Comparative Evaluation of Implant Placement with Conventional and Digital Surgical Guide Techniques: Two Case Reports

Background: Implant placement using a conventional surgical guide and digital surgical guide techniques is well documented in the literature. The most frequently reported disadvantages of conventional surgical guide placement are lack of accuracy in implant placement when compared to three-dimensional assessment in digital technique. Other factors listed are longer time duration and the need for impression techniques. In this case report, the authors present a comparison between the two techniques and the time taken between both cases one done conventionally and another case by digital technique. Case Presentation: For the digital surgical guide, a 44-year-old, male reported with the chief complaint of missing teeth needing replacement was considered. For the conventional technique, a female patient aged fifty-seven who had gone through various dentists with an existing bridge was considered. This patient wanted a good outcome at a reasonable cost. In both cases, molars were missing and needed replacement. The steps for digital flow for a surgical guide and step-by-step conventional methods are both highlighted in this article. Conclusion: Hence the digital technique saved time and was accurate when compared to the conventional in our experience.

Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors

Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. CA 19-9 < 37 U/mL, smaller tumor size, and R0 resection were found to be independent factors for long-term survival ≥ 5 years.

Biliary leakage following pancreaticoduodenectomy:Prevalence,risk factors and management

Background: Few studies investigated biliary leakage after pancreaticoduodenectomy(PD) especially when compared to postoperative pancreatic fistula(POPF). This study was to determine the incidence of biliary leakage after PD, predisposing factors of biliary leakage, and its management. Methods: We retrospectively studied all patients who underwent PD from January 2008 to December 2017 at Gastrointestinal Surgery center, Mansoura University, Egypt. According to occurrence of postoperative biliary leakage, patients were divided into two groups. Group(1) included patients who developed biliary leakage and group(2) included patients without identified biliary leakage. The preoperative data, operative details, and postoperative morbidity and mortality were analyzed. Results: The study included 555 patients. Forty-four patients(7.9%) developed biliary leakage. Ten patients(1.8%) had concomitant POPF. Multivariate analysis identified obesity and time needed for hepaticojejunostomy reconstruction as independent risk factors of biliary leakage, and no history of preoperative endoscopic retrograde cholangiopancreatiography(ERCP) as protective factor. Biliary leakage from hepaticojejunostomy after PD leads to a significant increase in development of delayed gastric emptying, and wound infection. The median hospital stay and time to resume oral intake were significantly greater in the biliary leakage group. Non-surgical management was needed in 40 patients(90.9%). Only 4 patients(9.1%) required re-exploration due to biliary peritonitis and associated POPF. The mortality rate in the biliary leakage group was significantly higher than that of the non-biliary leakage group(6.8% vs 3.9%, P = 0.05). Conclusions: Obesity and time needed for hepaticojejunostomy reconstruction are independent risk factors of biliary leakage, and no history of preoperative ERCP is protective factor. Efficacy of octreotide in the prevention of complications after pancreaticoduodenectomy in patients with soft pancreas and non-dilated pancreatic duct: A prospective randomized trial

Background: The efficacy of octreotide to prevent postoperative pancreatic fistula(POPF) of pancreaticoduodenectomy(PD) is still controversial. This study aimed to evaluate the effect of postoperative use of octreotide on the outcomes after ***: This is a prospective randomized controlled trial for postoperative use of octreotide in patients undergoing PD. Patients with soft pancreas and pancreatic duct < 3 mm were randomized to 2 *** I did not receive postoperative octreotide. Group II received postoperative octreotide. The primary end of the study is to compare the rate of ***: A total of 104 patients were included in the study and were divided into two randomized *** were no significant difference in overall complications and its severity. POPF occurred in 11 patients(21.2%) in group I and 10(19.2%) in group II, without statistical significance(P = 0.807). Also, there was no significant differences between both groups regarding the incidence of biliary leakage(P = 0.083), delayed gastric emptying(P = 0.472), and early postoperative mortality(P = 0.727).Conclusions: Octreotide did not reduce postoperative morbidities, reoperation and mortality rate. Also, it did not affect the incidence of POPF and its clinically relevant variants.

Modeling of secondary treated wastewater disinfection by UV irradiation: Effects of suspended solids content

This work aimed to study UV-resistant strains of Pseudomonas aeruginosa, to propose a formulation of the kinetics of secondary treated wastewater disinfection and to underline the influence of suspended solids on the inactivation kinetics of these strains. Some investigations were carried out for the validation of some simulation models, from the simplest, the kinetics model of Chick-Watson reduced to first order, to rather complex models such as multi-kinetic and Collins-Selleck models. Results revealed that the involved processes of UV irradiation were too complex to be approached by a simplified formulation, even in the case of specific strains of microorganisms and the use of nearly constant UV radiation intensity. In fact, the application of Chick-Watson model in its original form is not representative of the kinetics of UV disinfection. Modification, taking into account the speed change during the disinfection process, has not significantly improved results. On the other hand, the application of Collins-Selleck model demonstrates that it was necessary to exceed a least dose of critical radiation to start the process of inactivation. To better explain the process of inactivation, we have assumed that the action of disinfectant on the survival of lonely microorganisms is faster than its action on suspended solids protected or agglomerated to each others. We can assume in this case the existence of two inactivation kinetics during the processes （parallel and independent） of the first-order. For this reason, the application of a new kinetic model by introducing a third factor reflecting the influence of suspended solids in water on disinfection kinetics appeared to be determinant for modeling UV inactivation of P. aeruginosa in secondary treated wastewater.

Simple and sensitive method for the determination of trace amounts of thorium using a benzoquinone derivative

A novel simple, sensitive, rapid, direct, and spectrophotometry-based procedure was investigated to determine Th(IV) at trace amounts. The new method is based on Th(IV) chelation with 3,6-dichloro-2,5-dihydr.xy-l,4-benzoquinone(DDBQ). The reagent reacts with Th(IV) in 0.1 M HCl to form an orange 1:2 complex. The stability constant value is 6.62×10~4 for the Th(IV)complex. The Th(IV)-DDBQ obtained shows one peak with a maximum at about 346 nm. The chelate forms immediately and the absorbance remains stable for over24 h. Beer's law was obeyed in the concentration range0–10 μg mL^(-1). The molar absorptivity and Sandell's sensitivity were 4.4×10~4 L mol^(-1)cm^(-1) and 0.0053 μg cm^(-2), respectively. Different analytical parameters were tested in detail. Interfering ion(cations and anions) effects were tested. Methods for Th(IV) determination by second and third-derivative spectrophotometry were also introduced at about 344 and 341 nm, respectively. These two derivative orders offer the feature of sensitivity without the necessity for solvent extraction,heating, or pre-concentration steps. Finally, the methods were successfully utilized for Th(IV) determination in monazite, environmental water, and wastewater samples.

Impact of cirrhosis on surgical outcome after pancreaticoduodenectomy

Antioxidant therapy in the management of acute,chronic and post-ERCP pancreatitis:A systematic review

We systematically reviewed the clinical trials which recruited antioxidants in the therapy of pancreatitis and evaluated whether antioxidants improve the outcome of patients with pancreatitis. Electronic bibliographic databases were searched for any studies which investigated the use of antioxidants in the management of acute pancreatitis （AP） or chronic pancreatitis （CP） and in the prevention of post-endoscopic retrograde cholangio-pancreatography （post-ERCP） pancreatitis （PEP） up to February 2009. Twenty-two randomized, placebo-controlled, clinical trials met our criteria and were included in the review. Except for a cocktail of antioxidants which showed improvement in outcomes in three different clinical trials, the results of the administration of other antioxidants in both AP and CP clinical trials were incongruent and ***, antioxidant therapy including allopurinol and N-acetylcysteine failed to prevent the onset of PEP in almost all trials. In conclusion, the present data do not support a benefit of antioxidant therapy alone or in combination with conventional therapy in the management of AP, CP or PER Further double blind, randomized, placebo-controlled clinical trials with large sample size need to be conducted.

Fabrication of Porous Mg-Zn Scaffold through Modified Replica Method for Bone Tissue Engineering

Biodegradable scaffolds are essential parts in hard tissue engineering. A highly porous magnesium-zinc （Mg-Zn 4 wt.%） scaffold with different Mg-Zn powder to liquid media ratios （50 wt.%, 70 wt.% and 90 wt.%） and different concentrations of ethanol （0 vol.%, 10 vol.%, 20 vol.% and 40 vol.%） were prepared through modified replica method. The mechanical properties were assessed through compression test and the structures of scaffolds were examined by Scanning Electron Microscope （SEM）. Results show that, the increase in Mg-Zn powder to liquid media ratio （50 wt.% to 90 wt.%） in ethanol free slurry, increases the thickness of struts （37 lam to 74 lam） and the plateau stress （0.5 MPa to 1.4 MPa）. The results obtained from X-ray Diffractometry （XRD） and compression test indicate that consuming ethanol in liquid media of replica, results in higher plateau stress by 46% due to less Mg-water reaction and no formation of Mg（OH）2 in the scaffold. The results of porosity measurement indicate that water-ethanol mixture composition and different solid fractions have no significant effects on true and apparent porosities of the fabricated scaffolds.

Erector spinae plane block at lower thoracic level for analgesia in lumbar spine surgery: A randomized controlled trial

BACKGROUND Patients undergoing lumbar spine surgery usually suffer severe pain in the postoperative *** erector spinae plane block(ESPB),first published in 2016,can anesthetize the ventral and dorsal rami of thoracic nerves and produce an extensive multi-dermatomal sensory *** To assess whether bilateral ultrasound-guided ESPB at a lower thoracic level could improve pain control and quality of recovery in patients undergoing lumbar spine *** A total of 60 patients aged 18-80 years scheduled to undergo lumbar spine surgery with general anesthesia were randomly assigned to two groups:ESPB group(preoperative bilateral ultrasound-guided ESPB at T10 vertebral level)and control group(no preoperative ESPB).Both groups received standard general *** main indicator was the duration to the first patient controlled intravenous analgesia(PCIA)*** In the ESPB group,the duration to the first PCIA bolus was significantly longer than that in the control group(h)[8.0(4.5,17.0)vs 1.0(0.5,6),P<0.01],and resting and coughing numerical rating scale(NRS)scores at 48 h post operation were significantly lower than those in the control group(P<0.05).There was no significant difference between the two groups regarding resting and coughing NRS scores at 24 h post *** consumption during the operation was significantly lower in the ESPB group than in the control group(P<0.01),while there was no significant difference between the two groups regarding morphine consumption at 24 or 48 h post *** the ESPB group,Modified Observer’s Assessment of Alertness/Sedation score within 20 min after extubation was higher and duration in the post-anesthesia care unit was shorter than those in the control group(P<0.01).CONCLUSION In patients undergoing lumbar spine surgery,ultrasound-guided ESPB at a lower thoracic level improves the analgesic effect,reduces opioid consumption,and improves postoperative recovery.
