BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS)is an effective intervention for managing complications of portal hypertension,particularly acute variceal bleeding(AVB).While effective in reducing portal pressure and preventing rebleeding,TIPS is associated with a considerable risk of overt hepatic encephalopathy(OHE),a complication that significantly elevates mortality *** To develop a machine learning(ML)model to predict OHE occurrence post-TIPS in patients with AVB using a 5-year *** This retrospective single-center study included 218 patients with AVB who underwent *** dataset was divided into training(70%)and testing(30%)*** features were identified using embedded methods and recursive feature *** ML algorithms-random forest,extreme gradient boosting,and logistic regression-were validated via 10-fold *** Additive exPlanations analysis was employed to interpret the model’s *** analysis was conducted using Kaplan-Meier curves and stepwise Cox regression analysis to compare overall survival(OS)between patients with and without *** The median OS of the study cohort was 47.83±22.95 *** the models evaluated,logistic regression demonstrated the highest performance with an area under the curve(AUC)of *** predictors identified were Child-Pugh score,age,and portal vein ***-Meier analysis revealed that patients without OHE had a significantly longer OS(P=0.005).The 5-year survival rate was 78.4%,with an OHE incidence of 15.1%.Both actual OHE status and predicted OHE value were significant predictors in each Cox model,with model-predicted OHE achieving an AUC of 88.1 in survival *** The ML model accurately predicts post-TIPS OHE and outperforms traditional models,supporting its use in improving outcomes in patients with AVB.

Background Non-interventional large-scale research on real-world patients who had a stroke requires the use of multiple data sources ensuring access to longitudinal data from large populations with clinically-detailed *** sought to establish a framework for longitudinal research on patients hospitalised with stroke by linking information-rich,deidentified inpatient data from the Paul Coverdell National Acute stroke Program(PCNASP)to commercial and Medicare Advantage longitudinal claims *** All stroke admissions in PCNASP between 2008 and 2015 were evaluated for linkage to longitudinal claims from a commercial insurer using an algorithm based on six available common data fields(patient age,gender,admission date,discharge date,discharge diagnosis and state)and a hospital *** evaluated the linkage quality(via the percentage of unique records in the linked dataset)and the representativeness of the linked *** also described medical history,stroke severity and patterns of medication use among the PCNASP-claims linked *** The linkage produced uniqueness equal to 99.1%.We identified 5644 linked and 98896 unlinked patients who had an ischaemic stroke hospitalisation in claims *** patients were younger than unlinked(69.7 vs 72.5 years),but otherwise similar by medical history,prestroke medication use or lab *** severity was mild and most patients were discharged *** and discharge use of antihypertensive and statins in the PCNASP were greater than their use as measured by filled prescriptions in *** High-quality linkage between the PCNASP and commercial claims data is *** linkage identified differences between reported or recommended versus actual out-of-hospital medication utilisation,highlighting the importance of longitudinal data availability for research aimed to improve the care of patients who had a stroke.

We assessed the global incidence,mortality,and disability-adjusted life years(DALYs)associated with various liver diseases,including alcohol-related liver disease(ALD),hepatitis B/C virus infections(HBV or HCV),liver cancer,metabolic dysfunction-associated steatotic liver disease(MASLD),and other chronic liver diseases,from the 2019 Global Burden of Disease ***,we analyzed the global trends in hepatology research and drug *** 2000 to 2019,prevalence rates increased for ALD,MASLD and other liver diseases,while they decreased for HBV,HCV,and liver *** with a high socio-demographic index(SDI)exhibited the lowest mortality rates and *** burden of liver diseases varied due to factors like sex and *** nine representative countries,MASLD,along with hepatobiliary cancer,showed highest increase in funding in hepatology ***,the major research categories in hepatology papers from 2000 to 2019 were cancer,pathobiology,and *** United States(U.S.)was at the forefront of hepatology research,with China gradually increasing its influence over *** worldwide are increasingly focusing on studying the communication between the liver and other organs,while underestimating the research on ***,HCV,and MASLD were the primary diseases targeted for therapeutic development in clinical ***,the proportion of new drugs approved for the treatment of liver diseases was relatively low among all newly approved drugs in the U.S.,China,Japan,and the European ***,there were no approved drug for the treatment of ALD in the world.

This review discussed experimental mouse models used in the pre-clinical study of liver fibrosis regression,a pivotal process in preventing the progression of metabolic dysfunction-associated steatohepatitis to irreversible liver *** models provide a valuable resource for understanding the cellular and molecular processes underlying fibrosis regression in different *** primary focus of this review is on the most commonly used models with diet-or hepatotoxin-induced fibrosis,but it also touches upon genetic models and mouse models with biliary atresia or parasiteinduced *** addition to emphasizing in vivo models,we briefly summarized current in vitro approaches designed for studying fibrosis regression and provided an outlook on evolving methodologies that aim to refine and reduce the number of experimental animals needed for these ***,these models contribute significantly to unraveling the underlying mechanisms of liver fibrosis regression and offer insights into potential therapeutic *** presenting a comprehensive overview of these models and highlighting their respective advantages and limitations,this review serves as a roadmap for future research.

Diabetic foot ulcers(DFUs)are a serious vascular ***,no effective methods are available for treating ***-protein convertase subtilisin/kexin type 9(PCSK9)regulates lipid levels to promote ***,the role of PCSK9 in DFUs remains *** this study,we found that the expression of PCSK9 in endothelial cells(ECs)increased significantly under high glucose(HG)stimulation and in diabetic plasma and ***,PCSK9 promotes the E3 ubiquitin-protein ligase NEDD4 binding to vascular endothelial growth factor receptor 2(VEGFR2),which led to the ubiquitination of VEGFR2,resulting in its degradation and downregulation in ***,PCSK9 suppresses the expression and activation of AKT,endothelial nitric oxide synthase(e NOS),and ERK1/2,leading to decreased nitric oxide(NO)production and increased superoxide anion(O_(2)^(·-))generation,which impairs vascular endothelial function and ***,using evolocumab to limit the increase in PCSK9 expression blocked the HG-induced inhibition of NO production and the increase in O_(2)^(·-)production,as well as inhibited the phosphorylation and expression of AKT,e NOS,and ERK1/***,evolocumab improved vascular endothelial function and angiogenesis,and promoted wound healing in *** findings suggest that targeting PCSK9 is a novel therapeutic approach for treating DFUs.

BACKGROUND Only one case of liver transplantation for hepatic adenoma has previously been reported for patients with rupture and uncontrolled *** present the case of a massive ruptured hepatic adenoma with persistent hemorrhagic shock and toxic liver syndrome which resulted in a two-stage liver *** is the first case of a two-stage liver transplantation performed for a ruptured hepatic *** SUMMARY A 23 years old African American female with a history of pre-diabetes and oral contraceptive presented to an outside facility complaining of right-sided chest pain and emesis for one *** was found to be in hemorrhagic shock due to a massive ruptured hepatic hepatic *** underwent repeated embolizations with interventional radiology with ongoing hemorrhage and the development of renal failure,hepatic failure,and hemodynamic instability,known as toxic liver *** the setting of uncontrolled hemorrhage and toxic liver syndrome,a hepatectomy with porto-caval anastomosis was performed with liver transplantation 15 h *** tolerated the anhepatic stage well,and has done well over one year *** When toxic liver syndrome is recognized,liver transplantation with or withouthepatectomy should be considered before the patient becomes unstable.
