a disinfectant has many side effects on respiratory function. This respiratory toxicity may be related to the exposure time. Materials and Methods: Measure the impact of the duration of chlorine gas exposure on the respiratory function of Senegalese workers who work in the national water production factory: Senegalaise des Eaux (Sde), aged 35-54 years. Twenty-four (24) male subjects exposed to chlorine gas were divided into two groups, one (35-44) and the other (45-54). Clinical examinations were performed on each employee, followed by chest x-rays and spirometry. We included all subjects whose first or last two spirometry results were normal. Results: In the 35-44 years group, 50% of subjects had disturbed spirometry including 41.6% of obstructive syndrome (OS), at the second spirometry and 8.3% had a restrictive syndrome FVC at -25%, at the 3rd spirometry 4/12 had a normal spirometry. The duration of exposure was 10 years. For subjects in 45-54 years, 8/12 had persistent OS or accentuated, then at the 3rd spirometry we had 10 subjects. The duration of exposure was 17.5 years. Conclusion: Compared with (35-44) group, older (45-54) subjects had significant OS, which may be related to duration of exposure.
