AIM: To study the interactions between prostacyclin and endothelium-derived nitric oxide in porcine pulmonary arteries. METHODS: Rings of 5th order of porcine pulmonary arteries were studied in vitro for the measurement of tension and the content in cyclic nucleotides. RESULTS: Prostacyclin, given exogenously, caused en-dothelium-potentiated relaxations (inhibition of phenyle-phrine contraction) that were inhibited by the inhibitors of the L-arginine nitric oxide pathway, oxyhemoglobin and Nω-nitro-L-arginine. These inhibitors did not affect the tension in rings without endothelium. Cyclic GMP-con-centrations were not increased above basal concentrations in the presence of prostacyclin. Increases were seen with acetylcholine and sodium nitroprusside. Prostacyclin-stimulated cyclic AMP concentrations did not reach statistical significance compared to controls. Intraductal papillary mucinous neoplasms(IPMN) are mucin producing cystic neoplasms of the pancreas histologically classified as having non-invasive and invasive *** five-year survival rates for non-invasive and associated invasive carcinoma are 90% and 40%,respectively in resected IPMN *** carcinoma within IPMN lesions can be further classified by histological subtype into colloid carcinoma and tubular *** five-year survival rates following resection of colloid carcinoma range from 57%-83% and estimated five-year survival following resection of tubular carcinoma range from 24%-55%.The difference in survival outcome between invasive colloid and tubular IPMN appears to be a function of disease biology,as patients with the tubular subtype tend to have larger tumors with a propensity for metastasis to regional lymph *** matched to resected conventional pancreatic adenocarcinoma lesions by the Memorial Sloan Kettering Cancer center pancreatic adenocarcinoma nomogram,the colloid carcinoma histological subtype has an improved estimated five-year survival outcome compared to conventional pancreatic adenocarcinoma,87% and 23%(P = 0.0001),*** lesions with the tubular carcinoma subtype overall have a similar five-year survival outcome compared to conventional pancreatic ***,when these groups were stratified by regional lymph node status patients with negative regional lymph nodes and the tubular subtype experienced significantly better survival than patients with a similar nodal status and ductal adenocarcinoma with estimated five-year survival rates of 73% and 27%(P = 0.01),respectively.

Background: Acute hyperinsulinemia has been shown to increase QTc interval in lean subjects, but data on obese subjects are still unclear. Aim of this study was to evaluate the effect of acute hyperinsulinemia on QTc interval and QTc dispersion in uncomplicated obesity. Methods: We calculated QTc duration and QTc dispersion in 30 uncomplicated obese subjects(mean age 32.2±7, BMI 36.7±9.4 kg/m2) by measurements of 12-lead ECG recording during a euglycemic hyperinsulinemic clamp. Results: Insulin infusion during the clamp did not significantly modify QTc interval and QTc dispersion in uncomplicated obese subjects(401.5±29.2 vs. 413.7±30.5; 35.4±10.5 vs. 38.7±14.5, respectively). Conclusions: Acute hyperinsulinemia seems to no significantly affect ventricular repolarization in uncomplicated obesity. Insulin resistance and the absence of diabetes and hypertension could explain, at least in part, this finding.

Objective. Ovarian low malignant potential (LMP) tumors have an excellent prognosis when treated by surgical resection. Conservative management usually involves leaving behind the uterus and contralateral adnexa to allow future childbearing. The purpose of this study was to determine the outcome of women treated with fertility-sparing surgery. Methods. All patients diagnosed with ovarian LMP tumors between 1984 and 2003 were identified at three institutions. Data were retrospectively extracted from clinical records. Results. Thirty eight (15% ) of 249 women with LMP tumors underwent fertility-sparing surgery. Twenty-three were nulliparous and four primiparous. Thirty-three (87% ) underwent unilateral salpingo-ophorectomy and five (13% ) cystectomy. Fourteen patients also had contralateral cystectomy or biopsy. Thirty four (89% ) were stage Ⅰ , one (3% ) stage Ⅱ and three (8% ) stage Ⅲ . Most tumors had serous (55% ) or mucinous (42% ) histology. No patients received adjuvant therapy. Six (16% ) of 38 recurred after a median follow-up of 26 months: five in the remaining ovary were salvaged with surgical resection alone, and none died from recurrent LMP tumor. Five women delivered six term infants during post-treatment surveillance. Conclusion. Fertility-sparing surgery for ovarian LMP tumors is an option for motivated patients. Preservation of the contralateral adnexa increases the risk of recurrence, but surgical resection is usually curative.
