Present review for the first time provides a complete botanical description and information on ethnomcdicinal uses of Clerodendron ***(CG:***).Recent studies conducted from our laborator) provide pharmacological evidence for its anti-hypertensive,antidiabetic and anti-obesity ***,its beneficial potential in preventing in vitro and in vivo non-alcoholic steatohepatitis and atherosclerosis and potent hcpatoprotective and free radical scavenging abilities along with its acute and sub- chronic toxicologiesl evaluations are also reported from our *** keeping with its traditional uses,CG extract was capable of ameliorating experimentally induced hypertension,diabetes and *** beneficial potential against NASH induced oxidative stress and atherosclerosis can be attributed to its potent free radical scavenging ***—toxic nature of CG leaf extract further provides added merit to its reported pharmacological *** present review summarizes the pioneering scientific evidence for the pharmacological effects of CG against related metabolic disorders like hypertension,diabetes and obesity along with anti oxidant potential and beneficial elicits against non alcoholic steatohepatitis.

Echinacoside Alleviates Cognitive Impairment in Cerebral Ischemia Rats through α7nAChR-Induced Autophagy

Objectives:To evaluate the effect of echinacoside(ECH)on cognitive dysfunction in post cerebral stroke model ***:The post stroke cognitive impairment rat model was created by occlusion of the transient middle cerebral artery(MCAO).The rats were randomly divided into 3 groups by a random number table:the sham group(sham operation),the MCAO group(received operation for focal cerebral ischemia),and the ECH group(received operation for focal cerebral ischemia and ECH50 mg/kg per day),with 6 rats in each group,The infarct volume and spatial learning were evaluated by triphenyl tetrazolium chloride staining and Morris water *** expression of α7 nAChR in the hippocampus was detected by *** contents of acetylcholine(ACh),malondialdehyde(MDA),glutathione(GSH),superoxide dismutase(SOD),activities of choline acetyltransferase(ChAT),acetylcholinesterase(AChE),and catalase(CAT)were evaluated by enzyme linked immunosorbent *** neural apoptosis and autophagy were determined by TUNEL staining and LC3 staining,***:ECH significantly lessened the brain infarct volume and ameliorated neurological deficit in infarct volume and water content(both P<0.01).Compared with MCAO rats,administration of ECH revealed shorter escape latency and long retention time at 7,14 and 28 days(all P<0.01),increased the α7 nAChR protein expression,ACh content,and ChAT activity,and decreased AChE activity in MCAO rats(all P<0.01).ECH significantly decreased MDA content and increased the GSH content,SOD,and CAT activities compared with MCAO rats(all P<0.05).ECH suppressed neuronal apoptosis by reducing TUNEL-positive cells and also enhanced autophagy in MCAO rats(all P<0.01).Conclusion:ECH treatment helped improve cognitive impairment by attenuating neurological damage and enhancing autophagy in MCAO rats.

In the 1970s, with the advent of biochemical multichannel screening in the United States and other western countries, the clinical presentation of primary hyperparathyroidism （PHPT） changed from a symptomatic to an asymptomatic disorder. However, in Asian countries, like China, PHPT did not show this evolution, but rather continued to be a symptomatic disease with target organ involvement. In this paper, we revisit the clinical features of PHPT in New York and Shanghai, representative United States and Chinese cites, over the past decade. The questions we address are whether the disease evolved in China to a more asymptomatic one and, whether in the United States further changes are evident. The results indicate that while PHPT con- tinues to present primarily as an asymptomatic disease in the United States, a new phenotype characterized by normal serum calcium and high parathyroid hormone levels, normocalcemic PHPT, has emerged. Data from Shanghai demonstrates a trend for PHPT to present more commonly as an asymptomatic disorder in China. However, most patients with PHPT in China still manifest classical symptoms, i.e. nephrolithiasis and fractures. A comparison of the two cohorts shows that Chinese patients with PHPT are younger, with higher serum calcium and PTH levels, and lower 25-hydroxyvitamin D levels than patients in New York. Normocalcemic PHPT has not yet been recognized in Shanghai. In summary, although the phenotypes of PHPT in both cities are evolving towards less evident disease, sharp clinical and biochemical differences are still apparent in PHPT as expressed in China and the United States.

Increased Preoperative Glucose Variability Is Associated with Adverse Perioperative Outcomes Following Orthopedic Surgery in Patients with Type 2 Diabetes Mellitus

The association between glucose variability(GV)and adverse perioperative outcomes in type 2 diabetes mellitus(T2DM)patients undergoing orthopedic surgery was investigated.A retrospective cohort study was performed by analyzing data on T2DM patients receiving continuous blood glucose(BG)monitoring and continuous subcutaneous insulin infusion treatment due to poorly controlled preoperative BG prior to orthopedic *** was assessed with coefficient of variation(CV).Postoperative and perioperative CV,hypoglycemia cases,and other perioperative outcomes(diabetes preparation time[DPT],length of stay[LOS],and perioperative and infective complication cases)were *** showed that a total of 168 patients were grouped into preoperative CV tertiles:1st(n=56):0–0.2921,2nd(n=58):0.2922–0.3779,and 3rd(n=54):0.3780–*** blood glucose(FBG),perioperative CV,rate of hypoglycemia cases(OR:5.53,95%CI:2.43–12.59)(all P<0.001)and DPT(P=0.024)were higher in the 3rd than in the 1st *** adjustments of covariates,regression analysis indicated that the 3rd tertile was associated with increased perioperative CV(adjusted coefficient=0.515,P<0.001),DPT(adjusted coefficient=0.169,P=0.073),rate of hypoglycemia cases(OR:6.72,95%CI:2.69−16.82,P<0.001)and perioperative complication cases(OR:2.50,95%CI:0.90−7.01,P=0.080).In conclusion,preoperative GV is associated with increased perioperative GV and adverse perioperative outcomes including longer DPT and higher rates of hypoglycemia and perioperative complications.

The aim of this study was to explore the associations of moderate-to-vigorous-intensity physical activity(MVPA)time and sedentary(SED)time with a history of cardiovascular disease(CVD)and multifactorial(i.e.,blood pressure(BP),body mass index(BMI),low-density lipoprotein cholesterol(LDL-C),and glycated hemoglobin A1c(HbA1c))control status among type 2 diabetes mellitus(T2DM)patients in China.A cross-sectional analysis of 9152 people with type 2 diabetes from the Multifactorial Intervention on Type 2 Diabetes(MIDiab)study was *** were grouped according to their self-reported MVPA time(low,<150 min·week−1;moderate,150 to<450 min·week−1;high,≥450 min·week−1)and SED time(low,<4 h·d–1;moderate,4 to<8 h·d–1;high,≥8 h·d–1).Participants who self-reported a history of CVD were identified as having a CVD *** ratios(ORs)and 95%confidence intervals(CIs)of CVD risk and multifactorial control status associated with MVPA time and SED time were estimated using mixed-effect logistic regression models,adjusting for China’s geographical region *** participants had a mean±standard deviation(SD)age of(60.87±8.44)years,44.5%were women,and 25.1%had *** adjustment for potential confounding factors,an inverse association between high MVPA time and CVD risk that was independent of SED time was found,whereas this association was not observed in the moderate-MVPA group.A higher MVPA time was more likely to have a positive effect on the control of *** with the reference group(i.e.,those with MVPA time≥450 min·week−1 and SED time<4 h·d–1),CVD risk was higher in the low-MVPA group:The OR associated with an SED time<4 h·d–1 was 1.270(95%CI,1.040–1.553)and that associated with an SED time≥8 h·d–1 was 1.499(95%CI,1.149–1.955).We found that a high MVPA time(i.e.,≥450 min·week−1)was associated with lower odds of CVD risk regardless of SED time among patients with T2DM.

Available data suggest a possible link between abnormalvitamin D level and abnormal glucose homeostasis,two of the most common chronic medical *** conditions are associated with inflammation,and the exact mechanism for role of either on the other is not well *** investigating the link between vitamin D and either pre-diabetic states or diabetes is *** D deficiency is detrimental to insulin synthesis and secretion in animal and human *** humans,it has been shown by majority of observational studies,that vitamin D is positively correlated with insulin sensitivity and its role is mediated both by direct mechanism through the availability of vitamin D receptors in several tissues and indirectly through the changes in calcium *** number of,but not all,variable samples cross sectional human trials have demonstrated an inverse relation between vitamin D status and impaired glucose tolerance,insulin resistance or *** compliment this conclusively,evidence from intervention studies is critically warranted before we can frankly state that vitamin D plays a role in diabetes prevention or *** of both sizable prospective observational trials utilizing 25(OH)D as the main variable and the non-availability of randomized studies specifically designed to assess the effects of vitamin D on pre-diabetes and diabetes states,are the main obstacles to draw solid and conclusive relationships.
