BACKGROUND Emphysema pyelonephritis(EPN)is a very dangerous type of urinary tract *** is a lethal disease that develops rapidly and causes the patient to deteriorate rapidly,and it can easily lead to systemic infections and even *** incidence is extremely low,and it is prevalent in patients with *** here report a case of EPN in a non-diabetic patient with autosomal dominant polycystic kidney disease(ADPKD).We share the diagnosis and treatment procedure for this extremely rare condition to make this disease easier to identify and address *** SUMMARY A 47-year-old woman presented to the emergency department of our hospital with a high fever and left back pain lasting 4 *** had a history of autosomal dominant polycystic kidney and polycystic *** was diagnosed with left type I EPN and her vital signs deteriorated so quickly that she underwent an emergency operation in which a D-J tube was inserted into her left ureter on the second day after *** months later,she underwent a second-stage flexible ureteroscopy and *** postoperative sepsis,she finally recovered after active symptomatic support treatment and effective anti-infective *** Although EPN is more likely to occur in diabetic patients,for non-diabetic patients with ADPKD and upper urinary tract obstruction,the disease also causes rapid *** and accurate diagnosis and timely removal of the obstruction by invasive means may be able to save the damaged kidney and the patient’s life.

Evidence-based interventions of threatened miscarriage

Threatened miscarriage is the commonest complication of early pregnancy and affects about 20% of pregnancies. It presents with vaginal bleeding with or without abdominal cramps. Increasing age of women, smoking, obesity or polycystic ovary syndrome(PCOS) and a previous history of miscarriage are risk factors for threatened miscarriage. The pathophysiology has been associated with changes in levels of cytokines or maternal immune dysfunction. Clinical history and examination, maternal serum biochemistry and ultrasound findings are important to determine the treatment options and provide valuable information for the prognosis. Bed rest is the commonest advice, but there is little evidence of its value. Other options include progesterone, human chorionic gonadotropin(HCG) and muscle relaxants. The complementary and alternative medicine(CAM) therapies such as acupuncture and Chinese herbs have also been tried. There is some evidence from clinical studies indicating that CAM therapies may reduce the rate of miscarriage, but the quality of studies is poor. Thus, further double-blind,randomized-controlled trials are necessary to confirm its effectiveness, especially acupuncture and Chinese herbs.

This paper applies bilinear immersed finite elements （IFEs） in the interior penalty discontinuous Galerkin （DG） methods for solving a second order elliptic equation with discontinuous coefficient. A discontinuous bihnear IFE space is constructed and applied to both the symmetric and nonsymmetric interior penalty DG formulations. The new methods can solve an interface problem on a Cartesian mesh independent of the interface with local refinement at any locations needed even if the interface has a nontrivial geometry. Numerical examples are provided to show features of these methods.

AbstractLet G be a finite group and H a subgroup of G. Then H is said to be S-permutable in G if HP = PH for all Sylow subgroups P of G. Let H sG be the subgroup of H generated by all those subgroups of H which are S-permutable in G. Then we say that H is S-embedded in G if G has a normal subgroup T and an S-permutable subgroup C such that T ∩ H ? H sG and HT = C.Our main result is the followingTheorem AA group G is supersoluble if and only if for every non-cyclic Sylow subgroup P of the generalized Fitting subgroup F*(G) of G, at least one of the following holds: Every maximal subgroup of P is S-embedded in G.Every cyclic subgroup H of P with prime order or order 4 (if P is a non-abelian 2-group and H ? Z ∞ (G)) is S-embedded in G.

Since the relationship between angiogenesis and tumor growth was established by Folkman in 1971, scientists have made efforts exploring the possibilities in treating cancer by targeting angiogenesis. Inhibition of angiogenesis growth factors and administration of angiogenesis inhibitors are the basics of anti- angiogenesis therapy. Transfer of anti-angiogenesis genes has received attention recently not only because of the advancement of recombinant vectors, but also because of the localized and sustained expression of therapeutic gene product inside the tumor after gene transfer. This review provides the up-to-date information about the strategies and the vectors studied in the field of anti-angiogenesis cancer gene therapy.

A series of novel atropisomeric diphosphine ligands termed Tan Phos were designed and synthesized,which has a smaller bite angle compared with that of other ligands such as *** Phos showed high reactivity and enantioselectivity in the rhodiumcatalyzed asymmetric hydrogenation ofα-dehydro amino ketones,and up 99%yield and 99%ee were obtained for a wide range of chiralα-amino ketones.

In this paper, we study optimal value functions of generalized semi-infinite min-max programming problems on a noncompact set. Directional derivatives and subdifferential characterizations of optimal value functions are given. Using these properties,we establish first order optimality conditions for unconstrained generalized semi-infinite programming problems.

Wear Evaluation on Ni_3Al/MnS Composite Related to Metallurgical Processes

Iron alloyed Ni3Al with composition of Ni-18. 8Ab10. 7Fe-0. 5Mn-0. 5Ti-0. 2B in atom percent （NAC alloy） showed attractive tribological properties under unlubrication condition at room temperature. The alloy was prepared by hot isostatic pressing （HIP） process. The wear properties were associated with its intrinsic deformation mechanism. Unfortunately, the single phase NAC-alloy worked inadequately with its counterpart disk, and also showed a poor machinability. In the present work, NAC-alloy matrix composite with 6 % （volume percent） MnS particle addi- tion was studied to improve its wear behaviors and performance on machining. Two metallurgical processes of HIP and vacuum casting were applied to produce the testing materials. Pin-on-disk （POD） measurements were carried out at room temperature. A commercial vermicular graphite cast iron was selected as a reference material. The counter- part disk was made of a grey cast iron as liner material in ship engines. The contact pressures of 2.83 MPa and 5.66 MPa were normally applied in the tests. The investigation indicated that MnS particle addition in the NAC-alloy composites functions as an effective solid lubricant, and improved wear properties and machinability of the materials. Obvi- ously, as-cast NAC-alloy with in-situ formed MnS-phase was working more effectively with the counterpart, compa- ring to the HIPed NAC-alloy composite with MnS particles. At the high contact pressure of 5.66 MPa, the specific wear rate of the as-cast NAC-alloy composite was high. The phenomenon of the negative effect is mostly due to the brittle second NiAl phase as evidenced in the microstructure analysis.

Anti-tumor immunity of BAM-SiPc-mediated vascular photodynamic therapy in a BALB/c mouse model

In recent decades, accumulating evidence from both animal and clinical studies has suggested that a sufficiently activated immune system may strongly augment various types of cancer treatment, including photodynamic therapy （PDT）. Through the generation of reactive oxygen species, PDT eradicates tumors by triggering localized tumor damage and inducing anti-tumor immunity. As the major component of anti-tumor immunity, the involvement of a cell-mediated immune response in PDT has been well investigated in the past decade, whereas the role of humoral immunity has remained relatively unexplored. In the present investigation, using the photosensitizer BAM-SiPc and the CT26 tumor-bearing BALB/c mouse model, it was demonstrated that both cell-mediated and humoral adaptive immune components could be involved in PDT. With a vascular PDT （VPDT） regimen, BAM-SiPc could eradicate the tumors of -70% of tumor-bearing mice and trigger an anti-tumor immune response that could last for more than 1 year. An elevation of Th2 cytokines was detected ex vivoafter VPDT, indicating the potential involvement of a humoral response. An analysis of serum from the VPDT-cured mice also revealed elevated levels of tumor-specific antibodies. Moreover, this serum could effectively hinder tumor growth and protect the mice against further re-challenge in a T-cell-dependent manner. Taken together, these results show that the humoral components induced after BAM-SiPc-VPDT could assist the development of anti-tumor immunity.
