A 72-year-old man was admitted to our hospital to undergo a novel small-intestinal endoscopic procedure. He had had occasional episodes of hematochezia over a 2-year period, during which he had been hospitalized twice previously. However, numerous investigations, including hematological and biochemical studies, gastroscopy, colonoscopy, computed tomography, scintigraphy, and angiography had failed to detect the source of bleeding in the gastrointestinal tract. On this a dmission,double-balloon enteroscopy was performed and revealed several ulcer scars with localized dilation of the ileum. Histopathological examination of the biopsy specimens reveled no abnormal finding. Partial resection of the ileum was performed to prevent further gastrointestinal bleeding, and histopathological examination of the resected specimen revealed aggregation of atypical lymphocytes, predominantly in the muscularis propria layer. Immunohistochemical examination demonstrated that the tumor cells were positive for CD20 and BCL2, but negative for UCHL1. Based on these findings, the lesion was diagnosed as a marginal-zone B-cell lymphoma of mucosa-associated lymphoid tissue. Eighteen months after surgery, the patient was still in complete remission.

The importance of interactions between allergen and IgE in allergen-mediated activation of T lymphocytes from patients with atopic dermatitis (AD) is unclear. A role for this interaction is implied by past evidence for IgE-facilitated presentation of allergen to T cells, but this phenomenon has only been demonstrated in specific in vitro systems biased to maximize the effect. It is therefore not known whether the process is relevant in patients. We now show that the responses to allergen of unmodified peripheral blood mononuclear cells (PBMC)from individual AD patients are significantly greater in the presence of fresh, unheated, IgE-containing autologous serum than the same serum heated under IgE-denaturing conditions or specifically depleted of IgE by immunoprecipitation. In six independent experiments, 59%-67%of the maximal in vitro PBMC response to allergen was found to be dependent upon the presence of IgE in autologous serum used at 5%final concentration. These data provide the first evidence that sufficient amounts of allergen-specific IgE and allergen-reactive T cells occur concomitantly in the blood of individual AD patients to allow IgE-enhanced T cell responses to allergen. We conclude that IgE-enhanced T cell responses are pathophysiologically relevant and a therapeutic target in AD.

In this study, genetic polymorphisms, NQO1 C609T, GSTM1 positive/null, and GSTT1 positive/null, were examined with reference to cervical cancer risk in a population- based incident case- control study in Japanese. The cases comprised 131 cervical cancer patients: 87 cases with squamous cell carcinoma (SCC) and 44 with adenocarcinoma (ADC) or adenosquamous carcinoma (ADSC). Controls were sampled from 320 healthy women who underwent a health checkup. The cervical cancer risk was substantially elevated with smoking for all cases, SCC cases, and ADC/ADSC cases (OR = 4.50, 95% CI = 2.48- 8.17, P cell carcinoma of the cervix.

Among multiple sclerosis(MS)susceptibility genes,the strongest non-human leukocyte antigen(HLA)signal in the Italian population maps to the TNFSF14 gene encoding LIGHT,a glycoprotein involved in dendritic cell(DC)*** fine-mapping in a large Italian dataset(4,198 patients with MS and3,903 controls),we show that the TNFSF14 intronic SNP rs1077667 is the primarily MS-associated variant in the *** quantitative trait locus(e QTL)analysis indicates that the MS risk allele is significantly associated with reduced TNFSF14 messenger RNA levels in blood cells,which is consistent with the allelic imbalance in RNA-Seq reads(Pcells from 84 Italian patients with MS and 80 healthy controls(HCs).Interestingly,patients with MS are lower expressors of TNFSF14 compared to HC(Pcells,which may play a role in MS pathogenesis.

OBJECTIVE: To review the treatment options presented on die New York State H ea lth Care Prosy for Jehovah’s Witnesses, which is signed by pregnant women when they present for care. METHODS: Chart reviews were performed for all women who p resented to labor and delivery at our institution from 1997 to 2002 and identifi ed themselves as Jehovah’s Witnesses. A patient was included in the study if a completed health care proxy was available in her chart Data were derived from th e health care prosy and from the Mount Sinai School of Medicine’s Blood Product Checklist for Jehovah’s Witness Patients. Variables of interest included age, race, parity, and antenatal and perinatal complications. RESULTS: A total of 61 patients were identified. Of these, 39.3%agreed to accept a variety of donated blood products, 9.8%would accept donated packed red blood cells, and 50.1%woul d accept neither from a homologous donor. With respect to nonstored autologous b lood, 55%of respondents would accept either intraoperative normovolemic hemodil ution or transfusion of their own blood obtained by a cell salvage system. No si gnificant differences in responses were noted for any of the above-mentioned va riables. CONCLUSION: This review refutes the commonly held belief that all Jehov ah’s Witnesses refuse to accept blood or any of its products. In this populatio n of pregnant women, the majority were willing to accept some form of blood or b lood products. This information can be used to help health care providers counse l a patient when she is initially faced with considering these issues and may he lp to remove the stigma of accepting one of the options.
